Impact of Medical Service Quality by Attribute on Overall Satisfaction -Focused on Out-patient and In-patient in High-level general Hospital- (의료서비스 속성에 따른 품질이 전체만족도에 미치는 영향 -일개 상급종합병원의 외래 및 입원환자를 중심으로-)

  • This study was aimed to identify the current status of medical service quality of medical institutes; propose improvements; and find out the impact of medical service quality by its attribute on overall customer satisfaction. The research was conducted on the out-patients and in-patients in a high-level general hospital located in A city. To examine the research, the IPA was utilized to identify the priority requirements for improving the medical service quality. The study also applied an extended theory to the analysis on mismatch between the level of satisfaction and importance recognized by the clients, to examine the impact of the above mentioned factors on the overall satisfaction and intention to revisit and recommend the hospital. For out-patients, the result showed that "hospital hygiene" had a positive and negative impact on the clients' intention to revisit the hospital in priority improvements. In terms of the sustainable improvements, "doctor's skill" had a negative impact on the overall service satisfaction, whereas "state-of-the-art facility" and "nurse's instruction" had a positive impact on the intention to revisit the hospital. In long-term improvements,"complaint remedy" had a positive impact on the intention to recommend the hospital but there were no relevant factors in excessive investment. On the other hand, for in-patients, the result demonstrated that there were no relevant factors in priority improvements and sustainable improvements. The factor of"service procedure speed"had a positive as well as negative impact on the intention to revisit the hospital. In excessive investment,"nurse's empathy"had a positive impact on the intention to recommend the hospital.

The Effects of MWM Taping and Diamond Taping on the Pain, Grip Strength and Functional Activity in Patients with Lateral Epicondylitis (멀리건 테이핑과 다이아몬드 테이핑이 외측상과염 환자의 통증, 악력, 기능수행능력에 미치는 영향)

  • Background: Lateral epicondylitis is the most common complaint with complex etiological and pathophysiological factors on the lateral side of elbow. Taping techniques commonly used for lateral epicondylitis. The purpose of this study was to investigate the effects of mobilization with movement taping and diamond taping on the pain, grip strength and functional activity in patients with lateral epicondylitis Methods: Twenty patients with lateral epicondylitis (mobilization with movement taping group: n=10, diamond taping group: n=10) were recruited. They were evaluated pre-treatment, after 1weeks, and after 3weeks, using visual analog scale, pain free grip strength test, patient-rated tennis elbow evaluation. Results: Analysis showed statistcally significant improvement in all time in both groups. and The mean improvement in pre-1weeks visual analog scale was significantly greater in the Diamond taping group than that in the mobilization with movement taping group. and the mean improvement in pre-1weeks pain free grip strength test was significantly greater in the mobilization with movement taping than that in the diamond taping group. Conclusion: Taping technique to patients with lateral epicondylitis can help improve pain, grip strength, functional activity and initial taping technique can be selected depending on the patient's condition and the desired goal.

  • Lumbar discs are one of the first parts of the musculoskeletal system to age in the human body. Lumbar disc for females start to age at twenty years of age and completely age between fifty and sixty. The number of lumbar herniated intervertebral disc patients are increasing rapidly; however, studies on protective inner wear are insufficient. This study was conducted to develop protective inner wear equipment that retains an aesthetic appreciation of the body. The methods and procedures of this study are as follows. First, foreign and domestic lumbar pads were researched and analyzed by visiting medical shops and internet shopping malls that sell lumbar pads. Second, the survey was conducted on forty patients with disc disease in hospitals located in Seoul and surrounding suburbs. The following are the study results and observations. First, the lumbar pads selling on the market had a variety of designs, textiles, and fabrics. The result of the research was to develop additional inner protective equipment for lumbar herniated intervertebral disc patients. Second, the survey results of the forty patients with disc disease were that housewives were the most vulnerable. All patients had experience purchasing lumbar pads. The prevention of pain and stress were the most selected responses for the motivation to buy lumbar pads. Most individuals said that they used lumbar pads for less than 1 year. Dissatisfaction came from fabric, activity and design. 'Being uncomfortable to wear' was the most common complaint. 'Protection and how easy the clothes were to wear' were the most selected for the necessity of developments. The Inner wear shape was the most selected in shape. Except for six respondents, all favored the lumbar protective inner wear design.

A Study on Farming Tool-Machinery Injuries in Chonnam Province (전라남도 농촌주민의 농기구 및 농기계 사고에 대한 실태조사)

  • This study was conducted to investigate the incidence and related factors of farming tool-machinery injuries developed in the Chonnam rural area in 1992. A total of 9,068 persons (4,571men and 4,497women)were selected in a stratified cluster sampling manner, and interviewed individually to complete a structured questionnaire covering farmer characteristics, duration of farm working, annual income, kind of farming tools and machinery, type of injuries and medical care. The results were as follows : 1. The incidence rate of farming tool-machinery injuries was 63 per 1,000 persons over all, 73 in males and 54 in females. The age standardized incidence rate was 69 per 1,000 persons over all, 83 in males and 65 in females. 2. The associated factors with farming tool-machinery injuries in univariate analysis were sex, age, duration of farming and monthly income. The occurrence of injuries was higher in men than in woman, and the higher in high-income group than in the low-income group. 3. More men than woman were involved in trunk injuries and treated at the large scale medical care units such as hospitals, and they paid much higher medical fees. 4. The complaint rates for each item of farmers' syndrome in the injured group were higher than those of the non injured group in 'shoulder stiffness', 'sleeplessness', 'dizziness', and 'gastric fullness'. 5. When multiple logistic regression was applied to farming tool-machinery injuries, the significant variables were farmer's syndrome, age, and duration of farming in both males and females.

Enforcement of Arbitration Agreement in the Dispute of Standby Letter of Credit (보증신용장거래 분쟁에서 중재합의의 이행가능성)

  • This article focuses on the enforceability of arbitration agreements m the dispute of standby letter of credit, especially with the case analysis of the leading case from the U.S. Bankruptcy Court. In Nova Hut a.s. v. Kaiser Group International Inc. case, while the underlying contract contained an arbitration clause, a guarantee to assure contractor's performance did not contain an arbitration clause. Nova Hut drew on the standby for the Contractor's failure to deliver contractual obligations. Against the Kaiser's action under US Bankruptcy law, Nova Hut moved to stay the proceedings pending arbitration, to compel arbitration, and to dismiss the complaint. The US Bankruptcy Court for the District of Delaware denied Nova Hut's motions. On appeal, Kaiser argued that it was not subject to arbitration since it was not a party to the contract. It also argued that Nova Hut had waived its right to arbitration by filing a proof of claim in the bankruptcy proceeding and commencing legal actions in other countries. The appeals court noted that in order to avoid arbitration on those grounds prejudice must be shown. It indicated that because there was no long delay in requesting arbitration and no discovery conducted m the course of litigation, the Kaiser could not demonstrate actual prejudice on the part of Owner. In light of public policy favoring arbitration, the nature of the claims in the parties' agreements, Kaiser's conduct in embracing the agreements, and their expectation of benefit, the appeals court ruled that the doctrine of equitable estoppel applied in requiring the Parent to arbitrate.

A Study on the Effect of Waiting Time factor of Hospitals on Customer Satisfaction and Service Values (의료서비스의 대기시간 요인이 고객만족과 서비스 가치에 미치는 영향)

  • One of the important complaint factors by the patients using medical institutions is various kinds of waiting time. This research is about the causal relationships among waiting time(application of waiting time, perceived of waiting time, fairness of waiting, explanation of the delay, acceptance possibility), service values, and customer satisfaction. The subjects for this study were 256 out-patients currently using seven general hospitals in the Busan area. The findings of the empirical analysis are as follows: First, the good application of waiting time or fair waiting time procedure didn't actually affect perceived waiting time. Second, though the application of waiting time didn't affect acceptance possibility, the fair procedure of waiting time had an affirmative effect on acceptance possibility. Third, even when there was enough explanation about the need to wait, it could shorten perceived waiting time but failed to get sufficient sympathy or agreement from the patients. Fourth, the possibility of acceptance following the sufficient sympathy of patients had an effect on shortening perceived waiting time. Fifth, perceived waiting time lowered customer satisfaction, but had no negative effect on service values. Finally, the ranges of acceptance possibility by the patients were found to increase customer satisfaction and maximize service values. In conclusion, every medical institution needs to focus on, first of all, shortening waiting time for more customer satisfaction and improved service values.

What are the Endoscopic and Pathological Characteristics of Colorectal Polyps?

  • Background: Colon polyps need to be excised upon detection during colonoscopy due to the risk of malignancy irrespective of their size. In our study, we retrospectively evaluated the clinicopathological characteristics of polyps detected during colonoscopy. Materials and Methods: We assessed 379 patients with polyps detected during colonoscopy between January 2010 and May 2012. The demographics, complaints, colonoscopy findings (shape, place and size of the polyp) and histopathological findings were recorded. We carried out statistical analysis using PASW 18.0 for Windows. Results: There were 227 males (59.9%) and 152 females (40.1%) in the trial. The mean age was 53.8 years (32-90). The most common complaint was rectal bleeding (36.1%), followed by abdominal pain (35.4%). Polyps were detected most commonly in the rectosigmoid region (43.8%), followed by the descending colon (17.4%). Some 239 patients had a single polyp (63.1%) while 140 were found to have multiple polyps (36.9%). While tubular adenoma was the most common pathological type, occurring in 181 patients (47.8%), tubulovillous adenoma (14.2%) and hyperplastic polyp (12.7%) followed, occurring in 54 and 48 patients respectively. While 313 patients (82.6%) did not feature dysplasia, 37 patients (9.7%) exhibited low-grade dysplasia, 28 (7.7%) had high-grade dysplasia and 4 had cancer (1.1%). The rates of villous components and dysplasia were detected to be high among pedunculated polyps and polyps larger than 1 cm (p<0.001). Conclusions: Due to the fact that large-diameter polyps with malignant potential are commonly located in the left colon and have a high prevalence among the middle-aged individuals, it would be appropriate to screen this population at regular intervals via rectosigmoidoscopy.

Comparison between Radiological and Invasive Diagnostic Modalities in Diagnosis of Breast Cancer

  • Background: Breast cancer is the most common cause of deaths of cancer in women. Nowadays, following completion of imaging methods, mainly fine needle aspiration biopsy (FNAB) and core biopsy methods have been used for establishing cytopathological diagnosis although discussions regarding superiority continue. Materials and Methods: Those with a complaint of "mass in breast" along with those diagnosed to have a mass as a result of routine physical examination among all patients applying to our clinic between 01.01.2009 and 31.12.2011 were retrospectively assessed. Totals of 146 and 64 patients with complete radiological observation who had undergone FNAB and core biopsies, respectively, were evaluated. Postoperative pathological results of patients of both groups receiving surgery were also taken into consideration. All results were compared in terms of false positivity/negativity, sensitivity/specifity, surgery types and distribution of postoperative results with regard to diagnoses along with those of malignant/benign masses with regard to quadrants determined. Results: Diagnostic malignancy power of mammographic BIRADS classification was 87.3%. However, the value was 75% in the core biopsy group. Sensitivity and specifity following comparison of FNAB and postoperative pathology results of those receiving surgery were 85.4% and 92.9% while they were 93.5% and 100% in the core biopsy group. Diagnostic malignancy power, calculated by determining AUC in ROC analysis, of FNAB was 89.1% while that of core biopsy was 96.7%. Conclusions: It was shown that core biopsy is superior to FNAB in terms of sensitivity, specificity and accurate histopathological classification. However; quick, cheap and basic diagnosis by means of FNAB should not be ignored. Sensitivity of FNAB is rather high in experienced hands and furthermore it would be expected to be lower than with core biopsy.

Analysis of Hotel Customer Complaint and Recovery Strategy Using Critical Incident Technique (결정적 사건기법을 이용한 호텔 고객불평과 복구전략 분석)

  • A critical incident technique(CIT) was employed to show the results of empirical findings regarding hotel services. The major purpose of this study was to describe and analyze service failures from the customers' point of view and thus suggest strategic implications for hotel service providers. Four-hundred sixteen service failure anecdotes from hotel customers were classified using Hoffman et al.'s(1995) approach. The CIT data showed the three major categories and eleven sub-categories and revealed that, in general, tangible recovery(e.g., upgrade, free meal, discount) was more effective than intangible one(e.g., explanation, canned apology, manager's apology). Interestingly, however, sincere apology seemed to be a better recovery strategy than other types of tangible recovery. Furthermore, the results showed that severity of service failure had its own impact on customers' perception regarding hotel's service recovery efforts. In particular, irrespective of service recovery, severity of service problem was negatively related to intention to repurchase. Managerial implications and future research agenda was also discussed.

A Clinical Study of the Medistinal Tumors and Cysts -27 cases analysis (종격동 종양 및 낭종 27례에 대한 임상적 고찰)

  • We have experienced 27 cases of primary medistinal tumors and cyst from April, 1986 to April 1996. At Masan Samsung General Hospital. The results of the anslysed were as follows 1. Of 27 medistinal tumors and c,pests, 9 patients were male and 18 patients were female(m : f= 1 : 2) 2. The most common chief complaint was chest discomfort or pain(8 cases 30 %) and 17 patients(63%) were asymptomatic. 3. The most common primary medistinal tumor was Neurogenic tumor in 9 cases(33 %) followed by Thymoma in 7 cases(26 %), teratoma in 6 cases(22 %) 4. The incidence of malignancy of all case was 3 cases(11 %), all cases were symptomatic and the most common malignancy was malig. thymoma(3 cases,43 %) 5. The anterior mediastinum was the most common tumor location with 15 cases(56 %) followed by posterior 11(41 %) and middle mediastinum 1(4 %). Anterior mediastinum tumors were predominantly thymomas and teratoma and posterior mediastinal tumors were neurogenic tumors 6. Complete removal of tumor was achieved in 26 cases(96 %) and open biopsy was done on 1 case 7. Postoperative complications were continued lumbar shunt drainage in 1 case and wound infection in 1 case 8. There was no case of postoperative mortality and good clinical course in surgically completely rejected cases
