Detailed description of malaria in low transmission areas is crucial for elimination. The current study aimed to provide a comprehensive description for malaria transmission in Jazan, a low transmission district, southwestern Saudi Arabia. Patients at a tertiary care hospital were recruited in our study between August 2016 and September 2018. Malaria diagnosis was performed through a species-specific nested polymerase chain reaction (nested PCR), microscopy and Paramax-$3^{TM}$ rapid detection test (RDT). Malaria was detected in 30 patients by the PCR, with point prevalence of 10.9%. Of these malaria infections, 80% was imported, 26.6% was asymptomatic and 23.3% was sub-microscopic. Malaria was reported throughout the year, with February/March and September/October peaks. Infection was significantly more in males than in females (P=0.01). Likewise, infections were detected more in febrile than in non-febrile patients (P=0.01). Adult aged 15-24 years, fever and travel were identified as high-risk factors. Malaria was primarily attributed to Plasmodium falciparum mono-infections, followed by P. vivax mono-infections and lastly to falciparum/vivax mixed infections accounting 76.6%, 16.6%, and 6.6% of PCR-confirmed malaria cases, respectively. The nested PCR was superior to the smear microscopy (sensitivity 76.6%; specificity 100%) and the RDT (sensitivity 83.3%, specificity 94.2%). The overall percent agreement between microscopy and the RDT was 92.7% (kappa=0.63). High proportion of imported malaria including sub-microscopic and sub-patent cases were described. We suggest that incorporation of molecular tool into the conventional malaria diagnosis is beneficial in Jazan district.

Lack of incident reporting culture has been pointed out as a potential risk factor across industries. Consequently, comprehensive understanding of the factors affecting the willingness to report (WTR) is necessary. However, our knowledge on the related studies are not enough to date and even most parts of the existing studies have focused on patient safety, which makes it difficult to universally apply the factors to all industries. Therefore, this study aimed to identify WTR factors in chemical plants and evaluate the influence degree of the factors. To do this, 45 candidate factors were summarized throughout the previous studies and surveying plant safety staff. A questionnaire survey was conducted for 614 all employees from 9 chemical plants which belong to one company, and finally 32 WTR factors were identified throughout 520 responses. Of these, 19 factors were consistent with the previous studies and 13 factors were newly identified. The most influential factor was 'Views on the necessity of incident reporting', and 'Reporting practice by outcome severity', 'Fear of vilification, conflicts, blame, or sanctions' were followed. This result not only suggests various WTR factors suitable for chemical plants, but also shows need to derive specific factors that are appropriate to each industry. An empirical study could be expected to increase incident reporting by using these factors and verify its effectiveness on injury rate.
Recently, it is expected for requirement for more scientific research of weapon systems acquisition alternatives with reliability and objectivity. For these needs, study on methodology of weapon systems acquisition alternatives is but on-going, but it is still in its early stages and is unprofessional. We compared general decision making methods, government development program decision making methods, methodologies of weapon systems acquisition alternatives research such as AHP, ANP, KAAM and proposed AHP as weapon acquisition analysis. This study guarantees objectivity and derives scientific research results reliability by analyzing and comparing comprehensive elements such as cost, effectiveness, deployment schedule, program management risk. Further study on AHP weight and other decision making methods is needed considering category of weapon systems and program characteristics.
Decommissioning efforts are underway at the reactor where the accident occurred, namely the damaged Tokyo Electric Power Company (TEPCO) Fukushima Daiichi Nuclear Power Plant (FDNPP). However, a large amount of groundwater flowing into the site has become contaminated with radioactive substances and is stored in tanks on site, which has hampered the decommissioning work. Although the inflow of groundwater has been greatly reduced through measures such as the construction of frost walls, approximately 170 ㎥ of water treated by the Advanced Liquid Processing System (ALPS) is being stored in tanks, each day. The tanks used to store this treated water are expected to become full by around the summer of 2022. It is not easy to get people to understand the efforts of all concerned parties, and providing clear information to these concerned parties is also a challenge. Questions have also been raised regarding whether other alternatives have been fully explored in the ALPS subcommittee. Some people have commented that the answers to the questions raised regarding the biological effects of tritium transmutation are inadequate. Some suspect that the answers are too detailed and incomprehensible, and that the respondents may be manipulating the public with some malicious intent. In any case, each possible plan presents both advantages and disadvantages, depending on the people who are involved. That makes it an ethical and vexing issue that can sway decisions, as perspectives change. While the environmental release plan is scientifically safe, it may represent a painful alternative. On the other hand, a more careful and imaginative approach to the idea of continued storage in tanks or other forms of storage may reveal some troublesome hidden disadvantages. Under these circumstances, experts must be prepared to answer people's questions in a comprehensive and robust manner.
Purpose: This study aimed to develop an integrated health promotion program for cancer survivors residing in the community based on the shared care model, and evaluate its effectiveness. Methods: A quasi-experimental trial was conducted. The participants consisted of 35 cancer survivors with completed intensive cancer therapy at the cancer hospital. The intervention group (n=20) and the control group (n=15) were recruited from among a district home cancer patient registrations. The intervention group participated in an integrated health promotion program based on the MAPP (Mobilizing for Action through Planning and Partnership) development process. The program consisted of physical, psycho-social and body image units. The participants were assessed before the program, and immediately after the program. Data were collected between July 1 and September 2, 2018 using FACT-G quality of life (QOL), distress thermometer (DT), and resilience. The data were analyzed by performing a χ2 test, Fisher's exact test, Mann-Whitney test, and ranked ANCOVA using SPSS. Results: The intervention group reported a higher QOL overall and significantly higher social/family well-being than the control group. Distress was significantly lower in intervention group than in the control group. Resilience had no significant difference between the two groups. Conclusion: These findings indicate that the integrated health promotion program base on the shared care model and MAPP development process could be effective intervention for improving social/family well-being and the QOL, and reducing distress of cancer survivors at home. Community health center nurses need to provide intervention to support self-care competency for cancer survivors' comprehensive care with physical, psycho-social, and body image to help them adjust their life to a moderate risk group in the community.
Microarray technology represents a critical new advance in molecular cytogenetics. The development of this approach has provided fundamental insights into the molecular pathogenesis in clinical cytogenetics and has provided a clue to many unidentified or unexplained diseases. The approach allows a comprehensive investigation of thousands and millions of genomic loci simultaneously and enables the efficient detection of copy number alterations. The application of this technology has shown tremendous fluidity and complexity of the human genome, and has provided accurate diagnosis and appropriate clinical management in a timely and efficient manner for identifying genomic alterations. The clinical impact of the genomic alterations identified by microarrays is evolving into a diagnostic tool to identify high-risk patients better and predict patient outcomes from their genomic profiles. The transformation of conventional cytogenetics into an automated discipline will improve diagnostic yield significantly, leading to accurate diagnosis and genetic counseling. This article reviews cytogenetic technologies used to identify human chromosome alterations and highlights the potential utility of present and future genome microarray technology in the diagnosis.
This study analyzed how KBS's 'Type and Format', 'Professionalism' and 'Infectious Disease-related Issues' appear in the media news related to Corona-19 under the three periods of 'discovery', 'diffusion' and 'recovery'. According to an analysis of a total of 473 media news, In all three periods, 'Straight Report' and 'Information Delivery Frame' were high to inform the truth of the infectious disease. In addition, the 'transmission of facts' frame was more used than the emotional transmission in reporting the risk situation. However, the proportion of 'moral evaluation frames' in the second phase was relatively high, and the proportion of non-professional journalists was still overwhelmingly higher than professional journalists. Meanwhile, infectious disease-related issues had the most content on 'infection control', and relatively little information on how to deal with them. Based on the above findings, this study suggested the implications of 'Increased in-depth and professional press coverage', 'Refrain from moral evaluation frames' and 'a comprehensive presentation of various infectious disease-related information'.

The quality of subgroup analyses (SGAs) in chronic non-cancer pain trials is uncertain. The purpose of this study was to address this issue. We conducted a comprehensive search in MEDLINE and EMBASE from January 2012 to September 2018 to identify eligible trials. Two pairs of reviewers assessed the quality of the SGAs and the credibility of subgroup claims using the 10 criteria developed by Sun et al. in 2012. The associations between the quality of the SGAs and the studies' characteristics including risk of bias, funding sources, sample size, and the latest impact factor, were assessed using multivariable logistic regression. Our search retrieved 3,401 articles of which 66 were eligible. The total number of SGAs was 177 of which 52 (29.4%) made a subgroup claim. Of these, only 15 (8.5%) were evaluated as being of high quality. Among the 30 SGAs that claimed subgroup effects using an appropriate method of performing interaction tests, the credibility of only 5 were assessed as high. None of the subgroup claims met all the credibility criteria. No significant association was found between the quality of SGAs and the studies' characteristics. The quality of the SGAs performed in chronic pain trials was poor. To enhance the quality of SGAs, scholars should consider the developed criteria when designing and conducting trials, particularly those which need to be specified a priori.
Background: Research in environmental health (EH) is of crucial strategic importance for contemporary society. It is becoming even more critical in light of the increasingly rapid pace of environmental changes, opportunities, and threats. Objectives: This study aimed to identify trends and the prospective of environmental health research using SWOT analysis. Methods: The trends in environmental health research were reviewed in previous studies and reports. Reviewed manuscripts were searched for using the keywords of 'environmental health' and 'environmental hygiene' in the KCI (Korean Journal of Citation Index), KISS (Korean Academic Information), PubMed, and Google Scholar. Results: It is essential to center the EH research agenda around key priorities focusing on technological innovation, job creation, and the increasingly prominent role of the private sector. Given the rapidly evolving global sustainability agenda, greater clarity on the ever-increasing sources of complexity and growing expectations of the public might be needed. This requires the identification of criteria to identify EH research priorities with the ultimate goal of maximizing societal benefit. Public health relevance, such as extent and severity of health impact, level of exposure, and inequalities of effects, could be included. Conclusions: Considering the recent interest in and importance of environmental health, a comprehensive approach to environmental health research should be required through the application of the latest science and technology, citizen participation, and environmental health surveillance systems.
Landslides triggered by the combination of heavy precipitation and anthropological disturbance in hilly areas cause severe damage to human lives, properties, and infrastructure constructions. A comprehensive investigation of the influencing factors and failure mechanisms of landslides are significant for disaster mitigation and prevention. This paper utilized the combination of detailed geological investigation, physical experimental testing as well as numerical modelling to determine the failure mechanism, and proposed a countermeasures of the Lantian landslide occurred on 2, July 2017. The results reveal that the Lantian landslide is a catastrophic reactivated slide which occurred in an active tectonic region in Southwest China. Because of the unique geological settings, the fully to highly weathered basalts in the study area with well-developed fractures favored the rainwater infiltration, which is the beneficial to slide reactivation. Engineering excavation and heavy precipitation are the main triggering factors to activate the slide motion. Two failure stages have been identified in the landslide. The first phase involves a shallow mass collapse originated at the upper slopes, which extends from the road to platform at rear part, which is triggered by excavation in the landslide region. Subjected to the following prolonged rainfall from 19 June to 2 July, 2017, the pore water pressure of the slope continually increased, and the groundwater table successively rise, resulting in a significant decrease of soil strength which leads to successive large-scale deep slide. Thereinto, the shallow collapse played a significant role in the formation of the deep slide. Based on the formation mechanisms of the landslide, detailed engineering mitigation measures, involving slope cutting, anchor cable frame, shotcrete and anchorage, retaining wall and intercepting ditch were suggested to reduce the future failure risk of the landslide.
