Background: Refractory empyemas with collapsed lung and persistent bronchopleural fistulas pose significant problems to thoracic surgeons and impose a substantial burden in terms of morbidity and mortality. The modified Eloesser flap procedure is a useful palliative option for clearing infections. Herein, we present our experiences with the modified Eloesser flap procedure in mixed suppurative lung pathologies with a new technique of irrigation for persistent infection. Methods: A retrospective review was carried out of 56 patients who underwent the modified Eloesser flap with continuous irrigation at Katurba Medical College. These patients had severe morbidities and were not suitable for major thoracic resection surgery, and electively underwent modified Eloesser flap surgery. Regular follow-up was done at 1, 3, 6, and 12 months. Patients with persistent infections were treated with our continuous irrigation technique. Results: The most important finding was that all patients with active sputum acid-fast bacilli-positive findings became sputum smear-negative during the first month of follow-up. Half (50%) of the patients had a patent stoma. Eleven patients had persistent infections, necessitating continuous irrigation. The infection was fully cleared after 1 month in 9 patients, while 2 patients required second irrigation and continued to receive follow-up. In the remaining 50% of the patients, the stoma closed completely, and the lung expanded fully. Conclusion: The modified Eloesser flap is a simple procedure. In suppurative pathologies, infections were well controlled and the general condition of the patients improved. Our continuous irrigation method showed promising results in patients with persistent purulent discharge.
수도에서 카드뮴의 흡수양상과 물관리 및 토양개량제 사용에 따른 흡수경감효과를 구명하기 위하여 카드뮴이 $0.01mg\;l^{-1}$로 함유된 물을 개량제인 유기물, 석회, 규산 및 인산을 처리한 모양에 상시담수 및 간단관수로 조절하여 포트 시험한 결과는 다음과 같다. 생육시기별 토양의 pH는 유수형성기에 가장 높았고 Eh는 생육이 진행될수록 낮았으며 상시담수구에서 낮게 나타났다. 생육중기의 벼 경엽 및 뿌리의 카드뮴함량은 간단관수보다 상시담수구에서 낮았고 상시담수구중 규산 및 석회시용구에서 흡수저감 효과가 크게 나타났다. 벼 생육기의 토양 pH는 경엽 및 현미의 카드뮴함량과 부의 상관, Eh는 정의 상관을 보였고 수확후 경엽과 현미중의 카드뮴함량은 간단관수보다 상시담수에서 낮았고 상시담수구중 석회 및 유기물 병용구에서 흡수 저감효과가 크게 나타났다. 시험후 토양의 pH는 3요소 및 인산시용구를 제외하고 pH6.0 이상이였고 카드뮴함량은 석회, 유기물, 규산 단독 및 병용처리구가 낮게 나타났다.

This project is designed with the aim to facilitate the farmer or gardener to engage in green house systems and to improve agricultural technology. In order to reduce continuous monitoring of the soil parameters, excess time consumption for the farmers and excessive usage of water, "Smart irrigation and temperature control for a greenhouse system" has been developed. There are two different ways to irrigate the land namely traditional irrigation methods and modern irrigation methods.
Purpose: Continuous irrigation method is an important step in managing wound infection. V.A.C. devices have been used in intractable wounds for reducing discharge, improving local blood flow, and promoting healthy granulation tissue. We expect synergistic effects of reduced infection and more satisfactory, accelerated wound healing when using both methods simultaneously. This study evaluated continuous irrigation combined with V.A.C. appliance for treatment of infected chronic wounds. Methods: We reviewed data from 17 patients with infected intractable chronic wounds. V.A.C. device (Group A) was used in 9 patients, and V.A.C. with antibiotics irrigation (Group B) was used in 8 patients. We placed Mepitel$^{(R)}$ on the surface of wound and placed an irrigation and aspiration tube on each side. A sponge was placed on the Mepitel$^{(R)}$ and covered with film dressing. The wound was irrigated continuously with mixed antibiotics solution at the speed of 200 cc/hr and aspirated through the wall suction at the pressure of -125 mmHg. V.A.C. applied time, wound culture and wound size were compared between the two groups. Results: No complication were seen in two groups. Compared with Group A, in the Group B, V.A.C. applied time was shortened from 32.7 days to 25.6 days and showed efficacy in the reduction rate of wound size. No statistical differences were shown in bacterial reversion. Conclusion: V.A.C. appliance with continuous irrigation is an effective new method of managing infected chronic wounds and useful to reduce treatment duration and decrease wound size. Moreover it could be applied more widely to infected wound.
Cranial implant removal is recommended if implants become exposed owing to scalp necrosis after cranioplasty. However, it carries the risk of extensive bleeding, and the resultant cranial defects can cause both aesthetic and functional problems. We present a case of a scalp defect exposing a cranial prosthetic implant that was reconstructed with a local flap and salvaged using an indwelling antibiotic irrigation system. A 73-year-old man presented with scalp necrosis after undergoing cranioplasty due to intracranial hemorrhage. The cranial implant was exposed through the scalp defect. Methicillin-resistant Staphylococcus aureus was detected in the culture from the open wound. After debridement of the necrotic tissue and burring of the superficial layer of the implant, a transposition flap was used to cover the defect and an indwelling antibiotic irrigation system was installed. Continuous irrigation with vancomycin was conducted for 5 days, and intravenous vancomycin was continued for 4 weeks. The flap was in good condition at 4 months postoperatively, with no infection. The convex contour of the scalp was well maintained. The patient's neurological status was stable. Exposed cranial implants can be salvaged with continuous antibiotic irrigation as an alternative to implant removal; thus, the risk of bleeding and possible disfigurement may be avoided.

Objective : Pyogenic spondylitis often results in acute neurological deterioration requiring adequate surgical intervention and appropriate antibiotic treatment. The purpose of this study was to conduct an analysis of the clinical effect of continuous irrigation via laminotomy in a series of patients with pyogenic spondylitis in thoracic and lumbar spine. Methods : The authors conducted a retrospective investigation of 31 consecutive patients with pyogenic thoracic and lumbar spondylitis who underwent continuous irrigation through laminotomy from 2004 to 2008. The study included 22 men and 9 women, ranging in age from 38 to 78 years (mean 58.1 years). The average follow-up duration was 13.4 months (range, 8-34 months). We performed debridement and abscess removal after simple laminotomy, and then washed out epidural and disc space using a continuous irrigation system. Broad spectrum antibiotics were administered empirically and changed according to the subsequent culture result. Clinical outcomes were based on the low back outcome scale (LBOS), visual analogue scale (VAS) score, and Frankel grade at the last follow-up. Radiological assessment involved plain radiographs, including functional views. Results : Common predisposing factors included local injection for pain therapy, diabetes mellitus, chronic renal failure, and liver cirrhosis. Causative microorganisms were identified in 22 cases (70.9%) : Staphylococcus aureus and Streptococcus spp. were the main organisms. After surgery, LBOS, VAS score, and Frankel grade showed significant improvement in most patients. Spinal stability was maintained during the follow-up period, making secondary reconstructive surgery unnecessary for all patients, except one. Conclusion : Simple laminotomy with continuous irrigation by insertion of a catheter into intervertebral disc space or epidural space was minimally invasive and effective in the treatment of pyogenic spondylitis. This procedure could be a beneficial treatment option in patients with thoracolumbar spondylitis combined with minimal or moderate destructive change of vertebrae.

The microsurgical anastomosis is integral to the success of autologous-free tissue transfer. Successful performance of this procedure relies strongly on operator dexterity, which can be made more challenging when blood and edematous fluids obscure the field of view. Workflow is impeded by intermittent irrigation and suctioning, necessitating presence of an assistant, with risk of arterial thrombosis, from vessels being drawn into suction drains. To negate these current disadvantages and minimize the barrier of entry to microvascular operations, we designed, manufactured, and patented a novel three-dimensional printed microsurgical background device with microfluidic capabilities that allow continuous suction and irrigation as well as provide platforms that enable multiangle retraction to facilitate operator autonomy. Rice cultivation is not only the main resource of farm income and staple food but also the root of cultural life of Korean people. Korean government has carried out irrigation water development with heavy investment to cope with water shortage in rice fanning as a link of the five years economic development plans. In spite of the continuous accomplishment of irrigation water development, the marginal benefits-costs of irrigation water has not been studied. Owing to the government full support for the operation and maintenance of irrigation facilities, price of irrigation water as a membership fee could not be formed as the municipal and industrial water prices. Accordingly this study is aimed at identifying firstly the marginal benefits-cost of irrigation water, secondly deriving the macro-econometrics models as supply and demand functions of irrigation water and thirdly examining the marginal benefits-cost ratio. The trends of supply and demand prices of irrigation water were estimated annually. Considering the marginal benefits-cost ratio as 1.3, it was identified that additional irrigation water development projects still have an economic feasibility under the present economic situation in Korea.
Eumsung TM/TC system is composed of one control center, four reservoirs and twenty irrigation canal systems. In this paper we analyzed operation frequencies of the irrigation system to verify the effect of TM/TC system. Large facilities's operation frequency were more low then small thing and irrigation type was continuous irrigation. operation frequency was most high in may and june. usually water gate was operated once per 5-10 day. With these analysis we concluded that operators should operate irrigation facilities based on scientific technique and designer should make a proper choice in TM/TC system selection fit to the field.
We present a modified method for the treatment of brain abscess. The double lumen extraventricular drainage (EVD) catheter which was developed for the intracerebral hematoma management, was applied for the treatment of brain abscess drainage. We placed the double lumen EVD catheter into the abscess cavity by free-hand technique and irrigated the abscess cavity continuously with antibiotics solution for 7 days. Simultaneous intravenous antibiotics was administered for 4 weeks. The continuous irrigation with double lumen catheter was found to be safe and effective treatment modality in the brain abscess.
