Comparison of Three Dimensional Conformal Radiation Therapy, Intensity Modulated Radiation Therapy and Volumetric Modulated Arc Therapy for Low Radiation Exposure of Normal Tissue in Patients with Prostate Cancer

  • Radiotherapy has an important role in the treatment of prostate cancer. Three-dimensional conformal radiation therapy (3D-CRT), intensity modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT) techniques are all applied for this purpose. However, the risk of secondary radiation-induced bladder cancer is significantly elevated in irradiated patients compared surgery-only or watchful waiting groups. There are also reports of risk of secondary cancer with low doses to normal tissues. This study was designed to compare received volumes of low doses among 3D-CRT, IMRT and VMAT techniques for prostate patients. Ten prostate cancer patients were selected retrospectively for this planning study. Treatment plans were generated using 3D-CRT, IMRT and VMAT techniques. Conformity index (CI), homogenity index (HI), receiving 5 Gy of the volume (V5%), receiving 2 Gy of the volume (V2%), receiving 1 Gy of the volume (V1%) and monitor units (MUs) were compared. This study confirms that VMAT has slightly better CI while thev olume of low doses was higher. VMAT had lower MUs than IMRT. 3D-CRT had the lowest MU, CI and HI. If target coverage and normal tissue sparing are comparable between different treatment techniques, the risk of second malignancy should be a important factor in the selection of treatment.

  • Purpose: The purpose of this research is to give basic data about a way of connection to individual centers and vitalization of visiting nursing program on Busan. Method: The research is done with survey for eight hospitals. sixteen public heath centers, forty-six social welfare centers to be practised visiting nursing program on Busan for actual condition of that. Results: 1. The average nursing career is below 5-10 years. But visiting nursing career is below 3 years(70%) in hospital. public, public heath center, and heath center. 2. Hospitals coverage of visiting nursing service is city as a whole. Whereas public heath center and social welfare center covered some local area. 3. Client of visiting nursing service possess in order of alone lived elderly, person in uncomplete movement, a disabled person, and a chronic disease person. 4. The main service in visiting is patient assessment, basic nursing activity, and treatment nursing activity with medication care, B.P check, dressing, bedsore care, catheter care and exchange, fluid therapy. Particularly, the hospital runs parallel to basic care and treat care in 100%. The social service center has 65.0% in a patient assessment and basic nursing activity. 5. The concern about services connecting with other center is very high. Conclusion: Actual conditions of visiting nursing program on Busan, which is presented in this study. The results of this study will become the pillar of visiting nursing program planning and application.

Characteristics of species richness and diversity of woody vegetation in the natural rivers in Korea and its meaning to restoration design in flood plains

  • In order to get basic data for flood plain restoration, we surveyed the woody vegetation in Korean natural rivers and analyzed the species' characteristics with regards to patterns of richness and diversity. These characteristics were higher in hard wood forests than those in soft wood forests, such as Salix spp. community. Futhermore, they were the highest in the Prunus sargentii-Pinus densiflora community(H' 1.095), and the lowest in the Carpinus laxiflora community(H' 0.118) among the hard wood forests. Species' richness diversity were the highest in the Salix gracilistyla community, but the lowest in the S. koriyangi community or S. koreensis community among the soft wood forests. With regards to the dominant index, just one community is over 0.9, 13 communities are between 0.3-0.7 and 15 communities are less than 0.3. The Salix koreensis community was the highest at 0.931, and Prunus sargentii-Pinus densiflora community was the lowest at 0.13. Species' richness and diversity was significantly correlated with tree layer coverages and degree of slope. These results mean that in order to increase plant species diversity in flood plains planted hard woody trees, such as oaks and fir, are needed to suit environmental conditions with steeper slope and lower canopy coverage.

  • In Korea, the ratio of population in urban areas used to be only 50.1% in 1970, but with the value risen to 90.8% in 2009, urbanization is going on rapidly. Urbanization, which occurs by the rampantly planted buildings, has become major source of raising building density, changing wind direction and reducing wind amount, and such reductions are affecting even inside the building. In each year, among the total energy consumption in Korea, residential portion takes up significant ratio, and specifically the ratio of apartment house is shown to be highest. In order to solve such problem, many studies are being conducted for the improvement of natural ventilation performance. The natural ventilation performance of apartment house are significantly determined by the characteristics of external and internal structure, but in macroscopic perspective, the performance is established fundamentally by the layout characteristics of the main building of the apartment house in preparation for wind conditions. So far researches on raising the thermal comfort through elevation of ventilation performance have been conducted actively, but many of them propose only theoretical concepts deduced through wind path analysis, and do not include any indicator to measure ventilation performance simply only with area data from layout planning stage. Therefore, in this study, gap ratio a wind field measuring indicator was developed, and after the ventilation characteristics by layout types and main building uniformity were identified, the scope of gap ratio efficient for ventilation and that of uniformity were clarified, followed by verification through simulation.

  • The Purpose of this study is to propose policy directions for residential level improvement of residents in Public Rental Housing. In this study, the contents coverage was limited to the policy proposal on the efficient management of public rental housing by reviewing papers, understanding present situations of supply and management of Public Rental Housing and investigating residential conditions. First, the residential satisfaction of physical factors were different from by the type of public rental housing. The satisfaction of physical factors was highest in the kookmin rental housing, permanent and 50 years rental housing were lower than kookmin rental housing. Second, resident's satisfactions of social factor such as neighborhood relationship and the image of apartment complex were differed by each housing types. However, there was no difference in satisfaction of community participation by each housing types.

  • Our faces can express a remarkable range of subtle emotions and silent messages. Because the face is so essential for complex social interactions that are part of our everyday lives, aesthetic repair and restoration of function are an important tasks that we must not take lightly. Soft-tissue defects occur in trauma patients and require thorough evaluation, planning, and surgical treatment to achieve optimal functional and aesthetic outcomes, while minimizing the risk of complications. Recognizing the full nature of the injury and developing a logical treatment plan help determine whether there will be future aesthetic or functional deformities. Proper classification of the wound enables appropriate treatment, and helps predict the postoperative appearance and function. Comprehensive care of trauma patients requires a diverse breadth of skills, beginning with an initial evaluation, followed by resuscitation. Traditionally, facial defects have been managed with closure or grafting, and prosthetic obturators. Sometimes, however, large defects cannot be closed using simple methods. Such cases, which involve exposure of critical structures, bone, joint spaces, and neurovascular structures, requires more complex treatment. We reviewed and classified causes of significant trauma resulting in facial injuries that were reconstructed by microsurgical techniques without simple sutures or coverage with partial flaps. A local flap is a good choice for reconstruction, but large defects are hard to cover with a local flap alone. Early microsurgical reconstruction of a large facial defect is an excellent choice for aesthetic and functional outcomes.

  • Most demand forecasting studies for telecommunication services have focused on estimating market size at the introductory stage of new products or services, or on suggesting improvement methods of forecasting models. Although such studies forecast business growth and market sizes through demand forecasting for new technologies and overall demands in markets, they have not suggested more specific information like relative market share, customers' preferences on technologies or service, and potential sales power. This study focuses on the telecommunication service industry and explores ways to calculate the relative market shares between competitors, considering competitive situations at the introductory stage of a new mobile telecommunication service provider. To reflect the competitive characteristics of the telecommunication markets, suggested is an extended conjoint analysis using service coverage and service switching rates as modification variables. This study is considered to be able to provide strategic implications to businesses offering existing service and ones planning to launch new services. The result of analysis shows that the new service provider has the greatest market share at the competitive situation where the new service covers the whole country, offers about 50% of existing service price, and allows all cellphones except a few while the existing service carrier maintains its price and service and has no response to the new service introduction. This means that the market share of the new service provider soars when it is highly competitive with fast network speed and low price.

Dosimetric Comparison of Noncoplanar and Coplanar Volumetric Modulated Arc Therapy Plans for Esophageal Cancer

  • Purpose: We compared noncoplanar volumetric modulated arc therapy (ncVMAT) plans to coplanar VMAT (cVMAT) plans by evaluating the dosimetric quality of each for esophageal cancer. Methods: Twenty patients treated for esophageal cancer with the cVMAT technique were retrospectively selected. The cVMAT plans consisted of three coplanar full arc beams. The ncVMAT plans consisted of two coplanar full arc beams and one noncoplanar partial arc beam ranging from 45° to 315° with a couch rotation angle of 315°±5°. For dosimetric evaluation, the dose-volumetric (DV) parameters of the planning target volume (PTV) and organs at risk (OARs) were calculated for all VMAT plans. Results: No clinically noticeable differences between the cVMAT and ncVMAT plans were observed in the DV parameters of the PTV. For the lungs, the V13 Gy and mean dose for ncVMAT plans were smaller than those for cVMAT plans, showing statistically significant differences. For the heart, the values of the maximum dose for cVMAT and ncVMAT plans were 53.8±2.9 and 50.9±3.3 Gy, respectively (P=0.004). For the spinal cord, the values of the maximum dose for cVMAT and ncVMAT plans were 37.1±5.1 and 34.7±5.7 Gy, respectively (P<0.001). Conclusions: The use of ncVMAT plans provides better PTV coverage and sparing of OARs compared to that of cVMAT plans for long, tube-like esophageal cancer. For esophageal cancer, the ncVMAT plans showed a more favorable plan quality than the cVMAT plans.

Clinical Impact of Patient's Head Position in Supraclavicular Irradiation of the Whole Breast Radiotherapy

  • Patients with breast cancer can be positioned with their head turned to the contra lateral side or with their head straight during the radiation therapy treatment set-up. In our hospital, patients with locally advanced breast cancer who were receiving radiation therapy have experienced swallowing difficulty after 2 weeks of irradiation. In this pilot study, the impact of head position on reducing dysphagia occurrence was dosimetrically evaluated. Patients were divided into two groups viz., HT (head turned to the contra lateral side of the breast) and HS (head straight) with 10 members in each. Treatment planning was performed, and the dosimetric parameters such as Dmin, Dmax, Dmean, V5, V10, V20, V30, V40, and V50 of both groups were extracted from the dose volume histogram (DVH) of esophagus. The target coverage in the supraclavicular fossa (SCF) region was analyzed using D95 and D98; moreover, the dose heterogeneity was assessed with D2 from the DVHs. The average values of the dose volume parameters were 27.6%, 58.6%, 35.4%, 19%, 13.8%, 14.1%, 11.8%, 8.4%, and 8.1% higher in the HT group compared with those in the HS group. Furthermore, for the SCF, the mean values of D98, D95, and D2 were 42.4, 47.5, and 54 Gy, respectively, in the HS group and 38.9, 45.35, and 55.5 Gy, respectively, in the HT group. This pilot study attempts to give a solution for the poor quality of life of patients after breast radiotherapy due to dysphagia. The findings confirm that the head position could play a significant role in alleviating esophageal toxicity without compromising tumor control.

Impact of COVID-19 pandemic on routine immunization services in a tertiary care hospital of Rajasthan, India

  • Purpose: Globally, 25 million children were either unvaccinated or under-vaccinated in year 2021. Among them, India had the highest number of children unvaccinated. Studies have shown impact of coronavirus disease 2019 (COVID-19) pandemic on routine health-care services. Present study aimed to estimate the impact of COVID-19 on utilization of routine immunization services during years 2020 and 2021 in tertiary-care facility. Materials and Methods: Record based descriptive analytic study was conducted at pediatric tertiary-care hospital, Jaipur. Data of children vaccinated as per the National Immunization Schedule for the period January 2019 to December 2021 were retrieved from the immunization records. Doses administered were assessed as a proxy measure of vaccine coverage. The vaccination trends of 2020 and 2021 were compared assuming base year 2019. Unpaired t-test of significance and Pearson correlation was used for correlation analysis. Results: There was significant drop in the vaccine counts after emergence of COVID-19 pandemic. In year 2020 and 2021, mean monthly vaccine count was 2,190±715.1 and 2,305±393.2, respectively, in which maximum drop was in April 2020 (-79.12%) and May 2021 (-57.16%) when it was compared with matched month of base year. There was negative correlation between percent change in vaccine count and COVID-19 cases in 2020 (r=-0.057, p=0.861) and 2021 (r=-0.827, p=0.001) as compared to year 2019. Conclusion: Study concludes that there was a significant gap in utilization of routine immunization services during the COVID-19 pandemic. This necessitates planning and management of routine immunization services in-case of future pandemics to avoid resurgence of vaccine-preventable diseases in the Rajasthan.
