Clinical Experience of the Brushite Calcium Phosphate Cement for the Repair and Augmentation of Surgically Induced Cranial Defects Following the Pterional Craniotomy

  • Objective : To prevent temporal depression after the pterional craniotomy, this study was designed to examine the safety and aesthetic efficacy of the brushite calcium phosphate cement (CPC) in the repair and augmentation of bone defects following the pterional craniotomy. Methods : The brushite CPC was used for the repair of surgically induced cranial defects, with or without augmentation, in 17 cases of pterional approach between March, 2005 and December, 2006. The average follow-up month was 20 with range of 12-36 months. In the first 5 cases, bone defects were repaired with only brushite CPC following the contour of the original bone. In the next 12 cases, bone defects were augmented with the brushite CPC rather than original bone contour. For a stability monitoring of the implanted brushite CPC, post-implantation evaluations including serial X-ray, repeated physical examination for aesthetic efficacy, and three-dimensional computed tomography (3D-CT) were taken 1 year after the implantation. Results : The brushite CPC paste provided precise and easy contouring in restoration of the bony defect site. No adverse effects such as infection or inflammation were noticed during the follow-up periods from all patients. 3D-CT was taken 1 year subsequent to implantation showed good preservation of the brushite CPC restoration material. In the cases of the augmentation group, aesthetic outcomes were superior compared to the simple repair group. Conclusion : The results of this clinical study indicate that the brushite CPC is a biocompatible alloplastic material, which is useful for prevention of temporal depression after pterional craniotomy. Additional study is required to determine the long-term stability and effectiveness of the brushite calcium phosphate cement for the replacement of bone.

Effectiveness of anchorage with temporary anchorage devices during anterior maxillary tooth retraction: A randomized clinical trial

  • Objective: This study evaluated the efficiency of anchorage provided by temporary anchorage devices (TADs) in maxillary bicuspid extraction cases during retraction of the anterior teeth using a fixed appliance. Methods: Patients aged 12 to 50 years with malocclusion for which bilateral first or second maxillary bicuspid extractions were indicated were included in the study and randomly allocated to the TAD or control groups. Retraction of the anterior teeth was achieved using skeletal anchorage in the TAD group and conventional dental anchorage in the control group. A computed tomography (CT) scan was performed after alignment of teeth, and a second CT scan was performed at the end of extraction space closure in both groups. A three-dimensional superimposition was performed to visualize and quantify the maxillary first molar movement during the retraction phase, which was the primary outcome, and the stability of TAD movement, which served as the secondary outcome. Results: Thirty-four patients (17 in each group) underwent the final analysis. The two groups showed a significant difference in the movement of the first maxillary molars, with less significant anchorage loss in the TAD group than that in the control group. In addition, TAD movement showed only a slight mesial movement on the labial side. On the palatal side, the mesial TAD movement was greater. Conclusions: In comparison with conventional dental anchorage, TADs can be considered an efficient source of anchorage during retraction of maxillary anterior teeth. TADs remain stable when correctly placed in the bone during the anterior tooth retraction phase.

In-House Developed Surface-Guided Repositioning and Monitoring System to Complement In-Room Patient Positioning System for Spine Radiosurgery

  • Purpose: This study aimed to develop a surface-guided radiosurgery system customized for a neurosurgery clinic that could be used as an auxiliary system for improving the accuracy, monitoring the movements of patients while performing hypofractionated radiosurgery, and minimizing the geometric misses. Methods: RGB-D cameras were installed in the treatment room and a monitoring system was constructed to perform a three-dimensional (3D) scan of the body surface of the patient and to express it as a point cloud. This could be used to confirm the exact position of the body of the patient and monitor their movements during radiosurgery. The image from the system was matched with the computed tomography (CT) image, and the positional accuracy was compared and analyzed in relation to the existing system to evaluate the accuracy of the setup. Results: The user interface was configured to register the patient and display the setup image to position the setup location by matching the 3D points on the body of the patient with the CT image. The error rate for the position difference was within 1-mm distance (min, -0.21 mm; max, 0.63 mm). Compared with the existing system, the differences were found to be as follows: x=0.08 mm, y=0.13 mm, and z=0.26 mm. Conclusions: We developed a surface-guided repositioning and monitoring system that can be customized and applied in a radiation surgery environment with an existing linear accelerator. It was confirmed that this system could be easily applied for accurate patient repositioning and inter-treatment motion monitoring.

CT 영상을 이용한 대퇴체부 휨의 3차원적 곡률 분석 (Three Dimensional Curvature Analysis of Femoral Shaft Bowing based on CT Images)

  • 류마티스성 관절염 등의 관절 질환이나 내반 변형 등을 동반한 환자에게 정형외과적으로 인공슬관절 치환술을 시행하는데 수술용 금속정을 삽입할 때 대퇴체부의 휨 정도로 인해 대퇴피질부에 손상을 주거나 2차 골절 등의 부작용을 유발하기도 한다. 이때 대퇴체부의 중심 좌표값을 알면 금속정이 해부학적 모양의 중심으로 삽입될 수 있어 이러한 부작용을 예방할 수 있을 것이다. 본 연구에서는 인공슬관절 치환술이 필요한 10명의 환자와 대퇴골 질환이 없는 정상군 10명의 대퇴부 단층촬영영상을 이용하여 3차원 좌표값을 찾은 후 Matlab을 이용하여 중심좌표값을 찾아 해부학적 모양을 반영한 그래프를 얻었고, 구간별 3차원 곡률반경 값을 구하였다. 그 결과 실험군의 대퇴골 평균 곡률반경과 표준편차는 $758.15{\pm}206.3mm$로 측정되었고, 정상군은 $1672.97{\pm}395.6mm$로 측정되었다. 측정된 결과는 통계적 유의성 확보를 위해 f-검정을 실행하여 두 집단간 평균의 차이를 검증하였다. 이 결과로 미루어 실험군의 대퇴체부 휨 정도가 더 심함을 확일할 수 있었으며 이러한 영상 분석방법을 이용하여 해부학적 중심점을 찾는다면 인공슬관절 치환술 같은 정형외과적 수술에 도움이 될 것이라 사료된다.

Quality of root canal fillings using three gutta-percha obturation techniques

  • Objectives: The goal of this study was to compare the density of gutta-percha root fillings obturated with the following techniques: cold lateral (CL) compaction, ultrasonic lateral (UL) compaction, and warm vertical (WV) compaction. Materials and Methods: Thirty-three extracted mandibular first molars, with two separate mesial canals in each, were selected. After instrumentation, the canals were stratified into three groups based on canal length and curvature, and underwent obturation with one of the techniques. No sealer was used in order to avoid masking any voids. The teeth were imaged pre- and post-obturation using micro-computed tomography. The reconstructed three-dimensional images were analyzed volumetrically to determine the amount of gutta-percha present in every 2 mm segment of the canal. P values < 0.05 were considered to indicate statistical significance. Results: The overall mean volume fraction of gutta-percha was $68.51{\pm}6.75%$ for CL, $86.56{\pm}5.00%$ for UL, and $88.91{\pm}5.16%$ for WV. Significant differences were found between CL and UL and between CL and WV (p < 0.05), but not between UL and WV (p = 0.526). The gutta-percha density of the roots treated with WV and UL increased towards the coronal aspect, but this trend was not noted in the CL group. Conclusions: WV compaction and UL compaction produced a significantly denser gutta-percha root filling than CL compaction. The density of gutta-percha was observed to increase towards the coronal aspect when the former two techniques were used.

Comparison of Soil Pore Properties between Anthropogenic and Natural Paddy Field Soils From Computed Tomographic Images

  • Human influence on soil formation has dramatically increased with human civilization and industry development. Increase of anthropogenic soils induced researches on the anthropogenic soils; classification, chemical and physical characteristics of anthropogenic soils and plant growth from anthropogenic soils. However there have been no comprehensive analyses on soil pore or physical properties of anthropogenic soils from 3 dimensional images in Korea. The objectives of this study were to characterize physical properties of anthropogenic paddy field soils by depth and to find differences between natural and anthropogenic paddy field soils. Soil samples were taken from two anthropogenic and natural paddy field soils; anthropogenic (A_c) and natural (N_c) paddy soils with topsoil of coarse texture and anthropogenic (A_f) and natural (N_f) paddy soils with topsoil of fine texture. The anthropogenic paddy fields were reestablished during the Arable Land Remodeling Project from 2011 to 2012 and continued rice farming after the project. Natural paddy fields had no artificial changes or disturbance in soil layers up to 1m depth. Samples were taken at three different depths and analyzed for routine physical properties (texture, bulk density, etc.) and pore properties with computer tomography (CT) scans. The CT scan provided 3 dimensional images at resolution of 0.01 mm to calculate pore radius size, length, and tortuosity of soil pores. Fractal and configuration entropy analyses were applied to quantify pore structure and analyze spatial distribution of pores within soil images. The results of measured physical properties showed no clear trend or significant differences across depths or sites from all samples, except the properties from topsoils. The results of pore morphology and spatial distribution analyses provided detailed information of pores affected by human influences. Pore length and size showed significant decrease in anthropogenic soils. Especially, pores of A_c had great decrease in length compared to N_c. Fractal and entropy analyses showed clear changes of pore distributions across sites. The topsoil layer of A_c showed more degradation of pore structure than that of N_c, while pores of A_f topsoil did not show significant degradation compared with those of N_f. These results concluded that anthropogenic soils with coarse texture may have more effects on pore properties than ones with fine texture. The reestablished paddy fields may need more fundamental remediation to improve physical conditions.

Computational analysis of the electromechanical performance of mitral valve cerclage annuloplasty using a patient-specific ventricular model

  • We aimed to propose a novel computational approach to predict the electromechanical performance of pre- and post-mitral valve cerclage annuloplasty (MVCA). Furthermore, we tested a virtual estimation method to optimize the left ventricular basement tightening scheme using a pre-MVCA computer model. The present model combines the three-dimensional (3D) electromechanics of the ventricles with the vascular hemodynamics implemented in a lumped parameter model. 3D models of pre- and post-MVCA were reconstructed from the computed tomography (CT) images of two patients and simulated by solving the electromechanical-governing equations with the finite element method. Computed results indicate that reduction of the dilated heart chambers volume (reverse remodeling) appears to be dependent on ventricular stress distribution. Reduced ventricular stresses in the basement after MVCA treatment were observed in the patients who showed reverse remodeling of heart during follow up over 6 months. In the case who failed to show reverse remodeling after MVCA, more virtual tightening of the ventricular basement diameter than the actual model can induce stress unloading, aiding in heart recovery. The simulation result that virtual tightening of the ventricular basement resulted in a marked increase of myocardial stress unloading provides in silico evidence for a functional impact of MVCA treatment on cardiac mechanics and post-operative heart recovery. This technique contributes to establishing a pre-operative virtual rehearsal procedure before MVCA treatment by using patient-specific cardiac electromechanical modeling of pre-MVCA.

3차원 전산화 단층 사진을 이용한 안면비대칭 환자의 악교정 수술 전, 후 교근 분석 (Analysis of masseter muscle in facial asymmetry before and after orthognathic surgery using 3-dimensional computed tomography)

  • 안면비대칭 환자에서 하악 우각부위의 연조직 형태에 결정적 영향을 미칠 수 있는 교근의 성상에 대한 평가는 경조직의 분석과 더불어 중요하다. 교근은 수술로 인한 하악의 후방이동 시 가장 많은 영향을 받는 구조 중 하나이며, 수술 후 교근의 상태는 환자의 저작력과 하악 우각부 외형에 영향을 줄 수 있다. 본 연구는 안면비대칭을 가진 골격성 III급 부정교합자의 편위, 비편위측 교근의 형태학적 차이와 하악골 후퇴술 이후 양측 교근의 변화를 비교 분석하고자 하였다. 또한 안면비대칭의 개선 전, 후 교근을 정상교합자와 비교하여 비대칭의 수술이 교근에 미치는 영향을 알아보고자 하였다. 안면비대칭으로 진단된 환자 12명의 양악수술 전후의 3차원 CT 영상과 정상교합자 10명의 3차원 CT 영상에서 하악골과 교근을 계측, 분석하였다. 연구 결과 비대칭군에서 교근의 편위, 비편위측 모두 정상교합군에 비해 부피가 작고, 최대 단면적 부위가 좁은 것을 알 수 있었으며, 편위, 비편위측의 교근의 주행각도 차이와 최대단면적 부위에서의 두께 차이가 정상 교합군보다 크게 나타났다. 양악 수술 전, 후에 교근의 주행각도는 유의성 있게 감소하였고, 편위, 비편위측 각도의 차이도 감소하였으며, 최대 단면적 부위에서의 교근의 두께가 유의성 있게 증가하였다. 비대칭 수술 후 좌우 교근은 너비를 제외하고는 정상 교합자와 유의차 없게 변화하였다. 이상의 연구 결과, 안면비대칭 환자는 교근의 성상이 분명히 정상 교합자와는 다르지만, 적절한 수술 후에 경조직뿐만 아니라 교근도 정상범주로 변화하였음을 알 수 있었다.

The influence of root surface distance to alveolar bone and periodontal ligament on periodontal wound healing

  • Purpose: The purpose of this animal study was to perform a 3-dimensional micro-computed tomography (micro-CT) analysis in order to investigate the influence of root surface distance to the alveolar bone and the periodontal ligament on periodontal wound healing after a guided tissue regeneration (GTR) procedure. Methods: Three adult Sus scrofa domesticus specimens were used. The study sample included 6 teeth, corresponding to 2 third mandibular incisors from each animal. After coronectomy, a circumferential bone defect was created in each tooth by means of calibrated piezoelectric inserts. The experimental defects had depths of 3 mm, 5 mm, 7 mm, 9 mm, and 11 mm, with a constant width of 2 mm. One tooth with no defect was used as a control. The defects were covered with a bioresorbable membrane and protected with a flap. After 6 months, the animals were euthanised and tissue blocks were harvested and preserved for micro-CT analysis. Results: New alveolar bone was consistently present in all experimental defects. Signs of root resorption were observed in all samples, with the extent of resorption directly correlated to the vertical extent of the defect; the medial third of the root was the most commonly affected area. Signs of ankylosis were recorded in the defects that were 3 mm and 7 mm in depth. Density and other indicators of bone quality decreased with increasing defect depth. Conclusions: After a GTR procedure, the periodontal ligament and the alveolar bone appeared to compete in periodontal wound healing. Moreover, the observed decrease in bone quality indicators suggests that intrabony defects beyond a critical size cannot be regenerated. This finding may be relevant for the clinical application of periodontal regeneration, since it implies that GTR has a dimensional limit.

In vivo 3-dimensional Kinematics of Cubitus Valgus after Non-united Lateral Humeral Condyle Fracture

  • Background: Nonunion of lateral humeral condyle fracture causes cubitus valgus deformity. Although corrective osteotomy or osteosynthesis can be considered, there are controversies regarding its treatment. To evaluate elbow joint biomechanics in non-united lateral humeral condyle fractures, we analyzed the motion of elbow joint and pseudo-joint via in vivo three-dimensional (3D) kinematics, using 3D images obtained by computed tomography (CT) scan. Methods: Eight non-united lateral humeral condyle fractures with cubitus valgus and 8 normal elbows were evaluated in this study. CT scan was performed at 3 different elbow positions (full flexion, $90^{\circ}$ flexion and full extension). With bone surface model, 3D elbow motion was reconstructed. We calculated the axis of rotation in both the normal and non-united joints, as well as the rotational movement of the ulno-humeral joint and pseudo-joint of non-united lateral condyle in 3D space from full extension to full flexion. Results: Ulno-humeral joint moved to the varus on the coronal plane during flexion, $25.45^{\circ}$ in the non-united cubitus valgus group and $-2.03^{\circ}$ in normal group, with statistically significant difference. Moreover, it moved to rotate externally on the axial plane $-26.75^{\circ}$ in the non-united cubitus valgus group and $-3.09^{\circ}$ in the normal group, with statistical significance. Movement of the pseudo-joint of fragment of lateral condyle showed irregular pattern. Conclusions: The non-united cubitus valgus group moved to the varus with external rotation during elbow flexion. The pseudo-joint showed a diverse and irregular motion. In vivo 3D motion analysis for the non-united cubitus valgus could be helpful to evaluate its kinematics.
