Acoustic Analysis of Speech Disorder Associated with Motor Aphasia - A Case Report -

  • Motor aphasia is an affection frequently caused by insult of the left middle cerebral artery and usually accompanied by a large lesion involving the Broca's area and the adjacent motor and premotor areas. Therefore, a patient with motor aphasia commonly shows articulatory disturbances due to failure of the motor programing of speech sound. Objective assessment and treatment of phonologic programing is one of the important aspects of speech therapy in aphasic patients. We analyzed the speech disorders acompanied with motor aphasia in a 45-year-old man using a computerized sound spectrograph, Visi-$Pitch{\circledR}$, and Multi-Dimensional Voice $Program{\circledR}$. We concluded that a computerized speech analysis system is a useful tool to visualize and quantitatively analyse the severity and progression of dysarthria, and the effect of speech therapy.

  • Studies of cry characteristics in the newborn infant were aimed to determine if cry analysis could be succesful in the early detection of the infant at risk for developmental difficulties. Crying presupposes functioning of the respiratory, laryngeal and supralaryngeal muscles. The nervous system controls the capacity, stability, and co-ordination of the movements in these muscles. Hence, the cry provides information about how the Nervous System is functioning. 3 patients(down syndrome, cornelia de lange syndrome, Patent ductus arteriosus) were assessed through a Computerized Speech Lab (CSL). Tests had been chosen to assess Fundamental frequency(mean, maximum, minimum values), Melody contour, NHR, Energy. We compared the data from patients and healthy volunteer. Variations in cry characteristics were documented in a number of medical abnormalities.

  • The human speech contains many redundancies. To economize communication channel or memory size for a computerized synthesis of human voices, it is necessary to compress the data before sending. We have treated human speech organ as an eighth order dynamic system which is time varying as the person speaks. Using an anaylyzer of our design, each eight parameters are obtained for the vowels [아], [어], [오], [우], [으], [이], [애], and (외) of korean language with considerable discrepancies between persons. Supplying those parameters to a synthesizer which we have made, we have sucoeeded in the simulation of human speech for the above mentioned vowels of Korean language and observed that they bear all the features of the original speakers.

  • The aim of this study is to develop an objectively method of speech evaluation for children with cleft palates. To assess velopharyngeal function, Visi-Pitch, Computerized Speech Lab. (CSL), Nasometer and Palatometer were used for this study. Acoustic parameters were measured depending on the diagnostic instruments: Pitch (Hz), sound pressure level (dB), jitter (%) and diadochokinetic rate by Visi-Pitch, VOT and vowels formant ($F_1\;&\;F_2$) by a Spectrography and the degree of hypernasality by Nasometer. In addition, Palatometer was used to find the lingual-palatal patterns of cleft palate. Ten children with cleft palates and fifty normal children participated in the experiment. The results are as follows: (1) Higher nasalance of children with cleft palates showed the resonance disorder. (2) The cleft palate showed palatal misarticulation and lateral misarticulation on the palatogram. (3) Children with cleft palates showed the phonatory and respiratory problems. The duration of sustained vowels in children with cleft palates was shorter than in the control groups. The pitch of children with cleft palates was higher than in the control groups. However, intensity, jitter and diadochokinetic rate of children with cleft palates were lower than in the control group. (4) On the Spectrogram, the VOT of children with cleft palates was longer than control group. $F_1\;&\;F_2$ were lower than in the control group.

Intrinsic Fundamental Frequency(Fo) of Vowels in the Esophageal Speech (식도음성의 고유기저주파수 발현 현상)

  • Background : It has been established that the fundamental frequency(Fo) of the vowels varies systemically as a function of vowel height. Specifically, high vowels have a higher Fo than low vowels. Two major explanations or hypotheses dominate contemporary accounts of fired to explain the mechanisms underlying intrinsic variation in vowel Fo, source-tract coupling hypothesis and tongue-pull hypothesis. Objectives : Total laryngectomy surgery necessiates removal of all structures between the hyoid bone and the tracheal rings. Therefore, the assumption that no direct interconnection exists between the tongue and pharyngoesophageal segment that would mediate systematic variation in vowel Fo appears quite reasonable. If tongue-pull hypothesis is correct, systemic differences in Fo between high versus low vowels produced by esophageal speakers would not Or expected. We analyzed the Fo in the vowels of esophageal voice. Materials and method : The subjects were 11 cases of laryngectomee patients with fluent esophageal voice. The five essential vowels were recorded and analyzed with computer speech analysis system(Computerized Speech Lab). The Fo was measured using acoustic waveform, automatically and manually, and narrow band spectral analysis. Results : The results of this study reveal that intrinsic variation in vowel Fo is clearly evident in esophageal speech. By analysis using acoustic waveform automatically, the signals were too irregular to measure the Fo precisely. So the data from automatic analysis of acoustic waveform is not logical. But the Fo by measuring with manually calculated acoustic waveform or narrowband spectral analysis resulted in acceptable results. These results were interpreted to support neither the source-tract coupling nor the tongue-pull hypotheses and led us to offer an alternative explanation to account for intrinsic variation of Fo.

  • Background : For example, aerodynamic study, vibratory study, acoustic study, neuro-muscular test and psychoacoustic evaluation, a number of objective methods are now available for assessing pathologic voice change. They help to differentiate pathologic condition from normal condition and to monitor pathologic and aging change. These laboratory analyses are used commonly to monitor speech therapy and to follow a patient's recovery after surgery. Objectives : We investigated the values of jitter, shimmer and NNE of normal person and hoarseness patients in Korea. The values of Jitter and shimmer might be meaningful parameters distinguishing pathologic vibration from normal and recovery after surgery. Materials and Methods : Statistical significance between normal control and 48 subjects taken microlaryngeal surgery were compared with Dr. speech science program that is computerized system for acoustic analysis of voice production employed to determine vocal characteristics of pitch perturbation(jitter) and amplitude perturbation(shimmer). Results : The mean normal values of jitter and shimmer were 0.226${\pm}$0.110(%), 2.200${\pm}$0.421(%) in male and 0.164${\pm}$0.060(%), 2.063 ${\pm}$0.575(%) in female. In patients with vocal nodule, the preoperative and postoperative values of jitter and shimmer were valueless. In patients with vocal polyps, the preoperative and postoperative values of jitter and shimmer were valuable. Conclusion : Dr. speech science program was effective to monitor laryngeal disease and aging changes.

  • Many tools have been used to evaluate the voice abnormalities of velopharyngeal insufficiency(VPI). The aim of study was to obtain the objective evaluation method of VPI by comparing the acoustic and nasalance data of experimentally induced VPI group and those of normal control group. Ten healthy young men were included in this study Mild and severe VPI were experimentally induced by retracting velopharyngeal movement. Using the nasometer, we obtained the nasalance score of the sustained oral vowels and those of three types of nasometer passages and the slope scores of nasogram of nasal words. And we analysed the change of formant frequencies for the sustained oral vowels and the changes of various parameters of hyper-tnasality by the computerized speech analysis system. The nasalance score of sustained /a/ was increased significantly in VPI conditions. There was no changes in the slope score of nasogram. On the acoustic speech analysis, the second formant frequencies of vowel /e/ and /i/ were decreased significantly in VPI conditions. This results suggested that the measurement of nasalance score and formant frequency might be useful in the evaluation of VPI.

A study on the determining of vertical dimension of occlusion of edentulous patients using korean phonetic patterns (한국어 음성모형을 이용한 총의치 환자의 교합고경 결정에 관한 연구)

  • This study was performed to offer convenience to determine the vertical dimension of occlusion of edentulous patients by investigating the interocclusal distances at physiologic rest position, at speaking of /m/ sound, and some korean short sounds, that is, /mem/ and /beb/ sounds, which were found in our previous study with dentulous subjects. Ten edentulous subjects - 6 men and 4 women - were selected for this study. The frequencies at speaking of /m/, /mem/, and /beb/ sounds were analyzed with Computerized speech lab($CSL^{TM}$, Model 4300B, Software version 5.X, Kay Elemetrics Co. U.S.A.). And the interocclusal distances at physiologic rest position and at speaking of /m/, /mem/, and /beb/ sounds were measured with K6 diagnostic system(Myo-tronics, Inc. U.S.A.). The results of this study were as follows ; 1. In the acoustic analysis by Computerized speech lab, frequencies of sounds of edentulous subjects with complete denture at speaking of /m/, /mem/, and /beb/ were similar to those of dentulous subjects. 2. In the linear correlation by Pearson's correlation coefficient, the interocclusal distance at physiologic rest position was most similar to those of speaking /mem/ sound, secondly /m/ sound, and thirdly /beb/ sound(p<0.05). In reliability by Cronbach's alpha, the results were reliable with alpha value 0.97. 3. It was found by Levene's test for equality of variance that the difference between men and women in the interocclusal distances at physiologic rest position and at speaking of /m/, /mem/, and /beb/ sounds was not statistically significant(p>0.05).

Voice Analysis of Chronic & Daily Voice Burden in Professionals (직업적인 음성과사용자들의 음성 부담에 대한 평가)

  • Aims of study : The purpose of this study is to measure the chronic and daily voice burden of the professionals in their actual working places. These will be a valuable guideline for preventing and controlling the voice production of professionals. Material and method : Our study was selected to the 10 female telephone operators in the Asan Medical Center, ages ranging from 22 to 38 years old. The symptoms and acoustic analysis of both telephone operators and the controls were evaluated before and after their working. The symptoms were evaluated with questionaires, and the acoustic analysis was measured by using CSL (computerized speech laboratory) system. Results : The symptoms of the professional voice abusers are same as those symptoms in laryngeal fatigue. The acoustic analysis before their working were significantly increased in jitter and shimmer, in comparison with the data of the control. This shows that the experimental group is exposed to the chronic burden of voice production. The jitter, shimmer, and NHR after their working are significantly increased in comparison with the data of the acoustic analysis before their working. This also shows that the experimental group is exposed to the daily burden of voice production. Conclusion : The acoustic analysis of the professional voice overusers has objectively measured that there are chronic and daily overloading to the voice of operators, and these will be a valuable data for preventing and controlling the professionals that abuse their voice.

The Study of Tonsil Affected Voice Quality after Tonsillectomy (편도적출술로 음성변화가 올 수 있는 편도 상태에 관한 연구)

  • Tonsillectomy is the one of operation that is performed the most commonly in otolaryngology field. Many changes that include range of voice, tone, voice quality and resonance were made by tonsillectomy. Sometimes, any patients taken tonsillectomy has suffer from these voice problem after tonsillectomy. However there are less study for these problems until now. Then, we studied to find the anatomical findings that affected the voice quality when tonsillectomy was performed. We evaluated the voice in 2 groups, one is the group showed the normal pharyngeal space by using the transnasal fiberscopy, the other is group showed medially bulging tonsil at pharyngeal cavity by using same method, with perceptual evaluation, nasalance score, nasality, oral formant and nasal formant. We used the computerized speech analysis system, the nasometer and the spectrogram in the CSL program. We could not find any differences in perceptual evaluation between two groups. But objective measures were provided. Nasalance score and nasality on the nasometric analysis were increased significantly and oral formant on the spectrogram was changed singnificantly after tonsillectomy in Group 2. Authors thought medially bulging tonsil in the pharynx is able to affect the voice quality after tonsillectomy when we evaluted through the nasal cavity by the using of fiberscopy and this evaluation would be important especially in singers.
