CT characteristics of normal canine pulmonary arteries and evaluation of optimal contrast delivery methods in CT pulmonary angiography

  • This study was performed to identify the normal anatomic orientation of pulmonary arteries and to obtain the normal baseline parameters and the optimal contrast material delivery methods of computed tomographic pulmonary angiography (CTPA) on normal beagle dogs. Based on the contrast injection flow rate, the contrast volume, and the administration methods, the experimental groups were divided into 4 groups such as group 1 : 2 ml/s, 3 ml/kg, and monophasic administration; group 2 : 5 ml/s, 3 ml/kg, and monophasic administration; group 3 : 5 ml/s, 4 ml/kg, and monophasic administration; group 4 : 5 ml/s and 2 ml/kg in first phase, 0.3 ml/s and 2 ml/kg in second phase, as biphasic administration. Normal anatomic orientation of pulmonary arteries in CTPA was evaluated through reformatted and 3D images after retro-reconstruction. Normal parameters for great arteries and peripheral pulmonary arteries were obtained on the factor of basement hounsfield unit (HU) values, contrast enhanced HU values, delay time, and peak time. And the optimal contrast delivery methods were evaluated on the factor of contrast enhanced HU values, image quality, and artifact. The monophasic administration with 5 ml/s contrast injection flow rate and 3 ml/kg contrast volume was optimal in canine CTPA.

네거티브 경구 조영제를 이용한 PET/CT 촬영시 나타난 종양성 섭취와 유사한 생리적 장관 섭취 (Colon Cancer Mimicking Physiologic FDG Uptake : with Using of Negative Oral Contrast)

  • A 64-year-old female with glioblastoma multiforme (GBM) was assigned to our department for whole body PET/CT scan. She ingested 1 liter of pure water as negative oral contrast just before PET/CT examination. FDG-PET/CT images showed a very intense hypermetabolic, focal lesion in the abdominal cavity around descending colon. The SUVmax of the lesion was 17.2. But there was no abnormal lesion corresponded to the area of PET scan in the combined contrast enhanced CT scan. We suggested considering a malignant lesion due to very intense glycolytic activity. Conventional abdominal CT scan & colonoscopy were accomplished within one week after PET/CT evaluation. There was no abnormality in both examinations. We executed follow-up PET/CT evaluation after 1 month and couldn't find any abnormality around the corresponding area. So we concluded the hypermetabolism was colonic physiologic uptake. A colonic physiologic uptake is a well known cause of false positive finding. Nuclear physicians should be considered the possibility of malignancy when interpret focal colonic uptake, especially incidental finding. There are a few reports that using of negative oral contrast is able to reduce gastrointestinal physiologic uptakes. But as we can see in this case, although we used negative oral contrast, intense physiologic uptake is detected and maxSUV is able to up to 17.2. So, it is important to keep a fact in mind. Even though there is a colonic physiologic uptake in PET/CT image, it may be able to show very intense hypermetabolism regardless of using negative oral contrast.

Dynamic Contrast-Enhanced Ultrasound of Gastric Cancer: Correlation with Perfusion CT and Histopathology

  • Objective: To assess the relationship between contrast-enhanced ultrasound (CEUS) parameters and perfusion CT (PCT) parameters of gastric cancers and their correlation with histologic features. Materials and Methods: This prospective study was approved by our Institutional Review Board. We included 43 patients with pathologically-proven gastric cancers undergoing CEUS using SonoVue® (Bracco) and PCT on the same day. Correlation between the CEUS parameters (peak intensity [PI], area under the curve [AUC], rise time [RT] from 10% to 90% of PI, time to peak [TTPUS], and mean transit time [MTTUS]) and PCT parameters (blood flow, blood volume, TTPCT, MTTCT, and permeability surface product) of gastric cancers were analyzed using Spearman's rank correlation test. In cases of surgical resection, the CEUS and PCT parameters were compared according to histologic features using Mann-Whitney test. Results: CEUS studies were of diagnostic quality in 88.4% (38/43) of patients. Among the CEUS parameters of gastric cancers, RT and TTPUS showed significant positive correlations with TTPCT (rho = 0.327 and 0.374, p = 0.045 and 0.021, respectively); PI and AUC were significantly higher in well-differentiated or moderately-differentiated tumors (n = 4) than poorly-differentiated tumors (n = 18) (p = 0.026 and 0.033, respectively), whereas MTTCT showed significant differences according to histologic types (poorly cohesive carcinoma [PCC] vs. non-PCC), T-staging (≤ T2 vs. ≥ T3), N-staging (N0 vs. N-positive), and epidermal growth factor receptor expression (≤ faint vs. ≥ moderate staining) (p values < 0.05). Conclusion: In patients with gastric cancers, CEUS is technically feasible for the quantification of tumor perfusion and may provide correlative and complementary information to that of PCT, which may allow prediction of histologic features.

Virtual Non-Contrast Computer Tomography (CT) with Spectral CT as an Alternative to Conventional Unenhanced CT in the Assessment of Gastric Cancer

  • Objective: The purpose of this study was to evaluate computed tomography (CT) virtual non-contrast (VNC) spectral imaging for gastric carcinoma. Materials and Methods: Fifty-two patients with histologically proven gastric carcinomas underwent gemstone spectral imaging (GSI) including non-contrast and contrast-enhanced hepatic arterial, portal venous, and equilibrium phase acquisitions prior to surgery. VNC arterial phase (VNCa), VNC venous phase (VNCv), and VNC equilibrium phase (VNCe) images were obtained by subtracting iodine from iodine/water images. Images were analyzed with respect to image quality, gastric carcinoma-intragastric water contrast-to-noise ratio (CNR), gastric carcinoma-perigastric fat CNR, serosal invasion, and enlarged lymph nodes around the lesions. Results: Carcinoma-water CNR values were significantly higher in VNCa, VNCv, and VNCe images than in normal CT images (2.72, 2.60, 2.61, respectively, vs 2.35, $p{\leq}0.008$). Carcinoma-perigastric fat CNR values were significantly lower in VNCa, VNCv, and VNCe images than in normal CT images (7.63, 7.49, 7.32, respectively, vs 8.48, p< 0.001). There were no significant differences of carcinoma-water CNR and carcinoma-perigastric fat CNR among VNCa, VNCv, and VNCe images. There was no difference in the determination of invasion or enlarged lymph nodes between normal CT and VNCa images. Conclusions: VNC arterial phase images may be a surrogate for conventional non-contrast CT images in gastric carcinoma evaluation.

Automatic Liver Segmentation of a Contrast Enhanced CT Image Using an Improved Partial Histogram Threshold Algorithm

  • This paper proposes an automatic liver segmentation method using improved partial histogram threshold (PHT) algorithms. This method removes neighboring abdominal organs regardless of random pixel variation of contrast enhanced CT images. Adaptive multi-modal threshold is first performed to extract a region of interest (ROI). A left PHT (LPHT) algorithm is processed to remove the pancreas, spleen, and left kidney. Then a right PHT (RPHT) algorithm is performed for eliminating the right kidney from the ROI. Finally, binary morphological filtering is processed for removing of unnecessary objects and smoothing of the ROI boundary. Ten CT slices of six patients (60 slices) were selected to evaluate the proposed method. As evaluation measures, an average normalized area and area error rate were used. From the experimental results, the proposed automatic liver segmentation method has strong similarity performance as the MSM by medical Doctor.

Remnant parietal serosa detection in a cat with true diaphragmatic hernia using computed tomography

  • A 4-year-old cat was referred for a suspected pulmonary mass. True diaphragmatic hernia presence was diagnosed via computed tomography (CT). There was a thin membrane covering the diaphragmatic defect. The membrane was thinner than the diaphragm. After contrast injection, the membrane was less enhanced than that of the normal diaphragm. The membrane was identified as a remnant of the parietal pleura. In addition, contrast-enhanced CT images provided clarity in viewing the herniated liver and falciform fat. A thinner membrane, covering the diaphragmatic defect, and attached to the thicker normal diaphragm, is considered a unique CT feature of true diaphragmatic hernia.

진단용 고선량 CT를 이용한 $^{18}F$-FDG PET/CT 촬영시 정맥 조영제가 SUV 값에 미치는 영향 (The Effect of Intravenous Contrast on SUV Value in $^{18}F$-FDG PET/CT using Diagnostic High Energy CT)

  • 목적 : 현재 PET/CT에서 CT의 역할은 단순히 감쇠 보정만을 위한 도구에서 진단을 위한 도구로 그 역할이 발전해 가고 있다. 따라서 양질의 CT 영상을 얻기 위한 정맥 조영제의 사용과 고선량의 CT 촬영이 요구되고 있으며 최근 조영제가 감쇠 보정한 PET 영상에 크게 영향을 미치지 않는다는 보고가 나오고 있다. 이에 본 연구에서는 고선량 CT에서 정맥 조영제의 사용이 감쇠 보정에 미치는 영향을 알아보았다. 대상 및 방법 : 총 13명의 환자에게 PET/CT 검사를 시행하였다. $^{18}F$-FDG를 주사하고 1시간 후에 촬영하였고, 첫 번째로 조영제의 사용 없이 CT(NECT-120 kVp, 130 mAs)를 촬영하고 이어서 바로 조영제를 사용하여 CT(CECT)를 촬영한 후 PET을 시행하였다. 각 환자마다 NECT와 CECT에 의해 각각 보정된 PET 영상에서 10곳의 신체 부위(병변이 없는 폐첨부, 폐기저부, 상행대동맥, 간의 상부 및 하부, 비장, 척추, 대퇴골두, 장요근과 10번 척추와 척추 인접 근육) 에서 HU와 maxSUV 값을 구하였고, 종양과 임파선 병변의 값도 함께 측정하였다. 결과: 조영제를 사용하였을 때 국소적으로 섭취가 증가된 곳은 관찰되지 않았다. 총 130개의 정상 조직의 maxSUV값을 측정하여 비교하였을 때, 조영제를 사용한 군과 사용하지 않은 군의 평균 maxSUV 값은 각각 $1.1{\pm}0.5,\;1.0{\pm}0.5$이고 두 군 사이에 p<0.001로 통계적으로 유의한 차이를 보였다. 정상 조직에 대해 Bland-Altman 분석을 시행하였을 때 일치의 한계 범위는 $0.1{\pm}0.3$ 이었다. 10개의 대상 영역 중 폐첨부, 폐하부, 상행대동맥, 간의 상부와 하부, 비장에서 maxSUV 값이 두군에서 유의한 차이(p<0.05)가 있었다. 총 39개 병변의 maxSUV 값을 측정하여 비교하였을 때, 조영제를 사용한 군과 사용하지 않은 군의 평균 maxSUV값은 각각 $4.7{\pm}2.0,\;4.4{\pm}2.0$이고 두 군 사이에 p<0.001로 통계적으로 유의한 차이를 보였다. 병변에 대해 Bland-Altman 분석을 시행하였을 때 일치의 한계 범위는 $0.4{\pm}0.8$ 이었다. 결론: 조영제로 인해 maxSUV의 값은 증가되었으나 정상 조직과 병변에서 일치의 한계 범위가 매우 좁았다. 따라서 진단용 고선량 CT를 이용한 PET/CT 촬영 시에 조영제의 사용이 임상 상황에서 판독에 큰 영향을 미치지 않는 것으로 생각된다.

Emerging Role of Hepatobiliary Magnetic Resonance Contrast Media and Contrast-Enhanced Ultrasound for Noninvasive Diagnosis of Hepatocellular Carcinoma: Emphasis on Recent Updates in Major Guidelines

  • Hepatocellular carcinoma (HCC) can be noninvasively diagnosed on the basis of its characteristic imaging findings of arterial phase enhancement and portal/delayed "washout" on computed tomography (CT) and magnetic resonance imaging (MRI) in cirrhotic patients. However, different specific diagnostic criteria have been proposed by several countries and major academic societies. In 2018, major guideline updates were proposed by the Association for the Study of Liver Diseases, European Association for the Study of the Liver (EASL), Korean Liver Cancer Association and National Cancer Center (KLCA-NCC) of Korea. In addition to dynamic CT and MRI using extracellular contrast media, these new guidelines now include magnetic resonance imaging (MRI) using hepatobiliary contrast media as the first-line diagnostic test, while the KLCA-NCC and EASL guidelines also include contrast-enhanced ultrasound (CEUS) as the second-line diagnostic test. Therefore, hepatobiliary MR contrast media and CEUS will be increasingly used for the noninvasive diagnosis and staging of HCC. In this review, we discuss the emerging role of hepatobiliary phase MRI and CEUS for the diagnosis of HCC and also review the changes in the HCC diagnostic criteria in major guidelines, including the KLCA-NCC practice guidelines version 2018. In addition, we aimed to pay particular attention to some remaining issues in the noninvasive diagnosis of HCC.

Diagnostic Value of Computed Tomography in Crohn's Disease Patients Presenting with Acute Severe Lower Gastrointestinal Bleeding

  • Objective: To investigate the diagnostic yield of contrast-enhanced computed tomography (CT) in Crohn's disease (CD) patients presenting with acute severe lower gastrointestinal bleeding (LGIB), and the role of CT in predicting the risk of rebleeding. Materials and Methods: A consecutive series of 110 CD patients presenting with acute severe LGIB between 2005 and 2016 were analyzed. Among them, 86 patients who had undergone contrast-enhanced CT constituted the study cohort. The diagnostic yield of CT for detecting contrast extravasation was obtained for the entire cohort and compared between different CT techniques. In a subgroup of 62 patients who had undergone CT enterography (CTE) and showed a negative result for extravasation on CTE, the association between various clinical and CTE parameters and the risk of rebleeding during subsequent follow-up was investigated using Cox regression analysis. Results: The diagnostic yield of CT was 10.5% (9 of 86 patients). The yield did not significantly differ between single-phase and multiphase examinations (p > 0.999), or between non-enterographic CT and CTE (p = 0.388). Extensive CD (adjusted hazard ratio [HR], 3.27; 95% confidence interval [CI], 1.09-9.80; p = 0.034) and bowel wall-to-artery enhancement ratio (adjusted HR, 2.81; 95% CI, 1.21-6.54; p = 0.016) were significantly independently associated with increased rebleeding risks, whereas anti-tumor necrosis factor-${\alpha}$ therapy after the bleeding independently decreased the risk of rebleeding (adjusted HR, 0.26; 95% CI, 0.07-0.95; p = 0.041). Conclusion: The diagnostic yield of contrast-enhanced CT was not high in CD patients presenting with acute severe LGIB. Nevertheless, even a negative CTE may be beneficial as it can help predict the risk of later rebleeding.

조영증강 전산화단층촬영검사에서 보이지 않는 이하선의 다형선종 (Invisible Parotid Gland Pleomorphic Adenoma on Contrast Enhanced-Computed Tomography)

  • Pleomorphic adenomas is the most common tumor of parotid gland and usually located and confined in superficial lobe of parotid gland. Computed tomography (CT) is commonly used to initially evaluate salivary gland lesion, but contrast-enhanced CT may sometimes fail to reveal lesions in spite of a high clinical suspicion. For this reason, ultrasonography (US) can be used as the first-line image work-up in some cases of parotid gland benign tumors. We experienced a case of a 60-year-old woman without underlying disease presenting a palpable parotid mass in which the initial CT examination was reported as 'no obvious mass detected'. However, the lesion was revealed in US and histologically confirmed as pleomorphic adenoma. The patient underwent superficial parotidectomy through modified facelift incision. To the best of our knowledge, this is the first report of invisible major salivary gland tumor on CT in the Korean literature.
