Compliance of mothers' breastfeeding and complementary feeding practices with WHO recommendations in Turkey

  • BACKGROUND/OBJECTIVES: This study aimed to evaluate how breastfeeding and complementary nutrition practices of mothers of 0-24-month-old children comply with the World Health Organization (WHO) recommendations for infant and young child feeding and to compare the results with selected demographic parameters related to the mother and child. SUBJECTS/METHODS: The research sample comprised mothers (n = 250) with children less than 2 years old. Data were obtained via questionnaire and were analyzed using SPSS 20.0 package program. The Pearson χ2 or Fisher's exact tests were used for assessing relationships between categorical variables. The one-sample t-test was used for comparisons with reference values. RESULTS: Most mothers (97.2%) breastfed their babies immediately after birth. The mean time to breastfeeding after delivery was 47.8 ± 14.8 minutes, and 40.8% of the mothers complied with the WHO recommendation. Furthermore, 59.8% of the mothers exclusively breastfed their children for 6 months (mean 5.2 ± 1.5 months). The mean duration to the start of providing complementary food was 5.8 ± 0.6 months, and 76.1% of mothers who complied with the WHO recommendation. Only 12.3% of mothers breastfed their children for at least 12 months (mean 7.7 ± 3.3 months). On average, mothers gave cow milk to their children for the first time at 10.1 ± 1.7 months and honey at 11.8 ± 2.3 months. The mothers' rates of compliance with the WHO recommendations on cow milk and honey feeding were 32.0% and 71.6%, respectively. The rate of mothers who complied with the WHO minimum meal frequency recommendation was 88.3%. CONCLUSIONS: We suggest that the WHO recommendations on this subject will be realized more fully by emphasizing the importance of the positive effects of breastfeeding until the age of 2 years and of a timely start of complementary food provision. Such changes will affect child health over the long term.

아토피피부염에서 모유를 통한 식품 알레르겐에의 감작 (Sensitization of Food Allergen in Breastfed Infant with Atopic Dermatitis)

  • Breastfeeding has been known as the best feeding practice to prevent allergies including atopic dermatitis (AD) However, the benefit on the prevention of allergic disease is still controversial. The objectives of this study were to examine the rate of sensitization to the protein of eggs, cow's milk and soy in exclusively breastfed infants and to evaluate antigen-antibody reaction between breast milk and serum of AD infant. Data on feeding and food hypersensitivity were obtained for 62 AD infants (32 male, 30 female) aged < 6 month who had visited Samsung Medical Center from September 2001 to May 2003. Food hypersensitivity was determined by measuring specific IgE to egg, cow's milk and soy. Specific IgE levels > 0.7 kU/L by CAP assay (Pharmacia, Uppsala, Sweden) were considered positive. The rates of sensitization in breastfed infants were $41.9\%$ (26/62) to egg, $30.6\%$ (19/62) to milk and $18.0\%$ (11/62) to soy. Immunoblotting analyses were performed using breast milk with the matched serum of seven AD infants (4 male/3 female). Binding patterns of AD infant's IgE to breast milk extract showed visible specific band for immunoglobulin, especially in case of a lactating mother who did not completely restricted ingestion of egg, milk and soy. These results indicate that sensitization to food allergen develops via breast milk feeding. Breast milk feeding should be recommended in infants at risk of developing allergic disease, but maternal intake of highly allergenic food might be restricted for prevention and treatment of food allergy among the babies with AD.

EFFECT OF FEEDING LEGUME FORAGE WITH STRAW SUPPLEMENTATION ON MILK PRODUCTION AT PABNA MILK SHED AREA

  • An experiment was conducted at Pabna milkshed area under Bathan condition during November, 1992 to February 1993 with 16 milking cows fed legume forage with straw supplementation (treated group) and 8 milking cows fed legume forage only (control group). From the study, the results revealed that supplementation of straw with leguminous diet increases the total dry matter intake (DMI) of 11.83 kg/d/cow for the treated group and 11.53 kg/d/cow for the control group. The average daily legume forage intake was $37.39{\pm}8.67kg/d/cow$ and $49.62{\pm}10.57kg/d/cow$ for the treated and control group respectively and the difference was significant (p < 0.05). The results also exhibited that due to the supplementation of straw, the legume forage intake reduced by 12 kg/d/cow. The forage dry matter intake (DMI) kg/d/cow for the treated and control groups were $6.18{\pm}1.44kg$ and $8.38{\pm}1.95kg$ respectively. The milk production was $8.64{\pm}1.15litre/day$ for the treated group which was significantly (p < 0.05) higher than the control group ($7.74{\pm}1.24litre/day$). The average initial body weight of the supplemented and control groups were $338.06{\pm}39.32kg$ and $329.87{\pm}48.03kg$ respectively. Whereas, the final body weight of supplemented group was $344.33{\pm}35.90kg$ and control group was $330.35{\pm}37.28kg$. It may be concluded that straw supplementation with legumes diet could save legume forage for further use as well as increase milk production.

Association of infant feeding practices in the general population with infant growth and stool characteristics

  • This was a prospective cohort study of 976 infants from birth to 12 months of age. Infants were fed breast milk, goat infant formula, cow infant formula, or a combination of formula and breast milk during the first 4 months of age. Data on type of milk feeding and infant growth (weight and height) were collected at birth and at 4, 8, and 12 months during routine clinical assessment. The number and consistency of bowel motions per day were recorded based on observational data supplied by the mothers. Infants fed breast milk or goat or cow infant formula during the first 4 months displayed similar growth outcomes. More of the infants fed cow infant formula had fewer and more well-formed bowel motions compared with breast-fed infants. The stool characteristics of infants fed goat formula resembled those of infants fed breast milk.

  • 우분에 왕겨와 볏짚을 1:1, 1:2, 1:3의 부피비율로 혼합한 먹이조건이 지렁이의 생육, 분립생산량, 체조직으로의 유기물 전환율 및 전환효율을 조사하여, 적정 먹이조건을 추정하였다. 1. 우분+왕겨 혼합구가 우분+볏짚 혼합구보다 지렁이의 증체속도, 증식효율 및 분립생산량이 많았다. 특히 우분+왕겨 1:3 혼합비율이 지렁이 생육에 가장 알맞은 먹이조건이었다(Table 2). 2. 우분+왕겨 혼합 구에서 지렁이 체조직으로의 유기물 전환율과 전환효율이 높았던 것은 먹이 중의 휘발성 고형분과 전탄소함량이 높았기 때문이었다(Table 1, 4). 4. 지렁이 분변토는 약알칼리성이고, 전질소함량, 유효인산함량, CEC 및 치환 성 양이온 함량이 높아서 친환경 농자재로 유용하게 활용될 수 있다고 판단된다(Table 5).

Cow's Milk-Related Symptom Score in Presumed Healthy Polish Infants Aged 0-6 Months

  • Purpose: The Cow's Milk-related Symptom Score (CoMiSSTM), which considers crying, regurgitation, stools, skin and respiratory symptoms, was developed as an awareness tool for evaluating cow's milk-related symptoms. The scoring ranges from 0 to 33. A score ≥12 was proposed as being likely cow's milk-related and suggestive of allergy to cow's milk. This study aimed to determine the age-related CoMiSSTM values in presumed healthy infants in Poland. Methods: This was a cross-sectional study conducted in well-child clinics in two locations. Parents of the presumed healthy infants aged ≤6 months were approached during a routine checkup/vaccination visit. The exclusion criteria were as follows: presence of acute or chronic diseases, preterm delivery, treatment with therapeutic formula, and use of any food supplements (except vitamins) or medications. Results: Data from 226 infants were obtained (median age [Q1-Q3], 4 months [3-4]). The overall median (Q1-Q3) and mean (standard deviation) CoMiSSTM values were 4 (2-7) and 4.7 (3.5), respectively. The 95th percentile was 11. Scores on some, albeit not all, components of the CoMiSSTM significantly differed between age groups (crying, stools) or feeding type groups (stools and skin symptoms). Eleven children (4.9%) scored ≥12. Conclusion: This study adds to earlier age-related CoMiSSTM data by providing CoMiSSTM values in presumed healthy infants in Poland.

우분에 톱밥 혼합 수준이 지렁이의 생육과 분립 생산에 미치는 영향 (Effects of Mixture Ratios of Cow Manure and Sawdust on the Growth of Earthworm(Eisenia foetida) and Cast Production)

  • 본 실험에서는 우분과 톱밥의 혼합수준을 달리하여 지렁이 먹이로 이용하였을 때, 지렁이 생육과 증식에 미치는 영향과 지렁이 분립의 생산량 및 화학적 조성을 조사하여 우분의 효율적인 Vermicomposting의 기초자료로 이용하고자 실시하였다. 그 결과를 요약하면 다음과 같다. 먹이의 탄질율은 $20.43{\sim}45.20$으로 톱밥 혼합 수준이 증가함에 따라 탄질비도 높아졌으며 실험기간 동안 지렁이의 생존율은 $97.6{\sim}100%$로 나타났다. 산자수는 톱밥을 30과 40% 혼합한 구에서 각각 899.6과 927.8 마리로 우분 100%구의 769 마리보다는 유의하게 높게 나타났다(P<0.05). 산자중은 $8.00{\sim}14.35g$으로 톱밥 혼합 수준이 높을수록 유의하게 높아져 톱밥 40% 혼합구가 가장 높았다(P<0.05). 지렁이 분립생산은 톱밥 혼합 수준이 높을수록 낮게 나타났으며 소화량은 이와는 반대로 톱밥 혼합 수준이 높을수록 유의하게 높게 나타났다.

Efficacy and Safety of Soy Protein Based Formula in Atopic Dermatitis

  • Soy protein based formula (SPF) has been developed for infants who are at a high risk for atopic dermatitis (AD) and cow's milk protein allergy (CMA). We performed this study to evaluate the therapeutic efficacy and safety of SPF compared to conventional hydrolyzed cow's milk formula (hCMF) in the feeding of infants with AD and CMA. 38 infants (12 to 24 months of age) diagnosed with CMA and AD were randomized to receive either SPF or hCMF for 12 weeks. Follow-up was conducted at 4, 8 and 12 weeks. Growth parameters of the infants were evaluated during each visit. Clinical evaluations, including AD severity scores, pruritus, specific immunoglobulin E (IgE) (cow's milk protein and soy protein) levels of peripheral blood, were made at enrollment and week 12. Analysis was performed on the 32 infants (SPF: n=16, hCMF: n=16) who completed the 12-week intervention. Eczema area and severity index (EASI) scores, a measure of the severity of AD, and pruritus were significantly reduced after 12 weeks compared to enrollment in the both groups; however, the median changes for EASI scores and pruritus were not statistically different between the two groups. The growth parameters did not differ significantly between both groups at any assessed time point. This study suggests that SPF could be useful in decreasing the severity of AD without affecting infant growth status. Therefore SPF could provide an adequate and safe alternative to hCMF in treating infants with AD and CMA during the first 12 to 24 months of their life.

Identification of bioactive components behind the antimicrobial activity of cow urine by peptide and metabolite profiling

  • Objective: Cow urine possesses several bioactive properties but the responsible components behind these bioactivities are still far from identified. In our study, we tried to identify the possible components behind the antimicrobial activity of cow urine by exploring the peptidome and metabolome. Methods: We extracted peptides from the urine of Sahiwal cows belonging to three different physiological states viz heifer, lactation, and pregnant, each group consisting of 10 different animals. The peptides were extracted using the solid phase extraction technique followed by further extraction using ethyl acetate. The antimicrobial activity of the aqueous extract was evaluated against different pathogenic strains like Staphylococcus aureus, Escherichia coli, and Streptococcus agalactiae. The safety of urinary aqueous extract was evaluated by hemolysis and cytotoxicity assay on the BuMEC cell line. The urinary peptides were further fractionated using high-performance liquid chromatography (HPLC) to identify the fraction(s) containing the antimicrobial activity. The HPLC fractions and ethyl acetate extract were analyzed using nLC-MS/MS for the identification of the peptides and metabolites. Results: A total of three fractions were identified with antimicrobial activity, and nLC-MS/MS analysis of fractions resulted in the identification of 511 sequences. While 46 compounds were identified in the metabolite profiling of organic extract. The urinary aqueous extract showed significant activity against E. coli as compared to S. aureus and S. agalactiae and was relatively safe against mammalian cells. Conclusion: The antimicrobial activity of cow urine is a consequence of the feeding habit. The metabolites of plant origin with several bioactivities are eliminated through urine and are responsible for their antimicrobial nature. Secondly, the plethora of peptides generated from the activity of endogenous proteases on protein shed from different parts of tissues also find their way to urine. Some of these sequences possess antimicrobial activity due to their amino acid composition.
