  • 저자들은 2년전 궤양성 대장염으로 진단 받고 Sulfasalazine, Mesalamine등을 투약 해 오던 중 호흡곤란을 주소로 내원한 환자에서 임상양상및 방사선학적 소견, 경기관지 폐조직 생검소견등을 실시하여 궤양성 대장염으로 진단하고 스테로이드 제재등을 정하였으나 호흡부전으로 사망한 증례를 경험 하였기에 이를 문헌고찰과 함께 보고 하는 바이다.

괴화산(槐花散)이 Dextran Sulfate Sodium으로 유도된 생쥐의 궤양성 대장염에 미치는 영향 (Effects of Goihwa-san on Ulcerative Colitis Induced by Dextran Sulfate Sodium in Mice)

  • Objectives : This study was carried out to investigate the effects of Goihwa-san(GHS) against ulcerative colitis induced by DSS (dextran sulfate sodium). Method : The sample group was divided into three. The control group consisted of mice that were not inflammation-induced. The pathological group was composed of untreated colitis elicited mice. The experimental group was administered GHS after colitis elicitation. The effects on ulcerative colitis were evaluated by the morphological change of colonic mucosa, the anti-oxidant effect, HSP 70, NF-${\kappa}$B, COX-1, COX-2 and iNOS. Results : In terms of immunohistochemical changes, the distribution of COX-1 in mice treated with GHS increased noticeably more than that in the pathological group. The distributions of HSP70, NF-${\kappa}$B, COX-2, iNOS in mice treated with GHS more decreased than those in the pathological group. Regeneration of surface epithelial cell and goblet cell in mucosa was observed by optical microscope. The colonic lengths in GHS-treated mice were more elongated than those of DSS only treated mice. Conclusion : GHS is a candidate treatment for ulcerative colitis.

Inhibition of LSD1 phosphorylation alleviates colitis symptoms induced by dextran sulfate sodium

  • Inflammatory Bowel Disease is caused by an acute or chronic dysfunction of the mucosal inflammatory system in the intestinal tract. In line with the results of our previous study, wherein we found that the PKCα-LSD1-NF-κB signaling plays a critical role in the prolonged activation of the inflammatory response, we aimed to investigate the effect of signaling on colitis in the present study. Lsd1 S112A knock-in (Lsd1SA/SA) mice, harboring a deficiency in phosphorylation by PKCα, exhibited less severe colitis symptoms and a relatively intact colonic epithelial lining in dextran sulfate sodium (DSS)-induced colitis models. Additionally, a reduction in pro-inflammatory gene expression and immune cell recruitment into damaged colon tissues in Lsd1SA/SA mice was observed upon DSS administration. Furthermore, LSD1 inhibition alleviated colitis symptoms and reduced colonic inflammatory responses. Both LSD1 phosphorylation and its activity jointly play a role in the progression of DSS-induced colitis. Therefore, the inhibition of LSD1 activity could potentially protect against the colonic inflammatory response.

The Improving Effect of Gastrodia elata Blume on DSS-induced Colitis in Mice

  • Ulcerative colitis (UC) is a chronic inflammatory bowel disease characterized by abdominal pain, rectal bleeding and diarrhea. Gastrodia elata Blume (GE) has been used for the treatment of various diseases including neurodegenerative diseases and inflammatory disease. However, there has been no information on whether GE regulates intestinal inflammation. The aim of this study is to elucidate whether GE can protect against dextran sulfate sodium (DSS)-induced colitis in a mouse model. The colitis mice were induced by drinking water containing 5% DSS for 7 days. Body weight, colon length and clinical score were assessed to determine the effects on colitis. The levels of inflammatory cytokines, tumor necrosis factor $(TNF)-{\alpha}$ and interleukin (IL)-6 in colitis tissue were also measured. The results showed that mice administrated with DSS showed clinical signs including weight loss and reduced colon length. GE inhibited the DSS-induced loss of body weight and shortening of colon and increased Disease activity index score. Additionally, we observed that GE suppressed the levels of $TNF-{\alpha}$ and IL-6 in DSS-treated colon tissues. Collectively, these findings provide experimental evidence that GE might be a useful therapeutic agent for patients with UC.

Changes of colonic endocrine cells in trinitrobenzene sulfonic acid (TNBS)-induced rat colitis

  • In this study, immunohistochemistry was used to examine the changes in the density of colonic endocrine cells - argyrophil and argentaffin cells, chromogranin A (CGA), serotonin, somatostatin and glucagon-containing cells in trinitrobenzene sulfonic acid (TNBS)-induced rat colitis. Ulcerative colitis was induced by the instillation of 10 mg of TNBS into the colonic lumen through the anus. To confirm the inducement of ulcerative colitis, the macroscopic and microscopic scores as well as the colonic myeloperoxidase (MPO) activities were monitored for 8 days after TNBS instillation in the colonic lumens. In addition, the number of argyrophil and argentaffin cells, CGA, serotonin, somatostatin and glucagon-immunoreactive cells were counted in the colonic mucosa, respectively. After TNBS instillation into the lumen of the colon from the anus in rats, increases in macroscopic and microscopic scores in the colon tissues were observed along with increases in the colonic MPO activities. Therefore, ulcerative colitis was relatively well induced by the TNBS instillations. Marked decreases in the number of colonic endocrine cells were detected in the TNBS-treated animal compared to the sham control. These results suggest that colonic endocrine cells were also disrupted by TNBS-induced ulcerative colitis.

Anti-inflammatory Effect of Lactic Acid Bacteria Isolated from Kimchi on Acid-induced Acute Colitis in Model Mice

  • Lactic acid bacteria (LAB) provide numerous beneficial effects on the host body, especially on the intestine. Two LAB strains isolated from Kimchi, Leuconostoc mesenteroides and Lactobacillus sakei, were studied for its anti-inflammatory activity in acidinduced acute colitis in mice. To induce acute colitis in model mice (C57BL/6), 3% of dextran sulfate sodium treatment was treated for 7 days. Assessment of necropsy and histopathology analysis showed that oral supplementation of both L. mesenteroides and L. sakei ameliorated the symptoms of acute colitis. Moreover, the mixture of L. meseneroides and L. sakei showed synergistic effect on colitis. The results suggest that the formulation of L. mesenteroides and L. sakei mixture could be used as an oral supplementation to decrease the inflammatory harmful environment associated with colitis.

소아에서 발생한 대장의 염증성 질환에서 E-cadherin의 발현 (E-cadherin Expression in Colonic Epithelium of Various Colitis in Children)

  • 목 적: 소아에서 다양한 원인에 의해 발생한 대장염에서 점막의 형태학적 변화와 세포 접합에 다양한 변화가 있으리라 예상되어 세포 간의 결합을 유지하는 E-cadherin의 변화를 살펴보았다. 방 법: 1998년 1월부터 2003년 8월까지 부산대학교병원 소아청소년과에서 하부 위장관 내시경술과 대장점막 조직 검사를 통해 대장염으로 진단된 39명을 대상으로 하였다. 파라핀 블록에서 면역조직화학염색법을 이용하여 E-cadherin의 세포 내 발현을 조사하였다. 주변의 정상 조직과 비교하여 E-cadherin의 발현이 동일한 강도와 양상을 가진 세포가 50% 이상인 경우를 정상으로 판정하였고, 발현이 정상인 세포가 50% 미만이거나 염색 분포의 이상이 있거나 전혀 염색되지 않은 경우를 이상으로 판정하였다. 결 과: 1) 본 연구에서 비특이성 대장염 15예(38.5%), 크론병 7예(17.9%), 감염성 대장염 5예(12.8%), 음식 단백 과민성 직결장염 5예(12.8%), 궤양성 대장염 3예(7.7%), Henoch-Schonlein purpura 대장염 2예(5.1%), 그외 베체트병, 허혈성 대장염 1예가 포함되었다. 2) 모든 종류의 대장염에서 상피세포 E-cadherin 발현 감소가 관찰되었으며, 77%의 대상 표본에서 E-cadherin 발현감소가 있었다. 3) 활동성 염증이 심한 부위에서 Ecadherin 발현 감소가 현저하였으며 병변부에서 떨어진 상피세포에서는 정상 발현을, 궤양 주위나 재생 상피가 있는 부위는 심한 발현 감소를 보였다. 결 론: 모든 종류의 염증성 대장 질환에서 E-cadherin 발현 감소가 있었다. 이러한 변화는 염증과 궤양이 있는 부위에서 상피세포 접합을 느슨하게 함으로써 상피세포의 재생을 위한 세포의 이동을 용이하게 하기 위한 작용이라고 판단된다.

ST25(천추(天樞))에 대한 황련해독탕 약침이 DSS로 유발된 대장염 백서 모델에 미치는 영향 (The Effect of Hwangnyeonhaedok-tang Pharmacopuncture on ST25 (天樞) in Rats with Dextran Sulfate Sodium (DSS)-Induced Colitis)

  • Objectives The purpose of this study is to find out the effect of various concentrations of Hwangnyeonhaedok-tang (HTT) pharmacopuncture on ST25 (天樞) in rats with dextran sulfate sodium (DSS)-induced colitis.Methods Colitis was experimentally induced by feeding rats with water mixed with 5% (w/v) DSS for 20 days. The rats were divided into 5 groups: the normal group (Nor, n=5), the control group - colitis induced rats with no treatment (Con, n=5), the acupuncture group - colitis induced rats with acupuncture applied on ST25 (Acu, n=5), the pharmacopuncture group 1 - colitis induced rats with 0.729 mg/250 g/40 μl of pharmacopuncture applied on ST25 (PA-1, n=5), the pharmacopuncture group 2 - colitis induced rats with 3.645 mg/250 g/40 μl of pharmacopuncture applied on ST25 (PA-2, n=5). The changes in weight, excrement concentration and hemafecia were observed 5 times every 2 days. The colon lengths were measured from appendix to the end of colon after the experiment. Hematological and serological exams were conducted the day after the last treatment by cardiac puncturing anesthetized rats.Results ST25 is the abdominal front point (募穴) of large intestine meridian and is known to have effect in colitis. Various concentrations of HTT pharmacopuncture (HTTP) applied on ST25, in rats with DSS-induced colitis inhibited decrease in colon lengths and body weight in both PA-1 and PA-2 groups. Hematological and serological exam results also showed that HTTP has significant effect on colitis in both PA-1 and PA-2 groups.Conclusions Colon lengths were significantly increased in the acupuncture group, PA-1 group and PA-2 group, compared to the control group. The body weight was significantly increased (p<0.05) in PA-2 group after the first treatment, compared to the control group. TNF-α, IL-6, AST were significantly decreased in PA-1 and PA-2 groups, compared to the control group.

Diagnostic value of serum procalcitonin and C-reactive protein in discriminating between bacterial and nonbacterial colitis: a retrospective study

  • Background: Differentiating between bacterial and nonbacterial colitis remains a challenge. We aimed to evaluate the value of serum procalcitonin (PCT) and C-reactive protein (CRP) in differentiating between bacterial and nonbacterial colitis. Methods: Adult patients with three or more episodes of watery diarrhea and colitis symptoms within 14 days of a hospital visit were eligible for this study. The patients' stool pathogen polymerase chain reaction (PCR) testing results, serum PCT levels, and serum CRP levels were analyzed retrospectively. Patients were divided into bacterial and nonbacterial colitis groups according to their PCR. The laboratory data were compared between the two groups. The area under the receiver operating characteristic curve (AUC) was used to evaluate diagnostic accuracy. Results: In total, 636 patients were included; 186 in the bacterial colitis group and 450 in the nonbacterial colitis group. In the bacterial colitis group, Clostridium perfringens was the commonest pathogen (n=70), followed by Clostridium difficile toxin B (n=60). The AUC for PCT and CRP was 0.557 and 0.567, respectively, indicating poor discrimination. The sensitivity and specificity for diagnosing bacterial colitis were 54.8% and 52.6% for PCT, and 52.2% and 54.2% for CRP, respectively. Combining PCT and CRP measurements did not increase the discrimination performance (AUC, 0.522; 95% confidence interval, 0.474-0.571). Conclusion: Neither PCT nor CRP helped discriminate bacterial colitis from nonbacterial colitis.

항염증조절을 통한 자금정의 DSS 유도 궤양성 대장염 완화 효과 (Mitigating Effect of Jageum-jung on Dextran Sulfate Sodium-induced Ulcerative Colitis through Anti-inflammatory Regulation)

  • Objectives: This study investigated the anti-inflammatory effects of Jageum-jung extract on Dextran sulfate sodium (DSS-induced) ulcerative colitis in mice. Methods: Ulcerative colitis was induced by DSS in Balb/C male mice. Ten mice were assigned to each of four groups: Ctrl (control), UE (ulcerative colitis-induced), PT (treated with pentasaccharide after induction of ulcerative colitis), and JT (treated with Jageum-jung extract after induction of ulcerative colitis). The effects of Jageum-jung extract were measured by restoration of the length of the intestine, degree of mucosal damage as seen with histochemistry, and changes of p-IkB, iNOS, COX-2, and caspase-3 determined by immunohistochemistry. Results: The recovered intestinal length of the JT group was longer than that of the UE group. In the colon mucosa of JT group, hemorrhagic lesions were reduced, and the mucus barrier was recovered. This group also showed inhibited production of inflammatory enzymes (iNOS, COX-2) through regulation of proinflammatory enzyme (NF-kB, p65) activity in the colon. In addition, caspase 3 activation induced apoptosis. By GC/MS analysis, azetidine was identified. Conclusions: This study confirmed the anti-inflammatory effects of jageum-jung extract, and suggests the possibility of using Jageum-jung extract to treat ulcerative colitis. Further experiments and research on the mechanism of Jageum-jung effects are needed.
