  • The objective of this study was to investigate the effects in auditory performance and vocal development of Onomatopoeia and Mimetic Word Productive Training Program in prelingually deafened children with cochlear implantation. The effects were measured with Lip-profile (Listening progress profile: LiP), the number of utterances, vocal developmental level and phonetic inventory. Subjects were four children with cochlear implants who were able to detect speech sounds and environmental sounds. The Onomatopoeia and Mimetic word Productive Training Program was made up of 3 steps with 24 Onomatopoeia and Mimetic words. This study was pre and post design. The results of the study were as follows: First, after Onomatopoeia and Mimetic word Productive Training Program was treated, LiP score was significantly higher. Second, after this program was treated, the number of utterances and emergence of both canonical and postcanonical utterances were increased. Emergence of vowel and consonant Features were increased and diversified. In conclusion, Onomatopoeia and Mimetic Word Productive Training Program appeared to facilitate efficient auditory performance and vocal development.

후천성 인공와우 이식 성인의 청능훈련 사례 연구 (Case Study of Auditory Training for the Acquired Hearing loss Adult with Cochlear Implant)

  • 최근 인공와우 이식 수술에 대한 건강보험이 확대 되면서 이식자들의 수는 늘어나게 되었다. 2005~2009년 사이 최근 6년간 인공와우 수술을 받은 환자는 약 3,300여명이 이르며 그 중 성인의 인공와우 이식 수가 늘어가는 양상을 보이고 있다. 어린 아동의 경우 인공와우 이식 후 청능훈련을 적극적으로 받으며 관련 연구도 많이 있지만 성인에 대한 이식 후 청능훈련에 대한 연구는 많지 않다. 본 연구는 언어습득이후 인공와우를 이식한 성인여자(54세) 1명을 대상으로 Ling 6 sound test, 표준화된 자음과 모음 듣기 검사, 문장 검사 그리고 실생활에 필요한 환경음과 단어의 인지 및 확인 평가 도구를 이용하여 10주간 청능훈련을 실시하였다. 10주간의 청능 훈련 결과, 대상자는 Ling 6 sound의 모든 음소를 확인하였으며 표준화된 자모음과 문장 듣기 검사에서도 100%에 가까운 수행력을 보였다. 또한 실생활에 환경음과 단어의 인지 및 확인은 57%에서 95%까지 수행력이 개선되었다. 본 연구 결과는 성인을 대상으로 한 청능훈련은 체계적이고 효과적인 계획과 개인의 특성을 고려한 재활 프로그램이 필요함을 보여주었다.

인공와우 전기 청각 인지에 대한 정현파 모델 적용에 관한 연구 (Application of sinusoidal model to perception of electrical hearing in cochlear implants)

  • 어음은 복잡한 정현파들의 합으로 구성된다. 본 논문은 정현파 모델을 인공와우 시뮬레이션에 적용하여 인공와우 사용자의 전기 청각 인지 메커니즘을 정상 청력인의 음향 청각 인지 메커니즘과 비교하였다. 이를 위해 정상 청력을 지닌 14명의 성인들이 본 연구에 참여하였다. 2, 4, 6, 8개의 정현파 성분만을 추출하는 정현파 모델이 적용된 문장과 정현파 모델과 인공와우 시뮬레이션(8 채널 보코더)이 함께 적용된 문장에 대한 어음인지도 검사를 실시하였다. 연구결과, 인공와우 시뮬레이션이 함께 적용된 정현파 문장에서 통계적으로 낮은 어음인지도를 보였으며, 특히 적은 수(2개)의 정현파 성분에서 큰 차이를 보였다. 본 연구는 정현파 모델에 대한 인공와우 사용자의 전기 청각 인지 패턴의 특성을 제시하며, 인공와우 어음처리 알고리즘의 개발에 대한 기초 자료를 제공한다.

인공와우이식 아동 말용인도의 예측 변인 (Variables for Predicting Speech Acceptability of Children with Cochlear Implants)

  • Purposes: Speech acceptability means the subjective judgement of listeners regarding the naturalness and normality of the speech. The purpose of this study was to determine the predicting variables for speech acceptabilities of children with cochlear implants. Methods: Twenty seven children with CI participated. They had profound pre-lingual hearing loss without any additional disabilities. The mean of chronological ages was 8;9, and mean of age of implantation was 2;11. Speech samples of reading and spontaneous speech were recorded separately. Twenty college students who were not familiar to the speech of deaf children evaluated the speech acceptabilities using visual analog scale. 1 segmental (articulation) and 6 suprasegmental features (pitch, loudness, quality, resonance, intonation, and speaking rate) of speech were perceptually evaluated by 3 SLPs. Correlation and multiple regression analysis were performed to evaluate the predicting variables. Results: The means of speech acceptability for reading and spontaneous speech were 73.47 and 71.96, respectively. Speech acceptability of reading was predicated by the severity of intonation and articulation. Speech acceptability of spontaneous speech was predicated by the severity of intonation and loudness. Discussion and conclusion: Severity of intonation was the most effective variable to predict the speech acceptabilities of both reading and spontaneous speech. A further study would be necessary to generalize the result and to apply this result to intervention in clinical settings.

인공와우 이식자에서 Positive Peaked 청신경 복합활동전위 (Positive Peaked Electrically Compound Action Potentials in Cochlear Implant Recipients)

  • Animal experiments have shown that the positive peaked electrically compound action potentials (ECAPs) can be recorded in round window, intracochlear, and nerve trunk by stimulating a monopolar pulse. However, positive peaked ECAPs of cochlear implant recipients have never been reported because ECAPs are recorded from intracochlear electrodes after bipolar stimulation. In our experiment, the positive peaked ECAPs were recorded from 18 intracochlear electrodes in cochlear implant recipients with multiple cochlear anomalies. Thresholds in each channel were measured and the latency of P-, N-wave, and amplitude of P-N were analyzed. These results were identical with the electrically auditory brainstem response (EABR) on the input-output characteristics. In conclusion, the positive peaked ECAPs from the cochlear implant recipients are antidromic ECAPs recorded by perimodiolar electrodes stimulating cochlear implants with multiple anomalies. Therefore, positive peaked ECAPs can be used as useful audiological tools to evaluate the eighth nerve ending.

Educational Status in Bilateral Prelingual Deaf Children with Cochlear Implantation

  • Background and Objectives: This study was undertaken to investigate the educational status in bilateral prelingual deaf children with a cochlear implant (CI), also known as early cochlear implantees (CIs). Type of schooling and enrollment rate of tertiary education were analyzed as primary results. Subjects and Methods: Participants in this study comprised a highly homogeneous group of deaf patients who underwent cochlear implantation at a similar age. Sixty-four Korean patients were enrolled. Statistical data for disabled populations and the general population were obtained from the National Statistics Korea. Results: Among 64 patients, 46, 8, and 10 attended mainstream, integrated, and special schools, respectively. Notably, there was a significant difference in the type of school between hearing-impaired and CI groups (p=0.007). Ten of 13 patients enrolled in tertiary education. Conclusions: CI users were more likely than hearing impaired students to attend mainstream school. The enrollment rate of CI users in tertiary education was the same as that of the general population.

전정감각 중심의 감각통합치료가 인공와우 수술 아동의 감각처리, 문제행동, 청각행동에 미치는 영향 (The Effect of Vestibular Sensory Integration on the Sensory Processing, Problem Behavior, and Auditory Behavior of Children With Cochlear Implants)

  • 목적 : 본 연구는 전정감각 중심의 감각통합치료가 인공와우 수술 아동의 감각처리, 문제행동, 청각행동에 미치는 영향을 알아보고자 한다. 연구방법 : 본 연구는 대전시에 거주하는 인공와우 수술 후 보조기기를 착용한 아동3명을 대상자로 모집한 후에 2019년 8월부터 2019년 11월까지 총 11주간 전정감각 중심의 감각통합치료 프로그램을 실시하였다. 연구 설계는 단일 사례 실험 설계방법(single subject design)으로 A-B 설계를 사용하였으며, 기초선 기간 4회, 중재기 15회로 진행되었다. 기초선 기간에는 감각통합치료를 제공하지 않고, 중재기에는 40분간 치료사와 함께 자유놀이를 진행하였다. 중재기에는 대전시의 아동발달센터에서 1회기 당 40분으로 전정감각 중심의 감각통합치료를 제공하였다. 중재 시작 전과 중재기 15회 직후 참여 아동의 감각처리 능력과 청각행동을 평가했다. 문제 행동 관찰은 기초선 자유놀이 시간에, 중재기에는 치료가 실시되는 시간에 아동을 관찰하여 기록하였다. 결과 : 전정감각 중심의 감각통합치료를 중재하기 전보다 후에 A, B, C 세 명의 아동에게서 감각처리와 청각행동의 긍정적인 변화가 있었고, 문제행동에서도 2표준편차 밴드 그래프에서 긍정적인 변화를 확인할 수 있었다. 결론 : 본 연구의 결과는 전정감각 중심의 감각통합치료가 인공와우 수술 아동의 감각처리, 문제행동, 청각행동에 긍정적인 효과가 있음을 제시한다.

인공와우 이식의 비용효용 분석 (Cost-Utility Analysis of the Cochlear Implant)

  • Objective : To determine the quality of life and cost consequences for deaf adults who received a cochlear implant. Methods : The data from 11 patients, post-lingual deaf adults who received cochlear implants from 1990 to 2002, underwent cost-utility analysis. The average age of the participants was 49.6 years. The main outcomes were direct cost per quality-adjusted life-year (QALY) using the visual analog scale (VAS), health utility index (HUI), EuroQol (EQ-5D), and quality well-being (QWB), with costs and utilities being discounted 3% annually. Results : Recipients had an average of 5.6 years of implant use. Mean VAS scores increased by 0.33, from 0.27 before implantation to 0.60 at survey. HUI scores increased by 0.36, from 0.29 to 0.65, EQ-5D scores increased by 0.26, from 0.52 to 0.78, and QWB scores increased by 0.16, from 0.45 to 0.61. Discounted direct costs were $22,320, yielding $19,223/QALY using VAS, $17,387/QALY using HUI, $24,604/QALY using EQ-5D, and $40,474/QALY using QWB. Cost-utility ratios using VAS, HUI, and EQ-5D were all below $25,000 per QALY, except using QWB. Conclusion : Cochlear implants in post-lingual deaf adult have a positive effect on quality of life at reasonable direct costs and appear to produce a net saving to society.

모음의 포먼트 변형에 따른 인공와우 이식 아동의 청각적 인지변화 (Perception Ability of Synthetic Vowels in Cochlear Implanted Children)

  • The purpose of this study was to examine the acoustic perception different by formants change for profoundly hearing impaired children with cochlear implants. The subjects were 10 children after 15 months of experience with the implant and mean of their chronological age was 8.4 years and Standard deviation was 2.9 years. The ability of auditory perception was assessed using acoustic-synthetic vowels. The acoustic-synthetic vowel was combined with F1, F2, and F3 into a vowel and produced 42 synthetic sound, using Speech GUI(Graphic User Interface) program. The data was deal with clustering analysis and on-line analytical processing for perception ability of acoustic synthetic vowel. The results showed that auditory perception scores of acoustic-synthetic vowels for cochlear implanted children were increased in F2 synthetic vowels compaire to those of F1. And it was found that they perceived the differences of vowels in terms of distance rates between F1 and F2 in specific vowel.
