목적: 운동부하 심전도검사에 나타나는 ST-분절의 하강은 운동부하의 정도에 영향을 받는다. 그러나 운동부하의 정도가 운동부하 심근관류영상의 관동맥 질환 진단능에 미치는 영향은 보고자마다 달라 아직도 논란이 되고있다. 본 연구는 운동부하의 정도가 심근관류 영상의 관상동맥 질환 진단능에 미치는 영향을 규명하고자 시행하였다. 대상 및 방법: 111명(남자 73명, 여자 38명, 평균연령: 56세)의 안정된 환자를 대상으로 운동부하검사와 함께 Tc-99m MIBI를 이용한 심근관류 스캔을 실시하고 3개월 이내에 관상동맥촬영술을 실시하였다. 운동부하의 정도는 운동시 최대 예측 심박수의 85%에 도달했는가를 기준으로 하고, 운동심전도 소견을 첨가하여 4군으로 구분하고 각각의 군에서의 진단능을 비교하였다. 결과: 1) Tc-99m MIBI 심근관류스캔의 예민도와 특이도는 심전도검사의 소견이나 심박수의 증가정도에 따른 차이는 없었다. 2) 운동부하 Tc-99m MIBI SPECT의 예민도는 87%, 특이도는 79%였고 전례를 대상으로 한 경우 운동부하 심전도는 예민도 55%, 특이도 74%로서 예민도는 SPECT가 운동부하 심전도보다 유의하게 높았으며 특이도는 유의한 차이는 없었으나 SPECT가 더 높은 경향을 보였다. 3) 최대 예측 심박수의 85% 이상의 운동량에 도달한 경우의 관동맥 질환 진단 예민도는 85% 이하의 운동량에 도달한 군과 유사하였다(88% vs 87%, p=NS). 4) 운동부하 정도가 적었으나 조기에 심전도 검사상 양성 소견을 보인 경우에는 다혈관 질환에서 개개 혈관의 협착 진단능은 낮은 경향을 나타내었다. 결론: 이상의 결과로 볼 때 운동부하 Tc-99m MIBI 심근관류 스캔의 관동맥 협착 진단능은 운동부하 심전도검사보다 높았고, 답차를 이용한 증상 제한적 운동부하검사에서 심박동수의 증가로 평가란 운동 정도가 운동 목표량에 도달하지 않아도 진단능이 감소되지 않음을 알 수 있었다.
관상동맥 조영술(CAG)의 씨네(Cine) 촬영에서 엑스선 촬영 조건의 변화가 입사 표면 선량(ESD)과 흡수선량(DAP)에 미치는 관계를 알아보고자 하였고 image J 프로그램을 통해 촬영된 혈관 조영 영상의 SNR과 CNR을 측정 분석하여 조건 변화가 선량관계 및 영상의 질에 미치는 유용성을 분석하고자 하였다. 2017년 11월부터 2018년 3월까지 본원에서 CAG를 시행한 33명(남24, 여9)의 데이터를 대상으로 하였고, 연령대는 37-76세(평균 $59{\pm}10$세), 몸무게53-104kg (평균 $72{\pm}10kg$), 키150-185cm (평균 $166.82{\pm}9.5kg$), BMI 18.3-33.2(평균 $25.8{\pm}3.2$)이었다. 촬영조건 및 데이터 획득은 관전류(mA)를 높게 한 A그룹(397.2mA)과 mA를 낮게 한 B그룹(370.7mA)의 ESD와 DAP를 후향적으로 획득하여 비교 분석 하였고 Image J를 통한 SNR과 CNR 측정 분석은 획득한 데이터를 공식에 대입하여 결과 값을 도출하였다. 통계프로그램은 SPSS (PASW)를 사용하여 촬영조건 변화에 따른 ESD와 DAP 및 SNR CNR의 상관관계 등을 분석하였다. 촬영조건의 mA에 변화를 주어 촬영한 A그룹과 B그룹의 ESD($A:483.5{\pm}60.1$, $B: 464.4{\pm}39.9$)와 DAP($A:84.3{\pm}10.7$, $B:81.5{\pm}7$)간의 관계는 통계적으로 유의하지 않았다(p>0,05). Image J를 통한 SNR과 CNR의 관계에선 B그룹의 좌심장동맥(LCA)을 촬영해 얻은 영상의 SNR($5.451{\pm}0.529$), CNR($0.411{\pm}0.0432$)이 A그룹 좌심장동맥(LCA)의 SNR($4.976{\pm}0.433$), CNR($0.459{\pm}0.0431$)보다 SNR $0.475{\pm}0.096$, CNR $-0.048{\pm}0.0$로 차이가 있었으나 통계적으로 유의하지는 않았다(p<0.05). 우심장동맥(RCA)을 촬영해 획득한 SNR과 CNR에선 A그룹의 SNR($4.731{\pm}0.773$)과 CNR($0.354{\pm}0.083$)이 B그룹의 SNR($3.24{\pm}0.368$), CNR($0.166{\pm}0.033$)보다 SNR $1.491{\pm}0.405$, CNR $0.188{\pm}0.005$로 증가된 수치를 나타냈으며 그 중 CNR이 통계적으로 유의했다(p<0.05). 상관관계 분석 결과에서는 SNR (LCA) & CNR (LCA), SNR (RCA) & CNR (RCA), ESD & DAP, ESD & sec, DAP & CNR (RCA), DAP & sec간에 통계적으로 유의한 차이를 나타냈다(p<0.05). SNR과 CNR이 높을수록 선명하고 좋은 화질을 나타냄을 의미하는데 화질평가 및 선량변화 유용성에 관한 분석 연구를 진행한 결과 mA를 증가시켜 촬영 한 CAG의 RCA영상에서 SNR과 CNR이 증가된 수치를 보였다. 특히, CNR이 통계적으로 유의한 차이를 나타낸 것을 보았을 때 RCA촬영 시 mA를 향상시켜 촬영 할 경우 영상의 질 중 대조도가 한층 더 향상 될 수 있을 것으로 사료된다.
심막 질환은 응급질환인 관상동맥, 대동맥 질환과 증상이 비슷하여 빠른 감별이 중요하다. 심막은 두겹으로 된 막으로서 다양한 해부학적 변이와 질환이 있다. 크거나 비전형적인 위치를 보이는 심막의 오목(recess)은 이상 소견으로 오인될 수도 있다. 심막에는 감염이나 종양 같은 질환이 생기며 또한 주변에서 생기는 이러한 질환의 전파경로가 될 수 있다. 심막을 평가하는데 있어 심초음파가 일차적인 검사방법이나 해상도가 떨어지고 주변의 폐나 뼈에 의한 음창이 제한될 수 있으며 수술 후 평가가 힘든 경우가 많다. 이때 CT나 MR에 의한 영상평가가 유용한 경우가 많아 검사가 점점 늘어나고 있다. 심막의 형태와 질환에 대한 지식 및 영상소견에 대한 이해는 환자 치료에 중요하다.
Objective: To study the prevalence and clinical characteristics of decreased myocardial blood flow (MBF) quantified by dynamic computed tomography (CT) myocardial perfusion imaging (MPI) in symptomatic patients without in-stent restenosis. Materials and Methods: Thirty-seven (mean age, 71.3 ± 10 years; age range, 48-88 years; 31 males, 6 females) consecutive symptomatic patients with patent coronary stents and without obstructive de novo lesions were prospectively enrolled to undergo dynamic CT-MPI using a third-generation dual-source CT scanner. The shuttle-mode acquisition technique was used to image the complete left ventricle. A bolus of contrast media (50 mL; iopromide, 370 mg iodine/mL) was injected into the antecubital vein at a rate of 6 mL/s, followed by a 40-mL saline flush. The mean MBF value and other quantitative parameters were measured for each segment of both stented-vessel territories and reference territories. The MBFratio was defined as the ratio of the mean MBF value of the whole stent-vessel territory to that of the whole reference territory. An MBFratio of 0.85 was used as the cut-off value to distinguish hypoperfused from non-hypoperfused segments. Results: A total of 629 segments of 37 patients were ultimately included for analysis. The mean effective dose of dynamic CT-MPI was 3.1 ± 1.2 mSv (range, 1.7-6.3 mSv). The mean MBF of stent-vessel territories was decreased in 19 lesions and 81 segments. Compared to stent-vessel territories without hypoperfusion, the mean MBF and myocardial blood volume were markedly lower in hypoperfused stent-vessel territories (77.5 ± 16.6 mL/100 mL/min vs. 140.4 ± 24.1 mL/100 mL/min [p < 0.001] and 6.4 ± 3.7 mL/100 mL vs. 11.5 ± 4 mL/100 mL [p < 0.001, respectively]). Myocardial hypoperfusion in stentvessel territories was present in 48.6% (18/37) of patients. None of clinical parameters differed statistically significantly between hypoperfusion and non-hypoperfusion subgroups. Conclusion: Decreased MBF is commonly present in patients who are symptomatic after percutaneous coronary intervention, despite patent stents and can be detected by dynamic CT-MPI using a low radiation dose.
Image fusion is fast catching attention as Wagner pointed out in his 2006 version of the recent progress and development presented at the annual meeting of Society of Nuclear Medicine. Prototypical fusion of bone scan and radiograph was already attempted at in 1961 when Fleming et al. published an article on strontium-85 bone scan. They simply superimposed dot scan on radiograph enabling simultaneous assessment of altered bone metabolism and local bone anatomy. Indeed the parallel reading of images of bone scan and radiography, CT, MRI or ultrasonography has been practiced in nuclear medicine long since. It is fortunate that recent development of computer science and technology along with the availability of refined CT and SPECT machines has permitted us to open a new avenue to digitally produce precise fusion image so that they can readily be read, exchanged and disseminated using internet. Ten years ago fusion was performed using Bresstrahlung SPECT/CT and it is now achievable by PET/CT and SPECT/CT software and SPECT/CT hardware. The merit of image fusion is its feasibility of reliable assessment of morphological and metabolic change. It is now applicable not only to stationary organs such as brain and skeleton but also to moving organs such as the heart, lung and stomach. Recently, we could create useful fusion image of cardiac SPECT and 64-channel CT angiograph. The former provided myocardial metabolic profile and the latter vascular narrowing in two patients with coronary artery stenosis and myocardial ischemia. Arterial stenosis was severe in Case 1 and mild in Case 2.
Angiography means that a check up to know an abnormal condition in all the blood vessels include from the heart, aortae, cerobrovascular and abdonominal artery to hands and feet. Main examples of this are cerebral angiography, abdominal, liver for urinary anomaly, renovascular angiography, and artery and vein in arms and legs. In general, it is performed by using equipment which is attached one or two C-arms and the method of inserting catheter in vein after anesthesia. For this reason, some rooms that consist of angiography room units should be planned not only for expensiveness equipment and facilities also to be germ-free. Nowadays, in the angiography unit case, it is placed independently as the central part of many hospitals. It does not belong to the imaging medical department any more as considering raising filming times and the relation between C.C.U.(coronary care unit) and operation unit. This means the acts performed are diversified and well-organized rooms in support of diagnosis are required. However, it is difficult to plan the angiography room unit due to domestic researches and data on this unit are not enough. Therefore, this study aims at bringing up basic issue for architectural planning of the angiography unit in general hospital.

Cardiac PET emerged as a powerful tool that allowed in vivo quantification of physiologic processes including myocardial perfusion and metabolism, as well as neuronal and receptor function for more than 25 years. Wow PET imaging has been playing an important role in the clinical evaluation of patients with known or suspected ischemic heart disease. This important clinical role is expected to grow with the availability of PET/CT scanner that allow a true integration of structure and function. The objective of this review is to provide an update on the current and future role of PET in clinical cardiology with a special eye on the great opportunities now offered by PET/CT.
The potential for recovery of left ventricular dysfunction after myocardial revascularization represents a practical clinical definition for myocardial viability. The evaluation of viable myocardium in patients with severe global left ventricular dysfunction due to coronary artery disease and with regional dysfunction after acute myocardial infarction is an important issue whether left ventricular dysfunction may be reversible or irreversible after therapy. If the dysfunction is due to stunning or hibernation, functional improvement is observed. but stunned myocardium may recover of dysfunction with no revascularization. Hibernation is chronic process due to chronic reduction in the resting myocardial blood flow. There are two types of myocardial hibernation: "functional hibernation" with preserved contractile reserve and "structural hibernation" without contractile reserve in segments with preserved glucose metabolism. This review focus on the application of F-18 FDG and other radionuclides to evaluate myocardial viability. In addition the factors influencing predictive value of FDG imaging for evaluating viability and the different criteria for viability are also reviewed.
