목적: 주관절 전지환 성형술의 최근 수술 결과와 합병증에 대하여 문헌을 토대로 기술하고자 한다. 대상 및 방법: 주관절 전치환술의 적응증은 다른 수술 방법으로 관절 기능의 회복이 불가능한 류마토이드 관절염, 외상후 관절염, 관절 강직, 종양 제거 후 상태, 그리고 고령의 원위 상완골 분쇄 골절 등이 있다. 합병증으로는 가장 심각한 합병증인 감염을 비롯하여 삽입물 이완, 삽입물 골절, 삽입물 주위 골절, 척골 신경병증, 이소성 골화, 상완 삼두근 손상, 탈구를 포함한 불안정, bushing의 마모 등이 있다. 결과 및 결론: 재치환술을 기준으로 한 주관절 치환술의 수명은 10년을 기준으로 85% 정도인 것으로 알려져 있다. 염증성 관절염이 가장 예후가 좋으며 외상후 관절염의 이완율이 비교적 높다. 합병증은 다른 관절의 치환술에 비해 많이 발생하는 경향이 있으며 특히 심부 감염은 3~5% 정도로 높은 편이다. 주관절 치환술은 재건술로 관절 기능을 회복할 가능성이 없는 활동력이 높지 않은 환자에 대해 신중하고 적절하게 시행한다면 만족스러운 기능 회복을 기대할 수 있다.
Purpose: The purpose of this study was to analyze the characteristics and severity of wrist injuries in snowboarding. Methods: December 2005 to February 2008, Snowboarders who experienced wrist injures were included in this study. On the basis of the medical records and radiographic evaluation, the severity of distal radius fracture was classified according to the Arbeitsgemeinschaft fur Osteosynthesefragen/Association for the Study of Internal Fixation (AO/ASIF) classification. Results: Most of the injured snowboarders were a either of the beginner (35 cases, 46.1%) or the intermediate (27 cases, 35.5%) level. The most common cause of injury in snowboarding was a slip down (60 cases, 78.9%). Comminuted and articular fractures classified as AO types A3, B, and C, which required surgical reduction, made up 42.3% of the distal radial fractures in snowboarders. When we analyzed the differences in severity between the educated and the non-educated groups, an A2 type injury in the AO classification was the most common type of injury in the educated group (20 cases, 38.5%), it means less severe fractures ocurred in the educated group (p=0.045). The most frequent injury mechanism of fractures was slip down (48 cases, 63.2%), and a slip down backwards was the dominant type of slip down (36 cases, 75.0%) (p=0.031). Conclusion: Among the snowboarders in this study who suffered self-down injury to the wrist, more fractures were associated with a backwards slip down than with a forward slip down due to over extension. For educated snowboarders the severity of fracture was lower than it was for uneducated snowboarders.

Background: Increased frequency of comminuted clavicle mid-shaft fractures and importance of functional satisfaction through early joint exercise has resulted in higher emphasis on surgical treatments. This study aimed to evaluate the clinical radiological results of treatment of clavicle mid-shaft fractures by open reduction and internal fixation using a plate with a small incision. Methods: The subjects of this study were 80 clavicle mid-shaft fracture cases treated with internal fixation using a plate from October 2010 to July 2014. Clavicle mid-shaft fractures were internally fixated using anatomical plates or locking compression plates. Achievement of bone union, union period, and clavicle length shortening were evaluated radiologically, and clinical assessment was done by using Constant and University of California at Los Angeles (UCLA) scores. Results: All 80 cases were confirmed to have achieved bone union through radiographs with an average union period of 10.9 weeks (range: 7-18 weeks). The average clavicle length of shortening in the affected side was 1.8 mm (range: 0-17 mm). The average UCLA score and Constant score were 33.6 (range: 25-35) and 92.5 (range: 65-100), respectively. Regarding complications, four cases reported skin irritation by metal plates, and one case reported a screw insertion site fracture due to minor trauma history. Conclusions: We were able to induce successful bone union and obtain clinically satisfactory results in displaced mid-shaft fractures of the clavicle without major complications such as nonunion through treatment of internal fixation using a plate.
목적: 견갑골 관절와 골절의 치료로 관혈적 수술을 시행했던 예들을 후향적으로 분석하여 치료 결과에 미치는 요인을 알아보고자 하였다. 대상 및 방법: 1999년 3월부터 2004년 2월까지 관혈적 정복술을 시행한 관절와 골절 9예를 대상으로 하였다. 골절의 분류는 변형 Ideberg씨 분류를 이용하였으며 제 1형이 1예, 2형 3예, 3형 2예, 5형 1예, 6형 2예였다. 정복에 사용된 내고정물은 재건 금속판 6예, 소형 금속판 1예, 1/3 원통형 금속판 3예, 소형 나사 3예, 유관 나사 1예였다. 수술 후 견관절의 기능 평가는 Constant score 및 Adam의 기능 평가 방법을 이용하였다. 결과: 수술 후 평균 7주에 골유합 소견을 보였다. 임상적 평가에서 우수가 4예, 양호가 3예, 보통이 2예 였다. 수술 후 합병증으로 나사의 관절와 침범, 관절와 하방에 안정성에 지장이 없는 골소실이 각각 1예가 있었다. 결론: 관절의 불안정성이 있거나 골편의 전위가 큰 관절와 골절의 경우에 수술적인 치료로 양호한 결과를 얻을 수 있었고 골절 양상이 분쇄가 심할수록 나쁜 결과를 보였다.
Purpose: Bone grafting is often necessary to maintain a reduction and prevent delayed collapse of reduced fracture in a treatment of severely displaced comminuted intra-articular calcaneal fractures. Herein, we analyzed the usefulness and necessary conditions to perform tricortical-allobone grafting in open reduction of calcaneal fracture via the Ollier approach. Materials and Methods: We performed a retrospective review of 57 intra-articular calcaneal fractures that underwent an operation via the Ollier approach between April 2009 and April 2015. They were divided into two groups: Group 1 (n=17) included those with tricortical-allobone grafts underneath the posterior facet fragment, and group 2 (n=40) included cases without a bone graft. We measured the $B{\ddot{o}}hler$ angle, Gissane angle, height, and width of the calcaneus at preoperative, postoperative, and final follow-up radiograph. We measured the sagittal rotational angle of the posterior facet fragment of preoperative computed tomography to analyze the effect and necessary conditions for bone grafting. We also reviewed the clinical results by the American Orthopaedic Foot and Ankle Society (AOFAS) scale, visual analogue scale (VAS), and any complications. Results: According to the Sanders classification, there were 3 type-II fractures, 12 type-III fractures, and 2 type-IV fractures in Group 1; whereas in Group 2, there were 26 type-II fractures, 13 type-III fractures, and 1 type-IV fracture (p=0.002). Regarding the preoperative radiologic parameters, there were significant differences in the $B{\ddot{o}}hler$ angle (p=0.006), Gissane angle (p=0.043), and rotational angle of the posterior facet fragment (p=0.001). No significant difference was observed in the preoperative calcaneal height and width, as well as postoperative radiologic parameters. There was no significant clinical difference between the two groups (p=0.546). Conclusion: We suggest that a tricortical-allobone graft may be useful in open reduction and screw fixation via the Ollier approach for displaced intra-articular calcaneal fracture with a bony defect after reduction of collapsed posterior facet fragment. This graft can contribute to the stable reduction via a small approach, even without a plate.
목적: 근위 상완골 골절의 치료 방법은 일반적으로 Neer 분류법에 의하고, 심하게 전위된 3분 골절 및 4분 골절의 경우 상완골두 치환술의 적응증이 된다. 저자들은 근위 상완골의 분쇄 골절로 인하여 상완골두 치환술을 시행 받고, 최소 1년 이상 추시 가능하였던 10례의 단기적 임상적 결과를 보고 하고자 한다. 대상 및 방법: 저자들은 1999년 7월부터 2005년 3월까지 상완골 근위부 골절로 상완골 두 치환술을 시행 받은 환자 중 최소 1년 이상 추시 가능 하였던 10명(10 견관절)의 환자를 대상으로 하였다. Neer 분류법에 의하면 5명은 3분 골절이었고, 나머지 5명은 4분 골절이었으며 외상 후 수술까지 소요 시간은 평균 6.1일 이었다. 남자 4명, 여자 6명이었고, 평균 연령은 67.4세(최소 56세, 최고 76세)였다. 술 후 견관절 기능은 Constant score와 SST(Simple shoulder test) 및 UCLA 평가법(modified UCLA score for hemiarthroplasty)을 이용하여 평가하였다. 결과: 최종 방문 시 Constant 점수는 평균 51.4(최저 34점, 최고 60점)점이었고, 변형된 SST의 경우는 12개 문항 중 평균 7.8개 문항이 가능하다고 하였다. 수상 당시 액와동맥 파열 및 상완신경총 손상이 있었던 1예를 제외한 경우 Constant score는 평균 53.5(최저 44점, 최고 60점)점 이었고, SST의 경우는 12개 문항 중 평균 7.2($1{\sim}8$)개 문항이 가능하다고 하였다. 최종 추시 UCLA score는 동통, 기능성, 근력 및 운동 범위 항목 각각이 평균은 8.2($6{\sim}10$)점, 6.6($2{\sim}8$)점, 6.9($4{\sim}8$)점 이었고, 총점 21.7($12{\sim}26$)점 이었다. UCLA 평가법에 의한 결과는 우수 3예, 양호 6예, 불량 1예 이었고 불량의 경우는 혈관 및 신경 손상이 있었던 1예 이었다. 환자의 주관적 만족도는 10명의 환자 중 2명의 환자는 흡족, 7명의 환자는 양호한 결과라고 하였으나, 1명의 경우는 불만이었다. 결론: 단기 추시 결과이지만, 상완골 두 치환술은 관혈적 정복술이 어려운 상완골 근위부 골절에있어서 일차적 치료방법이라고 생각된다. 상완골 두 치환술은 고령의 환자에게 있어서 강직을 예방할 수 있고 일상적인 생활이 가능하도록 하는 수술 방법이나, 관절운동 범위 및 근력의 회복은 충분하지 않았다.
1989년 12월부터 1992년 1월까지 영남대학교 의과대학 부속병원 정형외과에서 치료한 대퇴골 과상부 골절 환자중 1년 이상 원격 추시 가능한 20례를 대상으로 다음과 같은 결론을 얻었다. 1. 활동성이 많은 10-40대가 전체의 85%를 차지하였으며 남자가 70%를 차지하였다. 2. 원인으로는 교통사고가 75%, 추락사고가 15%였다. 개방성골절은 천체의 35%였으며 교통사고 군에서 약 60%를 차지하여 강력한 외상에 의한 것으로 추정되었다. 3. 골절분류는 ASIF group에 의한 분류를 하였으며 Type A가 4례, Type B가 4례, Type C가 12례로 이중 $C_2$, $C_3$가 10례로 분쇄상의 정도 및 관절면 침범정도가 심한 골절의 형태가 많았다. 4. 동반손상은 다발성 골절이 9례로 가장 많았으며 대퇴 동맥 손상이 1례 있었다. 5. Schatzker criteria에 따르면 보존적 치료 결과는 40%에서, 수술적 치료결과는 67%에서 만족할 만한 결과를 얻었다. 6. 술후 합병증으로는 관절강직 및 지연 유합등이 있었으나, 관절강직이 대부분이었고 골절부의 심한 손상으로 조기 관절 운동이 불가능 했던 경우에서 주로 발생하였다.
목적: 방사선 소견에서 슬개골 골절이 발견되었을 때 치료 방침 결정에 초음파 영상의 임상적 유용성을 알아보고자 하였다. 대상 및 방법: 2006년 3월부터 2009년 9월까지 슬개골 골절로 치료받은 20명 20예를 대상으로 하였으며, 남자가 8예, 여자가 12예였고, 우측이 13예, 좌측이 7예였으며, 평균 연령은 43세(14~72세)였다. 초음파를 이용하여 굴곡-신전시 슬개 지대 파열 여부를 보는 Drayer 검사와 연계하여 치료 방침을 결정 하였다. 횡 골절이 10예, 종 골절이 8예, 분쇄 골절이 2예가 있었으며, 초음파를 이용하여 슬개 지대의 파열 유무와 Drayer 검사가 양성인지 음성인지를 보았다. 결과: 슬개 지대가 파열되지 않은 12예의 경우 전예에서 Drayer 검사는 음성이었으며, 별도의 고정 없이 보행을 허용하여 보존적 치료를 하였다. 슬개 지대가 완전 파열된 4예는 Drayer 검사가 양성이었으므로 관혈적 정복술을 시행하였고, 슬개 지대가 부분적으로 파열되어 Drayer 검사가 음성이었지만, 골절의 전위가 2 mm 이상인 2예는 비관혈적 나사 고정술로 좋은 임상적 결과를 보였다. 결론: 슬개골 골절에서 초음파는 방사선 소견, Drayer 검사와 연계하여 치료 방침을 결정 하는데 유용한 검사 방법 중의 하나로 생각된다.
발목 병변을 관찰하는 단순 방사선촬영은 여러 가지가 있다. 그 중에서 Ankle Broden법은 발꿈치뼈(Calcaneus) 및 발목뼈(Ankle Joint)의 결합상태와 발목뼈의 발꿈치뼈 골절(Calcaneus Fracture), Subtentaculum Tali Frature를 관찰하기 위하여 촬영을 한다. 이 촬영법은 전 후면과 측면뿐 아니라, 사면 상(oblique view)이나 축 상(axial view)의 촬영을 하며 잦은 부상이 발생하는 목말뼈 발꿈치뼈간, 발꿈치뼈 발배뼈간 결합상을 보기 위해 주로 촬영을 한다. 본 연구는 석고처치 환자들은 발목관절이 영상에 잘 묘출되기가 어렵기 때문에 발목뼈 사이의 관계 및 연부조직의 구조적 변화와 Subtalar joint, Calcaneus Fracture, Subtentaculum Tali Frature에 따른 목말뼈 발꿈치뼈의 골 결합 (talocalcaneal coalition)을 관찰하기 위한 Harris-Beath View($30^{\circ}{\sim}55^{\circ}$) 촬영 시 평가에 유용한 발목뼈의 결합상을 얻을 수 있는 각도를 알아보고자 하였다. 연구의 진행은 40명의 환자에게서 획득한 영상을 평가하였으며, 평가결과 $25^{\circ}$의 촬영상은 목말밑 관절(subtalar joint)이 열려져 분리되지 않았고 또한, Subtentaculum Tali Frature에 중복되어 보였다. $30^{\circ}$의 촬영상은 발꿈치뼈 골절(Calcaneus Fracture), Subtentaculum Tali Frature, 목말밑관절(subtalar joint과 후방 관절면의 앞부분이 가장 잘 나타나고 Calcaneo Navicularcoalition, Talocal Canealcoalition, Naviculo Cuneiform coalition의 결합상태가 명확하게 보였다. $35^{\circ}$의 촬영상은 목말밑관절(subtalar joimt), 목말뼈(talus), 발뒤꿈치뼈의 뒤쪽관절면이 명확하고 발목발꿈치관절(talocalcaneal joint)의 분리정도가 좋고 발목뼈굴(sinus tarsi)이 넓게 나타나며 좋은 평가가 되었다. $45^{\circ}$영역에서는 연부조직과 석고 분리를 판별할 수 있었고 가쪽복사와 뼈 밀도가 명확하게 보였다. $35^{\circ}$에 비해 발꿈치뼈(Calcaneus)이 분리되었지만 영상이 왜곡되어 보였다. Calcaneus, Subtentaculum Tali Frature는 $25^{\circ}$에서 $1.20{\pm}0.414$, $30^{\circ}$에서는 $2.47{\pm}0.516$, $35^{\circ}$에서는 $2.87{\pm}0.352$, $45^{\circ}$에서는 $2.27{\pm}0.458$로 나타났고 통계적으로 유의한 차이가 나타났다(p<0.05). 왜곡의 정도를 포함시키면 $30^{\circ}$영역의 왜곡이 적게 나타났고, $40^{\circ}$ 영역은 심한 왜곡현상을 보였다. 따라서 Ankle(Broden)촬영에서 $30{\sim}35^{\circ}$상이 가장 좋은 영상이었으며, $30{\sim}40^{\circ}$상에서는 Calcaneus Fracture, Subtentaculum Tali Frature의 진단에 가장 좋은 영상을 묘출하였다.
Purpose: This study was performed to analyze the characteristics of calcaneocuboid joint involvement in intraarticular calcaneal fractures. Materials and Methods: Total number of 92 patients (111 cases) who underwent operation for intraarticular calcaneal fractures between Jan. 2000 and Oct. 2005 were included in this study. The preoperative computed tomographs of the subjects were retrospectively reviewed to analyze calcaneocuboid joint involvement. Results: It was revealed that 63 cases (56.8%) involved calcaneocuboid joint; 29cases (46.0%) showed type 1 (undisplaced or minimally displaced type, articular gap ${\le}1\;mm$), 16 cases (25.4%) exhibited type 2 (moderately displaced type, articular gap ${\ge}2\;mm$), 7 cases (11.1%) were included in type 3 (comminuted type) and 11 cases (17.5%) belonged to type 4 (fracture and dislocation). 48 out of 63 cases belonged to Sanders classification II and III that involved calcaneocuboid joint and included 25 cases (52.1%) of type 1 and 14 cases (29.2%) of type 2. Among 15 out of 63 cases included in Sanders classification IV, 4 (26.7%) showed type 1 and 6 (40.0%) belonged to type 4. According to our results, Sanders classification allowed to predict pattern of the involvement of calcaneocuboid joint (P<0.05). However, there was no statistically significant relationship between degree of posterior facet joint injuries and calcaneocuboid joint involvement (P>0.05). Conclusion: Calcaneocuboid joint involvement in intraarticular calcaneal fractures was common and more than half showed severe injuries. We concluded that further studies on the involvement of calcaneocuboid joint should be performed prior to surgical treatment of intraarticular calcaneal fractures.

저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에

제 5 장 계약 해지 및 이용 제한

일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
