한국 요실금 여성노인의 생애이야기-내러티브 분석적 접근 (The Life Stories of Elderly Korean Women with Urinary Incontinence: A Narrative Study Approach)

  • Purpose: The purpose of this study was to investigate the experience of urinary incontinence (UI) for elderly Korean women using a narrative approach. Methods: The data were collected using individual in-depth interviews with 15 communitydwelling elderly women who had UI for at least 1 yr. The narratives of the life stories of these women were analyzed from the actor’s perspective, motivation and purpose of actions, and action toward goal achievement. Also the narratives on UI were analyzed according to cognition, behavior, and evaluation of UI. Results: Three major types of the life stories emerged from the analysis. First, the conquest narrative type reflects active characteristics of narrators within the circumstances of the sociocultural context. Second, the patience narrative is characterized as having flexibility between self determination for goal achievement and the boundary of the sociocultural context. Lastly, the compliance narrative reflected characteristics of narrators who easily adapt their way of life to circumstances. In terms of UI, the narrators in all three types lacked awareness of UI as an illness condition. Three different reasons are specifically identified according to the narrative types. Conclusion: The results of this study indicate that the concept and the illness behaviors related to UI in elderly Korean women with UI were closely related to the way of women's lives within the sociocultural and historical context.

고위험 미숙아 자녀의 아버지 되어감 경험 (Experience of Becoming a Father of a High Risk Premature Infant)

  • Purpose: This study was performed to identify the experience of becoming a father of a high risk premature infant. Methods: Grounded theory was used for this research. The participants were 12 fathers who had premature infants lighter than 2,500g of birth weight, less than 37 weeks of gestational age and having stayed 2 weeks or longer in a NICU right after birth. Theoretical sampling was done to identify participants and indepth interviews were done for the data collection. For data analysis, the process suggested by Corbin and Strauss was used. Results: For these participants the core phenomenon of the experience of becoming a father of a high risk premature infant was 'striving through with belief and patience'. The phenomenon was 'being frustrated in an unrealistic shock'. Contextual conditions were 'uncertainty in the health status of the premature baby' and 'no one to ask for help' and intervening conditions were 'possibility in the health recovery of the premature baby' and 'assistance from significant others'. Action/interaction strategies were 'withstanding with belief in the baby' and 'enduring with willpower as head of the family' and the consequence was 'becoming a guardian of the family'. Conclusion: For the participants, the process of becoming the father of a high risk premature infant was striving through the situation with belief in their babies' ability to overcome the crisis and waiting for the babies' recovery with patience.

욕창 환자들의 질병 체험에 관한 현상학적 연구 (A Phenomenological Study on Illness Experience of Patients with Pressure Ulcer)

  • Purpose: The purpose of this study was to describe the illness experience of patients with pressure ulcer. Methods: A phenomenological methodology was used for the study. The data were collected by individual in-depth interview with seven participants with pressure ulcer during 2013~2014. All interviews were audio-taped and verbatim transcripts were made for the analysis. The data were analyzed using Colaizzi's phenomenological method. Results: All participants had underlying disease, such as spinal paralysis and diabetes. Average period of having pressure ulcer was 18 months, ranged from 3 to 36 months. A total of seven theme clusters were derived from the analysis; unexpected wound, inherent vulnerability to infection, reversal of the treatment policy, unpleasant and strange feeling of wound, sweeping fear and helplessness, socioeconomic burden, and healing through specific actions and reflection. The participants faced various contradictory and paradoxical situations in managing their pressure ulcers as well as underlying diseases in their everyday life. However, they slowly overcome these situations by strictly practicing concrete action-oriented strategies that they have learned through suffering and appreciating miraculous wound healing. Conclusion: The results of this study can help developing a patient-specific intervention program with sufficient emotional support by providing insights of the paradoxical illness experience of patients with pressure ulcer.

혈우병 환자의 질병 경험 연구 (A study on the lived experience of patients with hemophilia)

  • This study was done to provide a firsthand description and analysis of phenomenological data of the singular, subjective and lived experience of patients with hemophilia. Sixe patients suffering from hemophilia were selected for this study. Data were collected from May to August, 1999 through in-depth interviews. The phenomenological method described by Colaizzi was used for the phenomenogical analysis of the data. After transcribing the recorded interview, the researchers read the data repeatedly to identify significant statements, restated them succinctly, and then formulated meanings, themes, theme clusters, and categories. The formulated meanings were grouped into six categories : fear, loneliness, frustration, sypmtoms of hemophilia, reception of support, and commitment. An exhaustive description of the experience of hemophilia can be deduced from this study. It is as follows: even though patients with hemophilia feel fear, loneliness and frustrations and suffer from symptoms of their disease such as bleeding, pain and functional disorders, they commit themselves to the maintenance of good health and enthusiastic life style with the help of support resources such as family members.

Factors Affecting Nurses' Performance of Cancer Pain Management in a Tertiary Hospital

  • Purpose: More than 60% of patients with advanced cancer experience pain, and uncontrolled pain reduces the quality of life. Nurses are the closest healthcare providers to the patient and are suitable for managing cancer pain using pharmacological and non-pharmacological interventions. This study aimed to identify factors affecting the performance of cancer pain management among nurses. Methods: This study was conducted among 155 participating nurses working at a tertiary hospital who had experience with cancer pain management. Data collection was performed between October 18, 2021 and October 25, 2021. Data analysis was conducted using descriptive statistics, the independent-sample t-test, one-way analysis of variance, and hierarchical regression analysis. Results: There were 110 subjects (71.0%) who had no experience of cancer pain management education. The results of regression analysis indicated that barriers included medical staff, patients, and the hospital system for cancer pain management (𝛽=0.28, P<0.001). The performance of cancer pain management was also affected by experience of cancer pain management training (𝛽=0.22, P=0.007), and cancer pain management knowledge (𝛽=0.21, P=0.006). The explanatory power of the variable was 16.6%. Conclusion: It is crucial to assess system-related obstacles, as well as patients and medical staff, in order to improve nurses' cancer pain management performance. A systematic approach incorporating multidisciplinary interventions from interprofessional teams is required for effective pain management. Furthermore, pain management education is required both for cancer ward nurses and nurses in other wards.

동아리 활동이 학습동기 및 대학생활 만족도에 미치는 영향 (치위생과 학생을 중심으로) (Influence on Academic Motivation and Satisfaction of College Life by Dong-A-Ry Activity (in students on department of dental hygiene))

  • 본 연구의 목적은 서울경기지역의 치위생과 학생들을 대상으로 동아리 활동이 학습효과와 학교생활에 대해서 유의한 차이가 있는지를 규명하려는 것이다. 이를 위해 약 3개 대학의 학생들을 대상으로 일대일 면접방식에 의해 설문지을 통하여 연구한 결과는 다음과 같다. 1. 삶의 만족도에 미치는 동아리 활동을 살펴보면 유형에 따라 유의한 차이를 보였으며, 통학시간 및 성적 등이 만족도에 큰 영향을 미치는 것으로 나타났다. 2. 동아리 활동이 미치는 학교생활에 대한 만족도는 동아리 활동을 하는 대학생일수록 학교에 대한 만족도 또한 높은 것으로 나타났다. 기타 경제형편, 통학시간, 성적 및 전공 선택의 유형에 따라 학교생활 만족도에 영향을 미치는 것으로 나타났다. 3. 동아리 활동은 학습동기 구성요인 중 수업동기 요인, 계속동기 요인, 비본질동기 요인, 본질동기 요인에 유의한 차이를 보였으나, 본질동기 요인은 유형에 따라 유의한 차이를 보인다고 할 수 없다. 4. 동아리 활동을 통해 얻고자 하는 것이 단순한 정신적물질적 보상의 개념이 아니라 자기가 배운 분야를 실제적인 적용의 한 부분으로서 활용하고자 하였다.

폐암 환자의 불확실성과 디스트레스가 삶의 질에 미치는 영향: 극복력의 조절효과 (Relationships among Uncertainty, Distress, and Quality of Life in Lung Cancer Patients: Mediating effect of Resilience)

  • Purpose: The purposes of this study were to examine health-related quality of life and to identify the mediating effect of resilience on the relationship among uncertainty, distress, and health-related quality of life in lung cancer patients. Methods: A total of 149 lung cancer patients visiting the D hospital in B city completed a questionnaire, including demographic and disease-specific characteristics, uncertainty, distress, resilience, and health-related quality of life. Data were analyzed with descriptive analysis, t-tests, ANOVA, and multiple regression analyses via SPSS 24. Results: Health-related quality of life was $81.00{\pm}21.39$ (range 0~136) in lung cancer patients. In the results of hierarchical regression analyses, the health-related quality of life was associated with education, uncertainty, distress, and resilience. However, there was no mediating effect of resilience on the relationship among uncertainty, distress, and health-related quality of life. Conclusion: Lung cancer patients with high uncertainty and distress and low resilience could experience low health-related quality of life. In order to reduce uncertainty and distress, it is necessary to provide more detailed, systematic information and support, while reinforcing positive thinking.

The Effect of Religious Activities on Boredom through the Meaning in Life: Focusing on Protestantism

  • Boredom is considered anxiety and is an experience accompanied by a lack of meaning because the need for meaning is not met. It is the pain that people feel without using their mind, and the suffering that people fall into the absence of desire. On the other hand, religious activities give people meaning in life, which reduces the tendency to feel bored. This study investigates whether there is a difference in boredom depending on whether individuals participate in religious activities, and to verify whether the meaning in life mediates the relationships between religious activities and boredom. We predicted that if individuals engaged in religious activities, the meaning in life would increase, and they would be less bored than those who did not. A total of 82 college students participated in this study. Results showed that there were significant differences in the meaning in life and boredom between the two groups according to whether they participated in religious activities or not. The group actively participating in religious activities had significantly higher meaning in life and lower boredom than the non-religious group. Second, the meaning in life significantly mediated the relationship between religious activities and boredom. We conclude that active participation in religious activities can increase the meaning in life and ultimately reduce boredom. Based on the results of this study, the implications and suggestions for follow-up studies are discussed.

상급종합병원 간호사의 공감 역량과 도덕적 민감성이 임종간호 수행에 미치는 영향 (Factors Influencing the Empathy Competence and Moral Sensitivity of Nurses at Tertiary General Hospitals Performing End-of-life Care)

  • Purpose: The goal of this study aimed to understand the factors influencing the empathy competence and moral sensitivity of nurses working in tertiary general hospitals who offered end-of-life care. Methods: Descriptive data were collected from self-reported questionnaires completed by 164 nurses with direct nursing care experience with end-of-life patients at two tertiary general hospitals located in Busan, Korea. The variables examined were empathy competence, moral sensitivity, and end-of-life care performance. Data were analyzed using SPSS version 27.0 software. Results: Participants scored 3.73±0.40 on empathy competence, 4.97±0.51 on moral sensitivity and 2.91±0.37 on end-of-life care performance. There was a significant positive correlation between empathy competence and end-of-life care performance, and between moral sensitivity and end-of-life care performance. The factors affecting nurses' end-of-life care performance, empathy competence and moral sensitivity, exhibited with an explanatory power of 31%. Conclusion: Programs to increase empathy competence and moral sensitivity should be developed to improve nurses' end-of-life care performance.

사회학적인 변인이 의복일탈 태도 및 경험에 미치는 영향 - 서울지역 여자고등학생을 중심으로 - (The Effect of Social Variables on the Attitude and Experience Toward Clothing Deviation - For Female High School Students in Seoul -)

  • Deviation behavior of adolescents is often expressed as clothing behavior being against the social or school rules. To understand the relationship between social variables and clothing behavior of adolescents, the study investigated the effect of social variables on the deviation attitude and behavior related to clothing. Survey was utilized to collect the data and subjects were 411 female high school students. Regression analysis were used to analyze the data. The results were as follows : The closer relationship female high school students had with their parents, the more negative attitude they took toward clothing deviation and the less they committed the deviant behaviors related to clothing. The results explained that the close relationships with adolescents and their parents kept adolescents from committing the deviant behaviors related to clothing. The subjects who were interested in studying and well adapted for their school life, not only took a negative attitude toward clothing deviation but also committed the deviant behaviors related to clothing less than the other subjects did. The level of social deviation of their friends had an effect on the attitude and experience toward clothing deviation more than the relationship with their parents or school life did. The understanding of the relationship between social variables and clothing deviation would help to decrease the clothing deviation of adolescents by controling the social variables.
