자석 압박을 이용한 다양한 이개 질환의 치료 (Treatment of Various Auricular Diseases Using Magnetic Disks)

  • Purpose: The earlobe is one of the areas which are most vulnerable to trauma. Various auricular diseases need compression treatment. We report a new compression method using magnetic disks. Methods: Seventeen patients with auricular diseases were treated from October 2002 to September 2006. The mean age was 29.1 years. The diseases details were osteochondroma in 2 patients; cauliflower's ears in 2 patients; acute otohematoma in 1 patient; and hypertrophic scars in 11 patients. The most common cause of their disease was ear piercing. The mean follow-up period was 8.9 months. All surgical procedures were performed under local anesthesia. To compress immediately, a pair of magnetic disks was applied to the anterior and posterior surface of the earlobe. Results: The results were generally good. Major complications, such as recurrence, necrosis, dehiscence, or infection, did not occur. Conclusion: A pair of magnetic disks are useful compression tool in various auricular diseases.

Mercerization of Wood: Formation and Reversibility of Na-cellulose I in Reaction Wood

  • The phase transformation from cellulose I into cellulose II in woods by way of Na-cellulose I was examined by x-ray diffraction analysis.The formation of Na-cellulose I in woods increased with the increase of treating time in alkali solution. When compression wood was treated with 20% NaOH solution at room temperature for 1 day, the x-ray diagram showed only Na-cellulose I. On the other hand, the x-ray diagram of tension wood showed a mixture of cellulose I and Na-cellulose I. Cellulose I of tension wood could not be transformed completely into Na-cellulose I even after 10-day treatment, but was transformed into Na-cellulose I after 30-day treatment. Na-cellulose I of compression and tension woods was converted to the cellulose I pattern and the mixture of cellulose I and cellulose II, respectively, after washing with water and drying at 20℃. Cellulose I regenerated from Na-cellulose I in wood could not be converted to cellulose II by delignification. Thus, it revealed that the delignification of the alkali-treated wood did not affect their cellulose structures. From the results, therefore, it can be concluded that lignin in woods prevents the formation of the stable Na-cellulose I and the conversion from cellulose I to cellulose II. This means that the conversion of chain polarity of wood cellulose hardly occurs during mercerization because cellulose microfibrils are fixed by lignin which not to be intermingled.

Mechanical Properties of the Apple Flesh According to the Specimen Size

  • Mechanical properties of the apple flesh were tested with compression test apparatus constructed by this study. The computer program was developed for measuring mechanical properties, and analyzing data obtained from the study. Compression tests on the apple flesh were performed at four levels of specimen diameter, three levels of specimen length, and at constant loading rate(25mm/min). Five replications were made at each treatment combination. Effect of specimen size on the mechanical properties of the apple flesh was investigated.

  • Acupuncture has been widely used to treat a variety of disease and symptoms. But various complications have been reported. Among them, peripheral nerve injuries have been reported less frequently than other complications. The purpose of this report is to describe what we believe to be the first case of delayed superficial and deep peroneal nerve compressive neuropathy caused by fibrotic mass formed by neglected broken acupuncture needle.

  • Stereotactic body radiation therapy (SBRT) appears an effective and safe treatment modality for spinal bone metastasis, which can enhance local control and improve quality of life. Life expectation, predicted fracture risk, localization, quality, size and number of metastasis and presence or absence of nerve compression seem to be important factors in decision-making for treatment. Further studies are needed to identify subsets of patient which will most benefit from treatment.

  • An estimated 250 million people worldwide suffer from lymphedema. In the past, the first-line option for treatment was nonsurgical management, either in the form of compression garments or wrapping, or comprehensive decongestive therapy, with debulking surgery reserved for the more advanced cases. However, with improvements in microsurgical techniques and imaging modalities, surgical intervention is increasingly being utilized. This review highlights recent advancements in the surgical treatment of lymphedema, specifically focusing on improvements in imaging, surgical techniques, and prevention of lymphedema.

Enhancement of Dimensional Stability of Compressed Open Cell Rigid Polyurethane Foams by Thermo-Mechanical Treatment

  • Thermo-mechanical treatment process of a compressed open-cell rigid polyurethane foam (OC-RPUF), which was fabricated for the vacuum insulation panel (VIP), was studied to obtain an optimum condition for the dimensional stability by the relaxation of compressive stress. Thermo-mechanical deformation of the sample OC-RPUF was shown to occur from about $120^{\circ}C$. Yield stress of 0.36 MPa was shown at about 10% yield strain. And, densification of the foam started to occur from 75% compressive strain and could be continued up to max. 90%. Compression set of the sample restored after initial compression to 90% at room temperature was ca. 82%. Though the expansion occurred to about twice of the originally compressed thickness in case of temperature rise to $130^{\circ}C$, it could be overcome and the dimensional stability could be maintained if the constant load of 0.3 MPa was applied. As the result, a thermo-mechanical treatment process, i.e, annealing process at temperature of $130{\sim}140^{\circ}C$ for about 20 min as is the maximum compressed state at room temperature, should be required for dimensional stability as an optimum condition for the use of VIP core material.

열처리에 따른 다공성 알루미늄 합금 재료의 미세구조와 기계적 성질 변화 (Evolution of Microstructure and Mechanical Properties of Porous Al Alloy Under Various Heat Treatment)

  • The relationships between evolution of microstructure and mechanical properties of porous Al-3Si-2Mg-2Cu alloy after the foaming and various heat treating were investigated. The foamed alloy having various densities were manufactured by powder compact foaming and heat treated. Then compression test was performed with deformation rate of 0.5/s. The ultimate compression strength was not changed after solution heat treatment but the flow curve after ultimate strength showed very smooth and uniform plateau region. This change of flow curve means that the deformation mechanism is altered from brittle fracture to ductile deformation and the energy absorption property of Al foam is dramatically improved. The improvement of energy absorption without any detriment of mechanical properties is due to that the very brittle precipitation like Al-Cu and Al-Mg was uniformly dissolved in Al matrix after solution heat treatment. And various mechanical properties of Al alloy porous material were improved by 40% with aging of $200^{\circ}C$ and 50min. These improvements are ascribe to the various fine precipitates like $\Omega$ and $\theta$'.

압박골절 후유증 관리 중 발생 한 섬망 환자 1례 증례보고 (A Case with Unspecified Delirium during Care for Sequela of Compression Fracture)

  • Delirium is a acute syndrome of disorientation caused by dysfunction of brain tissue and has a many varied symtome. It is characteristic of consciousness disturbance with disorientation and dys-mnesia and emotional disturbance. We experienced a 91 year-old woman who had a sequela of compression fracture as well as unspecified delirium, and whose condition was improved through oriental medical treatment. We had given herb medication, acupuncture treatment and decreased medication to this patient, and observed. The patient's delirium symptom had improved through oriental medical treatment and decreasing medication. It is able to improve a delirium caused by not specified origin and side effect on drug abuse through oriental medical treatment and decreasing medication.

골다공증성 추체골절에 대한 경피적 척추성형술 : 초기성적 (Percutaneous Vertebroplasty in Treatment of Osteoporotic Vertebral Body Fractures : Early Result)

  • Objective : Percutaneous vertebroplasty has recently been introduced as an interesting therapeutic alternative for the treatment of thoracolumbar vertebral body fractures in elderly persons with osteoporosis. The authors present the early results of this method. Method and Material : From July 1999 to April 2000, percutaneous transpedicular technique was used in 20 patients (2 men and 18 women) whose mean age was 67.5 years old(range 59-79) with painful vertebral compression(22) and burst(2) fractures. The interval between fracture and vertebroplasty ranged 1 day to 4 months. The procedure involved percutaneous puncture of the injured vertebra via transpedicular approach under fluoroscopic guidance, followed by injection of polymethylmetacrylate(PMMA) into the vertebral body through a disposable 11-guage Jamshidi needle. Result : The most common cause of fracture was slip down and the most frequent injured level was the twelfth thoracic spine. The procedure was technically successful bilaterally in 18 patients(9 thoracic and 15 lumbar spines) with an average injection amount of 7.7ml PMMA in each level. Seventeen(94.4%) patients reported significant pain relief immediately after treatment. Two leaks of PMMA were detected with postoperative CT in spinal epidural space and extravertebral soft tissue without clinical symptoms. Conclusion : Although this study represents the early results, percutaneous vertebroplasty seems to be valuable tool in the treatment of painful osteoporotic vertebral body fractures in elderly, providing acute pain relief and early mobilization.
