짧은 코 교정술과 비첨성형술에 있어서 동종늑연골 (Tutoplast®) 이식의 유용성 (The Usefulness of Allogenous Costal Cartilage Graft for Correction of Short Nose and Tip Plasty)

  • Purpose: Autogenous cartilage is generally first choice in rhinoplasty because of its biocompatibility and resistance to infection. On the other hand, allogeneous cartilage graft might preferred over an autogenous graft to avoid additional donor site scars, morbidity and lengthened operating time. Allogenous costal cartilage ($Tutoplast^{(R)}$) not only have the advantage of averting donor site morbidity but also are resistant to infection, resembling autogenous cartilage graft. We report here a technique for rhinoplasty by using allogenous costal cartilage graft. Methods: Through open rhinoplastic approach, alar cartilage is released from upper lateral cartilage and relocated caudally. After relocation of alar cartilage, allogenous costal cartilage is immobilized by nonabsorbable suture material at caudal aspect of septal cartilage. Caudal end of allogenous costal cartilage is sutured between medial crura of alar cartilage. Tip projection is improved by using interdormal suture, transdormal suture and shield-shape cartilage graft which is harvested from concha Results: No significant resorption and infection was detected in any of patients. Aesthetic and functional results were satisfactory. Conclusion: The low incidence of major complication and versatility of allogeneous costal cartilage graft make safe and reliable source of cartilage graft in rhinoplasty.

Polydeoxyribonucleotide and Microlens Array-type, Nanosecond-domain Neodymium:Yttrium-aluminum-garnet Laser Treatment for Scars from Costal Cartilage Harvest Surgery: Case Series of 9 Patients

  • Background and Objectives Surgery for harvesting costal cartilage is often required for revision septorhinoplasty due to a lack of septal cartilage in patients with a severely contracted nose, and postoperative scarring on the anterolateral rib cage commonly requires additional treatment. This study aimed to evaluate the therapeutic efficacy and safety of combined polydeoxyribonucleotide (PDRN) and microlens array (MLA)-type nanosecond-domain neodymium (Nd):yttrium-aluminum-garnet (YAG) laser treatment for postoperative scars after costal cartilage harvest surgery. Materials and Methods Nine Korean patients with scars after costal cartilage harvest surgery treated with PDRN injections and MLA-type Nd:YAG laser treatments were retrospectively reviewed. Results Most of the scar lesions exhibited clinical improvement at 2 weeks after PDRN and MLA-type nanosecond-domain laser treatments, and the lesions further improved after adding more treatment sessions. The median Vancouver Scar Scale (VSS) score decreased from 6 (interquartile range [IQR]: 6-7) before combined intralesional PDRN injection and MLA-type, nanosecond-domain Nd:YAG laser treatments to 3 (IQR: 2-4) thereafter. Patient satisfaction after the combination treatments was rated as satisfactory. None of our patients reported major adverse events. Conclusion This case series study demonstrated that combined PDRN and MLA-type, nanosecond-domain Nd:YAG laser treatments are effective and safe for treating scars from costal cartilage harvest surgery.

Rib Fixation for a Patient with Severely Displaced and Overlapped Costal Cartilage Fractures

  • Rib fixations for flail chest or displaced rib fractures are not a new technique. However, reports on rib fixations involving costal cartilage fractures are very few and surprisingly there are no reports of internal fixations involving only the costal cartilage in the English literature. The diagnosis is difficult and the necessity of the procedure may be quite controversial. Placing plates in screws into the costal cartilage alone may seem unstable and easily dislodged or stripped through the cartilage. We report a 31-year-old male scuba diver instructor who underwent rib fixations over his 7th and 8th costal cartilage ribs for severe pain. The procedure was done with conventional plates and screws. He had the plates and screws removed 2 months later due to lingering pain, but with them removed he is now quite happy with the results without pain. The procedure for fixation of painful overlapped costal cartilage is quite simple and can be done with the usual conventional methods, fixating plate and screws directly over the cartilage alone without fixation over the bony rib.

성문하협착증에서 윤상연골절개 및 윤상연골 후벽 늑연골 이식술과 T-tube stenting (Posterior Cricoid Split with Costal Cartilage Grafting and T-tube Stenting for Treatment of Subglottic Stenosis)

  • OBJECTIVES : To determine the results of posterior cricoid split with costal cartilage graft in the treatment of moderate and severe subglottic stenosis in adults, and to assess the effect of T-tube as a stent in this surgery. PATIENTS AND METHODS : Four adults with moderate and severe subglottic stenosis who underwent posterior cricoid split with costal cartilage graft from 1994 to 1995. Three patients were male and one was female. All of the patients had acquired subglottic stenosis, 2 of whom had a bilateral vocal folds paralysis. The surgical procedure we used included a cricoid lamina split with costal cartilage grafting and T-tube stenting. Arytenoidectomy was a added in 2 patients with bilateral vocal folds paralysis. RESULTS : Three of the 4 patients underwent decannulation, and 1 is still undergoing treatment. 3 patients who underwent decannulation demonstrated marked improvement in their symptoms of airway obstruction and good postoperative voice quality. CONCLUSIONS : The posterior cricoidotomy lumen augmentation with costal cartilage grafting is a safe and effective technique for the treatment of moderate and severe subglottic stenosis in adults and T-tube is a good alternative stenting material for this procedure.

늑골 Turnover 방법을 이용한 소이증 재건 시 흉벽 함몰 예방술 (Using Rib Bone Turnover Technique, Prevention of Chest Wall Depression after Microtia Reconstruction)

  • Purpose: Reconstruction of microtia using costal cartilage graft is commonly used technique nowadays. The chest wall depression at the donor site after the graft, however, has been noticed in many articles. Prevention or correction technique for the depression at the donor site also has been reported and we also have been concerned about the problem. This article is a case report about a new technique preventing chest wall depression after costal cartilage graft. Methods: We selected total 15 microtia patients who visited our clinic, from December 2005 to July 2007. They were 10 male and 5 female patients and the mean age was 11.9 years. The average follow up period was 9.2 months(2 to 15 months). We used 6, 7, and $8^{th}$ costal cartilage for microtia reconstruction. And then we turned over pivot of cartilage resection margin, after bihalving costal cartilage involving about 5-6 cm of $6^{th}$ and $7^{th}$ rib bone. After microtia reconstruction, chest donor sites were evaluated by physical examination and radiography. Results: Postoperative depression at the donor site was much less when the costal cartilage turnover technique was performed. Postoperative physical examination and three dimensional reconstruction CT showed that the rest part of rib bone was turned over and it supported the soft tissue defect during respiration. Conclusion: We expect that the turnover rib bone will not be absorbed after graft, as well as offering mechanical support, compared to the other reports.

Reconstructive rhinoplasty with costal cartilage grafting: A case report of relapsing polychondritis

  • Relapsing polychondritis (RP) is a rare autoimmune disorder of unknown etiology characterized by recurrent episodes of inflammation and the destruction of cartilaginous tissues, primarily involving the ear, nose, and the respiratory tract. Nasal chondritis is present in 24% of patients at the time of diagnosis and develops subsequently in 53% throughout the diseases progress. Progressive destruction of nasal cartilage leads to the characteristic flattening of the nasal bridge, resulting in the saddle nose deformity. In patients with RP, surgical management for saddle nose is carefully decided due to the disease relapsing characteristics. We present a RP patient with a saddle nose deformity who underwent reconstruction rhinoplasty with autologous costal cartilage grafting. At 6-month follow-up, the patient retained good esthetic results and showed neither complication nor relapse of RP.

사람에서의 연골막 유무에 따른 자가늑연골이식의 부피 및 무게 변화 (Volume and Weight Changes of Autologous Costal Cartilage Grafts with and without Perichondrium in Human)

  • Autologous cartilage grafts have become an integral part of aesthetic and reconstructive plastic surgery. However, little objective information is available about the actual quantitative resorption of cartilage in human. This study sought to objectively quantify and compare the resorption of costal cartilage in human. To compare the resorption characteristics of rib cartilage autografts, we harvested rib cartilage grafts from 37 microtia patients. All autografts were implanted subcutaneously on chest and then removed after 6 to 17 months. Graft mass and volume were compared before and after implantion. Rib cartilage grafts with perichondrium averaged $10.8{\pm}7.4%$ resorption by volume, On the other hand rib cartilage grafts without perichondrium $25.5{\pm}6.8%$. There was no evidence of necrosis or inflammatory changes. The rib cartilage is the preferred source of autogenous cartilage for auricular reconstruction. Short-term resorption of rib cartilage without perichondrium appears to be higher than with perichondrium. The low resorption of cartilage with perichondrium may be due to in part to cartilage forming capacity of the perichondrium. It remains to be seen whether these differences in resorption persist in the long term.

경미한 둔상에 의하여 야기되는 늑연골 골절 진단에 있어서 흉벽 초음파 검사의 임상적 유용성 (Clinical Usefulness of Chest Wall Ultrasonography for Detecting Fractures of Costal Cartilage due to Minor Blunt Chest Trauma)

  • 배경: 늑골 골절은 흥부 외상 중에 둔상으로 인한 손상 중 가장 흔한 질환이다. 하지만 늑연골 골절은 연골 부위의 석회화가 동반되지 않는 한 고전적인 흉부 방사선 검사에서 잘 관찰되지 않으며, 이러한 이유로 고전적인 흉부 방사선 검사로는 늑연골 골절의 진단에 한계가 있다 이에 저자들은 고전적인 흉부 방사선 검사에서 간과 되었던 늑연골 늑골 골절의 진단을 위하여 경미한 흉부 둔상 환자에게 흉벽 초음파 검사를 시행하였다. 대상 및 방법: 2008년 4월부터 2009년 3월까지 본원을 내원한 환자를 대상으로 하였고, 고전적 방사선 검사에서 이상 소견이 없었으며 중증 손상을 의심할 만한 병변이 없는 총 45명의 환자를 대상으로 하였다. 이들 중 여성은 24명, 남성은 21명이었으며, 평균 연령은 50.4$\pm$15.91세(17$\sim$76)였다. 상기 대상자들은 7.5-MHz 직선 탐촉자를 지닌 초음파 기기를 이용하여 흉벽 초음파 검사를 시행하였다 결과: 대상이 된 45명의 환자들 중 30예(67%)에서 늑연골 골절을 관찰할 수 있었고, 15예(33%)에서는 특별한 이상 소견이 관찰되지 않았다. 골절을 지닌 30명의 평균적인 골절 개수는 1.6$\pm$0.81 부위(1$\sim$4)였다. 늑연골 골절의 가장 흔한 동반 소견은 골막 주위 혈종으로 7예(17%)에서는 골절 소견과 동반되어 관찰되었고 1예(2%)에서는 골절 소견 없이 관찰되었다. 두 번째 흔한 동반 소견은 흉골 골절이었고, 5예(12%)에서는 연골 부위 늑골 골절과 동반되어 관찰되었고, 1예(2%)에서는 흉골 골절만 관찰되었다. 결론: 흉벽 초음파 검사는 경미한 흉부 외상 환자에게서 간과되었던 늑연골 골절의 진단에 유용한 방법으로 사료된다.

Auricle reconstruction with autologous costal cartilage versus polyethylene implants in microtia patients: a meta-analysis

  • Background: Auricle reconstruction is among the most challenging procedures in plastic and reconstructive surgery, and the choice of framework material is a critical decision for both surgeons and patients. This meta-analysis compared the outcomes of autologous auricle reconstruction using costal cartilage with those of alloplastic reconstruction using porous polyethylene implants. Methods: A literature review was conducted using the PubMed and Embase databases to retrieve articles published between January 2000 and June 2024. The outcomes analyzed included postoperative complications such as framework exposure, infection, skin necrosis, hematoma, and hypertrophic scars, as well as patient satisfaction. The proportions of reconstructive outcomes from each selected study were statistically analyzed using the "metaprop" function in R software. Results: Fourteen articles met our inclusion criteria. The group undergoing polyethylene implant reconstruction exhibited higher rates of framework exposure, infection, and skin necrosis, whereas the autologous reconstruction group experienced higher rates of hematoma and hypertrophic scars. Of all the complications, framework exposure was the only one to show a statistically significant difference between the two groups (p< 0.0001). In terms of patient satisfaction, those who underwent autologous cartilage reconstruction reported a higher rate of satisfaction, although this difference did not reach statistical significance in the meta-analysis (p= 0.076). Conclusion: There is no statistically significant difference in postoperative complications such as infection, hematoma, skin necrosis, and hypertrophic scars between auricle reconstructions using autologous costal cartilage and those using polyethylene implants. However, reconstructions with polyethylene implants show a significantly higher rate of framework exposure.

자가연골이식을 이용한 안장코의 교정 (Correction of Saddle Nose Deformity using Autogenous Cartilage Graft)

  • Purpose: Correction of saddle nose remains problematic both in the technique of reconstruction and in the choice of implant material for nasal augmentation. A large variety of graft materials have been used for the reconstruction of the saddle nose deformity. The purpose of this study is to determinate an algorithm for deciding alternative graft materials used in correction of saddle nose deformity. Methods: Six patients with saddle nose who were corrected using by auricular cartilage and costal cartilage at Chosun university hospital were analyzed. Results: After a mean interval of 12 months, all patients were satisfied with the esthetic and functional result. Conclusion: Auricular cartilage is an excellent graft material for esthetic and functional reconstruction of mild to moderate saddle nose deformity.
