손목 터널 증후군에 대한 추나요법의 효과: 체계적 문헌 고찰 (Chuna Manual Therapy for Carpal Tunnel Syndrome: A Systematic Review)

  • Objectives This systematic review aimed to provide evidence for the effectiveness and safety of Chuna manual therapy (CMT) for carpal tunnel syndrome (CTS). Methods We searched 12 electronic databases (PubMed, Ovid-MEDLINE, EMBASE, The Cochrane Library, CNKI, CiNii, ScienceON, KMbase, KISS, OASIS, KMTANK, and RISS) up to the end of October 2021. We only included randomized controlled trials (RCTs) investigating CMT for the treatment of CTS. The methodological quality of the included RCTs was evaluated using the Cochrane Risk of Bias Tool 1.0. Results Six RCTs were eligible for inclusion in our study. A meta-analysis of four studies demonstrated positive results for the use of CMT when used in conjunction with Korean medicine treatment compared to those for Western conservative treatment for CTS. Conjunction treatment significantly improved the total efficacy rate compared to conservative treatment alone (P=0.0007, n=4). Conclusions There is reliable evidence for the use of CMT in treating CTS based on a published meta-analysis. To measure only the effect of Chuna, an experimental group should be designed with Chuna alone to evaluate its effectiveness. However, it should be noted that the studies included in this systematic review were heterogeneous and of low quality, thus warranting further investigation using well-designed RCTs.

폐색성 혈전 혈관염의 침 치료에 대한 체계적 문헌고찰 (A Systematic Review of Acupuncture Treatment for Thromboangiitis Obliterans)

  • Objectives The purpose of this study is to present evidence by analyzing the research trends in acupuncture treatment in the last 10 years for thromboangiitis obliterans. Methods Randomized controlled trials about acupuncture on thromboangiitis obliterans were searched China National Knowledge Infrastructure (CNKI), PubMed, EMBASE, Cochrane Library, Oriental Medicine Advanced Searching Integrated System (OASIS), ScienceON, Koreanstudies Information Service System (KISS) and Research Information Sharing Service (RISS) from January 1, 2011 to November 13, 2021. Only randomized controlled trials were selected and we assessed the risk of bias (RoB) according to the revised Cochrane RoB2 criteria. Results A total of 5 randomized controlled trials (RCTs) were selected in this review and all were conducted in China. A total of 326 participants were in 5 RCTs. SP10, ST36, GB34 was the most commonly used treatment point. There were four types of acupuncture used: manual acupuncture, embedding acupuncture, pharmacopuncture, electroacupuncture. The most commonly used indicator for evaluation was the total efficacy rate, and all five studies were significantly higher. Conclusions All selected studies showed the group treated with acupuncture treatments on thromboangiitis obliterans were statistically more effective than the control group. However, the number of studies is too small and the interpretation of the results is limited due to the inclusion of subjective evaluation. So more objective and systematic studies should be conducted continuously.

퇴행성 슬관절염에 대한 약침 및 봉독 요법의 효용성 및 안전성: 체계적 문헌 고찰 및 메타 분석 (Efficacy and Safety of Pharmacopuncture and Bee venom Acupuncture for Knee Osteoarthritis: A systematic review and meta-analysis)

  • Objectives: The purpose of this study was to evaluate the efficacy and safety of pharmacopuncture and bee venom acupuncture for knee osteoarthritis. Methods: We searched for randomized controlled trials that investigated the effects of pharmacopuncture and bee venom acupuncture on knee osteoarthritis through the electronic databases including Pubmed, EMBASE, Cochrane, CiNii, CNKI, KMBASE, KISS, NDSL, and OASIS. Meta-analysis was performed by Review Manager software and the quality of included studies were assessed by the Cochrane risk of bias tool. Results: A total of 20 articles with 1536 participants were identified. 12 trials about phamacopuncture and 8 trials about bee venom acupuncture showed significant improvement than sham treatment, western medicine treatment, and other Korean medicine treatment such as acupuncture in diverse scales. Six trials reported adverse events. Conclusions: These findings showed certain efficacy and safety of pharmacopuncture and bee venom acupuncture. It would be helpful for patients and Korean medicine doctors in the choice of the treatment for knee osteoarthritis. Well-designed studies with long term follow up and more number of participants should be conducted to strengthen the evidence of the use of pharmacopuncture and bee venom acupuncture.

소아 말초성 안면마비에 대한 추나요법의 효과 : 체계적 문헌고찰 (Effect of Chuna Manual Therapy for Peripheral Facial Paralysis in Children : A Systematic Review)

  • Objectives : The purpose of this study is to review clinical studies about the effect of chuna manual therapy (CMT) for peripheral facial paralysis in children. Methods : In this review, we searched 11 electronic databases (Pubmed, Cochrane Library, EMBASE, CINAHL, CAJ, Oasis, NDSL, KISS, RISS, KISTI, Dbpia); We concluded our literature search in April 23, 2019. We included only randomized controlled trials (RCTs) of testing CMT for peripheral facial paralysis in children. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. The meta-analysis was performed by synthesizing outcome data of total efficacy rate (TER). Results : After screening papers, a total of 6 RCTs were selected and analyzed. In the 6 RCTs, patients(n=15-60 per study) were randomized into groups for treatment and control. Specifically, the treatment group received CMT, while the control group was concurrently given usual care, such as acupuncture and medicine. The meta-analysis showed that the treatment group receiving CMT alone showed significant improvement in TER, compared to the control group receiving acupuncture therapy alone(P<0.05). And the treatment group receiving CMT combined with usual care showed positive results, in terms of TER, compared to the control group receiving usual care, but was not statistically significant(P>0.05). Conclusions : Our analysis suggests that CMT has therapeutic effects for peripheral facial paralysis in children. However, to confirm this result, further investigation accompanied by high quality studies is required.

뇌졸중 환자의 실어증에 대한 전침 치료 : 체계적 문헌 고찰 (Electro-Acupuncture on Aphasia after Stroke: A Systemic Review of Randomized Controlled Trials)

  • Objectives: This study investigates the effect of electro-acupuncture on aphasia after stroke. Methods: A search of OASIS, NDSL, PubMed, Cochrane, and CNKI was executed between 4 January 2021 and 4 February 2021, with no limitation on publication year. Extraction and selection from the studies were made by 3 authors. The quality of the studies was evaluated using Cochrane's risk of bias (RoB) tool. Results: 10 studies met the selection criteria. As the treatment site for electro-acupuncture, GV20 (Baihui) was used the most. In all studies, the region located on the head was used for treatment without distinguishing between acupoints and areas of scalp acupuncture, and the stimulation was organized into 3 conditions: speed, intensity, and time. The outcome indicators used before and after treatment focused on the evaluation of language function and the degree of aphasia. The results showed that using electro-acupuncture with speech rehabilitation therapy for aphasia after stroke was more effective than using speech rehabilitation therapy alone. Conclusions: In this review, electro-acupuncture for aphasia after stroke was found to have a significant effect compared to the previous treatment alone. However, because of limitations, information was not reliable enough. Additional research is needed to produce more objective evidence.

  • Objectives: Cupping therapy (CT) has been widely used in traditional medicine worldwide for various indications, including stroke. The aim of this study was to systematically review the clinical evidence of CT for stroke. Methods: To identify randomized controlled trials (RCTs) reporting the effectiveness and/or safety of CT, seven databases including PubMed, EMBASE, and Cochrane Library were searched for articles published from January 2000 to February 2021 without language restrictions. Meta-analysis was performed using Review Manager 5.4 software and the results were presented as mean difference (MD) or standard mean difference (SMD) for continuous variables and odds ratio (OR) for diverse variables with 95% confidence intervals (CIs). Assessment of the methodological quality of the eligible trials was conducted using the Cochrane Collaboration tool for risk of bias in RCTs. Results: Twenty-two RCTs with 1653 participants were included in the final analysis. CT provided additional benefit in improving upper limb motor function (Fugl-Meyer assessment for upper limb motor function, MD 6.91, 95% CI 4.64 to 1.67, P<0.00001) and spasticity (response rate, OR 3.28, 95% CI 1.31 to 8.22, P=0.08) in stroke survivors receiving conventional medical treatment. These findings were supported with a moderate level of evidence. CT did not significantly increase the occurrence of adverse events. Conclusions: This study demonstrated the potential of CT to be beneficial in managing a variety of complications in stroke survivors. However, to compensate for the shortcomings of the existing evidence, rigorously designed large-scale RCTs are warranted in the future.

유착성 관절낭염에 대한 추나요법의 효과: 체계적 문헌고찰과 메타분석 (Chuna Manual Therapy for Adhesive Capsulitis : A Systematic Review and Meta Analysis)

  • Objectives : To evaluate the effectiveness of Chuna Manual Therapy for adhesive capsulitis by a systematic review manner. Methods : Five foreign electronic databases (Pubmed, Ovid-medline, Embase, Cochrane library, Chinese Academic Journals (CAJ)) and two Korean medical electronic databases (Oriental Medicine Advanced Searching Integrated System (OASIS) and Research Information Sharing Service (RISS)) were searched to find all randomized controlled trials (RCTs) using Chuna Manual Therapy as a treatment for adhesive capsulitis. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results : Twenty-one RCTs met our inclusion criteria. The meta-analysis showed positive results on the use of Chuna Manual Therapy combined with acupuncture or exercise therapy in terms of the efficacy rate and pain using the Visual Analogue Scale (VAS), compared to acupuncture or exercise alone [SMD-1.81 (95% CI & -2.54, -1.08), P<0.001, RR 1.19 (95% CI 1.14, 1.23), P<0.001)]. Positive results in terms of efficacy rate were also obtained comparing Chuna Manual Therapy to acupuncture alone [RR 1.10 (95% CI 1.02, 1.18), P=0.01]. Conclusions : Our systematic review found favorable results on the effectiveness of Chuna Manual Therapy for pain and efficacy rate of adhesive capsulitis. However, evidence was limited due to the lack of well-designed RCTs. More qualified clinical trials are needed to obtain stronger evidence.

슬개대퇴통증증후군의 침 치료에 대한 체계적 문헌고찰의 프로토콜 (Acupuncture Treatment for Patellofemoral Pain Syndrome: A Protocol for a Systematic Review)

  • Objectives The purpose of this systematic review is to analyse randomized controlled trials about acupuncture for patellofemoral pain syndrome (PFPS) and evaluate it's pain improvement effect. Methods Studies will be searched from ten databases (Medline/PubMed, Cochrane Library, EMBASE, China National Knowledge Infrastructure [CNKI], WanFang Data, Citation Information by NII [CiNII], ScienceON, Koreanstudies Information Service System [KISS], KMBASE, and KoreaMed). Participants will be patients diagnosed with PFPS, regardless of cause, gender, age or race, and interventions will be acupuncture, electroacupuncture, dry needling, fire needle, and acupotomy. Other treatments than interventions will be the control group and the main outcome will be reviewed by visual analogue scale (VAS). Results Data will be extracted and assessed according to the Preferred Reporting Items for Systematic Review and Meta-Analysis flow chart and Cochrane risk of bias tools. Meta-analysis will be conducted using random effect model within 95% confidence interval. Conclusions We expect this systematic review to provide useful data about the pain improvements of patellofemoral pain using acupuncture and expect to stimulate and systematize acupuncture treatment on PFPS in the future.

소아·청소년 천식에 대한 수기요법의 효과: 체계적 문헌 고찰 (Effectiveness of Manual Therapy for Asthma in Children and Adolescents: A Systematic Review)

  • Objectives This systematic review aimed to provide evidence of the effectiveness and safety of manual therapy for asthma in children and adolescents. Methods We queried eight electronic databases (PubMed, Ovid-MEDLINE, EMBASE, The Cochrane Library, CNKI, ScienceON, KISS, and RISS) up to the end of April 2022. We included only randomized controlled trials (RCTs) investigating manual therapy for the treatment of asthma in children and adolescents. The methodological quality of the included RCTs was evaluated using the Cochrane risk-of-bias tool 1.0. Results Four RCTs were eligible for inclusion. Meta-analysis could not be performed due to the heterogeneity of the studies. One study showed the statistical significance of SMD 0.41 (95% CI: 0.06, 0.76, p<0.05) in the comparison with the osteopathic therapy group and the sham control group before and after treatment on the same day of peak expiratory flow. Conclusions This possibility was observed in a non-pharmacological approach to manual therapy. Manual therapy can be considered as an adjunct therapy for asthma treatment in children and adolescents who have difficulty with drug therapy.

수술 후 폐 합병증 예방을 위한 심호흡 운동의 효과에 대한 체계적 고찰 (Effectiveness of Deep Breathing Exercise for Postoperative Pulmonary Complications Prevention: A Systematic Review)

  • Purpose: This study was done to evaluate effectiveness of deep breathing exercise as a postoperative intervention to prevent pulmonary complications. Methods: A search of databases from 1990 to 2012 was done including MEDLINE, EMBASE, CINAHL, Cochrane Library and eight Korean databases. Ten studies met eligibility criteria. Researchers trained in systematic review, independently assessed the methodological quality of selected studies using the Cochrane's risk of bias tool. Data were analyzed using RevMan 5.2 program. Results: Among ten RCTs in four studies, deep breathing exercise was compared with an instrument using interventions such as incentive spirometry, in the other four studies deep breathing exercise was compared with non-intervention, and in last two studies bundles of interventions including coughing and early ambulation were assessed. A significant difference was found between deep breathing exercise group and non-intervention group. The odds ratio (OR) of occurrence of pulmonary complications for deep breathing exercise versus non-intervention was 0.30. However, there was no significant difference between deep breathing exercise group and incentive spirometry group (OR=1.22). Conclusion: Deep breathing exercise is vital to improving cost-effectiveness and efficiency of patient care in preventing postoperative pulmonary complications. For evidence-based nursing, standardized guidelines for deep breathing in postoperative care should be further studied.
