소아 횡격막 질환의 임상적 고찰 (Clinical Study of Congenital Diaphragmatic Diseases in Neonates and Infants)

  • Congenital diaphragmatic disease is one of the common major congenital anomalies, and its mortality remained still high despite recent medical advances. The aim of this study is to examine the clinical characteristics of congenital diaphragmatic diseases. A total of 39 patients with congenital diaphragmatic disease that underwent surgery from January, 1997 to December, 2009 at Pusan National University Hospital were included in this study. Medical records were retrospectively reviewed. The male to female ratio was 30:9. Six out of 39 cases died (NS) before surgery, 17 patients had Bochdalek's hernia (BH), 11 patients hiatus hernia (HH), 4 diaphragmatic eventration (DE), and 1 Morgagni hernia (MH). There were no differences in mean birth weight and mean gestational age. NS (83.3 %). BH (35.3 %) was diagnosed more frequently than other diseases in the prenatal period. Three patients (17.6 %) of BH expired due to pulmonary hypoplasia and 1 patient had co-existing congenital heart disease. BH was diagnosed more frequently in the prenatal stage and had a higher motality rate than other conditions. Therefore, BH needs to be concentrated more than other anomalies.

성인에서 발생한 Bochdalek 탈장;수술 치험 2례 (Surgical Repair of Bochdalek Hernia in Adult; A Report of Two Cases)

  • Congenital diaphragmatic hernia through Bochdalek foramen, posterolateral diaphragmatic hernia, is the result of a congenital malformation of the posterolateral region of the diaphragm. Bochdalek hernia is the most common type of congenital diaphragmatic hernia, but it is a extremely rare anomaly which is detected in adulthood because it has severe cardiopulmonary and gastrointestinal symptoms after birth immediately.We experienced two cases of Bochdalek hernia which were detected in adulthood. Among them, one was a 43 years old man who was treated with simple closure in left Bochdalek hernia and the other was 41 years old woman who was treated with patch closure using silastic sheet in right Bochdalek hernia. Their postoperative courses were uneventiful. So we report two cases of rare Bochdalek hernia which was detected in adulthood with review of literatures.

  • Fourteen cases of diaphragmatic hernias surgically treated at the Dept. of Thoracic & Cardiovascular Surgery in Kosin Medical Center from Jan. 1979 to Feb. 1991, were reviewed in this study. This report includ 9 cases of traumatic diaphragmatic hernia, 5 cases of congenital diaphragmatic hernia. Among the traumatic hernias, 5 cases were blunt trauma and 4 cases were penetrating trauma. Five cases of congenital diaphragmatic hernias were presented, including 2 cases of esophageal hiatal hernia, 2 cases of Bochdalek's hernia and a case of Morgagni's hernia. Operation was performed in all patients. Results of all treated cases were excellent.

성인에 발생한 Bochdalek 허니아 2례 보 (Bochdalek hernia: 2 cases report)

  • Congenital diaphragmatic hernia through of Bochdalek foramen, posterolateral diaphragmatic hernia, is the result of a congenital malformation of the posterolateral region of the diaphragm. Bochdalek hernia is not a rare anomaly and is the most common type of congenital diaphragmatic hernia. The defect is more frequently on left [about 5 times]. The purpose of this paper to present the Authors recent experience with two cases of the congenital posterolateral diaphragmatic hernia adult women which was treated surgically in the Dept, Thoracic Surgery, Chosun University Hospital. The postoperative course was uneventful and both of them were discharged without any other complications. Our study and surgical experience was discussed and the literature reviewed.

  • Congenital diaphragmatic hernia is an important cause of respiratory distress in the newborn. Eventration, with which these conditions are easily confused, may also Produce severe respiratory distress in infancy or be an asymptomatic radiographic finding. Harrington`s classification of diaphragmatic hernias into two categories, traumatic and nontraumatic, is most widely accepted. Nontraumatic hernias are [1] the congenital types, composed of the posterolateral [Bochdalek], those through the esophageal hiatus, the parasternal [/Morgagni], and those through a defect left by partial absense posteriorly, and [2] the acquired types, composed of those through the esophagea/hiatus [sliding and paraesophageal] and those the sites mentioned above under the congenital hernias. During the period from 1970 up to October 1978, 21 cases of diaphragmatic hernia were treated in department of cardiovascular and thoracic surgery. 11 cases of Bochdalek hernias, 1 case of Morgagni hernia, 5 cases of diaphragmatic eventration and 3 cases of hiatal hernia [2 cases of paraesophageal and 1 cases of sliding type], were experienced. 3 cases of 20 died of respiratory insufficiency, 2 cases of mortality were combined with left lung hypoplasia with Bochdalek hernia.

  • 선천성 횡격막탈장은 신생아기에 응급 수술을 요하는 질환으로 주로 좌측에 많이 발생한다. 우측 선천성 횡격막 탈장은 드물며 증상이 경미하여 신생아기 이후에 발견되는 경우가 많다. 치료는 전통적으로 개흉술을 통한 횡격막 봉합술이 시행되지만 흉강경을 이용한 수술이 적용되어 좋은 결과들이 보고되고 있다. 우리는 우측 선천성 횡격막 탈장을 가진 5개월된 여아를 흉강경을 이용하여 수술하였기에 보고하는 바이다.

  • Congenital diaphragmatic hernia remains a disease with high neonatal mortality rate despite recent advance in neonatal intensive care. We experienced one case of the congenital diaphragmatic hernia with acute respiratory distress and left pulmonary hypoplasia in the neonate. The simple closure was performed through left paramedian approach after diagnosis. The postoperative course was uneventful except wound disruption. The patient was follow-up with good general condition.

횡경막 탈장의 임상적 고찰 (Clinical Review of Diaphragmatic Hernia)

  • Between June 1981 and April 1994, 15 patients underwent surgical repair of diaphragmatic hernia. The ages ranged from 1 day to 60 years, with a mean age of 34. There were 5 cases of congenital diaphragmatic hernia; Bochdalek hernia in 4 cases and Morgagni hernia in 1 case. There were 10 cases of traumatic diaphragmatic hernia;blunt trauma in 8 cases and stab wounds in 2 cases. The chest X-ray findings were abnormal in 10 cases. Operations were performed in all patients and there was only one death, who was a newborn with left Bochdalek hernia and pulmonary hypoplasia.

  • A 2.5 kg, three-month-old. intact male Shih-tzu was referred to the Veterinary Teaching Hospital of Kyungpook National University to examine the postoperative inflammation and suspected diaphragmatic defect which had been found during the surgical correction of umbilical hernia by the referral veterinarian. An umbilical hernia had been surgically repaired one month earlier at the time. Radiographic findings were enlarged cardiac silhouette containing soft tissue and gas densities and overlapping of cardiac and diaphragmatic borders. Ultrasonographic findings revealed the discontinuity of the diaphragm and the partial herniation of the liver into the pericardial sac. According to these findings, it was diagnosed as congenital peritoneopericardial diaphragmatic hernia(PPDH). Because the dog showed no serious clinical signs of PPDH and the owner didn't want a surgery to correct it, the dog was discharged the day of the examination after treatment of the inflammatory surgical site. On the follow-up after three months, the dog showed no distinct clinical signs of PPDH and was in good physical condition.

  • A diaphragm is a musculotendinous partition that separates abdominal and thoracic organs and assists in ventilation. Three dogs were presented after being hit by a car, and one dog was presented with the history of respiratory problems including intermittent dyspnea and coughing. Survey thoracic radiography, positive contrast celiography and sonography revealed traumatic diaphragmatic hernia in three dogs and congenital peritoneopericardial diaphragmatic hernia in one dog. In three dogs including a dog with congenital peritoneopericardial diaphragmatic hernia, herniorrhaphy was performed. Among the three dogs underwent surgery, two dogs with traumatic diaphragmatic hernia recovered uneventfully. However the dog with congenital peritoneopericardial diaphragmatic hernia died from pulmonary edema at 2 days after surgery. Positive-contrast ceilography and ultrasonography provides to investigate the integrity of the diaphragm. Chronic diaphragmatic hernia and concurrent injuries are considered as prognostic factors.
