The effect of aprotinin for hemostasis in open heart surgery (개심술시 Aprotinin의 지혈효과에 대한 고찰)

  • The 26 patients were randomly selected among patients who underwent valve replacement from February 1992 through September 1993 at National Medical Center. They were divided into two groups, one control group[n=13], the other aprotinin group[n=13]. The aprotinin group recieved 15000 KIU/kg aprotinin in the CPB priming volume and 35000 KIU/kg aprotinin intravenously during operation. We checked preoperative and postoperative hemoglobin, platelet, prothrombin time, activated partial thromboplastin time, creatinine phosphokinase and lactic dehydrogenase. There was no difference in the patient`s above clinical parameters between both groups. The platelet count in both groups decreaced after operation. These findings demonstrated that the effectiveness of aprotinin was not associated with platelet number but probably associated with a protection of platelet function and a prevention of hyperfibrinolysis. The intraoperative and postoperative blood loss and requirement of blood were significantly reduced in aprotinin group.

  • Obesity is known as a cause of variousdisease. Sterility is one of disease related to obesity. We had experienced an Obese patient accompanied by sterility. From January. 28. 2004 to April. 30. 2004. We applied Kobonkunyangdan and Ojayounjonghuan(固木健陽丹 合 五子衍宗丸), electrolipolysis and LCD(low alorie diet), behavioral theraphy, auricular acupuncture. Through this obesity treatment during 3 months, total weight loss was 23.8kg, reduction of percent body fat was 5.5% (from 22.8 to 17.3), reduction of BMI was $7.3kg/m^2$ (from 32.6 to 25.3), increase in sperm count and sperm motility was $50{\sim}60%$, hepatic index was improved.

  • The major economic health problem of dairy cattle is mastitis which can affect 10 to 50% of cow-quarters. This health problem is difficult for many dairy farmers and health advisors to understand, diagnose and control. Without special laboratory testing, most mastitis is overlooked. Estimates of annual mastitis cast per cow vary from $50 to $200. For the nearly 9 million cows in the United States, annual loss to the dairy industry amounts to over one billion. A knowledge-based decision aid has been developed to evaluate mastitis data retrieved electronically from two of nine U. S. regional dairy records processing centers. Heuristic rules to diagnose herd mastitis problems were collected and incorporated into the system from various domain experts. This system information. It allows users to select mastitis control schemes with various degrees of aggressiveness and teaches commonly accepted mastitis control practices.

Effect of Cardiopulmonary Bypass on Platelet (개심술시 체외순환이 혈소판에 미치는 영향)

  • The effect of cardiopulmonary bypass on platelet count and function was studied in 20 patients who underwent cardiac operation from April 1991 to August 1991 at the Department of thoracic and Cardiovascular Surgery, School of Medicine, Keimyung University. Ten patients were perfused with a bubble oxygenator, 10 with a membrane oxygenator. During and after bypass, platelet counts decreased in both groups and significantly reduced in those perfused with a bubble oxygenator. All 20 patients studied for platelet functions had an abnormal postoperative aggregation response to collagen and epinephrine, but no significant difference in both groups. One hour after bypass, bleeding times increased in both groups but did not differ significantly between groups. Postoperative 24 hour blood losses were significantly higher in patients perfused with a membrane oxygenator. Platelet damage and postoperative blood loss are less severe after cardiopulmonary bypass performed with a membrane oxygenator than with a bubble oxygenator.

Optimal Welding Condition of Dissimilar Friction Welded Materials and Its Real Time Evaluation by Acoustic Emission (이종마찰용접재의 최적용접조건과 음향방출에 의한 실시간 품질평가)

  • In this paper, dissimilar friction welding were produced using 15 mm diameter solid bar in chrome molybedenum steel(SCM440) to stainless steel(STS316L) to investigate their mechanical properties. Consequently, optimal welding conditions were n=2000 rpm, HP=70 MPa, UP=140 MPa, HT=10 sec and UT=10 sec when the metal loss(Mo) is 8.6 mm. In addition, an acoustic emission technique was applied to evaluate the optimal friction welding condition. AE parameters including the cumulative count, amplitude and energy showed a various changes according to the friction condition. A continuous type waveforms and low frequency spectrum was presented in friction time. On the other hand, a burst type waveform and high frequency spectrum was exhibited in pressing time.

  • Pituitary pars intermedia dysfunction (PPID), often referred to as equine Cushing's disease, is a common endocrine disorder often diagnosed in older horses. A 13-year-old 460 kg Warmblood gelding showed clinical signs suggestive of PPID, including hypertrichosis, fat redistribution, polyuria and polydipsia (PU/PD), and weight loss. Physical examination, complete blood cell count, and serum chemistry results were normal. However, dexamethasone suppression and plasma adrenocorticotropic hormone (ACTH) level tests confirmed PPID. Three months after the confirmed diagnosis, the horse was referred again with symptoms of laminitis. Radiography and venography were performed to evaluate the laminitis severity level. However, the foot condition continued to worsen, and the horse was eventually euthanized. The purpose of this case report is to describe clinical signs and diagnosis of PPID with laminitis.

Application and Efficacy Evaluation of Nutritional Screening Tool (영양부족 환자의 조기발견을 위한 선별검사의 적용 및 효용성 평가)

Purpose: Malnutrition has been frequently reported for patients on their admission to the hospital and it has been associated with an increase in morbidity, mortality and the length of the hospital stay. Although a number of screening tools have been developed to identify those patients at risk for malnutrition, there is no' gold standard' for defining malnutrition and the malnourished patients remain largely unrecognized. The aim of this study is to evaluate the efficacy of a nutritional screening tool for use in Dankook University Hospital. Methods Nutritional evaluation was performed for 53 patients who were admitted to the department of surgery and internal medicine between October and December 2004. The screening tool was completed by the ward nurse and the nutritional support team nurse on the same patients within24 hours of admission. The nutritional support team nurse performed the full assessment. The screening sheet included 4 questions regarding body mass index, recent unintentional weight loss, food intake and disease severity. Each answer was scored and a total of 5 was tested as the criterion fey malnutrition. The full assessment included current body weight, recent weight loss, triceps skinfold thickness, mid-arm muscle circumference, serum albumin)in and total lymphocyte count. Malnutrition was defined by 3 or more values below the reference values. The reliability of the screening tool was assessed using kappa statistic. Sensitivity, specificity and accuracy were calculated to evaluate the validity of the screening tool. The receiver operating characteristic(ROC) curve was drawn to choose a cutoff valve that maximizes sensitivity and specificity. Results' The level of agreement between the ward nurse and the NST nurse was good for BMI and food intake and moderate for weight loss and disease severity. The full assessment identified7 patients(13.2%) as malnourished. The screening sheet had a sensitivity of 86% and a specificity of 80%. According to the ROC curve, a score of 5 points provided the best validity. Conclusion The nutritional screening tool is reliable when completed by different observers and it is valid for nutritional assessment.

Effect of Tedding Time and Frequency on the Feed Value and Drying Rate of Rye(Secale cereale L.) Hay

  • Hay-making is one of the most common way for forage preservation in livestock industry. The quality and production of hay could be affected by various factors. This experiment was conducted to investigate the effect of tedding time and frequency on drying rate and feed value of forage rye (Secale cereale L.) hay. Rye was harvested on heading stage using mower conditioner. Hay was tedded at each set hour(09:00, 13:00 and 17:00) and sampled at each set hour to determine dry matter (DM) content. After two months' preservation, CP (crude protein), ADF (acid detergent fiber), NDF (neutral detergent fiber), IVDMD (in vitro dry matter digestibility), TDN (total digestible nutrient), RFV (relative feed value), DM loss, visual scores and total fungi count were determined for estimation of hay quality. Tedding was necessary for both speeding up drying rate and improving forage quality. Tedding at 17:00 showed lower NDF content (p<0.05), and also higher RFV value was found compared with tedding at 9:00 and 13:00 (p<0.05). On the other hand, it was observed that more DM losses would be found when tedding later (p<0.05). Tedding in 1~3 times per day were lower in ADF and NDF content (p<0.05), increased CP, TDN and RFV (p<0.05), got less DM loss (p<0.05), and contained less fungi during conservation compared with no tedding (p<0.05). On the other hand, tedding too frequent caused more DM loss (p<0.05). In conclusion, for shorter drying process and higher quality of forage rye hay, tedding at 13:00~17:00 for 1~2 times per day was recommended in this study.

Physicochemical and microbial characteristics of longissimus lumborum and biceps femoris muscles in Korean native black goat with wet-aging time

  • This study examined the effects of different wet-aging times on the physicochemical characteristics and microbial profile of longissimus lumborum (LL) and biceps femoris (BF) muscles from Korean native black goat (KNBG) meat. The water holding capacity (WHC), pH, cooking loss, shear force, meat color, free amino acid, total bacteria, and coliform count of KNGB meat were analyzed at 0, 5, 10, and 15 days of wet-aging at 4℃ under vacuum packaging. The results showed that different wet-aging times led to significant pH variations between the muscles throughout the aging period. The wet-aging time did not affect the WHC and cooking loss in meat from the LL muscle. In the BF muscle, however, meat wet-aged for five days had a significantly higher WHC and less cooking loss than meat aged for 0, 10, and 15 days. The meat from the LL muscle wet-aged for five days produced tenderer meat (low shear force value) than the unaged meat (p < 0.05). Moreover, the color was similar in the LL muscle regardless of the number of aging days. In the BF muscle, the redness (a*) was higher in the meat wet-aged for 15 days compared to that aged for 0, 5, and 10 days (p < 0.05). Regardless of the muscles, an increase in wet-aging time led to an increase in the total free amino acids contents in both muscles (p < 0.05). On the other hand, the tasty/bitter amino acid ratio was significantly higher for five days of wet-aged meat than 10 and 15 days of aging from the BF muscle. In addition, regardless of the muscles, the total bacteria and coliform counts were significantly lower for five days of wet-aged meat than 10 and 15 days of aging (p < 0.05). Therefore, chevon wet-aged for five days is an optimal aging period under vacuum packaging that fortifies meat quality with a minimal microbial negative defect.

Investigation of Antioxidant Activity of Cynanchi Wilfordii Radix and Inhibitory Effect of 5α-reductase mRNA in Human Dermal Papilla Cells (백수오(白首烏)의 항산화 효능 및 모유두 세포의 5α-reductase mRNA 발현 억제 효능)

  • Hair loss affects interpersonal relationships and causes psychological stress. In this study, we investigated the antioxidant activity of Cynanchi Wilfordii Radix (CWR) and its effects on dermal papilla (DP) cells. Antioxidant efficacy was examined by ABTS assay. To confirm the effect on cell activity, MTS assay was performed and cell count was directly measured by hemocytometer. The mRNA expression of genes involved in hair formation and hair loss formation was confirmed by quantitative RT-PCR. CWR has a strong antioxidant activity. Cell viability of DP cells was increased to 118.5% by treatment of 0.5 mg/ml CWR for 24 hours, but the effect on the cell number was insignificant. These results suggest that CWR increases mitochondrial activity without promoting cell proliferation. Treatment of DP cells with 0.5 mg/ml CWR resulted in 48.5% reduction of mRNA expression of type 2 $5{\alpha}$-reductase, a major cause of male hair loss. In addition, mRNA expression of bone morphogenetic pretein (BMP), fibroblast growth factor (FGF)7, and FGF10, which are closely related to hair growth, was also decreased. Reactive oxygen species (ROS) acts as a cause of hair loss. The excellent antioxidant efficacy of CWR is thought to be able to effectively remove ROS. The dihydrotestosterone produced by type 2 $5{\alpha}$-reductase in DP cells is a potent inducer of male pattern hair loss. The inhibitory effect of type 2 $5{\alpha}$-reductase mRNA on DP cells induced by CWR may induce a positive therapeutic effect of male pattern hair loss.
