Takayasu`s disease is an arteritis of unknown etiology involving larger elastic arteries such as aorta and its branches, pulmonary arteries and rarely coronary arteries. Especially, aortic root involvement with the valvular leaflets has been reported in several cases of Takayasu`s arteritis. Recently we have experienced one case of Takayasu`s arteritis involving left subclavian artery, descending aorta, left renal artery and multiple valvular leaflets. The patient was 33 year-old female and admitted with complaints of cough, dyspnea and general weakness. Aortogram revealed extensive type of arteritis showing dilatation of ascending aorta, segmental narrowing of thoracic aorta and Riolan`s anastomosis. Double valve replacement [mitral and aortic valve] and tricuspid valve annuloplasty were performed. The patient made an excellent postoperative recovery and has shown striking improvement in cardiac status, NYHA functional class II eight months after operation.
급성 관동맥 증후군(acute coronary syndrome)은 대부분 죽상경화 관상동맥 질환(atherosclerotic coronary artery disease)에 의해 발생하지만, 비죽상경화 관상동맥 질환에서도 발생할 수 있다. 고식적 관상동맥 혈관조영술은 동맥 내강의 협착이나 확장 등의 형상에 대한 정보만을 제공하고, 동맥경화반이나 동맥벽에 대한 평가가 어려워 관상동맥 이상의 원인 질환의 진단에 낮은 특이도를 보인다. 반면, 관상동맥 전산화단층촬영 혈관조영술은 혈관경화반의 특징, 혈관벽의 조영증강뿐 아니라 연접한 대동맥이나 폐동맥의 변화 등도 함께 관찰할 수 있어, 비죽상경화 관상상동맥질환의 진단 및 다양한 원인 감별에 도움이 된다. 따라서 이종설에서는 다양한 비죽상경화 관상동맥 질환들을 소개하고, 이의 병태생리 및 대표적인 관상동맥 전산화단층촬영 혈관조영술의 영상 소견에 대해 설명하고자 한다.

We report on two women who underwent myocardial revascularization associated with antiphospholipid syndrome (APS) with different pathogenic patterns. The first woman presented with acute myocardial infarction, and preoperative angiograms demonstrated rapidly progressing coronary lesions, presumptive unstable plaque, and dissection. Operative findings, however, showed fresh thrombi in the coronary arteries, and she was diagnosed postoperatively as having APS. Her one-year angiogram demonstrated improved coronary lesions and a competitive flow pattern in the grafts. The second woman presented with unstable angina and had been treated for systemic lupus erythematosus and secondary APS for more than 14 years. She underwent myocardial revascularization due to accelerated coronary atherosclerosis. Her one-year angiogram demonstrated patent grafts.
본 논문에서는 조영 영상을 받아들여 관상동맥을 효과적으로 추출하는 방법을 소개한다. 혈관 추출에 일반적으로 사용되는 디지털 혈관조영술(DSA : Digital Subtraction Angiography)은 조영제 투입 전에 촬영된 마스크 영상과 조영제 투입 후의 혈관 대비가 나타나는 라이브 영상과의 차이를 이용하여 빠르게 혈관 영역만을 검출하는 방법이다. 그러나 이 방법은 배경의 움직임에 민감하고 두 영상간의 지역적인 배경 명암 분포의 변화에 따라 오 검출이 발생할 수 있다는 단점을 가진다. 따라서 본 논문에서는 배경 텍스쳐의 유사도를 분석하여 움직임의 차이가 가장 작은 영상을 선택함으로써 배경의 움직임에 기인하는 구조적인 문제를 해결하고, 선택된 영상의 지역적 명암 보정을 통해 혈관 영역만을 효과적으로 추출하는 방법을 제안한다. 실험 결과는 제안된 방법이 기존의 방법보다 오 인식률은 감소하고 정확도는 증가함을 보여준다.
배경 : 관상동맥 결찰법에 의한 만성 심부전 모델은 실험 사망률이 비교적 높음에도 불구하고 임상적 연관성때문에 지속적인 연구 대상이 된다. 특히 대동물에서 만성 심부전 모델 확립은 소동물에서는 검증할 수 없는 기계 또는 생물학적 순환보조시스템의 연구분석에 대단히 유용하게 사용될 수 있다 이런 관점에서 본연구는 양에서 homonymous artery(사람의 좌전하행지에 해당)와 diagonal branch를 순차적으로 결찰 하는 방법으로 신뢰성 있는 만성 심부전 모델을 확립하고자 하였다. ·재료 및 방법: 모두 9마리의 Corridale 양을 사용하였다. 2마리 양에서 homonymous artery 와 diagonal branch를 동시에 결찰 하였고 7마리에서는 homonymous artery와 diagonal branch를 1시간 간격으로 결찰 하였다. 좌전 개흉술로 심장을 노출시킨 후 심첨부에서 기저부까지의 거리를 기준으로 하여 심첨부에서 40% 위치에서 homonymous artery를 결찰 하였다. diagonal branch는 homonymous artery와 동일한 위치에서 결찰 하였다. 관상동맥 결찰 전, homonymous artery 결찰 후,그리고 diagonal branch 추가 결찰 후 각각 동맥압, 심전도, 그리고 thermodilution catheter로 중심정맥압, 폐동맥압, 폐동맥쐐기압, 심박출량을 측정하였다. 같은 시점에 초음파 검사를 통해 좌심실 이완말기 크기, 좌심실수축말기 크기, 좌심실 구혈률, 좌심실 벽두께, 좌심실 구획단축률과 좌심실 벽운동을 분석하였다. 실험 동물은 2개월 또는 3개월 사육한 뒤 같은 분석 과정을 거친 후 희생시켰다. 실험양의 심장은 적출하여 병리조직학적 검사를 하였다. 결과 Homonymous artery 와 diagonal branch를 동시에 결찰 한 2마리는 결찰 직후 모두 사망하였다. 반면 homonymous artery 와 diagonal branch를 순차적으로 결찰 한 7마리는 모두 생존하였는데 이중 5마리는 2개월 그리고 2마리는 3개월 사육후 심장을 적출하였다. 결찰 2~3개월 후 양에서 중심정맥압, 폐동맥압, 폐동맥쐐기압, 좌심실 이완말기 및 수축말기 크기의 유의한 증가가 관찰되었다(p<0.05) 반면 homonymous artery 결찰 직후에는 동맥압의 유의한 감소(p<0.05)가 diagonal branch의 추가 결찰 후에는 동맥압, 심박출량의 감소 및 폐동맥쐐기압의 유의한 증가가 관찰되었다(p<0.05). 좌심실 벽운동은 결찰 직후부터 다양한 정도의 anteroseptal akinesia또는 dyskinesia가 관찰되었다. 적출 심장의 병리 분석 결과 주위와 잘 구별되는 섬유화 경색 부위가 관찰되었다. 결론: 양에서 homonymous artery와 diagonal branch를 순차적으로 결찰하는 방법으로 신뢰성 있는 만성 심부전 모델을 확립할 수 있었다.

Background: This study aimed to investigate sternal healing over time and the incidence of poor sternal healing in patients undergoing coronary artery bypass graft (CABG) surgery using bilateral internal thoracic arteries. Methods: This study enrolled 197 patients who underwent isolated CABG using skeletonized bilateral internal thoracic arteries (sBITA) from 2006 through 2009. Postoperative computed tomography (CT) angiography was performed on all patients at monthly intervals for three to six months after surgery. In 108 patients, an additional CT study was performed 24 to 48 months after surgery. The axial CT images were used to score sternal fusion at the manubrium, the upper sternum, and the lower sternum. These scores were added to evaluate overall healing: a score of 0 to 1 reflected poor healing, a score of 2 to 4 was defined as fair healing, and a score of 5 to 6 indicated complete healing. Medical records were also retrospectively reviewed to identify perioperative variables associated with poor early sternal healing. Results: Three to six months after surgery, the average total score of sternal healing was $2.07{\pm}1.52$ and 68 patients (34.5%) showed poor healing. Poor healing was most frequently found in the manubrium, which was scored as zero in 72.6% of patients. In multivariate analysis, the factors associated with poor early healing were shorter post-surgery time, older age, diabetes mellitus, and postoperative renal dysfunction. In later CT images, the average sternal healing score improved to $5.88{\pm}0.38$ and complete healing was observed in 98.2% of patients. Conclusion: Complete sternal healing takes more than three months after a median sternotomy for CABG using sBITA. Healing is most delayed in the manubrium.
Sixty four children [aged 2 days to 9 years] , 58 with complete transposition of the great arteries, 5 with Taussig-Bing double outlet right ventricle, and 1 with double outlet left ventricle plus left ventricular type single ventricle, have undergone anatomic correction from November 1987 to August 1992. Eleven underwent previous operations: pulmonary artery banding[7], modified Blalock-Taussig shunt[2], coarctoplasty[2], aortic arch reconstruction[1] . Of 58 patients with TGA, Type A coronary arteries of Yacoub were seen in 50[86%]. U-shaped coroanry arterial flaps were transfered to the neoaorta using trap door technique, and neopulmonary arterial tract was constructed using glutaraldehyde fixed autopericardium with Lecompte maneuver. There were 18 hospital deaths [28.1%] with no late mortality. Mean follow-up of 20.4\ulcorner11.9 months were achieved in all survivors. Postoperative cardiac catheterizations were done in 14 cases. Mean pressure gradients of pulmonary and aortic outflow tract were 15.0 $\pm$2.6 and 4.2$\pm$1.4mmHg, mild aortic valve insufficiencies were found in 2, and mean cardiac index was 5.18$\pm$0.19 L/min/M2. We conclude that we should continue anatomic correction for the complex congenital heart anomalies with the malposition of the great arteries because myocardial function seems to be well preserved, though we are still on the learning curve.
흉부 외상은 단순 부정맥에서 심근 파열 등 심장에 다양한 합병증을 일으킬 수 있다. 그중 관상동맥 손상은 매우 드문 합병증이고 특히 좌측 관상 동맥의 동맥류는 우측에 비해서 발생빈도가 작은 것으로 되어있다. 흉부 둔상 후 관상 동맥 내벽의 동맥류 형성, 박리, 열상, 동정맥루 및 혈전 등이 매우 드물게 생길 수 있으며, 혈관이 막히고 심근 경색이 발생하면 환자에게 치명적인 손상을 일으킨다. 교통사고로 인한 흉부의 둔상 후 발생한 심근경색증을 진단받은 33세 남자에서 관상동맥 우회로술을 통해 좋은 결과를 얻었기에 보고하는 바이다.
Purpose: Kawasaki disease (KD) is an immune-related multisystemic vasculitis that occurs in children, especially ensuing from a coronary artery abnormality. Sodium level is known to be related to vascular injury, which could affect the progress of KD. The purpose of this study was to determine the serum sodium levels that could predict the occurrence of cardiac and coronary artery events in KD. Methods: We conducted a retrospective review of medical records for 104 patients with KD from January 2015 to December 2015. Patients with serum Na levels of <135 mEq/L at the time of initial diagnosis were assigned to the hyponatremia group. Laboratory findings and echocardiographic data were analyzed for various aspects. Results: Among the 104 patients with KD, 91 were included in the study, of whom 48 (52.7%) had hyponatremia. The degree of fever, white blood cell count, percentage of neutrophils, percentage of lymphocytes, total bilirubin level, brain natriuretic peptide level, erythrocyte sedimentation rate, and C-reactive protein level were higher in the patients with hyponatremia. They also demonstrated a trend of larger coronary artery diameters based on Z scores. Conclusion: The severity of vascular inflammation in acute KD with hyponatremia might worsen the prognosis of coronary vasculature. Although no statistically significant correlation was found between the initial serum sodium levels and coronary arteriopathy in the patients with KD in this study, a long-term follow-up study with a larger number of enrolled patients should be designed in the future to elucidate the relationship between serum sodium level and coronary arteriopathy in patients with KD.
Background: As the internal mammary artery is far superior to the vein in the patency rate recently there has been a tendency to use the arterial graft as much as possible in coronary artery bypass grafts with the expectation of better the short- and long-term patency rate. Material and Method: We sequentially grafted the diagonal and the left anterior descending artery significantly influencing the cardiac function with the internal mammary artery. There were 32 cases of sequential grafts from July 1993 to December 1998: 21 men and 11 women. The age range was from 43 to 69 years with a mean age of 56.64$\pm$6.41 years. There were 22 unstable angina 7 stable angina and 3 acute myocardial infarction. 8 cases of them were accompanied by stenosis of the left main coronary artery. The grafts for coronary artery bypass surgery included the great saphenous vein at 60 the right gastroepiploci artery at 5 and the left internal mammary artery at 64 coronary arteries. Result: One patient died from sepsis and multiorgan failure. Complications included wound infections in two cases and gastrointestinal bleeding in one patient. All patients showed decrease or disappearance of angina after operation. The postoperative coronary angiogram performed in 9 patients showed neither occlusion nor stenosis of the grafts. Conclusion: This study suggests that sequential anastomosis of the internal mammary artery to the diagonal and the left anterior descending artery may result in excellent short-term patency diagonal and the left anterior descending artery may result in excellent short-term patency rate and be useful for the coronary artery bypass graft using only arterial grafts
