콜라겐을 알긴산으로 교차 결합시킨 인공진피의 창상치유 효과 (The Wound Healing Effects of Alginate- crosslinked Collagen Dermal Substitute of Artificial Skin)

  • Alginate, which is isolated from brown seaweed, is a bioabsorbable long chain polysaccharides, ${\beta}$-D-mannuronic acid and ${\alpha}$-L-guluronic acid. The authors produced alginates and alginate-colllagen as a disc form. Then, to evaluate the efficacy of alginate and alginate-collagen complex as a wound healing material, three full-thickness skin defects of 2 cm in diameter were made at the back of the New Zealand white rabbits. Three groups of dressing materials-alginate, alginate-collagen complex and vaseline gauze as control group - were applied on the wound and the results were evaluated grossly and histopathologically. The authors compared gross findings of sizes of healed wound, wound epithelization and wound contraction by tracing the remaining wound area at 5th, 10th, 15th, 20th, 25th day after wound introduction, and wound biopsy was performed at 3rd, 7th, 14th, 21st day, respectively. Alginate and alginate-collagen complex showed statistically higher percentage of wound contraction and wound healing compared to control group(p<0.05). Alginate-collagen complex showed statistically higher percentage of wound contraction, epithelization and wound healing compared to alginate alone. In conclusion, the result suggests that alginate has a good effect of wound healing and that alginate-collagen complex is more effective in wound healing than alginate alone.

알지네이트 진피지지체 인공피부 개발: 예비보고 (An Developmental Study of Artificial Skin Using the Alginate Dermal Substrate: Preliminary Report)

  • Alginate, a polymer of guluronic and mannuronic acid, is used as a scaffolding material in biomedical applications. The research was to produce highly-purified alginate from seaweeds and to evaluate the efficacy of alginate as dermal substrate. Our alginate purification method showed a production rate as high as 25%. The purified alginate contained little polyphenol contents and endotoxin, proteins. For study of wound healing, full thickness skin defects were made on the dorsal area of the animal models. And then alginate, fibroblast-growth-factor mixed alginate, alginate-collagen complex, vaseline gauze as control were applied on the wound, respectively, and were evaluated grossly and histopathologically. For biocompatibility test, alginate and alginate-collagen complex discs were implanted on the back of Sprague-Dawly rats. Four weeks after implantation, the animals were examined immunologically against alginate and collagen. Alginate and FGF-mixed alginate, alginate-collagen complex group showed statistically higher percentage of wound contraction and wound healing than control group(p<0.05). Alginate-collagen complex group and FGF-mixed alginate group showed statistically higher percentage of wound healing than alginate group. The experiment of biocompatibility and immunologic reaction against impanted alginate or collagen needs more investigation. Highly-purified alginate from seaweeds by our purification method, showed the effect of wound healing, and addition of FGF or collagen increases the alginate's wound healing effect. It shows the possibility of alginate as a dermal substrate.

중·고등학생의 성장과정 상처와 열등감, 성격장애, 정신건강과의 관계 연구 (A Study on the Relationship of Wounds in the Growth Period, Inferiority Complex, Personality Disorder and Mental-Health among Secondary Students)

  • 이 연구는 청소년학생들의 정신건강에 영향을 주는 성장과정의 상처, 열등감, 성격장애 간 관계를 분석하고자 하였다. 최종 분석대상은 중·고교생 2,119명을 표집, 조사·수집된 자료를 분석했다. 차이분석, 중다회귀분석, 경로분석 결과, 성장과정의 상처, 열등감, 성격장애, 정신건강 요인 면에서 여학생들과 고등학생들이, 그리고 가정경제 환경이 곤란한 학생들과 학업성적 수준이 하위권 학생들이 상대적으로 문제가 있음이 의미 있게 밝혀졌다. 열등감에 영향을 주는 성장과정의 상처 요인은 행복감, 안정감, 인정감의 상처 순으로 유의미하게 영향을 주는 것으로 확인되었고, 성격장애에 영향을 미치는 성장과정의 상처 요인은 행복감, 안정감, 자신감, 사랑, 평등감의 상처 순으로 유의미하게 영향을 주는 것으로 밝혀졌다. 또한 정신건강에 유의미하게 영향을 주는 성장과정의 상처 요인은 행복감, 안정감, 자신감, 사랑, 평등감의 상처 순으로 밝혀졌다. 그리고 정신건강에 미치는 관련변인들의 영향력은 전반적으로 성장과정 상처의 직접효과보다 성격장애와 열등감을 통한 간접효과가 높게 나타났다. 이는 성장과정의 상처가 열등감과 성격장애로 이어지고 궁극적으로 정신건강으로 악화될 수 있다는 교육적 시사점과 함께 가정환경의 건강성과 교육적 성장환경의 중요성을 일깨워 준다.

Recent Advancements in Smart Bandages for Wound Healing

  • Wound healing is a complex and dynamic process, making the accurate and timely assessment of skin wounds a crucial aspect of effective wound care management, especially for chronic wounds. Unlike conventional wound dressings that simply cover the wound area once some form of medicine is administered onto the wound, recent studies have introduced versatile approaches to smart wound dressings capable of interacting with wound fluids to monitor physicochemical and pathological parameters to determine the wound healing status. Such electrochemical wound dressings can be integrated with on-demand, closed-loop drug delivery or stimulation systems and ultimately expanded into an ideal technological platform for the prevention, treatment, and management of skin wounds or illnesses. This article briefly reviews the wound healing mechanism and recent strategies for effective wound care management. Specifically, this review discusses the following aspects of smart wound dressings: sensor-integrated smart bandages to detect wound biomarkers, smart bandages developed to accelerate wound healing, and wireless, closed-loop automatic (on-demand) wound healing systems. This review concludes by providing future perspectives on effective wound care management.

Differential Induction of PepTLP Expression via Complex Regulatory System against Fungal Infection, Wound, and Jasmonic Acid Treatment during Pre-and Post-Ripening of Nonclimacteric Pepper Fruit

  • Ripe fruit of pepper (Capsicum annuum) showed resistance to Colletotrichum gloeoporioides, but unripe fruit was susceptible. We previously isolated the PepTLP gene that induced in both unripe and ripe fruit by fungal infection and wound, and only in ripe fruit by jasmonic acid (JA) treatment. To examine further regulation of PepTLP, the action of specific agonist and antagonists of known signaling effector on the .PepTLP expression by fungal infection, wound, and JA was investigated. A similar dephosphorylation event negatively activated all the PepTLP expression in the ripe fruit by fungal infection, wound, and JA. The induction of PepTLP expression by wound is differentially regulated via phosphorylation and dephosphorylation step during pre- and post-ripening, respectively. In addition, the induction of PepTLP expression in the ripe fruit by wound and JA is differentially regulated via dephosphorylation and phosphorylation step, respectively. Only both wound and JA treatment has synergistic effect on the PepTLP expression in the unripe fruit. Both SA and JA treatments on the unripe fruit, and both wound or JA and SA on the ripe fruit could not do any effect on the expression of PepTLP. These results suggest that the induction of PepTLP expression is differentially regulated via complex regulatory system against fungal infection, wound, and JA treatment during pre- and post-ripening of pepper fruit.

Synthesis and optimization of immunomodulating hydrogel for biomedical application

  • Treatment towards wound healing, a complex and dynamic process, has been given a great deal of efforts in the last few decades. Focus has been imposed on developing wound dressings that meet the requirements for proper wound healing. In this study, hydrogel made from blends of poly (vinyl alcohol) and ${\beta}$-1,6-branched-${\beta}$-1,3-glucan (beta-glucan) were synthesized by modified solvent casting method for wound dressing application. Optimization of hydrogel composition and analysis of wound dressing parameters such as stability and fluid uptake capacity (in the presence of water, saline and different pH solutions) has been studied. The result indicated that the PVA/beta-glucan hydrogel hold its structural integrity even at alkaline pH (pH~9) and upholds fluids four times of its original weight. Thus, the developed hydrogel is expected to be a promising candidate as wound dressing.

  • Wound healing is a complex multi-step process that requires spatial and temporal orchestration of cellular and non-cellular components. Hypoxia is one of the prominent microenvironmental factors in tissue injury and wound healing. Hypoxic responses, mainly mediated by a master transcription factor of oxygen homeostasis, hypoxia-inducible factor-1 (HIF-1), have been shown to be critically involved in virtually all processes of wound healing and remodeling. Yet, mechanisms underlying hypoxic regulation of wound healing are still poorly understood. Better understanding of how the wound healing process is regulated by the hypoxic microenvironment and HIF-1 signaling pathway will provide insight into the development of a novel therapeutic strategy for impaired wound healing conditions such as diabetic wound and fibrosis. In this review, we will discuss recent studies illuminating the roles of HIF-1 in physiologic and pathologic wound repair and further, the therapeutic potentials of HIF-1 stabilization or inhibition.

음압 창상 처치(Negative pressure wound therapy)에 대한 문헌적 고찰 (Review of negative-pressure wound therapy)

  • NPWT는 90년대 이후 많은 연구가 이루어지며 발전해왔다. 이 기술은 사용하기에 간단하고 다양한 상처에 적용할 수 있고 효용성과 비용-효과가 뛰어나며 부작용이 적다. 따라서 NPWT를 일반적인 드레싱 대신에 사용하면 육아조직의 빠른 성장과 상처 수축을 통해 피부 이식이나 피판이 필요한 부위를 줄일 수 있고 봉합이 불가했던 부위를 가능하게 하므로 창상 환자에 대한 의료 비용을 감소시키고 만성 창상 환자가 입원하지 않고 움직이면서 창상 관리가 가능하도록 할 수 있는 유용한 방법이다.

고압전기화상 환자에서 손바닥에 비골동맥천공지유리피판수술과 등의 이물질제거수술: 증례 보고 (Peroneal Artery Perforator Free Flap on the Palm and Removal of Back Foreign Body in High Voltage Electrical Burn Patient: A Case Report)

  • Wound caused by high-tension electrical burns is difficult to manage because the wound is deep and complex. The wound is progressively necrotic due to microvascular injury resulting in deep tissue exposure. So, coverage of the wound at the entry point and the exit point is cumbersome, often requiring flap coverage. We experienced a case of one patient for peroneal artery perforator free flap coverage on the palm of the right hand of the entry point of electrical burn. The left foot wound of electrical exit point was covered by full thickness skin graft. Also a small wound was on the left side of the lower back was the exit point of electrical burn. The lower back wound was healed and recurred repeatedly after burn. On postburn day 6 month, through the radiologic exam, metal shadow was identified in the left gluteus muscle forming chronic sinus. We explored the wound of sinus and a foreign body was identified in the sac as multi braid wires thin as hair. According to the patient's past history, we suspected that the back wound was caused by electrical burn injury through the wires.

백서의 창상치유에 미치는 연교액기스의 효과 (The Effect of Forsythia suspensa Extract on the Wound Healing of Rats)

  • 연교액기스가 창상치료제로서의 사용가능성을 검증하기 위하여 연교의 창상치유 효과와 독성에 대하여 조사하였으며 아울러 연교액기스가 항생작용이 있는지도 조사하였다. 연교의 창상치유 효과를 Tetrachlorodecaoxygen anion complex(TCDO) 및 단백질을 제거한 혈액투석액(Haemyl)과 생리식염수의 창상치료 효과와 비교하였다. 4일, 11일, 14일째의 창상면적은 연교액기스, TCDO, Haemyl의 치료에 의하여 대조구인 생리식염수의 치료구에 비해 유의하게 적었다. 그러나 8일째의 창상면적은 다만 연교액기스의 치료구만 유의하게 적었다. 조직학적 검사에서도 연교액기스, TCDO, Haemyl의 치료구가 생리식염수의 치료구에 비해 표피화가 촉진되었다. 간 효소활성도(GOT, GPT), 혈구의 수에 있어서 연교액기스의 투여가 대조구에 비하여 어떤 유의적 차이가 보이지 않았으며 그외에도 연교액기스는 병원균의 증식을 억제하지 않았다. 이런 결과들은 연교액기스가 장차 창상 치료제로서의 개발 가능성이 시사하였으며 그의 약리작용은 항생작용과 무관한 것으로 추정된다.
