부식성 식도협착 후 발생한 식도암 1례 (Carcinoma of the Esophagus after Corrosive Esophageal Stricture - One case report -)

  • A 72-year-old women whose symptom was severe dysphagia and x-ray film revealed esophageal stricture and dilatation. She had attempted suicide by swallowing lye liquids 50 years ago. A conclusive clinical and histological diagnosis of esophageal carcinoma after corrosive stricture was made following a esophagectomy with esophagogastrostomy, I experienced one case of esophageal carcinoma after corrosive stricture and reviewed it with references.

NaOH 용액을 이용한 부식성 식도 협착 동물모델 형성에 관한 연구 (Corrosive Stricture Model Induced-Esophageal Burn : Animal Pilot Data)

  • 본 연구의 목적은 부식성 식도 협착 모델을 만들어 전임상연구 모델로서의 가능성과 활용성을 알아보고자 한다. 20마리의 백서를 Healthy group / Corrosive group으로 나누고, 부식성 화상은 하부 식도에 30% NaOH를 사용하여 모델링하였다. 수술 후 행동 및 체중 변화를 매주 모니터링했습니다. 수술 후 3주 뒤에 식도조영술을 시행한 후 모든 쥐를 안락사 희생시키고 병리학적 분석을 위해 하부식도를 파라핀블럭 제작하였다. 부식성 협착에 대한 기술적 성공은 100%였습니다. 총 2마리의 백서가 연하곤란과 관련된 부식성 화상으로 Corrosive group에서 14일 이내에 사망했다. 식도 협착 비율은 Corrosive group이 Healthy group보다 유의하게 높았다(각각 40.1 ± 9.2% 및 1.4 ± 7.2%; p = 0.001). 조직 증식 비율도 Corrosive group에서 유의하게 더 높았다(각각 62.5 ± 9% 및 22.08 ± 6%; p = 0.001). 결론적으로, 30% 농도의 NaOH를 이용한 백서 부식성 모델은 전임상 연구 평가 툴로서 다양한 부식성 식도 협착 치료 전임상 연구를 수행할 수 있을 것이라 사료된다.

경열공 식도절제술을 이용한 부식성 식도협착증의 치료 (Treatment of Corrosive Esophageal Stricture by Transhiatal Esophagectomy and Esophagogastrostomy)

  • Background: Surgical treatment of corrosive esophageal stricture with colon interposition was very widely used. The colon interposition advantage is low reflux esophagitis risk and preservation of gastric capacity and peristalsis. This procedure was introduced by Orsoni and much improved. But, if stomach injury was minimal, gastric interposition is useful due to simple technique and low complication. Material and Method: Esophageal reconstruction by the transhiatal esophagectomy and intracervical esophagogastrostomy was done in 7 patients of corrosive esophageal stricture at Dong-San medical center from January 1998 to December 2007. Conclusion: Transhiatal esophagectomy and intracervical esophagogastrostomy is safety and useful method at selection case even though corrosive esophageal resection is debated.

소아의 산성 부식성 식도협착의 외과적 치료 (Surgical Treatment of Acid Induced Corrosive Esophageal Stricture in Children)

  • Accidental ingestion of caustic substance is one of the common problems among children around the world. Acid intake accounts for a mere 5% of all reported cases of corrosive ingestion in the West. Because of the esophageal sparing effect of acid, clinically significant esophageal involvement after acid ingestion occurs in only 6 to 20 percent of the instances. Despite effort of prevention, 7% to 15% of children sustaining caustic esophageal burns develop esophageal strictures. If balloon dilatation or bougie dilatation fails to resolve the esophageal strictures, successful outcome following replacement by colon or stomach has been reported in children. But the complications and morbidity following these operations are still relatively high. Seven patients with corrosive-acid induced esophageal strictures who were operated upon at the Department of Pediatric Surgery, Seoul National University Children's Hospital from 1991 to 1995 were reviewed. Primary resection and anastomosis was performed in all of 7 patients. The stricture involved short segments of the esophagus at the level of the lower cervical and the upper thoracic vertebra. The operations were approached through a left cervical incision or a left thoracotomy. In one patient, operative repair of anastomotic leakage was done, and three patients required re-resection of anastomotic strictures postoperatively, and one patient required a third operation(reversed gastric tube) due to an anastomotic stricture. The other anastomotic leaks, strictures or pulmonary complications were resolved with conservative treatment. In conclusion, primary resection and anastomosis of the esophagus was performed successfully on the 6 of 7 children with acid induced corrosive esophageal strictures. This approach is physiologic, especially in children who should have a long life expectancy, and recommended for the treatment of short-segment acid induced corrosive esophageal strictures.

재수술을 요한 식도재건술 환자의 원인분석과 임상적 고찰 (Clinical Experience of Stenotic Anastoma of Neck after Reconstuctive Surgery for Corrosive Esophageal Stricture)

  • Reconstructive surgery for corrosive esophageal stricture was performed in 392 patients at National Medical Center from 1959 to 1990 Between Jan. 1971 and Dec. 1990, 23 cases were experienced stenotic anastoma of neck after reconstructive surgery for corrosive esophageal stricture. The major procedure of esophageal reconstruction was colon interposition without resection of the strictured esophagus except jejunal interposition in 1 case. There were 12 males % 11 females, and mean age was forty years. The caustic materials were 16[70%] alkali and 7[30%] acid. Half of the cases had hypopharyngeal injury. After reconstructed surgery, dysphagia was developed immediate in 65%, from 2 months to 5 months in 31%, and from years in 4%[1 case]. The complications were anastomatic leakage in 13 cases, anastomatic stenosis in 8 cases, graft gangrene in 1 case, and cancer development in 1 case. The therapeutic procedures were end-to-end anstomolis & partial resection of stenotic anastoma in 18 cases, bourgination in 2 cases, and coin interposition with graft removal in 3 cases. The therapeutic results were excellent in 16 cases, mild discomfort in 3 cases, poor in 3 cases, and death in 1 cases.

양성 식도 협착의 외과적 치료 (Surgical Treatment of Benign Esophageal Stricture)

  • During a ten-year period from August, 1978 to September, 1989 45 patients with benign esophageal stricture were surgically evaluated. The results are as follows; l. Out of 45 patients, there were 26 males and 19 females ranging from 2 to 70 years of age with a mean of 31.9 years. 2. The most common cause of benign esophageal stricture was corrosive burn due to caustic agents[40 cases, 88.9 %]. Corrosive agents were 15 cases of lye, 22 cases of acid and 3 cases of other agents. Other causes were two cases of esophageal web and each one case of previous surgical result, inflammation and idiopathic mediastinal fibrosis respectively. 3. The most frequent stricture site was whole esophagus as 21 cases[46.7 %] and the next was lower a third thoracic esophagus[10 cases, 25.0%]. 4. In 33 of total 45 cases, colon interposition with right colon was performed without resection of the strictured esophagus except one case which was complicated esophageal cancer. Other procedures were 4 cases esophagogastrostomy with segmental resection, 2 cases of plastic repair and so on. 5. Major postoperative complications which were needed for secondary operation were 5 cases[11.1 %]. [2 cases of stenosis, ileus and ulcer bleeding respectively] Overall mortality rate was 4.4 %.

부식성 식도협착 환자의 비개흉적 식도 적출술 - 2례 보고 - (Esophagectomy without thoracotomy in corrosive esophageal stricture - case report -)

  • The strictured esophagus is not removed usually in corrosive injury. But corrosion carcinoma and the late complications such as esophago-bronchial fistula, chronic mediastinal abscess, diverticulum can occur in case that strictured & inflamed esophagus be remained. Recently transhiatal esophagectomy is being done with low mortality and morbidity. So we are reporting 2 cases of esophageal stricture in young patients which were treated with transhiatal esophagectomy and esophagocologastrostomy. All of 2 cases were successfully treated and recovered. Postoperative esophageal function tests showed the mild reflux but it did not count clinically.

유리공장이식편을 이용한 합병된 부식성 식도협착의 재건술 (Jejunal Free Graft for Complicated Corrosive Esophageal Stricture)

  • 부식성 식도협착의 재건수술시 대장의 혈액순환이 나쁜 경우 또는 대장치환술 후 허혈성 부전이 나타난 경우 가장 적합한 재건장기로는 유리공장이식편을 들 수 있다. 복잡하게 합병된 부식성식도협착 환자에서 유리공장이식편을 이용하여 식도재건술을 시행한 4예를 보고하고자 한다. 일차식도재건술로서 4예 중 3예는 대장치환술을, 1예는 장천공에 의한 복막염으로 Whipple's 술식 및 공장루설치술을 받았다. 1예는 인두대장문합부의에 유리공장첩포 이식을, 2예는 대장이식편 부전 부위에 유리공장이식편 치환을 받았다. 1예는 대장의 일부와 유리공장이식편을 이용하여 인두공장대장공장문합술을 시행하였다. 수술 후 문합부위 누출로 인한 재수술이 1예 있었다. 장기추적 관찰상 모두 연하곤란이 없었다. 여러 가지 원인으로 인하여 합병된 식도재건술이 필요하여 유리공장이식편을 사용했던 경우 만족할 술 후 이환율과 연하기능을 보였다.

부식성 식도협착부위에 발생한 식도암 - 14례 보고 - (Esophageal Corrosion Carcinoma at the Site of Caustic Stricture - A Report of 14 Cases -)

  • Between Dec. 1958 and Oct. 1989, we had been experienced 14 cases of the esophageal corrosion carcinoma among 562 cases of corrosive esophageal stricture, which incidence was revealed 5.6 % of total esophageal carcinoma cases and 2.5 9o of total benign esophageal stricture cases. The age distribution was from 15 to 64 years, and the mean age was 45.6 years. Ten of the patients were women. The caustic agents were lye in 12 patients and hydrochloric acid in 2 patients. The latent time from injury to the development of carcinoma was from 12 years to 45 years, and the mean latent time was 29 years. In majority of cases, the developing site of carcinoma was just or just above the benign stricture portion of the esophagus. Corrective surgery were made in seven cases; three cases were made esophagectomy and esophagogastrostomy, and the other four cases were made esophagectomy and colon interposition. Palliative surgery including feeding gastrostomy were made in seven cases. All cases had a longstanding history of difficult oral swallowing especially at the site of stricture. We think chronic food irritations of stricture portion of the esophagus induced the development of corrosion carcinoma. In conclusion, we had to suspect possibility of carcinoma in patient with long standing history of benign esophageal stricture. So we recommended preoperative esophagoscopic biopsy in such cases.

  • Various flaps are using for reconstruction of esophageal defect. The choice of reconstruction is depended to the oncologic needs of the situation. If the entire esophagus or significant part of the thoracic esophagus is involved by tumor, then total esophagectomy and gastric pull-up or colon transposition is indicated. But for most hypopharyngeal tumors, laryngopharyngeal tumors, and cervical esophageal tumors, segmental resection of these area and replacement with a jejunal fee or forearm free flap has become the standard technique. The authors have experienced a case of total pharyngo-laryngo-esophago-gastrectomy and colon transposition in a patient of esophageal cancer following partial esophagectomy and gastic pull-up due to corrosive esophageal stricture. We report this case with brief review of the literatures.
