Treatment Outcome of Postoperative Radiotherapy in Extrahepatic Bile Duct Cancer (간외담관암의 수술 후 방사선치료 성적)

  • Purpose : To evaluate the efficacy of radiotherapy, and the factors affecting survival in patients of extrahepatic bile duct cancer, by analyzing the results of postoperative radiotherapy Materials and Methods : Between October 1991 and July 2001, 21 patients with extrahepatic bile duct cancer, who received radiotherapy after a radical resection, were retrospectively reviewed. The patients' ages ranged from 39 to 75 years, with a median of 61 years, and a male to female ratio of 16 to 5. The numbers of patients with proximal and distal bile duct cancer were 14 and 7, respectively. From the postoperative pathological examination, 19 of the patients were found to have microscopic residues, and 7 to be lymph node positive. Patients with AJCC stages I, II and III were 10, 10 and 1, respectively. The total radiation dose administered was 4,500$\~$6,300 cGy, with a median dose of 5,040 cGy. The follow up period was 20$\~$81 months, with a median of 57.5 months. Results : The overall and disease free survival rates at 3 and 5 years were 41.0 and 29.3$\%$, and 41.6 and 29.7$\%$, respectively. The influences of age, sex, tumor location, differentiation, microscopic residue, neural invasion, 7 and N stage, the stage itself, the dose of radiation and chemotherapy, on survival were evaluated. The T stage and the stage itself were found to be significant from a univariate analysis (p<0.05), but the degree of significance was limited by the small number of patients. A recurrence occurred in 12 patients (57.1$\%$), 5 in locoregional sites, 4 in distant sites and 3 in a combination of locoregional and distant sites, and the sites of distant metastasis were the liver, 6, and the bone, 1 Grade 2 or 3 acute leucopenias occurred in 2 patients and grade 2 chronic peptic ulcers occurred in 4, who were all recovered by conservative management. Conclusion : Postoperative radiotherapy is feasigbls in extrah데atic bile duct cancer, with tolerable toxicity, but prospective studies, with a large patient enrollment, are needed for the evaluation of the effects of postoperative radiotherapy and the related prognostic factors.

Chest Computerized Tomographic Scan and Flexible Fiberoptic Bronchoscopy in the Diagnosis of Middle Lobe Syndrome (페중엽증후군에서 흉부 전산화 단층촬영 및 굴곡성 기관지경검사의 의의)

  • Background: As well as fiberoptic bronchoscopy, chest computerized tomographic scan can now differentiate the benign from malignant causes and the obstructive from non-obstructive causes of lung collapse. This study was designed to evaluate the usefulness of chest CT scan and fiberoptic bronchoscopy in the diagnosis of middle lobe syndrome. Method: We reviewed the clinical features, roentgenographic changes, pathologic findings and bronchoscopic findings in 16 patients with middle lobe syndrome who were admitted to Severance Hospital during period of January, 1987 through January, 1992. Results: The male to female ratio was 1:1. The most common symptoms were cough and sputums. Crackle was the most common physical finding. Underlying disease was lung cancer, pulmonary tuberculosis and endobronchial tuberculosis in 3 each other, benign stenosis in 2, lung abscess, broncholithiasis, bronchial chondroma, pneumonia and nonspecific inflammation in 1 each other. Conclusion: We conclude that the combination of chest computerized tomogram and fiberoptic bronchoscopy was most desirable for the diagnosis of middle lobe syndrome.

The Study on Correlation between the Degree of Herniated Intervertebral Lumbar Disc at L4~5 Level and Improvement of Low Back Pain Treated by Korean Medicine Therapy (제 4~5번 요추 추간판 탈출 정도와 요통의 한의학적 치료 효과의 상관성 연구)

  • Objectives The purpose of this study was to compare the effects between the degree of herniated intervertebral lumbar disc (HIVD) at L4-5 level and improvement of low back pain treated by Korean Medicine therapy. Methods 567 patients who received inpatient treatment from May 2014 to December 2015 in the Daejeon-Jaseng of Korean Medicine Hospital were divided into 6 groups by the degree of HIVD at L4-5 level confirmed with a Lumbar spine magnetic resonance imaging. All patients received a combination of treatment including acupunture, chuna manual therapy, pharmacopunture, herbal medication. They were compared and analyzed on the basis of improvement between measuring Numeric Rating Scale (NRS), Oswestry Disability Index (ODI), EuroQol-5 Dimension Index (EQ5D Index) as they were hospitalized and as they were discharged. The statistically significance was evaluated by SPSS 23.0 for windows. Results After treatment, Normal stage on Intervertebral Lumbar Disc at L4-5 level group's Numeric Rating Scale (NRS), Oswestry Disability Index (ODI), EuroQol-5 Dimension Index (EQ5D Index) improvement was $1.30{\pm}1.62$, $4.52{\pm}11.82$ and $0.04{\pm}0.11$ respectively. Bulging group's improvement was $3.25{\pm}2.81$, $8.28{\pm}13.02$ and $0.09{\pm}0.17$ respectively. Spinal canal occupying ratio (SOR) less than 20 group's improvement was $2.15{\pm}1.92$, $11.79{\pm}17.81$ and $0.13{\pm}0.23$ respectively. SOR 20 to less than 40 stage group's improvement was $2.13{\pm}1.92$. $10.79{\pm}15.93$ and $0.10{\pm}0.26$ respectively. SOR 40 to less than 60 group's improvement was $2.16{\pm}2.24$, $9.80{\pm}16.62$ and $0.15{\pm}0.25$ respectively. Surgery group's improvement was $2.47{\pm}2.21$, $11.64{\pm}18.53$ and $0.15{\pm}0.27$ respectively (p<0.03). But there was no statistically significance between 6 group's improvement after treatment (p>0.05). Conclusions After inpatient treatment by Korean Medicine therapy, Most patient's pain, disability and Health Related Quality of Life was improved significantly. But there was no statistically correlation between the degree of HIVD at L4-5 level and improvement of low back pain. So We think that future research of higher quality and correct statistics shall be necessary.

Comparative Analysis on the Biomass Production between Machine-Transplanted and Water-Broadcast Seeded Silage Rice in the Central Plain Area of Korea (중부 평야지에서 사료용 벼 기계이앙과 담수산파 재배의 건물 생산성 비교 분석)

  • A field study was conducted over a 3-year period from 2014 to 2016 in the central plain area, Suwon, Korea, 1) to compare the biomass production between machine-transplanted and water-broadcast seeded silage rice and 2) to elucidate the growth factors that affect the difference in biomass production between the two cultivation practices. The heading date for the water-broadcast seeded silage rice was September 11-18, which was delayed by 11-17 days when compared to that for the machine-transplanted silage rice. On average, water-broadcast seeded silage rice had a shorter plant height, more panicles per area, and a greater biomass production because of the increased straw dry weight. However, the difference in dry weight of each plant organ between the two cultivation practices exhibited yearly variation. When the data were pooled across experimental years, cultivation practices, and varieties, biomass production was highly positively correlated with straw dry weight but not with panicle dry weight. When the ratio of water-broadcast seeding to machine-transplanting was analyzed, total dry weight and straw dry weight were positively associated with each other, whereas no relationship was found between total dry weight and panicle dry weight. Despite that water-broadcast seeded silage rice produced more panicles per area than machine-transplanted silage rice, the two cultivation practices had a similar dry weight per culm. Therefore, we conclude that the silage rice from the water-broadcast seeding, compared to the machine-transplanting, produced more biomass because of the combination of the increased panicle number per area and a similar dry weight per culm. These results suggest that silage rice could be produced through water-broadcast seeding to increase biomass production with low labor and cost input.

Effects of Intensive Weight Bearing Treadmill Training and Electrical Stimulation on Skeletal Muscle Properties in Hindlimb Suspended Rats (흰 쥐의 뒷다리 현수 후 집중 체중부하 트레드밀 훈련과 전기자극이 골격근 특성에 미치는 효과)

  • The purpose of this study was to investigate the treatment effect of three interventions on the disuse atrophy of rat hindlimb after two weeks suspension. Forty-eight 11~12 weeks old female Sprague-Dawley white rats were divided into four intervention groups: 1) suspension only (S; n=10), 2) intensive weight bearing treadmill (IWBT; n=10), 3) electrical stimulation (ES; n=9), 4) 2)+3) (ES/IWBT; n=9). Another 10 rats received no intervention or hindlimb suspension and served as controls (C). After the interventions, 1) the cross-sectional area (CSA), 2) the ratio of white muscle fiber composition (WMFC), 3) isometric tetanic tension (ITT), and 4) muscle weights (MWs) were measured from the four calf muscle specimens. The results were as follows: 1. In all intervention groups, the CSAs of medial and lateral gastrocnemius (MG LG), soleus (SOL), and flexor digitorum superficialis (FDS) decreased when compared to the control (C) group (p<.05). The CSA increased in FDS and LG for the IWBT group, in SOL for the ES/IWBT group compared to the S only group (p<.05). 2. The ratios of WMFC in MG, LG, SOL, and FDS increased compared to the C group for all interventions (p<.05). The ratios of WMFC decreased in SOL and LG for the IWBT group, in SOL for the ES/IWBT group compared to the S only group, and decreased in SOL for the ES/IWBT group compared to the IWBT group (p<.05). 3. The ITT in the MG, LG, SOL, and FDS decreased compared to the C group for all interventions (p<.05). The ITT increased in MG LG/FDS, SOL, and the whole calf muscles (WCMs) in the IWBT, ES and ES/IWBT groups compared to the S only group (p<.05). 4. The MWs in MG LG/FDS, SOL, WCMs decreased compared to the C group for all interventions (p<.05). The MWs increased in MG LG/FDS and WCMs for the IWBT group, in SOL for the ES group, and in SOL for the ES/IWBT group compared to the S only group (p<.05). 5. In atrophied muscles, the IWBT group showed the best recovery and the ES/IWBT and ES groups followed in decreasing order. The most susceptible muscle to disuse atrophy was the SOL. But conversely, it showed the best recovery in the ES/IWBT group. After two weeks of hindlimb suspension, the calf muscles of rats atrophied and their isometric tension decreased. These changes were best reversed by hindlimb-focused treadmill activity. The next best results were achieved by electrical stimulation combined with the treadmill followed by only electrical stimulation. These findings indicate that full weight bearing treadmill activity alone or in combination with electrical stimulation are effective treatments for non-weight bearing induced muscle atrophy. Further study of the effect of different intensities of electrical stimulation and variations in the duration period of full weight bearing treadmill activity on disuse atrophy is recommended.

  • The critical need of forest road for enchanting the additional values of various forest products, in addition, giving more recreational opportunity to citizen, has been recognized. In this study the present author aimed to ascertain the most effective construction working plan of forest road being tit to Korean geographic condition. To execute this research program, four locations in national forest of Kangweon-do district and other four locations in private forest in Chollabuk-do district both where forest roads have previously been constructed were selected to analyze the effectiveness basing upon the various factors separately or in combination. The results are summarized as follows ; 1. The investment efficiency in forest road construction showed to increase in the area where terrain factors and district social factors rate is high, and to decrease in the area where forest status factors and forest road structure factors rate is high. So in future the Forest Resource Development Model of forest road should take more importance particularly on those area having terrain factor ratio is low. The extractable value of constructed forest road based on forest status factors rate is expected to increase in case of high considerably. 2. To construct of forest road for increasing multiple use of forests, forest road should be construct with priority on area where obtained total score by evaluation factors is high. And these evaluation factors should take possible determine the position of forest road construction. 3. The following five types of forest road basing upon function performance are suggested with regard to the place where road is constructed. (1) Forest Utilization Model ; where forest status factors and forest road structure factors rate are over 60%. (2) Forest Resource Development Model ; where terrain factors, forest status factors, forest road structure factors and district social factors rate are less than 60%. (3) Community Development Model ; where terrain factors, forest road structure factors and district social factors rate are over 60% but forest status factors rate are less than 60%. (4) Recreation and Health Model ; where terrain factors, forest status factors, forest road structure factors and district social factors rate are over 60%. (5) Multiple Use Model ; where both forest status factors and district social factors rate are over 60%.

Effects of Antibiotics, Copper Sulfate and Probiotics Supplementation on Performance and Ammonia Emission from Slurry in Growing Pigs (사료에 대한 항생제, 황산동 및 생균제 첨가가 육성돈의 생산성 및 슬러리의 암모니아 발생에 미치는 영향)

  • An experiment was conducted to determine the effect of supplementation of chlortetracycline (CTC,110ppm), copper sulfate(Cu, 125ppm) and two levels of probiotics(Prob I, 0.04%, Prob II, 0.1%), Bacillus licheniformis and Bacillus subtilis) on growth performance, nutrient digestibility and manure characteristics in growing pigs. A total of 50 pigs that averaged 20.78$\pm$0.35kg BW and 50$\pm$2.3d age were allocated in a randomized block design with two pigs per pen and 5 pens per treatment. Pigs and feeders were weighed 10-days interval for the 40-d trials to determine ADG, ADFI and feed:gain ratio(F:G). Average daily gain, feed intake, feed/gain and nutrient digestibility were not improved(P>0.10) by the supplementation of CTC, Cu and two levels of probiotics. There were significant(P<0.05) Cu effects on digestibility of crude protein, and probiotics effects on digestibilities on organic matter, crude protein and energy between first 20 days and subsequent 20 days. Fecal concentrations of copper were highly increased(P<0.001) by the copper supplementation. Total bacteria and coliform counts were not altered by the supplementation of CTC, Cu and two levels of probiotics. Ammonia emission from slurry, measured during first 3 weeks, was reduced (P<0.001) in pigs fed diet with 125 ppm copper from copper sulfate. Results indicate that CTC, Cu and Probiotics supplementation had a little or not positive effect on grower pig performance under sound environmental conditions. Further studies may be warranted to investigate the effects of dietary copper-either reduced or in combination with dietary ammonia control agents-on the ammonia emission characteristics of swine manure.

A Study of Clinical Model for Radiation Therapy in Lung Cancer Patients of Busan and South Gyeongnam Province (부산, 경남지역 폐암 환자의 방사선치료 이용에 대한 임상 결정 모델 연구)

  • Radiation therapy for lung cancer is an effective treatment during monotherapy or combination therapy. Studies have reported that the optimum utilization rate of radiation therapy is estimated at 61% to 74%. Radiation therapy in Korea has been investigated to be low; further studies are needed. This study was intended to assess the appropriateness of the use of radiation and to reveal the use of radiation therapy-related factors by examining radiation therapy in lung cancer patients of Busan and South Gyeongnam Province. This study was aimed at the population diagnosed with lung cancer in Busan and South Gyeongnam Province. To conduct the study, 1036 patients enrolled in two hospitals were collected and 897 appropriate as subjects were selected. We compared the optimum utilization rate and actual rate of radiation therapy, and revealed the adequacy and related factors for use of radiotherapy. Of 897 patients, 503 (56%) were treated with medical therapy and 394 (44%) were given radiotherapy. The radiotherapy utilization rate of all lung cancer patients was 42%. The proportion of non-small cell lung cancer by histologic type was 33% and that of small cell lung cancer was 90%. Factors related to radiation therapy used in cancer were age, histological type, clinical stage, doctor refereed to, and clinical examination. Compared to radiation utilization by region (site), curative chest therapy was 42%; palliative treatment was 26%. In the comparison of histologic types, utilization of small-cell lung cancer is lower; the lowest especially in the stage III. Utilization of radiation therapy in Busan and South Gyeongnam Province was lower than the reasonable one. Utilization difference could be explained by patient factors, tumor factors, and health service factors. To improve utilization,development ofoutreach service programs and activation of the multidisciplinary team are required.

Drug resistance of Shigella and Salmonella and the inhibition and elimination of drug resistance (이질균(痢疾菌) 및 살모내라의 약제내성(藥劑耐性), 내성화방지(耐性化防止) 및 제거(除去))

  • Ninety-five strains of Shigella, 70 of Salmonella paratyphi A, and 230 of Salmonella typhi were tested for their resistance to drugs. Also studied was the inhibition and elimination of drug resistance. All except one strain of Shigella consisted of 79 Sh. flexneri and 16 Sh. sonnei were multiply resistant to chloramphenicol, tetracycline, streptomycin, and splfisomidine. Among them, 70 strains were resistant to ampicillin and carbenicillin, 80 to trimethoprim-sulfamethoxazole, 22 to nalidixic acid, and one to kanamycin, but strain resistant to gentamicin, cephaloridine, and rifampin was not encountered. All strains of S. paratyphi A and S. typhi were susceptible to drugs tested, except sulfisomidine and rifampin, for which all S. paratyphi A were slightly resistant to sulfisomidine and the majority of S. paratyphi A and S. typhi were slightly resistant to rifampin. Approximately 80% of multiply drug-resistant Shigella transferred their resistance to E. coli by conjugation, and the resistance was considered to be mediated by R plasmids. The frequency of transfer of drug resistance varied by donor strains and recipients, but not by selecting drugs. Resistance to nalidixic acid was not transferred by conjugation to the recipients. Drug-resistant Shigella strains successively subcultured in nutrient agar stabs contained clones resistant to drugs and those susceptible to drugs, but the ratio of resistant and susceptible clones varied by strains. The multiply drug-resistant S. typhi and Shigella strains were found to not lose completely their drug resistance by subculture in media. Acriflavine has some effect on the elimination of drug resistance mediated by R plasmids, but the effect varied markedly by strains. Atabrine has no effect among strains tested. The combination of drugs increased the drug actions in majority of cases with synergistic or additive effects.

Morphology, Mineralogy and Genetic Implication of Placer Gold from the Huongkhe Area, Vietnam (베트남 홍케 지역 사금의 산상과 생성연구)

  • Placer gold in collected heavy minerals from several localities in Huongkhe area, is consistently very finegrained (${\leq}100$ to $400{\mu}m$). The size and size distribution show somewhat differences at Dongdo and Hoahai : at Dongdo, predominant relatively larger and wide distribution; at Hoahai, characteristic relatively finer and narrow distribution range. The morphology of gold grains is divided into the four groups assumed by the dimension ratio : spherical, subprismoidal, prismoidal, and irregular. The gold grains at Dongdo show wide morphological distribution, whereas, at Hoahai, spherical form is predominant (${\approx}75%$). Three main types of gold are classified based on their chemical composition and mode of occurrence: type I (electrum; fineness=568~931), type II (amalgam; fineness=671~927), and type III (native gold; fineness=923~999). Type I gold contains, relatively high and variable silver contents (${\approx}11$ to 58 atomic % Ag), and has been classified into two subtypes based on their silver contents (type IA, ${\approx}11{\sim}39$ atomic % Ag; type IB, ${\approx}40{\sim}58$ atomic % Ag). However, type I gold would have been generally original compositions of electrum which originated at the provenance deposits. Mercury reacts with gold and silver to form amalgam (type II gold) which has variable Hg contents (1.2~30.5 atomic % Hg). The mercury contents in gold grains at Hoahai (10.9~30.5 atomic % Hg) are higher than those at Dongdo (5.8~21.1 atomic % Hg). The gold grains from the area generally exhibit a high-purity gold (type III) rim. The individual rims on the various grains range from <1 to $80{\mu}m$ in thickness and have silver contents of <10 atomic percent Ag, even though the core compositions range from ${\approx}11$ to 58 atomic percent Ag. The rim of gold most likely is responsible for the commonly cited cases of gold from placer deposits assaying at higher values of fineness than the gold in the corresponding source lode. The gold-rich rim in the Huongkhe area apparently forms by a combination of self-electrorefining and preferential dissolution of silver under oxidizing nature during the weathering and transport process. All data of gold grains in the Huongkhe area suggest that the transport distances and/or time of placer gold at Hoahai are generally farther than those at Dongdo. The mercurian gold bearing provenance deposits at Dongdo and Hoahai would be suggest nearest epithermal gold-silver vein-type.
