The Effect of PNF Pattern for Upper Extrimity on the Multifiudus (PNF 상지 운동이 다열근에 미치는 영향)

  • Purpose : The purpose of this study is to define the change of lumbar asymmetry multifidus muscles when upper extremity PNF pattern. Methods : For this research, twenty-nine asymptomatic participants were voluntary attended. Under the identical conditions, Multifidus muscles were measured standing position and on L4,L5 vertebral level by MyLabOne (ESAOTE, Netherland with a 3.5MHz covex array transducer). The upper extrimity PNF pattern used to activate the multifidus ipsilateral and cotralateral. Results : Results of analysis showed that at the L4, L5 vertebral level, healthy asymtomativ subjects had asymmetry multifidus muscles size. The depth of the multifidus muscles were significantly increased at contralateral upper extrimity PNF pattern, but no significant differences were comparison ipsilateral with contralateral. Conclusion : The multifidus muscle in asymptomatic subjects clinical significance asymmetry. Contralateral upper extrimity PNF pattern seems to be the most suitable exercise for strenthen the smaller size of the multifidus. This study will be used as a prevention method of LBP.

Contralateral Breast Dose Reduction Using a Virtual Wedge (가상쐐기를 이용한 반대측 유방선량감소)

  • Purpose: To evaluate the contralateral breast dose using a virtual wedge compared with that using a Physical wedge and an open beam in a Siemens linear accelerator. Materials and Methods: The contralateral breast dose was measured using diodes placed on a humanoid phantom. Diodes were placed at 5.5 cm (position 1), 9.5 cm (position 2), and 14 cm (position 3) along the medial-lateral line from the medial edge of the treatment field. A 6-MV photon beam was used with tangential irradiation technique at 50 and 230 degrees of gantry angle. Asymmetrically collimated $17{\times}10cm$ field was used. for the first set of experiment, four treatment set-ups were used, which were an open medial beam with a 30-degree wedged lateral beam (physical and virtual wedges, respectively) and a 15-degree wedged medial beam with a 15-degree wedged lateral beam (physical and virtual wedges, respectively). The second set of experiment consists of setting with medial beam without wedge, a 15-degree wedge, and a 50-degree wedge (physical and virtual wedges, respectively). Identical monitor units were delivered. Each set of experiment was repeated for three times. Results: In the first set of experiment, the contralateral breast dose was the highest at the position 1 and decreased in order of the position 2 and 3. The contralateral breast dose was reduced with open beam on the medial side ($2.70{\pm}1.46%$) compared to medial beam with a wedge (both physical and virtual) ($3.25{\pm}1.59%$). The differences were larger with a physical wedge ($0.99{\pm}0.18%$) than a virtual wedge ($0.10{\pm}0.01%$) at all positions. The use of a virtual wedge reduced the contralateral breast dose by 0.12% to 1.20% of the proscribed dose compared to a physical wedge with same technique. In the second experiment, the contralateral breast dose decreased in order of the open beam, the virtual wedge, and the physical wedge at the position 1, and it decreased in order of a physical wedge, an open beam, and a virtual wedge at the position 2 and 3. Conclusion: The virtual wedge equipped in a Siemens linear accelerator was found to be useful in reducing dose to the contralateral breast. Our additional finding was that the surface dose distribution from the Siemens accelerator was different from a Varian accelerator.

Electromyographic Activity, Firing Time and Sequence of the Anterior Temporalis and the Masseter on Chewing Stroke (저작운동시 전측두근과 교근의 근활성 및 활성시기에 관한 연구)

  • This study was performed to investigate the electromyographic(EMG) activity, firing time and sequence of the mandibular elevator muscle on gum chewing. For this study, 28 patients with temporomandibular disorders(TMD), especially internal derangement of TM joint, and 16 dental students without any signs and symptoms in the masticatory system were selected as the patients group and as the normal group, respectively. The patients group was composed of 14 right and 14 left side affected patients. For recording of EMG activity(${\mu}V$) of the anterior temporalis(TA) and the masseter muscle(MM), and measuring of firing time(millisecond) from the start of mandibular opening movement to the firing of the muscles, BioEMG, BioEGN, and Combo program integrated in the Biopak system(Bioresearch Inc., Milwaukee, USA) were used. Gum chewing stroke was performed in both right and left side for several times, and the first and the second chewing strokes were analysed and compared with regard to EMG activity, firing time, firing sequence, correlation between EMG activity and firing time. The data obtained were analysed by SPSS windows program and the results of this study were as follows : 1. In the patients group, chewing on the affected side showed higher EMG activities in the masseter of the side than those of the contralateral side, but chewing on the unaffected side showed higher EMG activity in the masseter and in the anterior temporalis of the side than those of the contralateral side. 2. There were no difference of firing time between both sides on chewing on the affected side, but firing time in the chewing side were earlier than that of contralateral side on chewing on the unaffected side in patients group. 3. In the normal group, EMG activities in the masseter and in the anterior temporalis in the chewing side were always higher than those of the contralateral side, and there were no difference of firing time between the two sides. 4. In general, firing sequence of four muscles of both sides were ipsilateral TA, ipsilateral MM, contralateral TA, and contralateral MM in earlier order of time in both groups. Correlation coefficients between EMG activity and firing time were negative value, and more significant correlation were appeared in the normal group than in the patients group.

Surgical Approaches to Paraclinoid Aneurysms (Paraclinoid 동맥류의 수술적 접근법)

  • Objective : "Paraclinoid" aneurysms include those aneurysms arising from the internal carotid artery between the site of emergence of the carotid artery from the roof of the cavernous sinus and the origin of the posterior communicating artery. The authors reviewed and analysed the results of surgical approaches to paraclinoid aneurysms treated with transcranial surgery and endovascular surgery. Methods : Between January 1998 and May 1999, 14 patients were treated surgically through ipsilateral and contralateral pterional approaches, and anterior interhemispheric approach, and endovascular surgery for paraclinoid aneurysms. All transcranial approaches were performed by same surgeon. The medical records, neuroimaging studies and videotapes which had been recorded operations were reviewed retrospectively. Results : Twelve patients presented with subarachnoid hemorrhage and ICH. Nine of fourteen patients had multiple aneurysms. Thirteen cases were small and one was a large aneurysm. Six patients were treated through ipsilateral approaches, six contralateral pterional approaches, one anterior interhemispheric approach and one primarily by GDC embolization. All aneurysms treated through contralateral approaches were multiple aneurysms. Neck clipping was performed in 9(69.2%) of the thirteen aneurysms, wrapping in four cases, among them three cases were followed by GDC embolization. The surgical outcomes were : Glasgow Outcome Scale(GOS) I 71.4%, GOS II 21.4% and GOS V 7.1%. Conclusion : The surgical approaches to paraclinoid aneurysms should be chosen after careful anatomical evaluation of aneurysm and its neighboring structures. 3D-CT angiography and/or the raw data of MR angiography were useful. This study supports the usefulness of the contralateral approach to paraclinoid aneurysm associated with multiple aneurysms, unruptured and small aneurysms whose dome projecting medially, superiorly and dorsally. The determination of contralateral approach to small and medially projecting paraclinoid aneurysm may be stressful to operator, thus we believe anterior interhemispheric approach is better alternated. Also we recommend the endovascular surgery after reinforcement of aneurym neck and dome in the case with difficulty in clipping.

CONTRALATERAL NECK LYMPH NODE METASTASIS OF EARLY TONGUE CANCER : A CASE REPORT (반대측 경부 림프절로 전이된 초기 설암의 치험례)

  • Likely to be the most common oral cancer, squamous cell carcinoma(SCC) of the tongue accounts for about 20% of all oral and pharyngeal cancers. SCC of the tongue frequently arises in the lateral border, and if it metastasize, it occurs on submandibular gland and neck lymph nodes. Location of the primary lesions and neck lymph node metastasis affect the prognosis and decrease survival rate of patients with carcinoma of the tongue. The authors experienced the patient with contralateral neck lymph node metastasis of SCC of the tongue. The patient came to our department with chief complaint of elevated lesion on left lateral border of the tongue. The mass was diagnosed as $T_2N_0M_0$, Stage II invasive SCC of oral tongue. Computed tomography(CT) & magnetic resonance imaging(MRI) which were taken before the operation showed no significant finding of metastasis. Surgical mass removal and preventive neck dissection on the left side were done. While follow up PET/CT, contralateral neck lymph node metastasis(right side, level II) was detected, and re-operation(Rt. side RND) was done. There are few studies concerning the contralateral neck lymph node metastasis related with SCC of the tongue. The purpose of this report is to introduce the uncommon case of contralateral neck lymph node metastasis occurred in the $T_2$-stage of SCC of the tongue treated by surgical resection.

Detection of Contralateral Breast Cancer Using Diffusion-Weighted Magnetic Resonance Imaging in Women with Newly Diagnosed Breast Cancer: Comparison with Combined Mammography and Whole-Breast Ultrasound

  • Objective: To compare the screening performance of diffusion-weighted (DW) MRI and combined mammography and ultrasound (US) in detecting clinically occult contralateral breast cancer in women with newly diagnosed breast cancer. Materials and Methods: Between January 2017 and July 2018, 1148 women (mean age ± standard deviation, 53.2 ± 10.8 years) with unilateral breast cancer and no clinical abnormalities in the contralateral breast underwent 3T MRI, digital mammography, and radiologist-performed whole-breast US. In this retrospective study, three radiologists independently and blindly reviewed all DW MR images (b = 1000 s/mm2 and apparent diffusion coefficient map) of the contralateral breast and assigned a Breast Imaging Reporting and Data System category. For combined mammography and US evaluation, prospectively assessed results were used. Using histopathology or 1-year follow-up as the reference standard, cancer detection rate and the patient percentage with cancers detected among all women recommended for tissue diagnosis (positive predictive value; PPV2) were compared. Results: Of the 30 cases of clinically occult contralateral cancers (13 invasive and 17 ductal carcinoma in situ [DCIS]), DW MRI detected 23 (76.7%) cases (11 invasive and 12 DCIS), whereas combined mammography and US detected 12 (40.0%, five invasive and seven DCIS) cases. All cancers detected by combined mammography and US, except two DCIS cases, were detected by DW MRI. The cancer detection rate of DW MRI (2.0%; 95% confidence interval [CI]: 1.3%, 3.0%) was higher than that of combined mammography and US (1.0%; 95% CI: 0.5%, 1.8%; p = 0.009). DW MRI showed higher PPV2 (42.1%; 95% CI: 26.3%, 59.2%) than combined mammography and US (18.5%; 95% CI: 9.9%, 30.0%; p = 0.001). Conclusion: In women with newly diagnosed breast cancer, DW MRI detected significantly more contralateral breast cancers with fewer biopsy recommendations than combined mammography and US.

The Risk Factors for Hydrocephalus and Subdural Hygroma after Decompressive Craniectomy in Head Injured Patients

  • Objective : The present study aims to investigate 1) the risk factors for hydrocephalus and subdural hygroma (SDG) occurring after decompressive craniectomy (DC), and 2) the association between the type of SDG and hydrocephalus. Methods : We retrospectively reviewed the clinical and radiological features of 92 patients who underwent DC procedures after severe head injuries. The risk factors for developing post-traumatic hydrocephalus (PTH) and SDG were analyzed. Types of SDGs were classified according to location and their relationship with hydrocephalus was investigated. Results : Ultimately, 26.09% (24/92) of these patients developed PTH. In the univariate analyses, hydrocephalus was statically associated with large bone flap diameter, large craniectomy area, bilateral craniectomy, intraventricular hemorrhage, contralateral or interhemisheric SDGs, and delayed cranioplasty. However, in the multivariate analysis, only large craniectomy area (adjusted OR=4.66; p=0.0239) and contralateral SDG (adjusted OR=6.62; p=0.0105) were significant independent risk factors for developing hydrocephalus after DC. The incidence of overall SDGs after DC was 55.43% (51/92). Subgroup analysis results were separated by SDG types. Statistically significant associations between hydrocephalus were found in multivariate analysis in the contralateral (adjusted OR=5.58; p=0.0074) and interhemispheric (adjusted OR=17.63; p=0.0113) types. Conclusion : For patients who are subjected to DC following severe head trauma, hydrocephalus is associated with a large craniectomy area and contralateral SDG. For SDGs after DC that occur on the interhemispherical or controlateral side of the craniectomy, careful follow-up monitoring for the potential progression into hydrocephalus is needed.

Renal mRNA Expression of Renin, $AT_1$ Receptor, TGF-${\beta}1$ and Fibronectin in Obstructive Nephropathy

  • The present study was designed to quantify the alterations of renal renin, angiotensin type I receptor ($AT_1$), $TGF-{\beta}1$, and fibronectin gene expression in rats with unilateral ureteral obstruction (UUO). We also investigated the change of $AT_1$ density during UUO. Reverse transcription-polymerase chain reaction (RT-PCR) technique and receptor binding assay were used to detect mRNA expression and receptor density, respectively. At one day after UUO, renin mRNA level of the obstructed kidneys was decreased transiently and then subsequently increased to the level of sham kidneys. In the contralateral kidneys of the same rats, on the contrary, renin mRNA level was gradually decreased. Then, at 9 days after UUO, it was significantly lower than that of sham kidneys. The expressions of both $AT_1$ subtypes, called $AT_{1A}$ and $AT_{1B}$, mRNAs did not change at any time. UUO led to a significant decrease in $AT_1$ density in the obstructed kidneys compared with the sham kidneys at 1 and 3 days $(66\;{\pm}\;11.6%\;(p<0.005)\;and\;73\;{\pm}\;4.0%$ (p<0.01), respectively). Thereafter, $AT_1$ density was gradually increased and at 9 days it showed a marked elevation in the obstructed kidneys compared to the sham kidneys. In contrast, in the contralateral kidneys $AT_1$ density was significantly reduced from 3 to 9 days after UUO. The $TGF-{\beta}$1 mRNA level of the obstructed kidneys was unexpectedly decreased at 6 days after UUO. Then, at 9 days it was followed by a significant increase in the obstructed kidneys, whereas it showed an obvious decrease in the contralateral kidneys. In addition, fibronectin mRNA level was also significantly increased in the obstructed kidneys after UUO compared to the sham or the contralateral kidneys of the same rats. These results suggest a differential regulation of renal renin, $AT_1$ receptor, $TGF-{\beta}$1 and fibronectin mRNA levels at different stages of UUO.

Effectiveness of Elective Irradiation Therapy on N0 Neck in Supraglottic Cancer (성문상부암에서 N0 경부에 대한 예방적 방사선치료의 효과)

  • Background and Objectives: Supraglottic cancer have a great tendency to spread cervical lymph nodes and lymph node metastasis is a well known prognostic factor. However the treatment for N0 neck in supraglottic cancer is still controversial. Materials and Methods: We retrospectively analyzed our neck management of supraglottic cancer patients who present with cN0 contralateral neck from 1989 through 1997. 36 patients were eligible for analysis. The primary site was surgically removed and the neck was managed by elective neck irradiation (ENI), elective neck dissection (END), or therapeutic neck dissection (TND) with postoperative radiation therapy (PORT). Results: Our results revealed that 18 of 36 patients have clinically negative neck, another 18 patients have clinically positive neck (N1-3). In clinically negative group, 12 of 18 patients were received ENI and there was 1 failure in contralateral neck area. Remaining 6 patients were received END with PORT and there was no failure. In clinically positive neck group, 3 of 18 patients were received ipsilateral TND and an additional contralateral END with PORT. Remaining 15 patients who were received TND with PORT, developed 3 neck failure. Conclusion: ENI or ipsilateral or bilateral END can be done in the cN0 neck of supraglottic cancer however ipsilateral TND and contralateral END with PORT is reasonable for the cN(+) neck.

Functional Relationship between the Vestibular Canals and the Extraocular Oblique Muscles (미노반규관(迷路半規管)과 외안사근(外眼斜筋)의 기능적(機能的) 관계(關係)에 관(關)하여)

  • This experiment was designed to explore specific functional relationship between the vestibular canals and the extraocular oblique muscles by observing the isometric tension responeses of the muscles to the selected vestibular canal excitation. The vestibular excitation was simulated by either stimulation of the individual canal nerve or endolymphatic fluid displacement in each canal. Each canal nerve was subjected to square wave pulses with a monopolar wire electrode placed closely to the ampullary nerve endings for electrical stimulation, and a fine stainless cannula was introduced into the each canal toward the ampulla and a minute amount $(0.5{\sim}3.5\;microliter)$ of fluid was injected in or ejected out by means of a microsyringe connected to the cannula to produce ampullopetal or ampullofugal displacement of endolymphatic fluid. The superior oblique muscle was contracted by the excitation of homolateral canals and was relaxed by contralateral canals. On the contrary, the inferior oblique was contracted by the contralateral canals and was relaxed by the homolateral canals. Summation of excitatory and inhibitory canal effects from the bilateral vestibular system was demonstrable on the tension changes of the oblique muscles. Excitation of either dual or triple canals of the unilateral vestibular system also caused summation effect on the tension response of the oblique pair; thus multiple signals from the different ampullary receptors seems to be converged into the relevant ocular motor muclei. Since the superior and inferior obliques are known to receive their motor fibers from the contralateral trochlear nuclei and intermediate nuclei of the homolateral oculomotor complex respectively, the above experimental evidences indicate that the ocular motor nuclei for oblique muscles receive excitatory signals from the contralateral vestibular canals and inhibitory signals from the homolateral canals.
