Computer-Aided Diagnosis in Chest CT (흉부 CT에 있어서 컴퓨터 보조 진단)

  • With the increasing resolution of modern CT scanners, analysis of the larger numbers of images acquired in a lung screening exam or diagnostic study is necessary, which also needs high accuracy and reproducibility. Recent developments in the computerized analysis of medical images are expected to aid radiologists and other healthcare professional in various diagnostic tasks of medical image interpretation. This article is to provide a brief overview of some of computer-aided diagnosis schemes in chest CT.

  • We describe two cases of tension pneumocephalus, one caused by ventriculoperitoneal shunt for communicating hydrocephalus, and the other caused by craniocerebral trauma. In the first case report, we examined the relationship between cerebrospinal fluid leakage and delayed onset tension pneumocephalus. The second case report, we addressed issues such as the diagnosis, management, and pathogenesis, as well as computerized tomography(CT) findings.

Surgical Experience of Pulmonary Blastoma -A Report of a Case - (폐아세포종 수술치험 1례)

  • We experienced very rare case of pulmonary blastoma in a 5 year old girl. She complained of right chest pain and productive cough for 3 months. With computerized tomography and echocardiography it is disclosed that huge mediastinal solid tumor is occupied to whole right thoracic cavity and compressed mediastinal structures to left and extended to left atrium. We removed the thoracic tumor and its extended intracardiac portion completely using the technique of intrapericardial pneumonectomy with cardiopulmonary bypass. Postoperatively the patient recovered without any problem and received adjuvant chemotherapy and radiotherapy. But she was succumbed 2 months later because of opportunistic pulmonary infection with pneumocystis carinii.

  • 안과 실명 질환의 가장 많은 부분을 차지하는 안구 망막의 실시간 3차원 계측기술은 아직 초보단계에 머물고 있다. 다른 장기나 조직에 비해 3차원계측기술이 늦어진 몇 가지 이유를 들면 다음과 같다. 첫째, 망막이 150$\mu$에서 400$\mu$의 얇은 막이어서 기존의 초음파(ultrasonography), CT(computerized tomography)나 MRI(magnetic resonance image)의 해상력으로는 영상화되지 않는다. 둘째로는 망막이 안구내의 뒤쪽에 위치하여 오직 동공을 통해서만 관찰 가능하여 3차원이 영상화가 어렵다. Tomography Reconstruction of Ionospheric Electron Density with Empirical Orthonormal Functions Using Korea GNSS Network

  • In South Korea, there are about 80 Global Positioning System (GPS) monitoring stations providing total electron content (TEC) every 10 min, which can be accessed through Korea Astronomy and Space Science Institute (KASI) for scientific use. We applied the computerized ionospheric tomography (CIT) algorithm to the TEC dataset from this GPS network for monitoring the regional ionosphere over South Korea. The algorithm utilizes multiplicative algebraic reconstruction technique (MART) with an initial condition of the latest International Reference Ionosphere-2016 model (IRI-2016). In order to reduce the number of unknown variables, the vertical profiles of electron density are expressed with a linear combination of empirical orthonormal functions (EOFs) that were derived from the IRI empirical profiles. Although the number of receiver sites is much smaller than that of Japan, the CIT algorithm yielded reasonable structure of the ionosphere over South Korea. We verified the CIT results with NmF2 from ionosondes in Icheon and Jeju and also with GPS TEC at the center of South Korea. In addition, the total time required for CIT calculation was only about 5 min, enabling the exploration of the vertical ionospheric structure in near real time.

  • In oncology various imaging modalities play a crucial role in diagnosis, staging, restaging, treatment monitoring and follow up of various cancers. Stand-alone morphological imaging like computerized tomography (CT) and magnetic resonance imaging (MRI) provide a high magnitude of anatomical details about the tumor but are relatively dumb about tumor physiology. Stand-alone functional imaging like positron emission tomography (PET) and single photon emission tomography (SPECT) are rich in functional information but provide little insight into tumor morphology. Introduction of first hybrid modality PET/CT is the one of the most successful stories of current century which has revolutionized patient care in oncology due to its high diagnostic accuracy. Spurred on by this success, more hybrid imaging modalities like SPECT/CT and PET/MR were introduced. It is the time to explore the potential applications of the existing hybrid modalities, developing and implementing standardized imaging protocols and train users in nuclear medicine and radiology. In this review we discuss three existing hybrid modalities with emphasis on their technical aspects and clinical applications in oncology.

IMAGE FUSION ACCURACY FOR THE INTEGRATION OF DIGITAL DENTAL MODEL AND 3D CT IMAGES BY THE POINT-BASED SURFACE BEST FIT ALGORITHM (Point-based surface best fit 알고리즘을 이용한 디지털 치아 모형과 3차원 CT 영상의 중첩 정확도)

  • Purpose: The goal of this study was to develop a technique for creating a computerized composite maxillofacial-dental model, based on point-based surface best fit algorithm and to test its accuracy. The computerized composite maxillofacial-dental model was made by the three dimensional combination of a 3-dimensional (3D) computed tomography (CT) bone model with digital dental model. Materials and Methods: This integration procedure mainly consists of following steps : 1) a reconstruction of a virtual skull and digital dental model from CT and laser scanned dental model ; 2) an incorporation of dental model into virtual maxillofacial-dental model by point-based surface best fit algorithm; 3) an assessment of the accuracy of incorporation. To test this system, CTs and dental models from 3 volunteers with cranio-maxillofacial deformities were obtained. And the registration accuracy was determined by the root mean squared distance between the corresponding reference points in a set of 2 images. Results and Conclusions: Fusion error for the maxillofacial 3D CT model with the digital dental model ranged between 0.1 and 0.3 mm with mean of 0.2 mm. The range of errors were similar to those reported elsewhere with the fiducial markers. So this study confirmed the feasibility and accuracy of combining digital dental model and 3D CT maxillofacial model. And this technique seemed to be easier for us that its clinical applicability can good in the field of digital cranio-maxillofacial surgery.

  • The development of cone beam computerized tomography (CBCT) allows three-dimensional analysis of the patient's anatomy. The surgical guide is a combination of CBCT, computer-aided design/computer-aided manufacturing (CAD/CAM) and implant diagnostics software, which allows well planned prostheses design and ideal implant placement. Guided surgery minimizes possible anatomical damage and allows for more reproducible treatment planning. In this case, the operation time was shortened by using a surgical guide for multiple implants placement in a fully edentulous patient. Immediate loading were performed more easily using preliminary preparation of provisional prosthesis. The patient was satisfied with improved esthetics and chewing function.

The Comparison of Computerized Tomography Utilization between before and after Coverage of Medical Insurance (전산화 단층촬영의 의료보험 급여전후 촬영건수 비교)

  • We analyzed volume changes in the computerized tomography(CT) and the magnetic resonance imaging(MRI) utilizations. These two volumes are comparative because CT has been covered by the Medical Insurance from the beginning of 1996, however, MRI has not been covered up to now. We investigated 80 hospitals which afforded a facility of CT by the end of 1994. For the comparison of CT utilization, we selected two periods. The first period was from January to June of 1995, in which CT had not been covered by the Medical Insurance and the second period was from January to June of 1966, in which CT had been covered. The information we obtained were characteristics of hospitals, patients numbers per month, monthly uses of CT and MRI, and payments of CT claims, etc. After the Medical Insurance coverage, the number of CT uses per 1,000 patients were increased from January to March, however, was decreased from April to June in 1996 compared to the same periods of 1995. the number of CT uses in clinics were large than those in tertiary hospitals. The number of CT uses in small cities were larger than those in big cities. On the other hand, the number of MRI uses per 1,000 patients did not changed from January to May and was increased from June in 1996 compared to the same periods of 1995. These results showed the substitutional relationship of CT and MRI uses. After the insurance coverage for CT, the utilization of CT was increased because patients payed less than before. However, insurers restricted the payments to the CT claims. This impact might explain the substitutional relationship between CT and MRI.
