The purpose of this study was to evaluate the effects of the chincap therapy on the craniofacial structure in persons with skeletal Class III malocclusion. The patients selected for this study were treated with extra-oral chincap therapy only. Both control and treatment samples were obtained from Seoul National University Hospital where these longitudinal data were gathered. 55 treated patients and 14 control patients were studied. The mean ages at the 1st evaluation was 8 years 3 months in the treatment sample and 9 years 4 months in the control sample. The duration of chincap therapy was variable but averaged 2 years of treatment. Post-treatment observation procedeeded for 1 year 2 months. Active treatment and post treatment effects were evaluated. The results were as follows: 1. Neither significant restraint nor acceleration of growth was found in the cranial base and maxilla during treatment. 2. A distal rotation of the mandibular complex was seen. 3. Some amount of restraint of growth was found in mandibular body length, ramus height, mandibular length during treatment. 4. The genial angle was reduced. 5. After removal of the chin-cap, forward displacement of the mandible took place.

Study of dentofacial structure relationships relative to TMJ internal derangement is required to increase the predictability of TMJ internal derangement. But few studies have been reported. The purpose of this study is to reveal any correlation of dentofacial characteristics with TMJ internal derangement by lateral cephalometric analysis. Patients were devided into two groups. (1) Symptomatic patients with TMJ internal derangement (2) Asymptomatic volunteers with no TMJ internal derangement. Twenty symptomatic patients with TMJ internal derangement(7male, 13female) were selected from our clinic and had undergone a standarized clinical examination, panorama, transcranical view, TMJ tomography. Twenty asymptomatic volunteers(9male, 11female) were selected from our clinic with no pain, no limitation of motion. All subjects had undergone lateral cephalometric analysis. The results were obtained as follows. 1. No significant difference between ID and normal group is detected in cranial base. 2. Maxilla position of ID group is located more posterioly than normal group. 3. Mandible position of ID group is located more posteriorly than normal group and facial profile is hyperdivergent. 4. Posterior facial height of ID group is less than normal group thus facial profile is hyperdivergent. The patients, as mentioned, have a high prevalance of ID thus it should be careful in TMJ ID diagnosis and treatment.
Objective : The authors attempted to confirm the risk factors for recurrent chronic subdural hematoma[CSDH] after simple burr-hole drainage. Methods : A total of 302 patients with CSDH who were treated at our hospital between January 1998 and May 2005 were studied. Various parameters considered for analysis of factors associated with CSDH recurrence; demographic and clinical findings [age, sex, history of seizures, diabetes, vascular diseases], initial and perioperative CT findings [hematoma density, location of catheter tip, post operative intracranial air, intracranial hematoma extension, hematoma width, hematoma site]. Results : Twenty-four patients [7.9%] experienced recurrence, whereas 278 patients [92.1%] did not. Five major risk factors should be considered : 1) layered type by hematoma density, 2) type I, II by location of catheter tip, 3) presence of postoperative intracranial air, 4) cranial base type of intracranial hematoma extension, 5) greater hematoma width. Conclusion : In this study, we report that the incidence of postoperative CSDH recurrence can be reduced by the examination of the hematoma characteristics on initial and perioperative CT findings and by preventing subdural air accumulation during operation. In addition, the location of the catheter tip can be used as a helpful factor in reducing the recurrence.
This study was performed to aid for the case analysis and diagnosis of the maxillofacial deformities. A cephalometric analysis was made from 101 subjects over 18 years old, that consisted of 46 males and 55 females with normal occlusion, acceptable profile. The results o f this study were obtained as follows: 1. The tables of normals in adult male and female were made. 2. The angles of SNO were $65.5^{\circ}$ in male and $65.3^{\circ}$ in female and the horizontal linear measurements from 0 to NA(O-NA) were 9.0 mm in male and 8.9 mm in female. 3. The cranial base angles (SN-FH) were $8.2^{\circ}$in male and $10.3^{\circ}$ in female and the mandibular plane angles (OP-HP) were $28.1^{\circ}$ in male and $26.2^{\circ}$ in female. 4. The correlation of (O-NA) to S-O/S-A and S-O/S-N were higher than SNO, and the correlation of (N-A) to SNA and (N-B) were high.
This study was performed to report the case of Korean medical treatment for the Patulous Eustachian tube patient with headache. The patient was treated by acupuncture, herbal medicine, bee venom injection, needle-embedding therapy, chuna therapy, release of cranial base and sacro-occipital therapy. Korean medical treatment was administered during 3 weeks. The improvement of clinical symptoms was evaluated by VAS (visual analogue scale) and SF-36 (36-Item Short Form Healthy Survey Instrument). After treatment, most symptoms decreased, headache VAS score changed 10 to 6 and tinnitus VAS score changed 10 to 5. Also, most SF-36 scores increased. Our study suggested that Korean medical treatments are effective in the patient with Patulous Eustachian tube. And further studies are required to identify underlying mechanism of treatment.
Moyamoya disease is a cerebrovascular condition that results in the narrowing of the vessels of the circle of Willis and collateral vessel formation at the base of the brain. Although relationships between Graves' disease and cerebrovascular accidents in Moyamoya disease are obscure, the coexistence of the two diseases is noteworthy. Moyamoya disease has been rarely reported in adolescent patients with thyrotoxicosis. Recently, we encountered two adolescent Korean patients with Moyamoya disease associated with Graves' disease who presented with episodic right-sided hemiparesis and syncope. These two girls who had Graves' disease had no history of other diseases or head trauma. A thyroid function test revealed a euthyroid state and a high thyroid-stimulating hormone (TSH) receptor antibody titer at that time. The patients were diagnosed with Moyamoya disease based on brain magnetic resonance angiography and cerebral four-vessel angiography. The patients underwent cranial revascularization by encephalo-duroarterio-synangiosis as soon as a diagnosis was made, which resulted in successful symptom resolution. They fared well and had no additional neurological symptoms as of their last follow-up visits. Here, we report these two cases of confirmed Moyamoya disease complicated by Graves' disease with a review of the literature, and discuss the possible association between the two diseases. To our knowledge, this is the first report in South Korea on Moyamoya disease associated with Graves' disease in adolescents with a euthyroid.
The CT findings of 46 cases of nasopharyngeal cancer were analyzed and compared with clinical tumor staging. They are composed of 28 cases $(61\%)$ of squamous cell carcinoma, 13cases $(28\%)$ of undifferentiated carcinoma, 4 cases $(9\%)$of lymphoma and 1 case $(2\%)$ of adenoid cystic carcinoma. The results were as follows : 1. The most common CT findings of nasopharyngeal cancers are air·way asymmetry including obliteration of Rosenmuller fossa, orifice of Eustachian tube and asymmetric obliteration of parapharyngeal fat. 2. Other involved anatomic sites are carotid sheath area, oropharynx, paranasal sinuses especially sphenoid sinus, cervical lymph nodes, nasal cavity and skull base or middle cranial fossa. 3. CT does significantly influence on the tumor staging of the nasopharynx cancers, but has a definite value in evaluating deep tissue invasion of the cancers especially to parapharyngeal space or carotid sheath area. 4. CT seems to be essential for staging work-up, estimating the prognosis, and assessing the effect of radiotherapy of the nasopharyngeal cancer because it clearly shows the whole extent of the tumors including deep tissue invasion.
A cephalometric radiographic cross sectional comparative study was undertaken to investigate craniofacial growth in cleft lip and palate individuals. The material for this study consisted of 43 subjects with operated cleft lip and palate.(29 males, 14 females). The range of age was from 6 years to 12 years. The roentgenocephalometric values of cleft individuals were compared with values of normal individuals reported by Lee. The following conclusions were obtained; 1) The pattern of cranial base of the cleft subjects was almost the same as that previously reported for the normal individuals. 2) Anterio-posterior length of the mandible did not show any significant difference but in the cleft subjects, that of the males was larger than that of the females. 3) The maxilla of the cleft subjects was very retruded and showed very concave profile. 4) Gonial angle of the cleft subjects was very high, especially in the female clefts. 5) The height of ramus was very poor in the cleft subjects. 6) The facial length was almost the same, but the facial depth of the cleft subjects was smaller than that of the normal individuals. 7) Maxillary and mandibular incisors were severely retroclined. 8) The range of values in the cleft subjects was very variable and the female clefts showed more distured growth than the male clefts.
This study was undertaken to investigate the inheritance in craniofacial complex among Korean familial members. The subjects were 160 lateral cephalometric radiographs from 45 families. Standard product-moment correlation coefficients(r) were calculated for the measurements between the familial pairings. Further, coefficients of $determination(r^2)$ were calculated and multiple regression analyses were performed to assess the use of parents' record for predicting an offspring's craniofacial features. The following results were obtained. 1. First-degree relatives had a high level of significant correlations which were compatible with a polygenic theory of inheritance than those of non-relatives. 2. Mother-daughter pair had the highest significant correlations, then mother-son and father-son pairs, and, finally, father-daughter pair had the lowest significant correlations. 3. The genetic influence was higher in anterior cranial base length and genial angle. In contrast, the environmental influence was higher in posterior border of ramus, maxillary ant. teeth, maxillary & mandibular apical bases and Gla-P. occ. 4. The predictability of offspring's cranio-facial growth could be improved by using multiple measurements from both parents than those from father or mother only.

The aim of this case report is to describe the treatment of a patient with skeletal Class III malocclusion with maxillary retrognathia using skeletal anchorage devices and intermaxillary elastics. Miniplates were inserted between the mandibular lateral incisor and canine teeth on both sides in a male patient aged 14 years 5 months. Self-drilling mini-implants (1.6 mm diameter, 10 mm length) were installed between the maxillary second premolar and molar teeth, and Class III elastics were used between the miniplates and miniscrews. On treatment completion, an increase in the projection of the maxilla relative to the cranial base (2.7 mm) and significant improvement of the facial profile were observed. Slight maxillary counterclockwise ($1^{\circ}$) and mandibular clockwise ($3.3^{\circ}$) rotations were also observed. Maxillary protraction with skeletal anchorage and intermaxillary elastics was effective in correcting a case of Skeletal Class III malocclusion without dentoalveolar side effects.
