Congenital Esophgeal Anomaly -6 Cases- (선천성 식도 폐쇄증 -치험 6례-)

  • Congenital esophageal anomaly with or without tracheoesophageal fistula was rare congenital disease.We experienced 6 cases of congenital esophageal atresia, with tracheoesophageal fistula [5 cases and one esophageal atresia without fistula, were treated in the Department of Thoracic and Cardiovascular Surgery, Kangnam Sacred Heart Hospital, Hallym university, from May, 1992 to July, 1994. The type of four cases were upper blind pouch and lower tracheo or broncho esophageal fistula and one case H-type fistula with no esophageal atresia, and the one case was esophageal atresia without fistula. We performed modified Haight`s method, one case was primary closure with feeding gastrostomy and stomach interposition. Three were died due to respiratory failure on 7 and 9th postoperative days.Three were recovered uneventfully.

  • Recently we successfully treated two cases of congenital bronchoesophageal fistula(BEF), communicating esophagus and right lower lobe, in adults by fistulectomy and right lower lobectomy. The fistulas were initially diagnosed on chest CT examination and confirmed by endoscopy and esophagography. The diagnosis of BEF is usually made by barium esophagography, esophagoscopy, and bronchoscopy. Although congenital BEF presented in adult life is a rare disorder, careful examination of chest CT films would disclose more cases of it , we think , than expected.

Congenital Bronchoesophageal Fistula in Adult - Report of a case - (성인의 선천성 기도-기관지루: 1례 보고)

  • We have experienced a case of 42-year-old woman with congenital broncho-esophageal fistula. The patient had productive coughing since childhood. A barium-swallowing examination showed a lower esophageal diverticulum communicating via a fistula with posterior basal segment of right lower lobe. Bronchography showed bronchiectasis in right middle and lower lobes. At thoracotomy resection of the diverticulum, bronchoesophageal fistula, and right middle and lower lobe of lung were performed. The postoperative course was uneventful.

  • There were so many causes of chronic coughing including postnasal drip, pneumonia, nasal polyp, asthma, interstinal lung disease etc. Congenital bronchoesophageal fistula was not usually thought as cause of chronic coughing. A 46-year-old female patient suffered from chronic coughing without usual causes. Her chest X-ray viewed normally. She coughed especially after swallowing foods. So we recommended her esophagogram and it revealed broncho-esphageal fistula. She underwent surgical resection of broncho-esophageal fistula. She was well without cough after the surgery. We reported a case of congenital broncho-esphageal fistula that had caused chronic coughing without any evidence of pneumonia, malignancy, tuberculosis, bronchiectasis, inflammation, asthma, nasal polyp, etc. So we should suspect the bronchoesophageal fistula when patients cough chronically with eating, and recommend the esophagogram.

  • The pedicled omental flap has been used for treatment of various kind of complications in thoraxcic surgery. Its property of promoting neovascularization , immunilogic properties that limiting the spread of infection, providing soft tissue coverage are very effective in treatment esophageal fistulas. Also, congenital broncho-esophageal fistula [ BEF ] is a rare disease entity which was reported about 100 cases around the world. We experienced 27 years old female patient with Braimbridge type I congenital BEF. We performed division of BEF using stapler and pericardial patch coverage of esophageal side with concomittent left lower lobectomy. This patient was complicated with postoperative esophageal leakage with empyema thoracis. We have successfully managed these problems with re-thoracotomy and re-closure of esophageal fistula using Right Gastroepiploic Artery based pedicled omental flap wrapping around the esophageal anastomosis site. It is felt that pedicled omental flap is a very effective method to manage esophageal complication such as postoperative esophageal leakage.

  • Congenital or acquired esophagotracheobronchial fistula are rare. The chief causes of the acquired form are malignancy developing on the esophagus or tracheobronchial system and infection, and trauma. The pathognomonic symptom is a paroxysmal cough occurring several seconds after ingestion of liquids. This report reviews a case of bronchoesophageal fistula of unknown origin accompanying bronchiectasis. The patient is 32 years old woman with excellent result by surgical intervention. But the fistula is accidently found in the operation field. The surgical procedures consissts of fistulectomy with Right lower lobectomy.

  • Tracheobronchial rupture following tracheal intubation is a rare complication. We experienced a case of tracheal rupture following double-lumen endotracheal tube intubation. A 76 year old female was admitted due to coughing and chest discomfort. The operation was performed with the diagnosis of congenital broncho esophageal fistula. During the operation, accidently the main trachea was ruptured longitudinally. There was no history of surgical trauma. The ruptured trachea was repaired with prolene and monofilament absorbable sutures. The cause of tracheal rupture was suspected overinflation of the cuff. The patient was discharged from the hospital without any significant complications.
