신체활동은 노화로 의한 인지기능 저하에 긍정적 영향을 미친다. 하지만 신체활동 참여가 정서인지기능에 미치는 영향을 과학적으로 밝혀 낸 연구는 드물다. 따라서 본 연구의 목적은 청년기와 노년기 성인의 정서인지 기능에 신체활동 참여가 어떠한 영향을 미치는지를 뇌 과학적으로 연구하는 것이다. 연구의 피험자는 D 광역시 성인 남성 60명으로 선정하였으며, 청년 운동 집단(25~35살), 청년 비 운동 집단(26~35살), 노년 운동 집단(60~70살), 노년 비 운동집단(60~70살) 4개의 집단으로 구성하였다. 실험도구는 EEG 측정 장비와 국제정서사진체계(International Affective Picture System: IAPS)를 사용하였다. 본 연구의 실험과제는 사진으로 제시하는 세 가지 정서자극(긍정적, 중립, 부정적)에 적합한 버튼을 눌러 정서적인지 검사를 실시하였다. EEG 측정은 8개(Fp1, Fp2, Fz, C3, C4, Cz, T3, T4)의 영역을 측정하였다. 통계처리는 4(집단)${\times}$3(자극)${\times}$8(영역)에 대한 삼원분산분석을 실시하였다. 결과: 본 연구 결과, 반응시간과 반응정확률 그리고 P3 잠재기는 집단간 주효과가 나타났다. 그리고 P3 진폭은 집단과 자극간 상호작용이 나타났다. 결론: 신체활동은 청년층과 노년층의 정서적 인지기능에 긍정적인 영향을 미치는 것으로 나타났다.
목표: 본 연구는 음악요법이 치매 노인의 인지기능, 초조행동, 불안, 우울에 미치는 영향을 규명하는 것을 목적으로 한다. 방법: 2010년부터 2019년까지 PubMed, EMBASE, Cochrane Library, CINAHL, Web of Science, Google scholar, PsycINFO에서 종합적인 문헌검색을 하였고 메타분석에서는 RevMan 5.4 프로그램을 사용하여 표준화된 평균 차이(Hedge's g)와 95% 신뢰 구간은 요약 측정으로 산출하고 랜덤 효과 모델과 역분산 방법을 적용하였다. 총 13개의 연구가 포함되었으며, 모두 오류 위험 평가를 위한 코크란 평가도구를 근거로 질 평가를 하였다. 결과: 효과 크기(Hedge's g)는 1차 결과 변수인 인지기능 0.31[95% CI:-0.02, 0.65], 초조행동 -0.03[95% CI: -0.17, 0.11], 2차 결과 변수인 불안 -0.61[95% CI: -1.17, -0.05], 우울 -0.44[95% CI: -0.88, 0.00]이었다. 음악중재 유형별 하위그룹 분석 결과 복합음악요법이 치매 환자의 인지기능(g=0.45[95% CI: 0.03, 0.87])에 유의한 증가 효과가 있는 것으로 나타났다. 결론: 음악요법은 불안과 우울을 감소시키는 데 유의한 효과를 보였으며, 복합음악치료는 치매 환자의 인지기능 개선 효과를 보였다.
본 연구는 가정방문 인지자극 활동이 치매특별등급(5등급) 노인의 인지기능 및 기억 자기효능감에 미치는 효과를 알아보고자 하였다. 연구기간 및 대상은 2019년 2월 11일부터 4월 26일까지 G광역시에 거주하는 65세 여성 1명으로, 개별대상연구의 A-B-A' 설계를 사용하였다. 초기 진단을 받은 치매특별등급(5등급)의 경증치매 노인에게 비교적 접근성이 쉽고, 스스로 생각하고 풀어갈 수 있는 수준의 신체·인지자극 활동을 병행함으로써 지적능력을 반복적으로 자극하여 저하된 인지기능을 완화시켰고, 일상생활에서 친숙한 글자, 숫자, 신체 움직임 등을 이용한 단기기억 및 회상 활동을 지속적으로 훈련함으로써 저하된 기억력과 집중력의 인지기능에 긍정적 영향을 미쳤다. 이는 인지자극 활동이 초기 경증치매 노인들에게 센터, 복지관 등의 특정장소를 벗어나 가정에서 손쉽게 접근 가능한 치매예방프로그램임을 확인하였다. 따라서 본 연구의 인지자극 활동은 치매특별등급(5등급) 노인의 인지기능 및 두드러지는 기억저하에 긍정적 영향을 미칠 수 있는 효과적 중재로의 가능성을 확인하였기에, 적극적인 실용 방안을 제안하는 바이다.
Purpose: The purpose of this study was to identify the prevalence of visual and hearing impairment and to investigate the effect of visual and hearing impairment on depression and cognitive function in community-dwelling Korean elderly. Methods: The study population consisted of a representative community sample of 4,028 persons aged 65 and older from the Korean Longitudinal Study of Aging 2008. Depression and cognitive function were measured by CES-D 10-item scale and K-MMSE. Visual and hearing impairment were measured by 5-Likert scale. Data were analyzed using Rao-Scott ${\chi}^2$-test, simple and multiple logistic regression. Results: The prevalence of visual and hearing impairment were 37.3% and 14.0%, respectively. Of the participants, the prevalence of depression was 58.4% and cognitive impairment was 50.6%. There were significant differences in depression and cognitive function according to visual and hearing impairment. Far vision and hearing impairment were predictors for both depression and cognitive impairment even after covariates had been adjusted. Conclusion: These data suggest that visual and hearing impairment in older adults may increase their probability of experiencing depression and cognitive impairment. Thus sensory impairment should be considered a risk factor for mental health and the cognitive function of elderly.
Purpose: In this study a cognitive enhancement group training program of 10 sessions was provided for communitydwelling elders and the effects on cognitive function, depression and quality of life were tested. Methods: A quasi-experimental study using a nonequivalent control group, pre-post design was used. The participants were 87 elders whose cognitive function was within the normal range. Of these elders, 45 were assigned to the experimental group and 42 to the control group. The intervention was conducted once a week for 10 weeks. Chi-square test, t-test, paired t-test, Wilcoxon rank sum test and Wilcoxon signed rank test were used to analyze the data. Results: After the program, the cognitive function (t=-2.85, p=.006), depression (z=2.82, p=.005) and quality of life (t=2.79, p=.007) of the experimental group was significantly better than those of the control group. Especially, immediate recall (z=2.45, p=.014) and concentration (z=2.58, p=.010) in the subcategory of cognitive function were significantly better than that of the control group. Conclusion: The findings indicate that the cognitive enhancement group training program was effective in enhancing the cognitive function, depression and quality of life for elders and could therefore be considered as a positive program for emotional and cognitive support for community-dwelling elders.
본 연구의 목적은 우리나라 재가 독거 여성노인의 우울 여부에 따른 인지기능, 도구적 일상생활 수행능력의 차이를 파악하고, 제 변수들 간의 상관관계를 확인하기 위함이다. 자료 수집은 2011년 4월부터 2012년 4월까지 구조화된 설문지로 면접 조사하였으며, 총 1,426명의 자료를 분석하였다. 제 변수들 간의 상관관계를 살펴본 결과, 우울과 인지기능(r=-.208, p<.001), 인지기능과 도구적 일상생활 수행능력(r=-.256, p<.001)은 약한 역상관관계를 보였고, 우울과 도구적 일상생활 수행능력(r=.222, p<.001)은 약한 순상관관계가 있는 것으로 나타났다. 이상의 연구 결과를 토대로 재가 독거 여성노인의 인지기능과 사회적 활동 수준의 향상을 통해 우울 수준이 저하될 수 있도록 앞으로 정신건강 유지 및 증진을 위한 방안 마련에 더욱 관심을 가져야 할 것이다.
Purpose: To investigate the influence of clinical and demographical variables on depression, instrumental activities of daily living (IADL), and cognitive function in patients with Parkinson's disease. Method: Using a structured questionnaire data were collected from 100 participants registered in the neurology department of C university hospital. Duration and stage of disease, fall history, vision impairment, duration and quality of sleep, orthostatic hypotension, ambulation impairment, and use of walking aid were included in clinical variables. Depression, IADL, and cognitive function were assessed using Kee's GDSSF-K, Cho's scale, and K-MMSE. Collected data were analyzed using the SAS program. Results: The depression score for the participants was 7.78, higher than cut-off score(5). Participants who were male, living with spouse, above high school education, high economic status, with no vision impairment, and no ambulation impairment revealed high cognitive scores. Average IADL score were significantly higher for participants who were male, who had high economic status, low stage of disease, and no ambulation impairment. Depression IADL & cognitive function scores were significantly different according to ambulation impairment. Scores for ADL and cognitive function were positively correlated. Conclusion: It is recommended that make programs for patients with Parkinson's disease, clinical and demographic variables should be considered according to their individual needs.
Objective To investigate the anatomical correlates of the neuropsychological deficits in patients with the cerebellar stroke. Methods We screened patients who were admitted to the National Rehabilitation Center with the cerebellar stroke between October 2012 and November 2016. The patients with the cerebellar stroke who underwent neuropsychological testing for which the Seoul Neuropsychological Screening Battery (SNSB) or the SNSB-II were enrolled. The neuropsychological function capacities were compared in accordance with the stroke type (hemorrhagic vs. ischemic) and the location (right/left anterior, right/left posterior intermediate, right/left posterior lateral lobe, and vermis). Mean z-scores were computed to compare the patient performances with the population averages. Results Twenty-six patients (15 with ischemic stroke and 11 with hemorrhagic stroke) with a mean age of $54.8{\pm}16.6$ years were assessed $8.8{\pm}9.2$ months after the stroke. Differences in the neuropsychological functioning according to the stroke type were not observed. All of the numerical subtests of the stroke patients showed significantly poorer performances compared with the population averages (mean z-score <0), and some of the subtests revealed abnormal performances in attention-, visuospatial function-, memory-, and frontal/executive function-related tasks (mean z-score <-1). The patients with the presence of a lesion in the right posterior intermediate lobe of the cerebellum showed a poorer performance in the subtests evaluating the executive function including the Korean-version Stroop Test (p=0.04), the Digit Symbol Coding Test (p=0.01), and the Korean-version Trail Making Test (p=0.02) compared with the patients without that lesion. Conclusion The present study confirms that the cerebellar stroke affects the neuropsychological functioning which is associated with the anatomical site of stroke.
본 연구는 독거노인의 인지기능과 삶의 만족도에 영향을 미치는 위험요인을 탐색하고자 하였다. 전국표본추출로 실시된 2014년도 노인실태조사 원시자료 중에서 만 65세 이상인 독거노인 2,449명의 자료를 분석하였다. 인지기능의 위험요인은 고령과 사별경험, 무학력, 운동 안함, 낮은 사회문화 활동만족도로 확인되었다. 삶의 만족도 위험요인은 이혼/미혼/별거경험, 무학력, 미취업, 부정적 건강평가와 우울진단, 운동 안함, 정서적 학대, 경제적 학대, 재정적 방임 경험과 낮은 인지기능 수준으로 확인되었다. 이는 고령의 사별한 독거노인 대상의 적극적인 인지기능 검사 및 강화 프로그램, 사회문화활동 참여를 위한 네트워킹, 우울 예방과 운동 및 건강증진 프로그램, 노인학대 예방을 위한 사회적 노력이 필요함을 시사해준다.
Purpose: The purpose of this study was to examine the effects of a smart-care services program for breast cancer survivors on cognitive function and physical health. Methods: A quasi-experimental control group pretest posttest design was used. Subjects were recruited in D city, and data were collected from July 2017 to February 2018. The experimental group (n=24) participated in the smart-care services program, whereas the control group (n=26) received conventional management. The smart-care services program consisted of addressing cognitive function problems arising from chemotherapy, diet, exercise, head/neck massage and self-monitoring using smartphone applications and smart bands. All participants underwent assessments at baseline, at 6 weeks, and at 12 weeks. Data were analyzed using descriptive statistics (frequency, percentage, mean and standard deviation), a chi-squared test, t-test, and repeated measures ANOVA. Results: After the smart-care services program, significant differences were found between the groups in cognitive function (F=18.91, p<.001) and sleep time (F=9.25, p<.001). No significant differences were found between the groups in caloric consumption after the program. Conclusion: The smart-care services program significantly improved the level of cognitive function and sleep time for breast cancer survivors. The use of this smart-care services program for breast cancer survivors might be an effective nursing intervention tool for improving cognitive function and health behaviors.
