Duodenal Perforation: Unusual Complication of Gastrostomy Tube Replacement

  • Feeding gastrostomy is widely used for children with feeding impairment. The replacement of gastrostomy tube is known as an easy and safe procedure. However, various complications associated with replacement of gastrostomy tube were reported, including fistula disruption and colo-cutaneous fistula. For replacement of gastrostomy tube in small children with small stomach, special cautions are needed. Here, we report a rare case of duodenal perforation as an acute complication after the replacement of gastrostomy tube for a 33-month-old girl.

  • Subcortical dementia may occur as a complication of cerebral malaria via deep white matter demyelination with encephalitis and diffuse small vessel vasculitis. In subcortical dementia, impairment in attention and frontal executive function are predominant. Patients are often inert, indifferent, and uninterested. They appear characteristically 'slowed up' with a marked deficit in the retrieval of information. Changes in mood, personality, and social conduct are very common. We describe a case of subcortical dementia, who has definite changes in brain MRI after cerebral malaria.

Dental care for patients taking antiresorptive drugs: a literature review

  • Antiresorptive drugs (ARDs), such as bisphosphonates or denosumab, that prevent bone resorption are widely used in patients with osteoporosis or with cancer that has metastasized to the bones. Although osteonecrosis of the jaw (ONJ) is a well-documented complication of ARD use, the benefits ARDs outweigh the complication. Thus, research has focused on finding ways to prevent or reduce the risk of developing ONJ. Dentists, as part of a multi-professional team, have a critical role in preventing ONJ. However, many dentists tend to hesitate to provide dental care to patients with ONJ, or tend to think that it is a problem to be dealt with by oral surgeons. This review gives an overview of ARD-related ONJ and provides the guidelines for dental care in patients taking ARDs to lower the risk of developing ONJ.

  • The proper management of mandibular fractures involves reduction, rigid fixation, and immobilization to allow bone healing. Nonunion or malunion at the fractured sites is a well-known complication of fracture when the treatments are inappropriate. We present a case of left mandibular fracture due to shrapnel during the Korean War. The patients did not receive appropriate treatment at that time, so nonunion and malunion developed. Sixty years after the accident, mandibular osteomyelitis on the fracture site developed due to dental-origin inflammation. The treatment was based on relatively conservative care, such as saucerization and administration of antibiotics. There was no complication during the short-term follow-up. We present the case with literature review.

  • Coronary stent dislodgement during percutaneous coronary intervention, which occurs when the stent is passed through tortuous and calcified lesions, is not a rare complication. Without proper treatment, such as fixing with another stent in the coronary artery or removing the undeployed stent from the coronary artery or systemic artery system, this complication can cause serious problems. We experienced the unusual situation of a dismounted and dislodged coronary stent, in which retrograde retrieval to the radial artery was impossible during transradial coronary intervention. We report on use of a rendezvous and snare technique at the brachial artery level via femoral puncture, which resulted in resolution without surgery.

  • This research aims to identify important variables and their effects on diabetes prevention or progression from prediabetes to diabetic complication using system thinking. Based on the existing studies, we have constructed a causal loop diagram explaining dynamics of diabetes and have found 7 important reinforcing loops and 3 balancing loops in the causal loop diagram. The CLD displays an effect of increasing prediabetes early detection and awareness on diabetes prevention. The findings indicate that the projects in Korea for reducing incidence of diabetes and potential risk of complication have focused on variables which are related only on diabetes even though prediabetes has been a critical point on diabetes prevention and management. Thus, it also concludes that the effect of projects focusing only on diabetes have faced limitation to manage diabetes accordingly.

  • Radiation-induced lumbosacral plexopathy (RILSP) is an uncommon complication of pelvic radiotherapy that can result in different degrees of sensory and motor deficits. An age 59 female with cervical cancer, who had received combined chemotherapy and radiation therapy two years before, presented with bilaterally symmetric lower-extremity weakness and tingling sensation. The magnetic resonance imaging showed diffuse T2 bright signal intensity and mild enhancement along the bilateral lumbosacral plexus with no space-occupying masses. RILSP was diagnosed after the exclusion of malignant and inflammatory plexopathies.

  • Plantar fibromatosis is a benign fibroproliferative disorder of plantar fascia with rare incidence, but there are locally invasive tendency. There has been no reports of the treatment of recurrent plantar fibromatosis and its complication except only 2 report about the plantar fibromatosis in Korea. Hereby we report three cases of recurrent plantar fibromatosis including treatment and complication.

  • Fine needle aspiration cytology(FNAC) holds a main role in assessing thyroid nodules. But nonnegligible rate of thyroid cytology is reported as uncertain, indeterminate or inadequate for diagnosis. Recently, the microhistologic evaluation by core needle biopsy(CNB) under ultrasound sonographical guidance has been reported to show high accuracy for the diagnose of thyroid nodules. Aim of this review was to furnish the state of the art of this topic by summarizing previous published data about indication, diagnostic performance, and complication of CNB in thyroid lesions compared with FNAC

A Case of Biliary Ascariasis in Korea

  • Biliary ascariasis is still the leading cause of surgical complication of ascariasis, though its incidence has been dramatically reduced. Herein, we report a case of biliary ascariasis for the purpose of enhancing awareness of parasitic infections as a possible cause. A 72-year-old male visited the emergency room of Dankook University Hospital on 12 July 2015, complaining of right-upper-quadrant pain. By endoscopic retrograde cholangiopancreatography (ERCP), a tubular filling defect in the right hepatic duct was detected. The defect was endoscopically removed and diagnosed as an adult female of Ascaris lumbricoides worm, of 30 cm length. Upon removal of the worm, the pain subsided, and the patient was discharged without any complication. When treating cases of biliary colic, physicians should not neglect biliary ascariasis as the possible cause.
