Associations between Airborne Manganese and Blood Manganese in the Korean General Population according to KNHANES 2008-2009 (한국인의 혈중 망간농도와 공기중 망간농도의 관련성)

  • The objective of this study was to evaluate associations between airborne manganese and blood manganese in a general population of South Korean adults. The concentrations of airborne manganese in total suspended particulate (TSP) were calculated from data obtained from ambient air-monitoring stations (AAMSs) located in South Korea. Blood manganese data obtained Korean National Health and Nutrition Examination Survey (KNHANES) using a rolling sampling design involving a complex, stratified, multistage, probability cluster survey of a representative sample of the non-institutionalized civilian population of South Korea. Airborne manganese geometric means was 46.10 $ng/m^3$, blood manganese geometric means were 1.19 ${\mu}g/d{\ell}$ for male and 1.40 ${\mu}g/d{\ell}$ for female. In multiple linear regression analysis of log transformed blood manganeseas a continuous variable on airborne manganese, after adjusting for covariates including gender, age, job, smoking and drinking status, education level, BMI (body mass index). Airborne manganese was positively associated with blood manganese with statistical significance. The present study confirms that airborne manganese is a possible contributor to the increase of blood manganese in the adult general population.

Relationship between asthma and dental caries in Korean adults: Data from the Sixth Korea National Health and Nutrition Examination Survey (한국 성인의 천식과 치아우식과의 관련성:제 6기 국민건강영양조사 자료 이용)

  • Objectives: The purpose of this study was to evaluate the association between asthma and dental caries by using data from the Korean National Health and Nutrition Examination Survey (KNHANES), from 2013 to 2015. Methods: The study included 11,731 subjects who were ${\geq}19$ years of age and analyzed their demographic-, socioeconomic-, health-, and oral health care-related data. The chi-squared test and logistic regression analysis were conducted using complex sample analyses to examine the relationship between asthma and dental caries. Results: Compared with the control group, the risk for dental caries in the asthma group was 1.207 times higher, regardless of various confounding factors (p<0.001). In the asthma group, the mean DMFT score was significantly higher than that in the control group (p<0.001). After adjusting for confounding factors, the mean number of DMFT in subjects with asthma was 7.67, which was higher than that in subjects without asthma (7.28) (p<0.001). Conclusions: The study results show that asthma may be a risk factor for dental caries. Therefore, in order to prevent dental caries in asthma patients, oral hygiene education is important.

The relationship between the numbers of natural teeth and nutritional status of elderly in Korea -based on 2007~2009 national health and nutrition survey data- (한국 노인의 자연치아 수와 영양소 섭취상태와의 관련성 -2007~2009년 국민건강영양조사 자료에 근거하여-)

  • Objectives : The aim of this study was to evaluate the relation between the state of dental health(number of natural teeth) and nutritional status of Korean elderly using Korean Dietary Reference Intakes, which was an objective standard for nutritional intake based on database of Korea National Health and Nutrition Examination Survey, large scale of sample obtained by the government. Methods : Complex sampling procedure was used to analyze the fourth data(2007-2009) of Korea National Health and Nutrition Examination Survey. When preparing planning file, the estimator of variance as a stratification variance (variance name : kstrata), population of survey unit as a cluster variance(variance name : PSU), and previous exam and nutritional related weighted as a weighted were analyzed by considering reproduced survey and nutritional related total weighted. Complex samples chi-square test was used to estimate the relation between number of natural teeth and inadequate intake and relation factors included in the model were analyzed by complex samples logistic regression analysis. Results : The group of edentulous had a higher risk to intake less nutrient, except calcium, riboflavin and vitamin C than recommended level comparing to reference group which had natural teeth more than 20(phosphate : OR=1.763; 95% CI=1.273-2.443, thiamine : OR=1.748; 95% CI=1.276-2.395, protein : OR=1.610; 95% CI=1.213-2.138). Conclusions : The number of teeth in Korean elderly over 65 years old had a relation with nutritional status in this investigation. Especially, intake level of nutrients was different between the edentulous group and the reference group. Therefore, dental health care is needed from young and middle age to keep health dental condition for through whole life as well as old age. Although the dental condition of the aged is not good, it is evitable to educate them about the relation between dental health and nutritional ingestion to take balanced nutrition, we think.

The Effects of Working Conditions and Job Satisfaction on the Mental Health and Presenteeism of Early Childhood Teachers (영유아 교사의 근무여건 및 직업만족도가 정신건강과 프리젠티즘에 미치는 영향)

  • Purpose: The purpose of this study was to examine the mental health and presenteeism of early childhood teachers and to explore the effects of working conditions and job satisfaction on these. Methods: Data from the 4thKorean Work Environment Survey were used. The sample consisted of 474 female early-childhood teachers from private facilities. Mental health was measured using the WHO-Wellbeing Index. Presenteeism was attributed to a participant upon indication that, during the past 12 months, she had ever attended work, despite being sick. The data by complex sample design were analyzed through the Rao-Scott $x^2$ test and logistic regression analysis. Results: 37.9% of early childhood teachers reported poor mental health and 34.2% had experienced presenteeism. Early childhood teachers in permanent employment, who did not work evening shifts, and with sustainable jobs, were more likely to have better mental health. Moreover, those in permanent employment and remunerated appropriately were less likely to experience presenteeism. Conclusion: Type of employment, number of days working in the evening, and job sustainability affect mental health. Further, type of employment, number of workers, and being well paid have an effect on presenteeism. We suggest that improved working conditions should precede the improvement of early-childhood teachers' mental health.

Effects of Atopic Dermatitis on Suicidal Ideation, Plans and Attempts in Young Adult (성인 초기의 아토피피부염이 자살생각, 자살계획 및 자살시도에 미치는 영향)

  • Purpose: This study aims to investigate the effects of atopic dermatitis on suicidal ideation, plans and attempts in young adult. Methods: Data are from the Sixth Korean National Health and Nutrition Examination Survey (KNHANES VI, 2013, 2015) in a sample of 2,983 respondents aged 19 to 39 years. Data were analyzed using Frequency, Percentage, $x^2$ test, Complex sample multivariate binary logistic regression analysis with SPSS version 18.0 software. Results: Firstly, factors affecting suicidal ideation are gender, education level, smoking, self-reported health status, asthma, depression and stress perception. Factors affecting suicidal plan are education level, self-reported health status, asthma and depression. Lastly, factors affecting suicidal attempt are gender, education level, smoking, self-reported health status, depression and stress perception. Compared to non-atopic group, the prevalence of suicidal ideation and plan of atopic group increased from odds ratio 2.14 to 2.48 and 1.90 to 2.50, respectively. Odds ratio for suicidal attempt was odds ratio 2.69 (10% error level). Conclusion: The results show that atopic dermatitis in young adult affects suicidal ideation, plans and attempts. Therefore, it is necessary to identify atopic patients who have suicidal thoughts and plans and develop an intervention program to prevent and manage suicide in young adult.

The Effect of the Strength of Physical Activity on Vitamin D Deficiency among Korean Adolescents (청소년의 신체활동 강도가 비타민 D 결핍에 미치는 영향)

  • Purpose: The purpose of this study was to identify the association between physical activity and vitamin D deficiency status among Korean adolescents. Methods: This is cross-sectional analysis of Korean adolescents aged 10-18 years from the 5th Korea National Health and Nutrition Examination Survey(KNHANES 2010-2012; n=2,384). We estimated the prevalence of vitamin D deficiency and identified the correlations with the strength of physical activity. $x^2$ test, t-test, and multiple logistic regression using complex sample analysis were done. The odds ratio of vitamin D deficiency by physical activity was calculated using complex sample multivariate logistic regression analysis. Results: The prevalence of vitamin D deficiency was 78.2%. The mean serum 25 (OH) D level in deficiency group and non-deficiency group was 14.4ng/mL, 23.7ng/mL respectively. Differences by age strata (the prevalence ranging from 68.5% to 86.3%) and gender (78.6% for boy and 84.5% for girl) were identified. Among adolescents aged 16-18 or girl in vigorous physical activity, a significant correlation between vitamin D deficiency and physical activity was observed, whereas no significant correlation were found among adolescents in moderate physical activity. Conclusion: In conclusion, vigorous physical activity was associated with vitamin D deficit in Korean adolescents. Thus, intervention programs enhancing vigorous activity than moderate activity need to be developed in schools and community.

Understanding Complex Design Features via Design Effect Models (설계효과모형을 통한 설계요소의 유용성 이해)

  • Survey research, data is commonly collected through a sample design with complex design features that allow the relative efficiency on the precision of an estimator to be measured using the concept of the design effect compared to simple random sampling as a reference design. This concept is most useful when the design effect can be expressed as a function of various design features. We propose a design effect formula suitable under a stratified multistage sampling by generalizing Gabler et al. (1999, 2006)'s approaches for multistage sampling. Its use can either guide improvement in the design efficiency when in design stage or enable the evaluation of the adopted design features afterwards.

A study of the factors influential on a health-related quality of life using complex sample design (복합표본 설계를 이용한 건강관련 삶의 질에 영향을 미치는 요인에 관한 연구)

  • Using complex sample design, this article analyzes the difference between sex and age groups of mental health, physical activity, suffering lifestyle diseases, drinking, and smoking using the fifth Korea national health and nutrition examination survey data (2011-2012), and then analyzes the effect of mental health, physical activity, suffering lifestyle diseases, drinking, and smoking on EQ-5D, a measure of health-related quality of life. The results show that mental health, physical activity, suffering lifestyle diseases, drinking and smoking are statistically different among gender and age groups, and that age group, education level, suffering diabetes, recognizing stress, thinking suicide are statistically influential on EQ-5D.

Complex Sample Analysis of the Effect of Alcohol Drinking and Smoking Period on Bone Mineral Densities in Korean Men Aged 50 Years and Over (50세 이상 한국 남성의 알코올과 흡연력이 골밀도에 미치는 영향에 대한 복합표본 분석)

  • Purpose: This study aimed to identify the effect of the level of alcohol drinking and smoking period on the occurrence of lumbar and femoral neck osteoporosis in men aged 50 years and over. Methods: This study analyzed the data of 1,410 men based on results of a National Survey on Nutritional Health for the period 2010-2012. Complex sample multi-variate logistic regression analysis was conducted to determine whether alcohol drinking and smoking period were associated with osteoporosis in men aged 50 years older. Results: The risk of osteoporosis in the femoral neck was 3.43 times (95% confidence interval [CI]=1.15~10.29) higher in men who consumed less alcohol and smoked for a long time compared to men who smoked more than 40 years and consumed much alcohol at once. The risk of osteoporosis in the lumbar spine was 3.45 times (95% confidence interval [CI]=1.69~7.04) higher respectively. Men who consumed much alcohol at a time and smoked less than 40 years were 1.54 (1/0.65) times (95% confidence interval [CI]=0.45~0.93) more likely to be classified as normal than the men who consumed much alcohol and had a long smoking period. Conclusion: The interaction of excessive drinking and smoking has a more detrimental effect on bone mineral density.

Effects of Oral Health Behavior and Mental Health on Metabolic Syndrome in Korean Adults

  • Background: Integrated management of general, mental, and oral health is necessary to improve an individual's quality of life. This study aimed to identify the effects of mental and oral health behaviors on metabolic syndrome. Methods: A total of 4,227 Korean adults aged 20 years or older were selected as study subjects using raw data from the first year (2019) of the 8th period of the Korea National Health and Nutrition Examination Survey (KNHANES). A complex sample chi-square test and a complex sample logistic regression analysis were performed using the PASW Statistics ver. 18.0 program. Results: The effect on metabolic syndrome was significantly higher in male than in 1.833 times in male, 2.914 times in 30~49 years old, and 3.855 times in 50~64 years old, and 3.929 times in people over 65 years old compared to 20~29 years old, and compared to those with a college degree or higher, those with a middle school degree or lower are 2.116 times, those with lower income levels are 1.507 times higher, those with middle-lower are 1.359 times higher, those with middle-high are 1.401 times. Compared to non-smokers, smokers were 1.570 times higher than non-smokers and compared to those without speech problem and chewing difficulty, they were 1.717 times and 1.397 times higher, respectively and 1.973 times higher in those with 0~1 brushing times per day. Mental health did not affect prevalence of metabolic syndrome. Conclusion: It is necessary to improve maintain a healthy lifestyle to lower the risk of metabolic syndrome. It is necessary to establish effective dental hygiene customized education and an efficient health management system at the national level that can induce improvement of oral health behavior for the prevention and management of metabolic syndrome.
