Extravasation Injury of Contrast Media in the Neck and Thorax During MDCT Scanning with 3D Image Reformation Findings (CT검사에서 조영제의 혈관외유출에 의한 목 및 흉부 손상의 3차원 재구성 영상)

  • Contrast media may cause tissue injury by extravasation during intravenous automated injection during CT examination. Here, we present a study in which contrast media extravasation was detected and localized in the neck and thorax by three-dimensional(3D) CT data reformation. The CT studies of the extavasation site were performed using a 3D software program with four different display techniques axial, multi planar reformation(MPR), maximum intensity projection(MIP), and volume rendering displays are currently available for reconstructing MDCT data. 3D image reconstructions provide accurate views of high-resolution imaging. This paper introduces extravasation with the MDCT and 3D reformation findings of contrast media extravasation in neck ant thorax. The followed injection of the external jugular vein into an existing intravenous catheter and a large volume of extravasation was demonstrated on by 3D MDCT.

Spinal Lymphoma in a Cat (고양이에서 척수림프종의 발생례)

  • A 3.5kg, 3-year-old castrated male Japanese domestic cat withpelvic limb ataxia and dysuria was referred to the Veterinary Medical Center of the Tokyo University. On the neurologic examination findings, both pelivic had a LMN paresis. The cat was FeLV positive and FIV negative. Radiographic findings did not identify the spinal lesions. In magnetic resonance images(MRI) of the lumbar spinal cord, the cat had the lesions in the lumbar(L) 1 and L3, characterized by hyperintensity on a transverse T2-weighted and T1-weighted images, and contrast enhancement was evident. The mass removed by dorsal laminectomy. Histopathological examination of the mass revealed spinal lymphoma. Three weeks after the surgery, the cat administered chemotherapy protocol for lymphoma by current protocol. Two weeks after chemotherapy, the cat had a metastasis to bone marrow and died.

Understanding of Nutrition Labelling Use and Related Factors among Korean Adults

  • This study was conducted to investigate that the nutrition labeling use is associated with demographic and psychosocial factors according to each nutrition information on the nutrition labeling in Korean adults. The study subjects (N=1,140) were individuals who were aged 20 years and more and answered on the question of nutrition label use and who participated in the Korean National Health Examination and Nutrition Survey (KNHANES) in 2010.As age older, there was more interest in information such as sugar, protein, fat, cholesterol than calories. In contrast, as age younger, there was more interest intrans-fat, sodium as well as calories. As higher education level, there were more aware of trans-fat, sodium and calories. From the result that the most interested nutrition items were significantly different by democratic factors, we could understand interested nutrient information on the nutrition labels could change according to individual specific education. Therefore, this can also provide basic data for systematic education program by nutrition label use.

  • Chest radiograms obtained by using Fuji Computed Radiography(FCR) system were compared to conventional film/screen radiograms. The FCR images showed better image quality in diagnostic informations than the conventional chest images. In FCR, the radiation exposure to patient for chest examination could be reduced up to one tenths of conventional chest examination. The main advantages or FCR were considered to depend on the contrast processing and frequency processing properties. The use of FCR in clinical work may improve both diagnostic quality and radiation exposure.

A Case of Ectopic Thyroid with Clinical Evaluation of Fifteen Cases (이소성 갑상선 1례 및 국내 증례의 임상적 고찰)

  • The authors recently experienced a case of ectopic thyroid. A 15-year-old female patient visited to ENT department with the complaint of the submental neck mass. On physical examination, the mass was relatively firm, non tender and 3$\times$2cm in size. Oral cavity examination revealed 0.5$\times$0.5cm sized pink colored mass near the foramen cecum area. Suspecting ectopic thyroid, thyroid function test, thyroid scan, neck computed tomogram scan were performed. Thyroid scan revealed a functioning thyroid on the lingual and submental area without normal uptake in the anterior neck area. Thyroid (unction test was normal. Pre-contrast computed tomogram scan revealed an ectopic thyroid in the lingual and submental area. A review of literature concerning ectopic thyroid was discussed.

  • Although small bowel the mainly occupies the most part of the gastrointestinal tract, small intestine tumors are rare, insidious in clinical presentation, and frequently represent a diagnostic and management challenge. Small bowel tumors are generally classified as epithelial, mesenchymal, lymphoproliferative, or metastatic. Familial adenomatous polyposis and Peutz-Jeghers syndrome are the most common inherited intestinal polyposis syndromes. Until the advent of capsule endoscopy (CE) and device-assisted enteroscopy (DAE) coupled with the advances in radiology, physicians had limited diagnostic examination for small bowel examination. CE and new radiologic imaging techniques have made it easier to detect small bowel tumors. DAE allows more diagnosis and deeper reach in small intestine. CT enteroclysis/CT enterography (CTE) provides information about adjacent organs as well as pictures of the intestinal lumen side. Compared to CTE, Magnetic resonance enteroclysis/enterography provides the advantage of soft tissue contrast and multiplane imaging without radiation exposure. Treatment and prognosis are tailored to each histological subtype of tumors.

The Effectiveness of CT and MRI Contrast Agent for SUV in 18F-FDG PET/CT Scanning (18F-FDG PET/CT 검사에서 정량분석에 관한 CT와 MRI 조영제의 효과)

  • In this study, among various factors having influence on SUV, we intended to compare and analyze the change of SUV using CT(4 type) and MRI(3 type) contrast agents which are commercialized now. We used Discovery 690 PET/CT(GE) and NEMA NU2 - 1994 PET phantom as experimental equipment. We have conducted a study as follows; first, we filled distilled water to phantom about two-thirds and injected radioisotope(18F-FDG 37 MBq), contrast agent. Second, we mixed CT contrast agent with distilled water and MRI contrast agent with that water separately. And then, we stirred the fluid and filled distilled water fully not to make air bubble. In emission scan, we had 15minutes scanning time after 40 minutes mixing contrast agent with distilled water. In transmission scan, we used CT scanning and its measurement conditions were tube voltage 120 kVp, tube current 40 mA, rotation time 0.5 sec, slice thickness 3.27 mm, DFOV 30 cm. Analyzing results, we set up some ROIs in 10th, 15th, 20th, 25th, 30th slice and measured SUVmean, SUVmax. Consequently, all images mixed 3 types of MRI contrast agent with distilled water have high SUVmean as compared with pure FDG image but there was no statistical significance. In SUVmax, they have high score and there was statistical significance. And other 4 images mixed 4 types of CT contrast agent with distilled water have significance in both SUVmean and SUVmax. Attenuation correction in PET/CT has been executed through various methods to make high quality image. But we figured out that using CT and MRI contrast agents before PET/CT scanning could make distortion of image and decrease diagnostic value. In that reason, we have to sort out the priority of examination in hospital not to disturb other examination's results. Through this process, we will be able to give superior medical service to our customers.

Ultrasonographic Evaluation of Surgically Induced Cranial Cruciate Ligament Rupture in the Dog (개에서 전십자 인대 단열의 초음파적 평가)

  • Ultrasonographic examination of the stifle joint four clinically and radiographically normal adult dogs was performed before and after inducing the cranial cruciate ligament (CrCL) rupture of the bilateral stifle surgically. At Pre- or post-surgery, the hyperechoic patella ligament and the echogenic infrapatella fat interfered the visualization of the CrCL by the plain scanning method. However when the stifle joint was imaged by the scanning method injecting normal saline within the joint capsule, the hyperechogenic ligament was visualized by the separation of the infrapatella fat and the CrCL and the contrast effect of anechoic saline. When the stifle joint was imaged by the real time scanning method after the induction of the surgical rupture, fluttering of the ligament and anechoic area between the bone and the CrCL was identified due to the lack of tonicity and continuity of the ligament. Enlargement of the ligament because of its increased diameter as well as fluttering of the CrCL and the increased joint space's thickness was visualized. Consequently, confirming fluttering of the CrCL, increased ligament diameter and anechoic region between the CrCL and the bone, ultrasonographic examination is a valuable diagnostic tool as well as cranial drawer test and radiographic examination.

Long-Term Chemotherapy with Lomustine of Intracranial Meningioma in a Dog (개에서 발생한 두개내 수막종에 대한 로무스틴 치료 증례)

  • A 13-year-old, neutered male, chihuahua was referred to the Veterinary Medical Teaching Hospital of Chungnam National University with a history of a bilateral mandibular mass, intermittent vomiting, cough, and left sided staggering starting ten days prior. Clinical examination along with radiography, neurologic examination and magnetic resonance imaging (MRI) demonstrated a well-circumscribed and solitary mass in the brain stem. The dura mater was thickened with marked linear enhancement after contrast administration. Based on diagnostic image analysis and histopathologic examination, this case was diagnosed as intracranial meningioma. The patient's symptoms were controlled for 18 months by a combination therapy of prednisolone and lomustine (CCNU), and then he died of at home.

Walled-off Pancreatic necrosis in a Dog

  • A 7-year-old, castrated, male Maltese dog presented with hyporexia and depression for 3 days. Elevated serum amylase, lipase activities, and liver enzyme values were found upon blood examination. An abdominal mass was seen on radiographs caudal to the gastric body in the left middle abdomen. In the left middle abdomen, abdominal ultrasonography also revealed a massive, irregularly marginated, heterogeneous mass of unknown origin, and in the right cranial abdomen, heterogeneously hypoechoic pancreatic tissue and hyperechoic change of adjacent mesenteric fat were observed. Contrast-enhanced computed tomography showed an irregular contour of the left pancreatic limb as well as heterogeneously enhanced parenchyma. A low-attenuating peripancreatic fluid collection with a thin and irregular wall was also seen. Based on these findings, an atypical pancreatic abscess with necrotizing pancreatitis which manifested as walled-off necrosis was suspected. The mass was excised, and the pancreatic abscess was confirmed by histopathologic examination. No complications were found in the patient after two months of follow-up examination.
