국내 고령자의 Y-DuCog 표준치, 인지기능에 관한 연구 (Normative Data of the Yonsei Dual Task Cognitive Screening Test (Y-DuCog) for Korean Older Adults and Characteristics of Cognitive Function)

  • Purpose : This study aimed to present normative data for older Korean adults completing the Yonsei dual task cognitive screening test (Y-DuCog) and identify changes in cognitive function on the Montreal Cognitive Assessment - Korean (MoCA-K) with age. Methods : From May 2019 to August 2019, 195 healthy adults aged ≥60 years participated in this study. All participants completed the Y-DuCog to assess their dual-task performance and the MoCA-K to assess their cognitive function. Participants were divided into three groups based on their age: 60~69 years, 70~79 years, and ≥80 years. Results : The results of the Y-DuCog showed that dual-task performance time, effect, and correct response rate decreased significantly with age (p<.001). Scores from the three groups showed differences on all items (p<.001). Cognitive function on the MoCA-K also decreased significantly with age (mean score ± standard deviation [SD]; 27.33 ± 2.61 in subjects aged 60~69 years; 24.82 ± 3.20 in subjects aged 70~79 years; and 22.10 ± 4.91 in subjects aged ≥80 years; p<.001). Conclusions : Occupational therapists should be aware of the decline in cognitive function and dual-task performance time, effect, and correct response rate in older adults and consider interventions to treat this decline. Further studies are needed with larger groups of participants to examine factors, such as sex and education, that may impact dual-task performance and cognitive function.

경증 외상성 뇌손상 환자에서 신체적 증상, 우울, 불안과 인지기능의 관계 (Relations between Somatic Symptoms, Depression, Anxiety, and Cognitive Function in Patients with Mild Traumatic Brain Injury)

  • Objectives : This study was aimed at evaluating the relationship between somatic symptoms, depression, anxiety and cognitive function in the patients with Mild Traumatic Brain Injury(MTBI). Methods : Thirty seven patients with MTBI were selected from those patients who had visited the Department of Neuropsychiatry of Wonkwang University Hospital from 2003 to 2007. To assess and quantify the somatic symptoms, depression and anxiety, Personality Assessment Inventory(PAI) was used. Assessment of cognitive function was carried out by using Korean Wechsler Adult Intelligence Scale(K-WAIS), Rey-Kim Memory Test, and Kims Executive Function Test. The effects of somatic symptoms, depression, and anxiety on the cognitive function were evaluated by Pearson correlation test. Results : Somatic symptoms, depression, and anxiety, all showed inverse correlation to cognitive function. Specifically, 1) an increase in somatic symptoms was associated with a decrease in attention, verbal short term memory, verbal recall and recognition, and visual memory. 2) An increase in anxiety was associated with a decrease in verbal recall and recognition. 3) An increase in depression was associated with a decrease in cognitive function that requires high attention and verbal memory. Conclusion : The patients with MTBI displayed diverse symptoms ranging from cognitive impairment to somatic symptoms, depression, and anxiety. Somatic and emotional symptoms were correlated with cognitive function(especially executive function). Importantly, this study raises the possibility of treating the cognitive impairment associated with MTBI by treating somatic symptoms, depression, and anxiety.

Effects of Combined Cognitive and Physical Exercise Program on Cognitive and Physical Functions in Older Adults with Mild Cognitive Impairment

  • Objective: This study aimed to investigate the impact of a 12-week combined cognitive and physical exercise program on cognitive and physical functions in older adults diagnosed with mild cognitive impairment (MCI). Design: A one-group pretest-posttest study. Methods: Twelve participants with MCI engaged in a weekly 60-minute session of combined cognitive and physical exercise program. Cognitive function was assessed using the Montreal Cognitive Assessment (MoCA), while physical function was evaluated through measures of muscle strength, postural balance, and walking capabilities. Muscle strength assessments included the arm curl test, handgrip strength, and the 5 sit-to-stand test. Postural balance was evaluated using the one-leg stance test, timed up-and-go test, functional reach test, and four square step test. Walking function was analyzed through a gait analysis device. Pre- and post-intervention measurements were compared to determine the effects of the exercise program. Results: The results demonstrated significant improvements in MoCA, arm curl test, timed up-and-go test, walking speed, and cadence following the 12-week intervention (p<0.05). MoCA scores revealed enhanced cognitive performance, while measures of muscle strength, including the arm curl test, exhibited significant changes. Improvements in timed up-and-go test scores indicated enhanced mobility, accompanied by increased walking speed and cadence, as evidenced by gait analysis. Conclusions: This study suggests that a structured 12-week program incorporating both cognitive and physical exercises can lead to meaningful improvements in cognitive and physical functions among older adults with MCI.

치매노인을 대상으로 한 인지 자극 훈련의 효과 (The Effect of Cognitive Stimulation Training on Elderly Persons with Dementia)

  • This study was conducted to test the effect of cognitive stimulation training on elderly persons with dementia. The design of the research was one group in pre-test-post-test design. The subjects were nine demented persons over sixty years, with mild to moderate cognitive impairment. Training was administered by research assistants on a one to one basis for thirty to forty minutes, three times a week for eight weeks. In order to evaluate the effect of cognitive stimulation training, we measured cognitive function before and after three training sessions each. Data were analyzed using descriptive statistics and a paired t-test analysis using a spss pc package, The results are as follows: 1) The recipients of the training program showed improvement in overall cognitive functioning. The MMSK - K score, recall levels of concepts, daily tasks, personal past history and performance of word fluency were significantly increased after training. 2) There was a significant improvement in cognitive functioning over the training period: recall levels of concepts, daily tasks, past personal history and performance of word fluency significantly increased over the training period progessively, The results suggest that cognitive stimulation training is effective in improving and maintaining overall cognitive function of elderly persons with dementia.

실버인지건강프로그램이 노인의 인지기능 향상에 미치는 영향에 대한 연구 (The Study of the Effect on the Improvement of Cognitive function by Cognitive Health Program)

  • 현 고령화 사회로 치매에 대한 관심이 높아지고 있다. 하지만 치매예방에 대한 국가 차원의 프로그램이 미흡한 실정이다. 치매 예방 프로그램과 보급을 위해, 인지 기능 향상 프로그램을 실시하고 인지 기능 변별력, 구성력, 사고력, 기억력, 집중력으로 나누어 평가하여, 이러한 프로그램이 노인의 인지 기능에 구체적으로 어떤 영향을 미치는지 알아보고자 한다. 실버인지 건강 프로그램을 편성하고, 실험 군과 대조군을 모집해 3개월 동안 프로그램을 실시하고 실험 군과 대조군을 대상으로 MMSE-DS, 노인 우울척도, 인지영역테스트 실시하여 설문조사 방법을 통해 전후를 비교, 분석한다. 연구결과는 다음과 같다. 1. 프로그램 진행 전과 후의 인지 기능, MMSE-DS, 노인 우울척도를 비교한 결과 인지건강프로그램이 인지기능, MMSE-DS 및 노인우울척도 개선에 유의한 효과가 있었다. 2. 인지 기능을 5개 영역으로 나누어 테스트한 결과 기억력, 사고력, 집중력, 변별력, 구성력 순으로 향상되었다. 3. 인지건강 프로그램 실시 후 기억력과 사고력이 가장 많은 변화를 보였는데 뇌의 노화가 기억력과 사고력 부분에 많이 일어나므로, 이에 대한 집중적인 교육이 더 좋은 효과를 가져 올 수 있다.

종합 신경인지기능 평가(Comprehensive Neurocognitive Function Test; CNFT)의 표준화 예비 연구 (A Study on the Standardization of Comprehensive Neurocognitive Function Test)

  • 목적 : 본 연구의 목적은 종합 신경인지기능 평가인 Comprehensive Neurocognitive Function Test(CNFT)의 표준화 자료를 얻고 신뢰도와 공인타당도를 검증하기 위함이다. 연구방법 : CNFT는 기억력, 집중력, 감각,운동협응, 전두엽 및 고위인지기능을 평가하는 것으로 결과는 전산화되어 일관성 있고 자동으로 처리된다. 연구대상은 표준화 자료를 얻고 신뢰도를 검증하기 위한 조사에서 20대부터 80대 이상까지 서울특별시와 대구시에 거주하는 정상 성인 140명이 참여하였다. Computerized Neurocognitive Function Test(CNT)와의 상관관계를 통하여 타당도를 검증하기 위한 조사에는 정상 성인 40명이 참여하였다. 결과 : 정확도는 연령이 증가할수록 낮아지며, 반응속도는 늦어졌다. 또한 영역별 점수 분석결과 기억력, 전두엽 및 고위인지기능 점수가 낮은 편으로 나타났다. 2주 간격의 검사-재검사 신뢰도는 r=.48~.85로 나타났고 두 검사 사이의 유의한 차이는 없었다(p>.05). CNT와의 세부 영역과의 상관계수는 r=.67~.79이다(p<.05). 결론 : 종합 신경인지기능 평가인 CNFT의 표준자료와 해석기준이 마련되었다. CNFT는 인지기능에 문제를 평가할 수 있는 신뢰도와 타당도를 갖춘 평가도구이다.

일부 치매 및 치매의심환자들의 인지기능에 따른 구강보건인식 조사 (Perceived oral health awareness in dementia and dementia-suspected depending on KMME)

  • Objectives: The purpose of this study is to investigate cognitive function, performance of activities of daily living, and recognition on oral health with the cognitive function testto dementia or dementia-suspected patients in the outpatients. Methods: The subjects were 94 dementia or dementia-suspected patients visiting C University hospital for the dementia test. Study instruments included Korea Mini-Mental State Examination KMMS, The Bayer-Activities of Daily Living Scale; B-ADL, Seoul-Instrumental Activities of Daily Living; S-IADL, Global Deterioration Scale; GDS, Korean Dementia Screening Questionnaire; KDSQ, and underlying diseases. Results: Dementia or dementia-suspected patients were 42 by KMMSE test, 25 patients had impaired functioning of daily living by B-ADL test, 27 patients showed the presence of depression by GDS test, and 45 patients showed impaired functioning of daily living. There was a statistically significant difference in the subjective recognition on oral health conditions. There was a statistically significant difference in the subjective recognition on oral health conditions by ADL. There was a positive correlation between the cognitive function and ADL performance. Higher cognitive function is proportional to ADL performance. Conclusions: The cognitive function was closely associated with ADL and subjective oral health conditions.

학습기술과 인지기능과의 관계 연구 (A Study on Relationship between the Learning Skills and the Cognitive Functions)

  • The purpose of this study is to investigate the relationship between learning skills and cognitive functions on elementary school students. In this study CAS and Learning Skills Test(LST) were administered with 3 to 6 grade, 60 students from 5 elementary schools. The data were analyzed according to Pearson's correlation and Stepwise Multiple Regression Analysis. The results are as follows. Firstly, girls and older students showed significantly higher ability than boys and younger students on the learning skills. And girls significantly outperformed boys on the planning function and attention function and on the simultaneous cognitive function was the other way round. Secondly, learning skills were explained 41% by two variables that the planning function and the successive function which are sub factors of the cognitive function. And then, planning and successive processing effected to self-management, attention and planning to test-taking skills, successive processing and attention to class-participation skills, and successive processing to information processing.

재가 노인을 위한 집단인지기능향상프로그램이 인지기능, 우울 및 삶의 질에 미치는 효과 (The Effects of a Cognitive Enhancement Group Training Program for Community-dwelling Elders)

  • Purpose: In this study a cognitive enhancement group training program of 10 sessions was provided for communitydwelling elders and the effects on cognitive function, depression and quality of life were tested. Methods: A quasi-experimental study using a nonequivalent control group, pre-post design was used. The participants were 87 elders whose cognitive function was within the normal range. Of these elders, 45 were assigned to the experimental group and 42 to the control group. The intervention was conducted once a week for 10 weeks. Chi-square test, t-test, paired t-test, Wilcoxon rank sum test and Wilcoxon signed rank test were used to analyze the data. Results: After the program, the cognitive function (t=-2.85, p=.006), depression (z=2.82, p=.005) and quality of life (t=2.79, p=.007) of the experimental group was significantly better than those of the control group. Especially, immediate recall (z=2.45, p=.014) and concentration (z=2.58, p=.010) in the subcategory of cognitive function were significantly better than that of the control group. Conclusion: The findings indicate that the cognitive enhancement group training program was effective in enhancing the cognitive function, depression and quality of life for elders and could therefore be considered as a positive program for emotional and cognitive support for community-dwelling elders.

보행기호잇기검사: 새로운 신체 및 인지 기능에 대한 이중 과제 평가도구로서의 효용성에 대한 예비연구 (Stepping Trail Making Test: Preliminary Study for the Effectiveness of the Novel Dual Task Assessment Tool for Physical and Cognitive Functions in Elderly )

  • PURPOSE: Falls are caused by a decline in physical and cognitive function. A quantitative evaluation tool that can comprehensively evaluate motor and cognitive functions for elderly people with an impaired physical function. This study assessed the clinical application potential by confirming the correlation between the physical function tests, cognitive and the stepping trail-making test (S-TMT). METHODS: Fourteen community-dwelling older patients (65-75 years) were recruited. The study conducted cognitive function tests with the trail-making test (TMT-A, B), and physical function tests (6-minute walking test (6MWT), short physical performance battery (SPPB), and timed up and go (TUG)). The results of were analyzed using SPSS version 21.0. Descriptive statistics were used for the general characteristics of the study subjects, and the correlations between S-TMT, other functional tests were examined through Pearson's correlation analysis. The statistical significance was set to .05. RESULTS: S-TMT had a significant positive correlation with the TUG (r = .588*) and trail-making test-B (TMT-B) (r = .689*, p < .05). Furthermore, S-TMT showed a negative correlation between SPPB (r = -.397) and 6MWT (r = -.422), but it was not statistically significant. CONCLUSION: S-TMT is a cognitive-gait dual-task performance evaluation tool that can be performed safely. A significant correlation was confirmed between the TUG test and the TMT-B. S-TMT is a dual-task screening tool that can evaluate both physical and cognitive functions simultaneously.
