Clinical implications of coronavirus disease 2019 in neonates

  • Since the emergence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in December 2019, a small number of coronavirus disease 2019 (COVID-19) cases in neonates have been reported worldwide. Neonates currently account for only a minor proportion of the pediatric population affected by COVID-19. Thus, data on the epidemiological and clinical features of COVID-19 in neonates are limited. Approximately 3% of neonates born to mothers with COVID-19 reportedly tested positive for SARS-CoV-2. Current limited data on neonates with COVID-19 suggest that neonatal COVID-19 shows a relatively benign course despite a high requirement for mechanical ventilation. However, neonates with pre-existing medical conditions and preterm infants appear to be at a higher risk of developing severe COVID-19. The greatest perinatal concern of the COVID-19 pandemic is the possibility of vertical transmission, especially transplacental transmission of SARS-CoV-2. Although direct evidence of the vertical transmission of SARS-CoV-2 is lacking, its possibility during late pregnancy cannot be ruled out. This review summarizes available case studies on COVID-19 in neonates and introduces what is currently known about neonatal COVID-19 with focus on its vertical transmission.

  • Controversy remains over whether the coronavirus disease 2019 (COVID-19) virus may have infectivity during the incubation period before the onset of symptoms. The author had the opportunity to examine the infectivity of COVID-19 during the incubation period by conducting an epidemiological survey on a confirmed patient who had visited Jeju Island during the incubation period. The epidemiological findings support the claim that the COVID-19 virus does not have infectivity during the incubation period.

  • Coronavirus disease 2019 (COVID-19) is a respiratory tract infection caused by a newly emergent coronavirus, that was first recognized in Wuhan, China, in December 2019. Genetic sequencing of the virus suggests that it is a beta coronavirus closely linked to the SARS virus. This disease has non-specific symptoms such as fever, dry cough, sore throat, and gastrointestinal symptoms. This virus can transmit via aerosol and need to droplet precaution to prevent spreading in public areas. Most people with COVID-19 develop only mild or uncomplicated illness. However, about 20% patients require hospitalization, oxygen supply, and intensive care. There is no currently effective treatment available for COVID-19 unresponsive to supportive care. This is review about the recently published epidemiologic, and clinical features, diagnosis, treatment and prevention of COVID-19.

  • 최근 세계보건기구(WHO)의 Coronavirus Disease-19(COVID-19)에 대한 팬데믹 선언으로 COVID-19는 세계적인 관심사이며 많은 사망자가 속출하고 있다. 이를 극복하기 위하여 국가 간 정보 교환과 COVID-19 관련 대응 방안 등의 공유에 대한 필요성이 증대되고 있다. 하지만 언어적 경계로 인해 원활한 정보 교환 및 공유가 이루어지지 못하고 있는 실정이다. 이에 본 논문은 COVID-19 도메인에 특화 된 인공신경망 기반 기계번역(Neural Machine Translation(NMT)) 모델을 제안한다. 제안한 모델은 영어를 중심으로 프랑스어, 스페인어, 독일어, 이탈리아어, 러시아어, 중국어 지원이 가능한 Transformer 기반 양방향 모델이다. 실험결과 BLEU 점수를 기준으로 상용화 시스템과 비교하여 모든 언어 쌍에서 유의미한 높은 성능을 보였다.

  • 코로나바이러스감염증-19 (coronavirus disease 2019, COVID-19)의대유행 동안 이에 대응하기 위해 학교폐쇄가 이루어졌고, 이에 대해 경제학, 사회학, 수학모델링, 역학 및 공중보건을 포함한 다양한 학문분야에서 연구가 진행되고 있다. 인플루엔자 대유행의 모형으로부터 파생된 학교 폐쇄의 COVID-19에 대한 수리 모형은 상충되는 결과들을 보였다. 초기 연구에서는 학교와 지역사회 전반에 걸친 전파를 모델링하여 개학의 위험을 평가했으나, 이 위험이 성인 또는 소아의 전파 중 어떤 측면에 인한 것인지 여부는 여전히 불분명하다. 경험적인 결과들을 종합했을 때, 학교 폐쇄가 COVID-19 유행의 통제에 명확한 효과를 보여주지 못하였으며 이는 지역사회에 따라 COVID-19 유행의 규모가 다르고, 학교 폐쇄 정책이 다양하며, 여러 가지 대응 전략이 함께 사용되었기 때문일 수 있다. 학교 폐쇄의 이익은 불분명하고 정량화하기 어려운 반면, 학교 폐쇄에 따른 교육 격차 및 사회경제적 부담 등 미래 세대에 치러야 할 잠재적인 사회적 비용이 매우 커질 것이다. 본 종설에서는 COVID-19 대유행에 대응하기 위한 학교 폐쇄의 영향에 대해 정리하고 소아청소년의 건강 영향에 대한 고찰을 통해 교육기관 등교 정상화에 대한 제언을 하고자 하였다.

Small Molecule Drug Candidates for Managing the Clinical Symptoms of COVID-19: a Narrative Review

  • Towards the end of 2019, an atypical acute respiratory disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was identified in Wuhan, China and subsequently named Coronavirus disease 2019 (COVID-19). The rapid dissemination of COVID-19 has provoked a global crisis in public health. COVID-19 has been reported to cause sepsis, severe infections in the respiratory tract, multiple organ failure, and pulmonary fibrosis, all of which might induce mortality. Although several vaccines for COVID-19 are currently being administered worldwide, the COVID-19 pandemic is not yet effectively under control. Therefore, novel therapeutic agents to eradicate the cause of the disease and/or manage the clinical symptoms of COVID-19 should be developed to effectively regulate the current pandemic. In this review, we discuss the possibility of managing the clinical symptoms of COVID-19 using natural products derived from medicinal plants used for controlling pulmonary inflammatory diseases in folk medicine. Diverse natural products have been reported to exert potential antiviral effects in vitro by affecting viral replication, entry into host cells, assembly in host cells, and release. However, the in vivo antiviral effects and clinical antiviral efficacies of these natural products against SARS-CoV-2 have not been successfully proven to date. Thus, these properties need to be elucidated through further investigations, including randomized clinical trials, in order to develop optimal and ideal therapeutic candidates for COVID-19.

항공종사자와 코로나바이러스감염증-19 백신 예방 접종 (COVID-19 Vaccination for Pilots and Air Traffic Controllers)

  • The coronavirus disease-19 (COVID-19) vaccine is expected to play an important role in stopping the pandemic. Studies show that COVID-19 vaccines are effective at keeping you from getting COVID-19. Getting a COVID-19 vaccine will also help keep you from getting seriously ill even if you do get COVID-19. Efforts to find an effective vaccine against severe acute respiratory syndrome coronavirus 2 have progressed unprecedentedly through active support from public research grants and private-public partnership programs. Clinical studies have been actively conducted, and some vaccines are being vaccinated with approval for urgent use. The WHO has approved and supplied the Pfizer-BioNTech COVID-19 vaccine and the Oxford-AstraZeneca COVID-19 vaccine. In Korea, the Oxford-AstraZeneca vaccine was approved for urgent use, and vaccination began on February 26, 2021. In this paper, the efficacy and side effects of each vaccines and the effect on pilots and air traffic controllers related to COVID-19 vaccination were investigated in terms of aviation medicine.

  • Coronavirus disease 2019 (COVID-19) is currently in progress. Although it is difficult to predict the end of currently increasing COVID-19, it is expected to last for a long time. The COVID-19 is making a lot of changes. Due to physical distancing and living distancing, non-contacts such as wearing facial masks, online lectures, online medical services, telecommuting, and telemarketing are becoming common. In the era of post-COVID-19, online and offline will coexist. Many countries following China's lockdown strategy, which is agreed with the World Health Organization, should be changed to Taiwan's facial mask strategy for reducing the economic problems. The prolonging COVID-19 will add to the economic difficulties, and the US-China confrontation will be difficult to rebound the global economy. COVID-19, such as plaque, smallpox, and Spanish influenza, will be a historical momentum. How to respond to the crisis of COVID-19 and post-COVID-19 will determine the future of the world and Korea.

  • 2019년 12월에 중국 후베이성 우한시에서 원인 미상 폐렴이 무리 지어 발생하기 시작하였다. 환자의 하기도에서 이전에 알려지지 않은 새로운 코로나바이러스가 분리되었으며 severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2)로 명명되었다. 세계보건기구는 SARS-CoV-2에 의한 질병을 코로나바이러스감염증-19(coronavirus disease 2019, COVID-19)로 명명하였다. 2020년 3월 11일에 COVID-19는 전세계 최소 114국으로 퍼졌으며 약 4,000명이 사망하여 세계보건기구는 COVID-19가 세계적 대유행임을 선언하였다. 지난 20년 동안 새로운 코로나바이러스에 의한 두 번의 유행이 있었다. 2002-2003년 중증급성호흡기증후군(severe acute respiratory syndrome, SARS) 유행 시에는 8,098명의 확진 환자와 774명의 사망자가 발생하였으며, 2012년에 사우디 아라비아에서 시작되어 현재까지 주로 아라비아 반도에서 발생하고 있는 중동호흡기증후군(Middle East respiratory syndrome, MERS)는 2019년까지 총 2,499명의 환자와 858명의 사망자를 발생시켰다. 본 종설의 목적은 2020년 3월 12일까지 알려진 SARS-CoV-2의 특징과 전파 양상 및 COVID-19의 임상 증상을 알아보고 SARS와 MERS와의 유사점 및 차이점에 대하여 간략하게 소개하는 것이다.

2019 Novel Coronavirus (COVID-19) Pneumonia: Serial Computed Tomography Findings

  • From December 2019, Coronavirus disease 2019 (COVID-19) pneumonia (formerly known as the 2019 novel Coronavirus [2019-nCoV]) broke out in Wuhan, China. In this study, we present serial CT findings in a 40-year-old female patient with COVID-19 pneumonia who presented with the symptoms of fever, chest tightness, and fatigue. She was diagnosed with COVID-19 infection confirmed by real-time reverse-transcriptase-polymerase chain reaction. CT showed rapidly progressing peripheral consolidations and ground-glass opacities in both lungs. After treatment, the lesions were shown to be almost absorbed leaving the fibrous lesions.
