Systematic Review of TCM on Moderate to Severe Obese Patients with Hypertension in Chinese Medical Journal (CNKI) (고혈압을 동반하는 중고도 비만 환자의 중의학 임상연구 체계적 고찰: CNKI 검색을 중심으로)

  • Objectives: The purpose of this study is to review clinical studies related to the effect of Traditional Chinese Medicine (TCM) on moderate to severe obese patients with hypertension. Methods: Clinical studies from the China National Knowledge Infrastructure (CNKI) were searched by specific keywords and criteria. Total of 7 randomized controlled trials were selected and analyzed. Results: Various evaluation methods were used to see the effect of TCM compared to conventional western medicine. Most of the studies used herbal medicine combined with conventional anti-hypertensive western medicine. The effective rate of anti-hypertension was higher when TCM was added to conventional anti-hypertensive medicine. Traditional medicine treatment showed significant effect on lowering blood pressure and body mass index. Conclusions: Based on the results of the clinical studies from China, TCM can be a valuable option for moderate to severe obese patients with hypertension. Applying Traditional medicine has a significant effect on lowering blood pressure and weight loss. Combining herbal medicine can be worthy of clinical promotion and application for moderate and severe obese patients with hypertension.

  • High-temperature superconducting rotors offer advantages in terms of output-to-weight ratio and efficiency compared to conventional phase conduction motors or generators. The rotor can be cooled by conduction cooling, which attaches a cryocooler, and by refrigerant circulation, which uses circulating liquid or gas neon, helium and hydrogen. Recent work has focused on environmental issues and on high-temperature superconducting motors cooled with liquid hydrogen that can be combined with fuel cells. However, to ensure smooth supply and return of the cryogenic cooling fluid, a cryogenic rotational coupling between the rotating and stationary parts is necessary. Additionally, the development of a sealing structure to minimize fluid leakage applicable to the coupling is essential. This study describes the design and performance evaluation of a non-contact sealing method, specifically a labyrinth seal, which avoids power loss and heat load caused by friction in contact sealing structures. The seal design incorporates a spiral flow path to reduce leakage using centrifugal force, and computational fluid dynamics (CFD) simulations were conducted to analyze the flow path and rotational speed. A performance evaluation device was configured and employed to evaluate the designed seal. The results of this study will be used to develop a cryogenic rotational coupling with supply and return flow paths for cryogenic applications.

Multi-scale context fusion network for melanoma segmentation

  • Aiming at the problems that the edge of melanoma image is fuzzy, the contrast with the background is low, and the hair occlusion makes it difficult to segment accurately, this paper proposes a model MSCNet for melanoma segmentation based on U-net frame. Firstly, a multi-scale pyramid fusion module is designed to reconstruct the skip connection and transmit global information to the decoder. Secondly, the contextural information conduction module is innovatively added to the top of the encoder. The module provides different receptive fields for the segmented target by using the hole convolution with different expansion rates, so as to better fuse multi-scale contextural information. In addition, in order to suppress redundant information in the input image and pay more attention to melanoma feature information, global channel attention mechanism is introduced into the decoder. Finally, In order to solve the problem of lesion class imbalance, this paper uses a combined loss function. The algorithm of this paper is verified on ISIC 2017 and ISIC 2018 public datasets. The experimental results indicate that the proposed algorithm has better accuracy for melanoma segmentation compared with other CNN-based image segmentation algorithms.

Ear lobule reduction using a sub-antitragal groove technique in patients with angiolymphoid hyperplasia with eosinophilia on the earlobe: a case report and literature review

  • Angiolymphoid hyperplasia with eosinophilia (ALHE) is a rare condition characterized by nodular lesions predominantly in the head and neck region, often causing discomfort or pain. Treatment remains challenging because of its rarity and the lack of established guidelines. This report presents a case of ALHE affecting the earlobes that was successfully managed using ear lobule reduction surgery and subsequent intralesional steroid injections. A 31-year-old woman with a history of recurrent earlobe masses underwent a partial excision to avoid the loss of the earlobe. Histopathological examination confirmed Kimura disease, a variant of ALHE. Subsequent local methylprednisolone injections effectively controlled the remaining lesions, resulting in significant size reduction without notching. Various treatment modalities have been attempted for this condition; however, recurrence rates remain high. Surgical resection combined with intralesional corticosteroid injections is the preferred approach. In this case, a sub-antitragal groove technique for earlobe reduction was employed to preserve the lateral edge of the ear lobule, minimize the risk of deformity, and achieve a predictable outcome. The sub-antitragal groove technique offers an approach to reduce earlobe size without compromising aesthetics. Further research is required to elucidate the pathogenesis of ALHE and establish standardized treatment protocols for this rare condition.

Failure simulation of nuclear pressure vessel under LBLOCA scenarios

  • This paper presents the finite element deformation and failure simulation of a typical Korean high-power reactor vessel under a severe accident characterized by large break loss of coolant (LBLOCA) with in-vessel retention of molten corium through external reactor vessel cooling (IVR-ERVC) conditions. Temperature distributions calculated using Modular Accident Analysis Program Version 5 (MAAP5) as thermal boundary conditions were used, and ABAQUS thermal and structural analyses were performed. After full ablation, the temperature of the inner surface in the thinnest section remained high (920 ℃), but the stress remained relatively low (less than 6 MPa). At the outer surface, the stress was as high as 250 MPa; however, the resulting plastic strain was small owing to the low temperature of 200 ℃. Variations in stress, inelastic strain, and temperature with time in the thinnest section suggest that the plastic and creep strains are saturated owing to stress relaxation, resulting in low cumulative damage. Thus, the lower head of the vessel can maintain its structural integrity under LBLOCA with IVR-ERVC conditions. The sensitivity analysis of internal pressure indicates the occurrence of failure in the thinnest section at an internal pressure >9.6 MPa via local necking followed by failure due to high stresses.

Nutrient variations from swine manure to agricultural land

  • Objective: Swine manure in Korea is separated into solid and liquid phases which are composted separately and then applied on land. The nutrient accumulation in soil has been a big issue in Korea but the basic investigation about nutrient input on arable land has not been achieved in detail. Within the nutrient production from livestock at the national level, most values are calculated by multiplication of the number of animals with the excreta unit per animal. However, the actual amount of nutrients from swine manure may be totally different with the nutrients applied to soil since livestock breeding systems are not the same with each country. Methods: This study investigated 15 farms producing solid compost and 14 farms producing liquid compost. Composting for solid phase used the Turning+Aeration (TA) or Turning (T) only methods, while liquid phase aeration composting was achieved by continuous (CA), intermittent (IA), or no aeration (NA). Three scenarios were constructed for investigating solid compost: i) farm investigation, ii) reference study, and iii) theoretical P changes (${\Delta}P=0$), whereas an experiment for water evaporation was conducted for analyzing liquid compost. Results: In farm investigation, weight loss rates of 62% and 63% were obtained for TA and T, respectively, while evaporation rates for liquid compost were 8.75, 7.27, and $5.14L/m^2{\cdot}d$ for CA, IA, and NA, respectively. Farm investigation provided with the combined nutrient load (solid+liquid) of VS, N, and P of 117.6, 7.2, and $2.7kg/head{\cdot}yr$. Nutrient load calculated from farm investigation is about two times higher than the calculated with reference documents. Conclusion: The nutrient loading coefficients from one swine (solid+liquid) were (volatile solids, 0.79; nitrogen, 0.53; phosphorus, 0.71) with nutrient loss of 21%, 47%, and 29%, respectively. The nutrient count from livestock manure using the excretion unit has probably been overestimated without consideration of the nutrient loss.

Facial Nerve Damage in Bell's Palsy according to When Korean Medical Treatment was Started : A Retrospective Study (벨마비 환자의 한의학적 치료 시작시기에 따른 신경손상률 비교 : 후향적 관찰 연구)

  • Objectives : Although integrative Korean medical treatments have been suggested to be effective for treating Bell's palsy, the effect of Korean medical treatment according to when treatment was started is unknown in the clinical field. Therefore, this study was conducted to determine the results of treating Bell's palsy according to different starting points of integrative Korean medical treatments. Methods : We screened patients who visited the Facial Palsy Center at Kyung Hee University Hospital for Korean and Western combined medical treatment from March 2011 to February 2016. A total of 821 out of 2086 patients were studied, including their basic characteristics and results of an electromyography. Patients who started Korean medical treatment within 3 days of onset were placed in group A, within 9 days of onset in group B, and patients who started treatment after 10 days of onset were placed in group C. We tried to compare the level of facial nerve damage by electromyography between groups. Results : The patients in group C had the highest axonal loss rates in all branches(frontal, oculi, nasal, oris). The post hoc analyses revealed the difference of axonal loss rates between group A and B was not statistically significant. Only group C showed statistically higher axonal loss rates in all branches. Conclusion : The study results showed that the patients who had delayed Korean medical treatments had a higher level of facial nerve damage by electromyography. According to the results of this study, early application of integrative Korean medical treatment is suggested.

Enhancement of the Performance a Centrifugal Compressor in an Automobile Turbocharger by Modifying the Circumferential Inlet Height of Volute (원주방향 볼류트 입구 높이를 수정한 자동차용 터보차저 원심압축기의 성능 향상)

  • To enhance the performance of an automobile turbocharger compressor, the circumferential inlet heights of the volute were modified and the flow field for the combined region of the diffuser and volute was numerically investigated using commercial software. Basically, a well-designed volute should have a high pressure recovery coefficient and a low loss coefficient for the total pressure. In this study, two circular volutes with the same cross sectional shape and tongue angle, but circumferentially different volute inlet heights, were selected. One volute had the middle inlet in the cross-section at the circumferential angle of $90^{\circ}$ but gradually lower inlet heights for the angles between $90^{\circ}$ to $360^{\circ}$ with respect to the cross sectional center of the volute, while maintaining the same height between the tangential line connecting the lowest positions of the cross section and the line connecting the volute inlets in the circumferential direction (case 1 volute). The other volute has an inlet height that is 2 mm lower than in case 1 volute such that the tongue section has a tangential inlet (case 2 volute). The results showed that the case 2 volute had a higher total pressure ratio because of its higher pressure recovery coefficient and higher isentropic efficiency, resulting from the lower loss coefficient along the circumferential position than the case 1 volute.

A Study on a Ku-Band High Power and High Efficiency Radial Combiner (Ku 대역 고출력 고효율 Radial Combiner에 대한 연구)

  • We have studied a combiner that can withstand high power while minimizing insertion loss in high frequency band. In particularly, because the output power that can be output per unit elements is much lower in the Ku band and above than in the low frequency band, it is necessary to combine many semiconductor elements in order to make a high power SSPA. A planar combiner such as a microstrip, as the number of elements to be combined increases, the insertion loss increases proportionally, resulting in a reduction in the overall system efficiency and an increase heating value also. The planar combiner also have some problems due to the low power handling rate. To improve these problems, we proposed a Cavity Radial Combiner. A Ku band 16-way Cavity Radial Combiner was fabricated and measured. As a result, it was tested 14dB return loss and over 94.5 % output combining efficiency in design band.

An Ultrastructural Study on the Effect of Exogenous $Ca^{2+}$ Stimulation to Ischemic Myocardium during Post-ischemic Reperfusion (재관류시 외인성 칼슘 자극이 허혈 심근에 미치는 영향에 대한 전자현미경적 연구)

  • The effects of exogenous $Ca^{2+}$ stimulation on the post-ischemic myocardial cells were studied using isolated Langendorff-perfused guinea pig hearts. At the starting point of reperfusion, Tyrode solutions, each containing 2.0mM, 4.0mM and 8.0mM $CaCl_2$ respectively, were administered for 2 minutes apart by descending, ascending, or by combined sequences followed by standard Tyrode solution containing 1.0mM $CaCl_2$. The minutes of global ischemia produced reversible but moderate to severe degree of myocardial ultrastructrual changes including focal destruction of sarcolemma, loss of nuclear matrix, clumping and margination of chromatins, mitochondrial swelling, destruction of mitochondrial cristae, shortening of sarcomeres, focal loss of myofibrils, and separation of cell junctions. In spite of reperfusion, the ultrastructure was more severely damaged and irreversible changes such as intracellular fluid accumulation, contracted sarcomeres, mitochondrial destruction, disruption of sarcolemma, loss of nuclear matrix, and separation of cell junction were observed in a large number of cells. In contrast, Tyrode-perfused $Ca^{2+}$-stimulated myocardial cells showed relatively well preserved ultrastucture, except slight changes including focal mitochondrial swelling, widening of T-tubule, and widening of cell junctions, especially at fasciae adherentes. The post-ischemic $Ca^{2+}$-stimulated reperfused myocardial cells produced focal changes such as mitochondrial destruction, disintegration of sarcolemma, widening of T-tubule, and intracellular fluid accumulation with slight variation in degree of changes by the method of $Ca^{2+}$ administration sequence. However, in a large number of the myocardial cells, chromatins were redistributed relatively evenly in the nuclear matrix, mitochondrial cristae were tightly packed, and a considerable number of intramitochondrial granules and glycogen granules reap-pealed. These results indicate that exogenous $Ca^{2+}$ stimulation in the initial period of reperfusion may be beneficial to salvage or to reduce the post-ischemic myocardium from further deleterious changes, and that the beneficial effects may be derived from the reserves of the function of the intracellular $Ca^{2+}$ regulating organelles and/or from the responsiveness of contractile apparatus to $Ca^{2+}$ stimulation.
