The helideck structure must satisfy the safety requirements associated with various environmental and accidental loads. Especially, there have been a number of fire accidents offshore due to helicopter collision (take-off and/or landing) in recent decades. To prevent further accidents, a substantial amount of effort has been directed toward the management of fire in the safety design of offshore helidecks. The aims of this study are to introduce and apply a procedure for quantitative risk assessment and management of fires by defining the fire loads with an applied example. The frequency of helicopter accidents are considered, and design accidental levels are applied. The proposed procedures for determining design fire loads can be efficiently applied in offshore helideck development projects.
The management of safety at sea is based on a set of internationally accepted regulations and codes, governing or guiding the design and operation of ships. The regulations most directly concerned with human safety and protection of the environment are, in general, agreed internationally through the International Maritime Organization(IMO). IMO has continuously dealt with safety problems and, recognized that the human element is a key factor in both safety and pollution prevention issues(IMO, 2010). This paper proposes a human error analysis methodology which is based on the human error taxonomy and theories (SHELL model, GEMS model and etc.) that were discussed in the IMO guidelines for the investigation of human factors in marine casualties and incidents. In this paper, a cognitive process model, a human error analysis technique and a marine accident causal chains focused on human factors are discussed, and towing vessel collision accidents are analyzed as a case study in order to examine the applicability of the human error analysis technique to marine accidents. Also human errors related to those towing vessel collision accidents and their underlying factors are discussed in detail.

Traffic accidents are normally caused by late or faulty judgements due to the driver's inaccurate estimation of the distance, velocity, and acceleration from the surrounding vehicles as well as his carelessness or inattention. Thus, the development of collision avoidance systems is motivated by their great potential for increased vehicle safety. A typical collision avoidance system consists of the forward-looking sensor, the criteria for activation of collision warming and avoidance, the collision avoidance maneuvers, and the user interface. This thesis is concerned with the development of a collision warning algorithm in which the driver is warned of approaching collision with the visual and/or the audible signals . The warning algorithm based on fuzzy logic is presented here based on new warning criteria. It has been newly derived from the conventional warning equation by adding a new input variable of the required deceleration to avoid collision. The algorithm is also able to adapt to the individual driver's taste along with the different road conditions by externally controlling the warning intensity. Finally , the proposed algorithm has been validated using computer simulation.
In order to reconstruct vehicle-pedestrian collision accidents, this paper presents a fuzzy tool to estimate accurately the impact velocity of the vehicle using parameters which could be easily collectable at the accident scene. The fuzzy rules and membership functions were set up using number of over 200 domestic and foreign data from accidents and empirical tests and 700 data from multibody simulation experiments. The developed fuzzy tool deduces the category of pedestrian trajectory and impact speed of the vehicle using 4 membership functions and 2 logic rules. The membership function of throw distance was differently set according to the deduced category of trajectories. The implemented fuzzy program was validated through comparing with the domestic and foreign empirical data. The output results agree very well in impact velocities of vehicle resulting the accuracy and usefulness of the developed tool in the reconstruction analysis of vehicle-pedestrian collision accidents.

The objective of this paper is to describe the result of development of collision avoidance supporting system, based on the electronic chart display and information system(ECDIS). In real ship operations, collision accidents happen frequently due to human errors such as the lax vigilance, misinterpretation of international regulations for preventing collisions at sea (COLREGs). We developed a system which will help to avoid these kind of accidents. This system can automatically recognize the risk of collisions, generate the safe alternative routes that comply with COLREGs, and then deliver the results into auto pilot. A virtual simulation assuming progressive collision situations revealed the usefulness of this system.

If we were in a head-on or crossing situation with a target ship and did not know the target ship's intention to change her course, we might be confused about our decision making to change our course for collision avoidance and be in a danger of collision. In order to solve these problems, we need to develop an automatic system which enables mariners to easily detect a change in the target ship's course and efficiently avoid being on a collision course. In this paper, we proposed an early detection method on altering course of a target ship using the steering wheel signal. This method will contribute to the reduction of collision accidents and also be used to the VTS system and the analysis of marine accidents.

Marine accidents caused by ships have brought about economic and social losses as well as human casualties. Most of these accidents are caused by small and medium-sized ships and are due to their poor conditions and insufficient equipment compared with larger vessels. Measures are quickly needed to improve the conditions. This paper discusses a video-integrated collision prediction and fall detection system to support the safe navigation of small- and medium-sized ships. The system predicts the collision of ships and detects falls by crew members using the CCTV, displays the analyzed integrated information using automatic identification system (AIS) messages, and provides alerts for the risks identified. The design consists of an object recognition algorithm, interface module, integrated display module, collision prediction and fall detection module, and an alarm management module. For the basic research, we implemented a deep learning algorithm to recognize the ship and crew from images, and an interface module to manage messages from AIS. To verify the implemented algorithm, we conducted tests using 120 images. Object recognition performance is calculated as mAP by comparing the pre-defined object with the object recognized through the algorithms. As results, the object recognition performance of the ship and the crew were approximately 50.44 mAP and 46.76 mAP each. The interface module showed that messages from the installed AIS were accurately converted according to the international standard. Therefore, we implemented an object recognition algorithm and interface module in the designed collision prediction and fall detection system and validated their usability with testing.

The purpose of Maritime Safety Tribunal is that experts about ship operation investigate and reveal causes of marine disasters and suggest plans to prevent recurrence in Korean Maritime Safety Tribunal which is a kind of administrative court. Despite the fact that 72% of marine disasters is fishing vessel accidents and 75% of collision accidents between vessels which most occur is related to fishing boats, it is the very serious problem that there is no any person with licenses of marine technicians for fishing vessels in judges and investigators of Korean Maritime Safety Tribunal. The operational characteristics of fishing vessels, their sizes, and shift patterns of duties in wheelhouses are completely and incommensurably different from those of merchant vessels. By the way, if the investigators and judges who just consist of merchant vessel experts investigate and judge marine accidents about fishing vessels, there could be errors in establishment of policies to investigate and reveal their causes and prevent recurrence of accidents. Especially, in case of collision accidents between fishing vessels and merchant vessels, it is thought that the marine accident interested provides causes which can be doubtful about fairness of judges. Therefore, it is thought that the ratio of judges and investigators is most desirable to compose it to be similar to the occurrence frequency of marine accidents. For this, the following solution plans are suggested. First, qualification for appointment requires first class marine technicians. But there is the only one vessel which needs the first class in fishing vessels. Therefore, it is thought that the provisory clause should be added so that the second class marine technicians can be used instead of the first class ones. Second, the marine accidents of fishing vessels reach 72% but the fact that there is no any judge and investigator with licenses of marine technicians for fishing vessels is thought to go against the purpose of establishment of Korean Maritime Safety Tribunal. Therefore, it is thought that there is, at least, one more judges and investigators in the central and local Korean Maritime Safety Tribunals. The same method should be applied to judge assistants and investigation assistants.
Objectives : The purpose of this study was to investigate accident cases involving electronic assistive devices, to determine how these accidents can be avoided and to devise preventative instructions for the handicapped who use these devices. Methods : This study was carried out from July 20 to October 3, 2015. A consent-based survey was conducted via mail and mobile phones targeting 700 electronic assistive device users, of which questionnaires from 290 users were collected and used in the analysis. Results : Accidents involving electronic assistive devices were investigated, especially the causes of the accidents and accident prevention education, as well as the status of the electronic assistive devices. The most common accident types were collision and falling, and it was found that the victims of the accidents usually suffered severe injury to their lower limbs. Most users used electronic assistive devices every day but rarely wore a safety belt because of discomfort. There were more incidents of collision and falling for older aged handicapped users, and the injury rate to the lower limbs was highest in handicapped elderly aged 50 years or older. Conclusions : In order to prevent accidents with electronic assistive devices in the future, a or management organizations must prepare specific safety guidelines and manage these accidents.
The transport of cargoes carried by coastal and ocean-going vessels has increased with the rapid growth of the Korean economy these days. This increase of the sea-borne cargoes has made the Korean coastal traffic so congested that this can be a cause of large pollution as well as great marine casualties such as loss of human lives and properties. Marine casualties generally result from the complicated interaction of natural and human factors; the former being the topographic, marine traffic volume and meteorological conditions, and the latter being the quality of seafares. In this paper, the authors analyse the trend of marine casualties in the Korean coastal and clear up the cause of accidents and examine closely the mutual relations among sea accidents, weather conditions, and marine traffic volume. These accidents are classified into several patterns on hte point of view of ship's size, ship's type and ship's age and its characteristics of each pattern are described in detail. Also, the authors estimate the amount of economical losses resulting from marine casualties which are classified into the accident patterns, and clarify the effects of those losses on B/B(Balance Sheet) and P/L(Profit & Loss) of Korean shipping companies and Korean national economy. The analyzed results of marine casualties are summarized as follows: 1) The average number of sea accidents is 248 cases per year with the loss of 107 persons during last 13 years. 2) Collision is the top of causes of sea accidents (approx. 36.4%), shipwreck the second (approx. 20.3%), agroung the third rank (approx. 18.2%). 3) The ship's number under 1, 000G/T is approx. 74% of total ship's number of accidents. 4) 80% of total number of marine accidents is taken plact at the coastal waters. (involved ports & narrow channels) 5) Marine casualties are occur likely to in the night, the winter and the summer. 6) The average amount of economical losses is approx. 18.5 billion won. (approx. 0.14% of GNP) 7) Shipwreck is the top of the amount of economical losses (approx. 60.4%), collision the second (aprox. 24.5%), aground the third (approx. 9.9%). 8) The amount of economical losses is approx. 5.24% of gross capital of shipping co., 1.24% of shipping revenue, 1.38% of shipping total income in 1983.
