악성 복수와 비악성 복수의 감별을 위한 복수액 분석 (Ascitic Fluid Analysis for the Differentiation of Malignancy-Related and Nonmalignant Ascites)

  • 악성 복수와 비악성 복수의 감별 진단은 진단과 치료 과정에서 매우 중요한 역할을 한다. 많은 계측치들이 감별을 위하여 사용되어져 왔는데 어느 것도 단독 사용시 두 군을 완벽히 구분짓지는 못한다. 이에 본 연구자들은 악성복수와 비악성 복수의 감별에 있어서 여러 계측치들을 비교하여 민감도와 특이도가 높은 계측치를 찾고 이 결과를 토대로 악성복수와 비악성 복수의 구분에서 간단하면서도 효과적인 진단방법을 찾고자 하였다. 1995년 1월부터 1997년 12월까지 영남대학교 의과대학 부속병원 내과에 복수를 주소로 내원하여 복수 천자를 시행한 80명을 대상으로 이들 환자들을 악성 복수 환자군과 비악성 복수 환자군으로 나누어 조사하였다. 악성 복수군은 다시 암종증이 동반된 21명의 환자와 암종증의 증거가 없는 6명의 환자로 구분하였고 비악성 복수군은 다시 52명의 간경변증 환자군과, 1명의 간경변증이 없는 군으로 구분하였다. 모든 환자에게서 복수와 혈액에서 동시에 검체가 얻어졌고 계측치로 복수액의 콜레스테롤, CEA, LDH, 단백치, 세포진 검사, 복수액과 혈청의 LDH 비, 단백비, 혈청-복수액 알부민 차를 구하였고, 각 계측치의 평균을 구하여 두 군을 비교하였는데, 모든 계측치가 통계적으로 유의한 차이를 보여 주었다. 저자는 각 계측치마다 각 측정치를 적용하여 가장 높은 민감도와 특이도를 보이는, 즉 가장 높은 진단 능력을 나타내는 측정치를 절단치로 정한 후각 계측치에 적용하여 진 양성, 진 음성, 가 양성, 가 음성으로 분류한 후 민감도, 특이도, 양성 기대도, 음성 기대도, 진단 능력을 산출하였고, 연구된 계측치 중 민감도가 높은 계측치인 복수액의 콜레스테롤을 선별검사의 단계에 적용하고, 특이도가 높은 계측치인 세포진 검사와 복수와 혈청의 단백비를 악성 복수로 분류된 군에 적용하여 악성 복수를 확진하는 2단계 진단 과정을 80명의 환자들에게 적용하여 다음 결과를 얻었다. 1) 복수액 분석 결과 복수액의 콜레스테롤치가(절단치: 30 mg/dl) 93%의 가장 높은 민감도를 보였고, 복수액의 세포진 검사와 복수액과 혈청의 단백비가(절단치: 0.5) 100%, 98%의 가장 높은 특이도를 나타내었다. 2) 2단계의 진단 과정을 통하여 복강내 암종 전이가 확인된 21명 모두 악성 복수군으로 분류되었다. 3) 광범위한 간전이가 있는 악성 복수 모두 비악성군으로 분류되었다. 4) 간 경변증으로 인한 52례에서 모두 비악성군으로 분류되었다. 5) 전체 진단 과정을 통해 총 27명의 악성 복수 중 81.5%인 22명이 악성 복수군으로 진단되었다. 이상의 결과를 토대로 본 연구자들이 제시한 진단 과정은 악성 복수와 비악성 복수의 감별 진단에 효과적인 방법으로 생각되며 더욱 정확한 진단을 위해서는 임상 양상, 방사선학적 검사, 기타 검사 등을 토대로 종합적인 판단이 필요하리라 생각된다.

내과계 중환자들의 예후 판정에 었어서 제 7병일 APACHE III 점수의 임상적 유용성 (The Prognostic Value of the Seventh Day APACHE III Score in Medical Intensive Care Unit)

  • 연구배경 : 중환자의 예후를 계량화 하려는 채점 체계 중 APACHE III 체계는 중환자실 제1병일 접수는 물론 일 갱선점수도 중환자의 예후를 예측할 수 있는 것으로 알려져 왔다. 평균 재원일이 외국과 비교하여 3-4배의 차이가 나는 점을 감안하면, 일 갱신점수는 예후를 판정하는 지표로서 경제적 효율성이 떨어진다. 이에 제7병일(평균 중간 재원일)의 APACHE III 점수의 임상적 유용성에 관해 알아 보고자 하였다. 방 법 : 1997년 6월부터 1998년 4월까지 한양대학교 구리병원 내파계 중환자실에 입원한 241명의 제1병일과 7병일 APACHE III 점수를 조사하여 생존군과 비생존군 간의 차이를 분석하였다. 결 과 : 전체 환자 수는 241명으로 이 중 사망자가 65명으로 26.6%의 사망률을 나타내었으며 평균 재원일 수는 $10.3{\pm}13.8$일이였다. 제1병일 APACHE III 점수는 $59.7{\pm}30.9$, 제7병일 APACHE III 점수는, $37.9{\pm}27.7$점이였다. 제1병일과 제7병일 APACHE III 점수는 생존군과 비생존군에서 $49.9{\pm}23.8$, $86.3{\pm}32.3$점, $30.1{\pm}18.5$, $81.1{\pm}30.4$점으로 유의한 차이를 보였다(P<0.0001, P<0.0001). APACHE III 점수가 사망률에 미치는 영향을 알아보기 위하여 로지스틱 회귀분석을 시행한 결과 제1병일과 제7병영일의 비차비(odds ratio)는 각각 1.0507, 1.0779로 유의한 결과를 나타내었다(P<0.0001). 결 론 : 이상의 결과로서 제1병일 APACHE III 점수 뿐 아니라 제7병일 점수 또한 사망률 예측과 입원 후 치료 경과에 의해 변화된 예후를 평가하기에 유용한 척도임을 알 수 있었다. 평균 중간 재원일인 제7병일 APACHE III 점수는 일 갱선점수가 경제적으로 물적, 인적 비용이 많이 드는 상황에서 비용효과면에서 임상의에게 도움을 줄 수 있다고 판단된다.

$Fe^0$, $Fe^{2+}$, $Fe^{3+}$ / $H_2O_2$ 시스템을 이용한 침출수의 Fenton 산화반응 (Fenton Oxidation of Landfill Leachate by $Fe^0,\;Fe^{2+},\;Fe^{3+}\;/\;H_2O_2$ Systems)

  • Fenton 산화공정을 매립지 침출수에 적용하여 최적의 촉매를 선정하고 최적의 반응조건을 도출하기 위해 Lab scale로 상온에서 실험하였다 본 실험의 연구 결과, 다음의 결과를 얻을 수 있었다. 1) TOC의 제거효율로 최적 pH를 살펴본 결과 $Fe^{2+}$는 pH 3.0, $Fe^{3+}$는 pH 4.5, $Fe^0$는 pH 4.0으로 각각 관찰되었다. 2) 각 촉매별 최적 주입량을 결정하고 반응특성을 살펴보기 위하여 TOC, $COD_{Cr}$, $UV_{254}$를 변수로 보았고, 2가철의 경우 $H_2O_2$ : $Fe^{2+}\;=\;1,200\;mg/L$ : 1,200\;mg/L로 결정되었다 또한 3가철의 경우 $H_2O_2$ : $Fe^{3+}\;=\;1,200\;mg/L$ : 1,200 mg/L, 0가철의 경우 $H_2O_2$ : $Fe^0\;=\;900\;mg/L$ : 1,200 mg/L로 각각 결정되었다. 3) 최적조건에서 3가철($Fe^{3+}$)이 TOC와 $COD_{Cr}$에서 가장 높은 제거효율을 나타냈지만 0가철($Fe^0$)과 큰 제거율 차이를 나타내지 않았다. 이에 따라 경제적인 측면을 고려할 때 0가철($Fe^0$)이 동일한 철염주입량에서 가장 적은 과산화수소 주입량이 필요하므로 상대적으로 우수한 것으로 나타났다. 4) 실험에 적용된 최적 pH를 검증하기 위해 처리수를 pH 중화제(NaOH)로 적정했을 때 각 촉매별로 실험에 사용된 pH가 최적조건임을 확인할 수 있었다. 5) 촉매별로 시간에 따른 redox potential을 사용하여 모든 촉매에 대해 산화반응의 정도 및 반응이 일어나고 있는 계의 산화환원력을 간접적으로 측정할 수 있었다. 이에 따라 실제 Fenton 공정을 적용할 때 "on-line monitoring"의 기초 자료로써 산화환원전위를 활용할 수 있을 것으로 판단된다.

멀티에이전트시스템(MAS)을 이용한 G2B 조달 프로세스 혁신의 효과평가에 관한 연구 : 나라장터 G2B사례 (A Study on the Performance Evaluation of G2B Procurement Process Innovation by Using MAS: Korea G2B KONEPS Case)

  • 방대한 규모와 복잡한 프로세스로 구성된 공공조달의 혁신성과를 평가하기란 매우 어려운 문제이다. 기존의 프로세스 혁신 평가는 주로 설문 및 인터뷰, 그리고 운영데이터를 분석한 정성적, 또는 통계적인 정량적 방법들이었다. 이에 본 연구에서는 공공조달 프로세스를 복잡계로 보고 이에 대한 시뮬레이션 방법으로 멀티에이전트시스템(MAS)을 이용하여 공공조달 프로세스 혁신효과 평가모델을 개발하였다. 그리고, 개발된 MAS 기반의 공공조달 혁신평가 모델을 우리나라 조달청의 G2B(나라장터, KONEPS) 시스템에 적용하여 혁신효과를 평가하였다. MAS 시뮬레이션 도구는 Northwestern University에서 개발된 Netlogo Version 4.1.3을 사용하였고, 모델링에서는 에이전트 정의, 에이전트 행동특성 정의, 에이전트 관계 정의의 세단계로 진행하였다. 첫째, 에이전트 정의에서는 에이전트가 될 대상을 선정하고, 에이전트가 가진 속성과 변수들을 정의하였다. 둘째, 행동특성 정의에서는 각 에이전트의 행동계획 및 자원할당을 설정하였고, 셋째, 관계정의 단계에서는 상태변화에 따른 행동 규칙을 설정하였다. 또한 프로세스 혁신의 목적에 맞는 성과를 측정하기 위하여 혁신 효과평가항목을 선정하였고, 데이터는 조달청의 협조를 통해 DB 데이터와 설문데이터를 활용하였다. 이를 통해 프로세스 전체 및 프로세스별 절감시간과 업무량의 절감율을 측정하였다. 실험결과 전체 프로세스의 효율성이 증대되었으며 '평균 업무처리 건수'의 절감율이 92.7%, '평균 업무처리 시간'의 절감율이 95.4%로 나타났다. 즉 공공조달분야는 G2B 시스템 도입을 통해 프로세스 혁신을 추진한 결과 매우 높은 효율성이 제고된 것으로 분석되었다. 또한 본 연구를 통해 '계약'과 관련한 업무프로세스에서 추가적인 개선이 이루어질 경우 프로세스 혁신효과가 더욱 향상될 수 있는 것으로 분석되었다. 본 연구는 MAS를 이용하여 프로세스 개선효과에 대한 평가모델을 제시하고 분석했다는데 의의가 있다.

다중 계층 웹 필터를 사용하는 웹 애플리케이션 방화벽의 설계 및 구현 (Design and Implementation of a Web Application Firewall with Multi-layered Web Filter)

  • 최근 인터넷 상에서 빈번하게 발생하는 내부 정보와 개인 정보 유출과 같은 보안 사고들은 보안을 고려하지 않고 개발된 웹 애플리케이션의 취약점을 이용하는 방법으로 빈번하게 발생한다. 웹 애플리케이션의 공격들에 대한 탐지는 기존의 방화벽과 침입 탐지 시스템들의 공격 탐지 방법으로는 탐지가 불가능하며 서명기반의 침입 탐지 방법으로는 새로운 위협과 공격에 대한 탐지에 한계가 있다. 따라서 웹 애플리케이션 공격 탐지 방법에 대한 많은 연구들이 웹 트래픽 분석을 이동하는 비정상행위 기반 탐지 방법을 이용하고 있다. 비정상행위 탐지 방법을 사용하는 최근의 웹 방화벽에 관한 연구들은 웹 트래픽의 정확한 분석 방법, 패킷의 애플리케이션 페이로드 검사로 인한 성능 문제 개선, 그리고 다양한 네트워크 보안장비들의 도입으로 발생하는 통합관리 방법과 비용 문제 해결에 중점을 두고 있다. 이를 해결하기 위한 방법으로 통합 위협 관리 시스템이 등장 하였으나 부족한 웹 보안 기능과 높은 도입 비용으로 최근의 애플리케이션 공격들에 대해 정확한 대응을 하지 못하고 있는 현실이다. 본 연구에서는 이러한 문제점들을 해결하기 위해 웹 클라이언트의 요청에 포함된 파라미터 값의 길이에 대한 실시간 분석을 이용하여 공격 가능성을 탐지하는 비정상행위 탐지방법을 제안하고, 애플리케이션 데이터 검사로 발생하는 성능 저하 문제를 해결할 수 있는 다중 계층 웹 필터를 적용한 웹 애플리케이션 방화벽 시스템을 설계하고 구현하였다. 제안된 시스템은 저가의 시스템이나 레거시 시스템에 적용 가능하도록 설계하여 추가적인 보안장비 도입으로 야기되는 비용 문제를 해결할 수 있도록 하였다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
