A Case of the Bleomycin-Induced Bronchiolitis Obliterans Organizing Pneumonia (Bleomycin 투여 후 발생한 폐쇄세기관지기질화폐렴)

  • A 34-year-old man was admitted to our hospital due to fever and cough. He received the combination anti-cancer chemotherapy for testicular tumor, including bleomycin. The chest X-ray showed consolidation and ground glass opacity on the right upper lobe and subpleural areas of other lobes. This condition was initially misdiagnosed as a pneuomonia, but consolidation did not disappear after antibiotics treatment. We performed transbronchial lung biopsy and bleomycin induced pulmonary toxicity was confirmed. The bleomycin induced lung injury is the most common chemotherapeutically induced pulmonary disease. Bleomycin induced Bronchiolitis Obliterans Organizing Pneumonia(BOOP) is less common than interstitial pneumonitis and responds well to corticosteroid treatment.

Study on Material Characteristics and Conservation Methods for Tracksite of Cretaceous Dinosaurs and Pterosaurs of Jeongchon area in Jinju, Korea (진주 정촌면 백악기 공룡·익룡발자국 화석산지의 재질특성 및 보존 방안 연구)

  • The Tracksite of Cretaceous Dinosaurs and Pterosaurs in Jeongchon, Jinju was discovered in late 2017 during the construction of the Ppuri industry complex. This site is a natural heritage site with a high paleontological value, as it preserves fossils of various types of dinosaurs, pterosaurs, and animal traces at a dense concentration. In this study, we surveyed that physical weathering such as joint, crack, scaling, exfoliation, and fragmentation occurred through field research in the fossil site, and conducted basic research on conservation science to reduce the damage. To this end, among the eight levels identified after excavation, the rocks of Level 3, which yielded a large number of theropod footprint fossils, and Level 4, which yielded pterosaur footprint fossils, were analyzed for material characteristics and evaluation of the effectiveness of consolidation and adhesion. This results showed that the rocks in the Level 3 stratum were dark gray siltstone and the rocks in the Level 4 stratum were dark gray shale, which contained a large amount of calcite and were composed of quartz, plagioclase, mica, alkali feldspar, and other clay minerals, which are likely to be damaged by rainfall under external conditions. As a result of conducting an artificial weathering experiment by dividing the probationary sample into four groups: untreated, consolidation treatment, anti-swelling treatment, and adhesive treatment, the consolidation and the swelling inhibitor showed an effect immediately after treatment, but did not show a blocking effect under a freezing-thawing environment. The adhesive showed that the adhesive effect was maintained even under freezing-thawing conditions. In order to preserve the fossil sites at Jeongchon in the future, in addition to temporary measures to block the inflow of moisture, practical measures such as the construction of protective facilities should be prepared.

Short-course versus long-course neoadjuvant chemoradiotherapy in patients with rectal cancer: preliminary results of a randomized controlled trial

  • Purpose: Colorectal cancer is becoming an increasing concern in the middle-aged population of Iran. This study aimed to compare the preliminary results of short-course and long-course neoadjuvant chemoradiotherapy treatment for rectal cancer patients. Materials and Methods: In this clinical trial we recruited patients with rectal adenocarcinoma located from 5 cm to 15 cm above the anal verge. Patients in group I (short-course) received three-dimensional conformational radiotherapy with a dose of 25 Gy/5 fractions in 1 week plus concurrent XELOX regimen (capecitabine 625 mg/㎡ from day 1-5 twice daily and oxaliplatin 50 mg/㎡ on day 1 once daily). Patients in group II (long-course) received a total dose of 50-50.4 Gy/25-28 fractions for 5 to 5.5 weeks plus capecitabine 825 mg/㎡ twice daily. Both groups underwent consolidation chemotherapy followed by delayed surgery at least 8 weeks after radiotherapy completion. The pathological response was assessed with tumor regression grade. Results: In this preliminary report on complications and pathological response, 66 patients were randomized into two study groups. Mean duration of radiotherapy in the group II (long-course) was 5 ± 1 days (range, 5 to 8 days) and 38 ± 6 days (range, 30 to 58 days). The median follow-up was 18 months. Pathological complete response was achieved in 32.3% and 23.1% of patients in the shortcourse and long-course groups, respectively (p = 0.558). Overall, acute grade 3 or higher treatment-related toxicities occurred in 24.2% and 22.2% of patients in group I and II, respectively (p = 0.551). No acute grade 4 or 5 adverse events were observed in either group except one grade 4 hematologic toxicity that was seen in group II. Within one month of surgery, no significant difference was seen regarding grade ≥3 postoperative complications (p = 0.333). Conclusion: For patients with rectal cancer located at least 5 cm above the anal verge, short-course radiotherapy with concurrent and consolidation chemotherapy and delayed surgery is not different in terms of acute toxicity, postoperative morbidity, complete resection, and pathological response compared to long-course chemoradiotherapy.

Improvement of Consolidation Characteristics around PVD Using the Thermal Method (열적 방법을 이용한 연직배수재 주변의 압밀특성 연구)

  • This paper is to investigate the removal of the smear at elevated temperatures. This study utilized both small cylinder cell and large consolidometer apparatus to investigate the combination of PVD with heat and without heat. Two types of heaters are used in this study. The heater drain made of copper tube is used for all tests except large consolidometer and flexible wire heater is used for large consolidometer. Specimens demonstrated volume contraction upon heat and without heat. This contraction is dependent on temperature magnitude. When the specimen is heated, the magnitude of settlements is higher and rate of consolidation is faster. After treatment using PVD combined without heat and with heat for undisturbed specimens the $C_h$ values obtained were $3.45m^2/yr$ and $3.83m^2/yr$, respectively, from $3.2m^2/yr$ before treatment. The $K_h/K_s$ ratios were 3 for the specimen without heat and 2 for the specimen with heat. Similarly, in reconstituted specimens without heat and with heat, the $C_h$ values were $2.1m^2/yr$ and $2.5m^2/yr$ with $K_h/K_s$ ratios of 1.75 and 1.5, respectively.

Preoperative short course radiotherapy with concurrent and consolidation chemotherapies followed by delayed surgery in locally advanced rectal cancer: preliminary results

  • Purpose: This study aimed to assess complications and outcomes of a new approach, that is, combining short course radiotherapy (SRT), concurrent and consolidative chemotherapies, and delayed surgery. Materials and Methods: In this single arm phase II prospective clinical trial, patients with T3-4 or N+ M0 rectal adenocarcinoma were enrolled. Patients who received induction chemotherapy or previous pelvic radiotherapy were excluded. Study protocol consisted of three-dimensional conformal SRT (25 Gy in 5 fractions in 1 week) with concurrent and consolidation chemotherapies including capecitabine and oxaliplatin. Total mesorectal excision was done at least 8 weeks after the last fraction of radiotherapy. Primary outcome was complete pathologic response and secondary outcomes were treatment related complications. Results: Thirty-three patients completed the planned preoperative chemoradiation and 26 of them underwent surgery (24 low anterior resection and 2 abdominoperineal resection). Acute proctitis grades 2 and 3 were seen in 11 (33.3%) and 7 (21.2%) patients, respectively. There were no grades 3 and 4 subacute hematologic and non-hematologic (genitourinary and peripheral neuropathy) toxicities and perioperative morbidities such as anastomose leakage. Grade 2 or higher late toxicities were observed among 29.6% of the patients. Complete pathologic response was achieved in 8 (30.8%) patients who underwent surgery. The 3-year overall survival and local control rates were 65% and 94%, respectively. Conclusion: This study showed that SRT combined with concurrent and consolidation chemotherapies followed by delayed surgery is not only feasible and tolerable without significant toxicity but also, associated with promising complete pathologic response rates.

Modification of the Hyperbolic Method for Staged Fill (단계성토 시 쌍곡선법의 개선된 해석방법)

  • The purpose of settlement management when treating soft ground through preloading is to determine the amount of settlement, check the progression of consolidation, and compare the settlement with the target settlement amount. Of the various methods available for predicting settlement based on measured data, the hyperbolic method was used in this study to analyze the settlement behavior of soft ground considering the creep behavior resulting from staged fill. Two versions of the method were used: the existing hyperbolic method, and a modified hyperbolic method. The existing hyperbolic method predicts the settlement amount using data for the final settlement section only during soft ground treatment through staged fill, for which the coefficient of consolidation behavior (k) was computed to give a predicted final consolidation settlement amount of Sr = 1.05 cm. In comparison, using the modified method, a predicted final consolidation settlement of Sr = 0.50 cm is obtained by considering the data for each staged fill section. These results show that the modified method considering data from the staged settlement was more accurate than the existing method considering data only from the final settlement section. This modification to the hyperbolic method therefore represents an improvement in performance over the existing method.

  • Parasomnias induced by hypnosedatives are rare but serious side effect. Such parasomnias have not been reported with all hypnosedatives. However, frequent use of hypnosedatives, particularly nonbenzodiazepine receptor agonists is associated with parasomnias. Associated symptoms are sleep eating, sleepwalking with object manipulation, sleep conversations, sleep driving, sleep sex and sleep shopping etc. Mechanisms include high affinity for $GABA_A$ receptor, interruption of the consolidation phase of memory formation by drug, pharmacokinetic or pharmacodynamic drug-drug interaction and concomitant administration with alcohol. Managements for parasomnias induced by hypnosedatives involve stopping medication, switch to other medications or nonpharmacological treatment, lowest effective dose of NBRAs (Non-Benzodiazepine Receptor Agonists), taking into consideration drug-drug interactions, identification and treatment of underlying disease states.

  • Ni(Fe)Al powders containing a homogeneous distribution of the in-situ formed AIN and $Al_2O_3$ dispersoids have been produced by mechanical alloying process in a controlled atmosphere using high energy attrition mill. The powders have been successfully consolidated by hot extrusion process. The phase information investigated by TEM and XRD analysis reveals that Fe can be soluble up to 20% to the NiAl phase ($\beta$) at room temperature after MA process. Subsequent thermomechanical treatment under specific condition has been tried to induce secondary recrystallization (SRx) to improve high temperature properties, however, the clear evidence of SRx was not obtained in this material. Mechanical properties in term of strength at room temperature as well as at high temperatures have been improved by the addition pf AIN, and the room temperature ductility has been shown to be improved after heat treatment, presumably due to the precipitation of second phase of $\alpha$ in this material.

  • There were excavated many relics about 200 pieces including gilt-bronze cap, gilt-bronze shoes, bronze mirror and armor, etc. in Ahndong tumulus, Gildu-ri, Goheung. The National Research Institute of Cultural Heritage was undertaking excavation of major damaged relics that were corroded by several environment and were destroyed by a lump of earth. Shoes are consolidated with soils in order to stop destroy during excavation. And these are relocated in order to treat for conservation. Before treatment, X-ray radiography and CT(Computed Tomography) are used in order to examine the original surface and the shape of shoes. As a result, we confirm the condition of shoes. If soils are removed, gilt-bronze shoes are crumbling into little pieces because gilt-bronze shoes are damaged by corrosion and deformation. So, shoes are consolidating with inner soils and are removing outer soils. Throughout conservation treatment, shoes recovered original form and inner soils are consolidated in order to keep the shape of shoes.

Options Study for the Neutralization of Elemental Sodium During the Pyroprocessing of Used Nuclear Fuel

  • An options study was performed for the treatment of residual elemental sodium in driver plenums following the chopping operation during the pyroprocessing of used nuclear fuel. Given the pending availability of a multi-function furnace for distillation and consolidation operations in the Fuel Conditioning Facility, the furnace was considered for the processing of driver plenums. Although two options (oxidation and distillation) could be performed in the multi-function furnace, neither option has been developed sufficiently to date to warrant the use of the furnace for treatment operations. Thus, it was decided to defer the treatment of elemental sodium from driver plenums in the multi-function furnace until more developed technologies and/or furnaces become available. In the interim, storage of the plenums and characterization efforts are recommended.
