말초동맥질환자의 불확실성, 사회적 지지, 환자역할행위 이행이 건강 관련 삶의 질에 미치는 영향 (Effects of Uncertainty, Social Support, and Sick Role Behavior on Health-Related Quality of Life in Patients with Peripheral Arterial Disease)

  • Purpose: This study was conducted to examine the effects of uncertainty, social support, and sick role behavior on health-related quality of life in patients with peripheral arterial disease. Methods: This study is a descriptive research using self-reporting questionnaire. Data were collected from 167 patients with peripheral arterial disease. Measurement tools were Multidimensional Scale of Perceived Social Support(MSPSS), Mishel's Uncertainty in Illness Scale (MUIS), Sick role behavior measurement tools and SF-36 Version I. The data were analyzed using descriptive statistics, correlation, and regression analysis by using SPSS/WIN 24.0. Results: Factors that significantly influenced physical health-related quality of life were age (β=-.19, p=.010), monthly income (β=.17, p=.027), uncertainty (β=-.29, p<.001), and exercise and rest (β=.28, p<.001) that all together accounted for 32.6% of the variance. Factors that significantly influenced mental health-related quality of life were monthly income (β=.20, p=.015), drinking (β=.17, p=.040), uncertainty (β=-.24, p=.001), and exercise and rest in sick role behavior (β=.26, p=.003) that all together accounted for 18.2% of the variance. Social support was an insignificant factor on physical and mental health-related quality of life. Conclusion: To improve the health-related quality of life of people with peripheral arterial disease, it is necessary to develop a systematic nursing intervention program including a strong support system, education, strategies for alcohol abstinence, and exercise and rest therapy.

약물복용 중인 고혈압 환자의 혈압관리양상 예측을 위한 의사결정나무분석 (Decision-Tree Analysis to Predict Blood Pressure Control Status Among Hypertension Patients Taking Antihypertensive Medications)

  • Purpose: This study was performed to analyze the levels of blood pressure and to identify good or poor blood pressure control (BPC) groups among hypertension patients. The study was based on the Korea National Health and Nutrition Examination Survey (KNHANES VI and VII) conducted from 2013 to 2016. Methods: The sociodemographic and clinical data of 4,151 Korean hypertension patients aged 20-79 years and who were taking antihypertensive medications was extracted from the KNHANES VI and VII database. Descriptive statistics for complex samples and a decision-tree analysis were performed using the SPSS WIN 24.0 program. Results: The mean age was $62.46{\pm}0.21years$. The mean systolic blood pressure (SBP) was $128.07{\pm}0.28mmHg$, and the diastolic blood pressure (DBP) was $76.99{\pm}0.21mmHg$. 71.9% of participants showed normal blood pressure (SBP < 140mmHg and DBP < 90mmHg). From the decisiontrees analysis, the characteristics of participants related to good BPC group were presented with 9 different pathways same as those from the poor BPC group. Good or poor BPC groups were classified according to the patients' characteristics such as age, living status, occupation, education, hypertension diagnosis period, numbers of comorbidity, perceived health status, total cholesterol, high density lipoprotein-cholesterol, alcohol drinking per month, and depressive mood. Total cholesterol level (< 201mg/dL or ${\geq}201mg/dL$ cutoff point) was the most significant predictor of the participants' BPC group. Conclusion: This decision-tree model with the 18 different pathways can form a basis for the screening of hypertension patients with good or poor BPC in either clinical or community settings.

강직척추염 환자의 일반적 특성에 따른 건강관련 삶의 질 차이 (The Difference of Health Related Quality of Life According to Ankylosing Spondylitis Patient's General Characteristics)

  • 본 연구는 강직척추염 환자의 일반적 특성에 따른 건강관련 삶의 질의 차이를 알아보기 위한 서술조사연구이다. 연구대상은 2015년 1월 13일부터 2월 12일까지 G광역시 C대학병원에 강직척추염으로 진단받고 최소 4주이상의 안정적인 약물을 유지중인 만 19세 이상의 환자를 대상으로 설문조사를 통해 자료를 수집하였으며 총 275명의 자료가 분석에 포함되었다. 수집된 자료는 기술적 통계, t-test, ANOVA, Scheff-test, multiple regression를 통해 분석하였다. 연구 결과 연령(p=.008), 직업유무(p<0.001), 동반질환 수(p<0.001), 수술유무(p=0.002), 증상 발현 후 기간(p=0.010), 진단 후 기간(p=0.027), 음주(p=0.002), 적혈구 침강속도(p=0.049)는 강직척추염 환자의 건강관련 삶의 질에 유의하게 영향을 미치는 것으로 분석되었으며, 다중회귀분석 결과 교육수준, 수입, 동반질환 수, 음주가 유의미한 변수였다. 결론적으로 강직척추염 환자의 건강관련 삶을 향상시키기 위해 일반적 특성을 개선시킬 수 있는 중재 프로그램을 개발함과 동시에 인구사회학적 특징과, 임상학적 특성을 고려한 중재프로그램이 제공되어야 할 필요가 있음을 알 수 있다. 따라서 본 연구의 결과가 이러한 통합적인 중재프로그램 개발에 실제적인 근거로 활용 될 수 있을 것으로 생각된다.
