무릎관절 통증 환자에서 멀리건 관절가동술과 맥켄지 운동이 통증과, 균형, 관절가동범위에 미치는 영향 (The Effects of Mulligan Mobilization with Movement and McKenize Exercise on Pain, Balance, Range of Motion in Patients with Knee Pain)

  • Background: This study aimed to compare the effects of the Mulligan mobilization with movement and McKenzie exercise after applying conservative physical therapy to patients with knee pain. Methods: Patients were randomly allocatied into two groups: the Mulligan mobilization with movement (10 subjects) and the McKenzie technique (10 subjects). Each group was givenr conservative physical therapy and manual therapy sessions, three times week, for four weeks. The pain intensity was measured using the visual analogue scale (VAS). The cervical range of motion (ROM) was measured with a goniometer. Balance was measured using the modified Berg balance scale (BBS). Results: After four weeks of therapy, VAS (p<.05) decreased significantly, and ROM and balance increased siginficantly in both groups(p<.05). There was a significant improvement in knee extension (p<.05) in the McKenzie group compared to the Mulligan group. No intergroup differences were found with respect to the knee flex, VAS, and BBS (p>.05). Conclusion: The McKenzie exercises are more effective than Mulligan mobilization with movement for improving knee extension. Both interventions have the same effects on pain relief, in increasing knee flexion ROM and improving balance in patients with knee pain.

마미 증후군 FBSS 증례보고 (The Clinical Report on 1 Case of Failed Back Surgery Syndrome Who were Diagnosed as the Cauda Equina Syndrome)

  • Objectives : The purpose of this study was to evaluate the oriental conservative treatment for FBSS patient who were diagnosed as the Cauda equina syndrome. Methods : The patient were treated by the oriental medical conservative treatment including herbal medication, dry needle acupuncture, direct moxibustion and indirect moxibustion, cupping therapy and, physical therapy. We estimated by Visual Analog Scale and Oswestry disability index for evaluate the effect of Oriental conservative treatment. Results : After treatment, patient's visual analogue scale score and Oswestry disability index score were generally decreased. Conclusions : The oriental medical conservative treatment might be an effective method to treat the FBSS patient who were diagnosed as the Cauda equina syndrome.

Surgical management of an accessory canal in a maxillary premolar: a case report

  • We report the surgical endodontic treatment of a maxillary first premolar with a lateral lesion that originated from an accessory canal. Although lesions originating from accessory canals frequently heal with simple conventional endodontic therapy, some lesions may need additional and different treatment. In the present case, conventional root canal retreatment led to incomplete healing with the need for further treatment (i.e., surgery). Surgical endodontic management with a fast-setting calcium silicate cement was performed on the accessory canal using a dental operating microscope. At the patient's 9-month recall visit, the lesion was resolved upon radiography.

두개하악장애환자의 교합안정장치에 의한 치료후 Sonopak을 이용한 악관절음 변화 (A Comparative Study on the Temporomandibular Joint Sounds before and after Occlusal Splint Therapy Using Electrovibratography)

  • This study was performed to compare the TMJ sounds by means of vibration-related items by Sonopak such as integral, high integral, above 300/(0-300) ratio, peak amplitude, peak frequency and median frequency before and after occlusa1 splint therapy as well as counselling, physical modalities. For this study 22 patients with craniomandibular disorders (CMDs) were selected and examined by routine diagnostic procedure for CMDs including Transcranial and Panoramic radiographs and were classified into 3 CMDs subgroups : disc displacement with reduction, disc displacement without reduction, and degenerative joint disease. Visual analogue scale (VAS) about joint sound was recorded during treatment period and VAS treatment index (VAS Ti) was calculated from the VAS data and treatment duration. The author evaluated and compared treatment results by several parmeters such as symptom duration, timing of joint sound, parafunctional habits, trauma, and diagnostic classification. The obtained results were as follows : 1. Before the treatment, the highest value of peak amplitude was observed in disc displacement with reduction group and value of median frequency was highest in degenerative joint disease group. In addition the highest values of peak frequency and ratio ware observed in degenerative joint disease group, though they were not significant. Furthermore the lowest value of high integral was observed in disc displacement without reduction group and though it was not significant, value of integral was lowest in that group. 2. Among 3CMDs subgroups disc displacement with reduction group showed the significantly decreased value of high integral and degenerative joint disease group had the significantly decreased value of integral after conservative treatment including occlusal splint therapy. Conclusively conservative treatment including occlusal splint therapy vay be effective in the treatment of CMDs including TMJ sound. 3. Fair prognosis for conservative treatment was observed in acute group under 6 months than chronic group, 6 months over in symptom duration but there was no statistical difference. The result for conservative treatment was observed slightly poor in subjects with bruxism, clenching, unilateral chewing habit and trauma history but there were no statistical differences.

상피성 임파부종 환자의 치료 -증례보고- (Treatment of Patients With Elephantiasis Lymphedema - Case Report -)

  • Lymphedema, regardless of etiology, is essentially incurable but different treatment approaches which serve to contain swelling exist. The objectives of treatment are to reduce swelling, restore shape, educate about the self-maintenance methods, and prevent inflammatory episodes, eg, recurrent cellulitis. The purpose of this report is to provide therapists and other medical staff with a general guideline through the example treatment procedure of two patients with lymphedema admitted to Samsung Medical Center. This study demonstrates the effects of the various treatments used and how they helped to achieve improvement in mobility and reduction in swelling of the lower limbs. The basic conservative treatments were sequential intermittent pneumatic pumps, elevation, and CDP (complex decongestive physiotherapy). The surgical procedures (Homan's operation) were carried out after maximal volume reduction through conservative programs. In these cases, we can see greater than 50% reduction in the lymphedema in those treated by conservative and surgical procedures. This presents a simple, reliable, variable method yielding satisfactory cosmetic and functional results for patients suffering from chronic both-leg lymphedema. Futhermore, I suggest that the outcomes are best when treatment is administered by a multidisciplinary team including a physiotherapist, surgeon, nurse, et al.

맥켄지운동이 턱관절 장애 및 신체균형에 미치는 효과 (Effect of McKenzie Exercise on Temporomandibular Joint Disorder and Body Balance)

  • Purpose : This study was aimed at investgating the effect of McKenzie exercise on temporomandibular joint disorder (TMJD) and body balance. Methods : Thirty subjects with TMJD were selected. They were randomly assigned to one of two groups (15 in each group) : namely the conservative treatment group and McKenzie exercise group. The conservative treatment group were treated physical therapy for 6 weeks (3 times a week). The McKenzie exercise group performed McKenzie exercise for 6 weeks (3 times a week). We measured mouth opening, pain, muscle activation and body balance before and after exercise by using the maximum mouth opening MMO), visual analog scale (VAS), electromyography and Biodex Balance System, respectively. Results : We found statistically significant differences in pain and mouth opening in the conservative treatment group and McKenzie exercise group before and after (p<.05). Conclusion : We confirmed the effect of McKenzie exercise on TMJD. Thus we thought these results could be used as basic data and reference for TMJD. But we need more study effect of McKenzie exercise on TMJD and body balance.

Is stopping of anticoagulant therapy really required in a minor dental surgery? - How about in an endodontic microsurgery?

  • Nowadays, oral anticoagulants are commonly prescribed to numerous patients for preventing cardiovascular accident such as thromboembolism. An important side effect of anticoagulant is anti-hemostasis. In a major surgery, the oral anticoagulant therapy (OAT) regimen must be changed before the surgery for proper post-operative bleeding control. However, in a minor dental surgery and endodontic surgery, the necessity for changing or discontinuing the OAT is open to debate. In this study, risks of the consequences were weighed and analyzed. In patients who stop the OAT, the occurrence of thromboembolic complication is rare but the result is fatal. In patients who continuing the OAT, post-operative bleeding can be controlled well with the local hemostatic measures. In the endodontic surgery, there are almost no studies about this issue. The intra-operative bleeding control is particularly important in the endodontic surgery because of its delicate and sensitive procedures such as inspection of resected root surface using dental microscope and retrograde filling. Further studies are necessary about this issue in the viewpoint of endodontic surgery.

추나요법으로 치료한 후방 종인대 골화증 1예 (A Case Report on the Ossification of the Posterior Longitudinal Ligament Treated by CHUNA Manual Therapy)

  • 후방 종인대 골화중의 치료에 수술적 처치 이전 단계의 환자에 대한 보존적 치료는 대증적 치료가 주가 되어 왔다. 본 증례에서는 추나요법을 주로 시행한 후방 종인대 골화증 환자(32세, 여자)에서 시각적 상사 척도의 각 치료기간별 감소 및 ROM의 증가와 더불어 X-ray상의 변화까지도 감소되는 현저한 치료효과를 나타내게 되었다. 향후 본 질환에 대한 보다 심도 있는 연구가 진행된다면 후방 종인대 골화증의 보존적 치료에 추나요법이 뚜렷한 역할을 공헌하리라 기대되며 이에 대한 관심과 연구가 진행되어야 한다.

요통환자의 물리치료에 관한 임상적 연구

  • A Clinical Study of physical Therapy on the Low Back Conditions J. B. Rhee, S. K. fang, D. J, Han Low back pain usually is due primarily to a faulty mechanics, ,dis'c disease and other contributing factors .Conservative treatment of low back pain is considered to be useful with applicationes physical therapeutic measures including elimination of gravity, but surgical treatment is alternative to conservative .This study 1s conducted to analyse and evaluate efficiency of physical therapy on the low back pain on 452 cases who were treated at the Korea University Hospital, from January 1, 1978 to December 31. 1978.The results obtained in this study are summarized as follows :1. Sex distribution of total rationㄷs surveyed showed that 5l.3% in male and 487% in female.2 . In the age distrbution. the mest Liedominant age group was 25-54 years with7O.8 % .3 . The most proportion of the cases by occupation was housewife 27.7%, clerical and office workers 2l.7% , students 12.8% and labours 12.2 % respectively .4 . The most common causes of low back conditions classified, the Strain or Trau-rla revealed the highest frequency with 58.4% and thc Herniated Nucleus Pulpe-sus with 29.2 % of the total.5 . The modalities of physical therapy suitable to each cases were applied : 94.0%of all cases were treated with deep thermotherapy, 91.4 % hydrocollator steampack, 86.7% strengthening exercise of the weak muscles, pelvic traction 34.1%.6 . In regard to the duration required for the physical therapy. Herniated Nucleus Pulposus were approximately 4 weeks and strain or trauma were 2 weeks.7 . The improvement by physical therapy on lcw back conditions was classified in-to 41grades and showed following reaulits , excellent 42.5%, good 37.8%, fair 13.7% and poor 6.0% respectively.

복부 기능적전기자극이 목척수손상환자의 최대기침유량과 노력성폐활량에 미치는 효과 (Effects of Abdominal Functional Electrical Stimulation on Peak Cough Flow and Forced Vital Capacity in Patients with Cervical Spinal Cord Injury)

  • Purpose : The purpose of this study was to investigate the effects of abdominal functional electrical stimulation on peak cough flow and forced vital capacity in patients with cervical spinal cord injury. Methods : The study examined 20 patients with cervical spinal cord injury. The subjects were randomly divided into two groups. All subjects performed conservative physical therapy for 30 minutes. The experimental group also underwent abdominal functional electrical stimulation for at least 20 minutes per day. Abdominal functional electrical stimulation was applied to the rectus abdominis muscle twice each day, three times a week, for four weeks. In all subjects, the peak cough flow was measured using a peak flow meter and forced vital capacity was assessed using a spirometer. Results : The experimental group showed a significant increase in peak cough flow and forced vital capacity in pre-post measurements (p<.05), while the control group showed a significant increase only in peak cough flow. Conclusion : These findings suggest that conservative physical therapy in combination with the abdominal functional electrical stimulation can improve peak cough flow and forced vital capacity in patients with cervical spinal cord injury.
