Streptozotocin유도 당뇨병이 백서하악 절단과두 치유에 미치는 영향의 실험적 연구 (REMODELING PROCESS OF THE STREPTOZOTOCIN-INDUCED DIABETIC RAT'S RESECTED CONDYLE)

  • The purpose of this study was to investigate the remodeling process of the streptozotocin-induced diabetic rat's resected condyle. This experiment was performed with male Sprague-Dawly strain rats weighing approximately 250 gm, which were rendered diabetic by an intravenous injection of streptozotocin(70㎎/㎏ body weight). After condylectomy, experimental rats were serially terminated on the 1st week, the 2nd week, the 3rd week, and the 4th week. The following termination, the mandibles were dissected out to make specimens. Each mandibular condyle was radiographed with Hitex HA-80(Hitex Co., Ltd. Japan). In addition to radiographic observation, the mandibular condyles, further decalcified and embedded in paraffin, were sectioned and stained with Hematoxylin and Eosin, Toluidine blue and Masson's trichrome. They were observed with a light microscope and a polarizing microscope. The results were as follows. 1. Soft X-ray radiograms revealed proliferation of bone after 1 week in both groups. Irregularly repaired bones and dense trabeculae were clearly observed in experimental group. 2. The resected condyles were repaired by intramembraneous and endochondral bone formation in both groups. 3. Bone tissue repair was initiated from the adjacent margin of resected bone, and cartilaginous tissues were observed at the top of repaired bone in both groups. 4. The number of osteoblasts of experimental group was small, compared with control group. Each osteoblast was small and flat. The thin trabeculae were irregularly formed. 5. Collagens of bone were gradually matured in both groups, but the degree of maturation was lower in experimental group. 6. Fibrous tissues covered the upper parts of repaired bone were densely arranged in the both groups. Conclusively, atrophied osteoblasts, immature collagen of bone, and thin and irregular trabeculae which were characterized in the diabetes experimental group showed diabetes disturbed osteoblastic function and caused disturbance of remodeling process of bone.

하악 과두골절에 관한 임상적 연구: 효과적인 체외고정법 (A CLINICAL STUDY ON FRACTURES OF THE MANDIBULAR CONDYLES: EFFECTIVE EXTRACORPOREAL FIXATION TECHNIQUE)

  • Purpose: Fractures of the mandibular condyle accounts for 15% to 30% of all the mandibular fractures and lots of complications are reported. Among these complications, condylar resorption is the most important one to be taken into account. The purpose of this study is evaluating condylar resorption pattern in relation to extracorporeal fixation technique. Patients & methods: Thirty four patients with 42 mandibular condylar fractures were retrospectively examined with an average follow-up of 18.5 months (6-66 months). Male/female ratio is 27 males (7 bilateral cases) and 7 females (1 bilateral case). Applicated surgical techniques were extracorporeal fixation via submandibular approach and extracorporeal fixation via intraoral approach. We compared radiographic views of post-operative periods with that of most long term follow up in 34 operated sites respectively. Result: There were 2 generalized condylar resorption sites and 3 partial resorption sites with clinical implications. Others (29 joints) showed good healing or satisfactory adaptive state of temporomandibular joints clinically. Radiologically, signs of partial condylar resorption and/or osteoarthrosis were diagnosed in 11 patients. But, 8 out of the 11 patients didn't show significant clinical symptoms, which means successful adaptive remodeling of the surgically located condylar head. Conclusion: From the above results, intentional ramal osteotomy, temporary detachment of the fractured condyle and extracorporeal fixation technique seems to be effective and useful in those cases of condylar fractures in which reduction of the dislocated condyle is very difficult or virtually impossible.

Three-dimensional assessment of condylar surface changes and remodeling after orthognathic surgery

  • Purpose: This study was performed to evaluate condylar surface changes and remodeling after orthognathic surgery using three-dimensional computed tomography (3D CT) imaging, including comparisons between the right and left sides and between the sexes. Materials and Methods: Forty patients (20 males and 20 females) who underwent multi-detector CT examinations before and after surgery were selected. Three-dimensional images comprising thousands of points on the condylar surface were obtained before and after surgery. For the quantitative assessment of condylar surface changes, point-to-point (preoperative-to-postoperative) distances were calculated using 3D processing software. These point-to-point distances were converted to a color map. In order to evaluate the types of condylar remodeling, the condylar head was divided into six areas (anteromedial, anteromiddle, anterolateral, posteromedial, posteromiddle, and posterolateral areas) and each area was classified into three types of condylar remodeling (bone formation, no change, and bone resorption) based on the color map. Additionally, comparative analyses were performed between the right and left sides and according to sex. Results: The mean of the average point-to-point distances on condylar surface was $0.11{\pm}0.03mm$. Bone resorption occurred more frequently than other types of condylar remodeling, especially in the lateral areas. However, bone formation in the anteromedial area was particularly prominent. No significant difference was found between the right and left condyles, but condylar surface changes in males were significantly larger than in females. Conclusion: This study revealed that condylar remodeling exhibited a tendency towards bone resorption, especially in the lateral areas. Condylar surface changes occurred, but were small.

Intracorporeal reduction of condylar fracture using both pedicled condylar and seperated ramal fragments after vertical ramal osteotomy

  • The aim of this study is to introduce a surgical technique that can maintain blood supply to prevent condylar resorption in the extracorporeal reduction of condylar fracture. Neither the medial pterygoid muscle on the ramal bone nor the lateral pterygoid muscle on the condylar fragment was detached after vertical ramal osteotomy. Thus, reduction was performed in the intracorporeal state. Therefore, blood supply was expected to be maintained to the fragments of both the condylar and ramal bones. On postoperative radiographs, the anatomical outline of the fractured condyle was well restored, and the occlusion was stable. In the unilateral case, there were no signs of mandibular condylar resorption until postoperative 3 weeks. In the 2 bilateral cases, condylar displacements with plate fractures and screw loosening were observed at postoperative 1 month or 5 months, but radiodensity at the displaced fracture site increased during the follow-up period. Finally, complete remodeling of the condylar fragments with restored anatomic appearance was observed on 8-month or 2-year follow-up radiographs. All cases exhibited good healing aspects with no signs or symptoms of mandibular condylar dysfunction during the postoperative remodeling period after intracorporeal reduction of condylar fracture.

비골 피판을 이용한 하악 및 하악과두 재건의 장기간 임상적 평가 (LONG-TERM ANALYSIS OF RECONSTRUCTED TEMPOROMANDIBULAR JOINT AND MANDIBLE USING FREE FIBULAR FLAP)

  • Purpose of study: The temporomandibular joint (TMJ) occupies a key functional role in mastication and contributes to normal deglutition, speech as well as cosmesis. When a large amount of mandible including the condyle head is resected, it is very difficult to reconstruct it as a functional unit. In this retrospective study, we present the functional, radiographic and cosmetic results of reconstructed temporomandibular joint using free fibular flap. Patients and Methods: Total 12 patients (M:F = 6:6) who underwent condylar reconstruction with the fibular flap were interviewed and examined by radiographs and Bio-PAK$^{(R)}$. Mean follow up periods was $47.7{\pm}20.0$ months and the average age was $38.7{\pm}15.3$ years. Remodeling of condyle and function of TMJ were evaluated and facial contour was judged subjectively. Results: All flaps were viable and no immediate postoperative complication had happened. One patient showed decreased mouth opening, so interpositional gap arthroplasty was performed. The resorption rates of reconstructed fibular were minimal and the condyle heads were changed into domeshaped neocondyle after 2 years. All patients had normal diet and no speech difficulty was reported. Nine patients were satisfied with their facial contour but three patients complained about the depression of cheek. Conclusion: The reconstruction of TMJ with free fibular flap was reliable methods and very effective means of restoring mandibular function. The functional and morphologic results were excellent and showed little complications.

Galvani전류가 백서의 하악골 성장에 미치는 영향에 관한 실험적 연구 (AN EXPERIMENTAL STUDY ON THE EFFECT OF THE GALVANIC CURRENT ON THE MANDIBULAR GROWTH IN RAT)

  • In almost all biologic systems, mechanically induced electric charge separation is a fundamental phenomenon. Since the hypothesis was established that the generation of electric potentials in bone by mechanical stress including muscular force might control the activity in bone by mechanical stress including muscular force might control the activity of osseous cells and their biopolymeric byproduct, the concept of electrically mediate growth mechanism, which involves biological growth and bone remodeling by any means, in living systems has been applied clinically and experimentally to orthopedic fracture repair, the regulation of orthodontic tooth movement, epiphyseal cartilage regeneration, etc. On the other hand, recent numerous research data available show apparently that the mandibular condyle has the characteristics of growth center as well as growth site. In addition, there exists a considerable difference of opinion as to the role of external pterygoid muscle in condylar growth. In view of these evidences, this. experiment was performed to investigate the effect of the galavic current on the growth of the mandible and condyle for elucidating the nature of condylar growth. The bimetallic device was composed of silver and platinum electrode connected with resistor (3.9 Mohm), which was expected to produce galvanic current of 23.6 nA according to the galvanic principle. The 25 Sprague-Dawley rats were divided into two group, 2 week group comprising 8 animals exposed to satanic current for 2 weeks and 3 control animals not exposed for 2 weeks, 4 week group comprising 10 animals in experimental group and 4 animals in control group applied for 4 weeks respectively. The experimental rats were subjected to application of the galvanic current invasively to codylar head surface and the control groups with sham electrode. On the basis of anatomic and histologic data from the mandibular condyle of experimental and control group, the following results were obtained. 1. After 2 weeks, there was no increase of mandibular size in experimental group over that of the control group. 2. After 4 weeks, the size of the condylar head was larger in experimental group than that of the control. 3. In 2 week group, the thickness of the mitotic compartment and hypertrophic chondroblastic layer was increased in experimental group. 4. In 4 week group, the number and the size of the hypertrophic chondroblasts were increased significantly on experimental group over that of the control group. 5. The application of the satanic current caused an increase in chondrocytic hypertrophy and intercellular matrix in both groups.

편측성 하악 과두 골절 후의 교합 관계 회복 (Re-establishment of occlusion after unilateral condylar fracture)

  • 본 증례의 편측성 하악 과두 골절 환자는 외상 후 1개월간 다수의 상악 전치부 치아 외상이 간과되어 교합이 붕괴된 상태로 내원하였다. 변위된 상악 전치부 치아들의 외과적 정복 후 지속적인 기능 운동을 진행하여, 10개월 후 정상 교합 및 하악 과두 골절부의 골유합이 확인되는 양호한 결과를 얻을 수 있었다.

백서의 하악골 전방 재위치 시 하악과두 조직에서의 nitric oxide synthases 발현 양상 (Expression of nitric oxide synthases in the mandibular condyle of anterior repositioned rat mandibles)

  • 성장기 아동의 하악과두 조직에서의 골개조 과정은 연골세포, 조골세포, 파골세포가 연관되며 nitric oxide synthase (NOS)가 중요한 매개자의 하나로 작용할 것으로 생각되나 기능성 악정형술을 이용해 하악골을 전방유도시켰을 때 하악과두에서의 골개조와 NOS의 발현에 대해서는 아직 자세히 연구되어 있지 않다. 본 연구는 하악골을 기능적으로 전방위치시켰을 때 하악과두 조직의 조직학적 변화양상에 대해 살펴보고 NOS의 발현 정도를 조사하여 상관관계를 알아보고자 하였다. 35일령의 24마리의 Sprague-Dawley 백서를 3군으로 나누었고 각 군에 있어서 5마리는 실험군으로, 3마리는 대조군으로 나누었다. 실험군은 하악골 전방유도장치를 24시간 장착한 후 3, 14, 30일에 희생하였으며 조직절편은 시상면에 평행하게 절단하였고 $NOS_2$$NOS_3$의 조직면역염색을 통해 다음과 같은 결과를 얻었다. 대조군 간에는 조직학적인 변화와 NOS의 발현 변화가 뚜렷이 관찰되지는 않았으며 실험기간에 따른 조직학적 변화는 실험 14일군에서 가장 많은 신생골 형성과 조골세포 활성을 보였으며, 실험 30일군에서는 감소하는 양상으로 나타났다. 또한 모든 실험군에서 대조군에 비해 $NOS_2$$NOS_3$의 발현이 많이 나타났으며 실험기간에 따른 비교 시 $NOS_2$$NOS_3$의 발현은 실험 14일군에서 가장 많이 나타났으며 실험 30일군에서는 감소하는 양상으로 나타났다. 전체 실험군과 대조군에서 $NOS_2$의 발현은 $NOS_3$의 발현보다 많이 나타났고 $NOS_2$는 실험군 과두의 비대연골, 연골하골에서 주로 발현되었다. $NOS_3$는 실험군 과두의 골수와 모세혈관에서 주로 발현되었다. 따라서 하악과두 내의 $NOS_2$$NOS_3$의 발현조절이 하악과두 조직의 골개조에 관여함을 시사한다.

하악골의 발육중인 생쥐에서 기능력의 변화가 특이-유전자 발현에 미치는 영향 (THE EFFECT OF ALTERED FUNCTIONAL FORCE ON THE EXPRESSION OF SPECIFIC MRNAS IN THE DEVELOPING MOUSE MANDIBLE)

  • 기능력은 뼈의 형성, 유지 및 개조 이외에도 치아이동, 교정치료, 기능성 악교정장치의 사용 등과 관련한 치주인대세포의 특성 변화 등에 중요한 영향을 미친다. 또한, 하악과두에서도 기능력의 변화에 따라서 다양한 특성의 변화가 나타난다. 본 연구에서는 ICR 생쥐를 8주 동안 soft-diet와 hard-diet의 식이 조절을 통해 기능력의 변화를 유도하여, 기능력의 변화와 관련한 특이 유전자를 검출하기 위하여 subtractive hybridization, northern 분석 및 mRNA in-situ hybidization 등의 실험을 시행하여 다음과 같은 결과를 얻었다. 1. Soft-diet군과 hard-diet군의 subtractive hybridization을 통하여 총 39개의 clone을 얻었고 이중에서 11개의 서로 다른 hard-diet군 특이 후보 유전자들을 분리하였다. 2. 11개의 후보 유전자들 중에서 homology 검색과 northern 분석을 통하여 기능력과 관련이 있을 것으로 생각되거나 hard-diet군에서 mRNA가 선택적으로 발현되는 FS-s2, FS-s5, FS-s18 및 Fs-s22 유전자를 선택하였다. 3. Soft-diet군과 hard-diet군의 형태학적 분석에서 soft-diet군은 hard-diet군에 비하여 골모세포의 활성과 골개조의 특성은 저하된 것으로 관찰되었으나, 하악과두의 연골성골화 과정은 오랜 시간동안 지속되는 것으로 나타났다. 4. FS-s2, FS-s5, FS-s18 및 Fs-s22 유전자의 cRNA 탐침자를 이용한 in-situ hybridization에서 4개의 유전자의 mRNA들은 hard-diet군의 세포들에서는 강하게 발현되었으나 soft-diet군의 세포들에서는 그 발현이 미약하거나 나타나지 않았다. 이상의 결과를 종합하여 FS-s2, FS-s5, FS-s18 및 Fs-s22 유전자들은 대부분이 기능이 거의 알려져 있지 않는 것들로 기능력의 변화 과정에서 중요한 역할을 할 것으로 생각되나 앞으로 보완 연구를 통하여 각 각의 유전자들의 보다 정확한 기능을 이해하는 것이 필요 할 것으로 사료된다.

악관절원판의 인위적 전방변위술시행후 악관절구성조직에서 Fibronectin의 분포변화 (DISTRIBUTION IN FIBRONECTIN OF THE RABBIT TEMPOROMANDIBULAR JOINT TISSUES FOLLOWING SURGICAL INDUCTION OF ANTERIOR DISK DISPLACEMENT : IMMUNOHISTOCHEMICAL STUDY)

  • The extracellular matrix(ECM) is a complex network of different combination of collagens, glycosaminoglycans, laminin, fibronectin, and many other glycoproteins including proteolytic enzymes. The composition and organization of the ECM contributes to the uniques physical or biomechanical properties of a tissue. Fibronectins(FN) are dimeric glycoproteins located on cell surfaces, in the matrix of connective tissue, and in blood. Fibronectins mediate cell attachment to collagen substratum and have been implicated in a variety of important biological processes, including embryogenesis and cell differentiation. The purpose of this study was to determine the effects of surgical induction of anterior disk displacement(ADD) on distribution of fibronectin in the rabbit temporomandibular joint(TMJ) tissues included the articular cartilage, disc, retrodiscal tissue, articular eminence using an immunohistochemical technique. The left TMJ was exposed surgically, and all discal attachments were severed except for the posterior attachment. The disk was then repositioned anteriorly and sutured to the zygomatic arch. The right TMJ served as a shamoperated control. Normal joints were used as a nonoperated control. Fourty-five rabbits were used for experiments in total. For fibronectin immunohistochemical study, eighteen rabbits (one normal group and 5 experimental groups, each group consists of 3 rabbits) were used. The experimental rabbits were sacrified after operation period of 2, 3, 4, 6 and 8 weeks on fibronectin. The obtained results were as follows ; 1. Fibronectin immunoreaction on all TMJ tissues(mandibular condyle, articular disc, retrodiscal tissue, articular eminence) in the normal rabbit was observed. Especially the reverse cell layer and proliferation zone of articular cartilage of condyle show strong positive reaction. 2. Depletion of fibronectin in the all TMJ tissues except hypertrophic zone of articular cartilage occurred at 2 weeks following induction of ADD. 3. The restoration of immunoreaction at 4 weeks was observed and a progressive increasing reaction at 6 weeks, 8 weeks also was found. Our study generally showed degenerative changes in TMJ tissues after ADD although TMJ tissues adapted or degenerated to abnormal loads and stress distribution according to the remodeling capacity of TMJ tissues.
