Deoxyribonucleic Acid Was Responsible for the Anticoagulatory Effect of an Earthworm, Lumbricus rubellus

  • Earthworm extracts are known for anti-inflammatory, analgesic. antipyretic, and anticancer effects but can also influence blood circulation. It was previously shown that an earthworm, Lumbricus rubelius. contained a water-extractable anticoagulant which was a heat- and acid-stable molecule with hydrophilic property. In order to uncover the biochemical nature of this molecule, the anticoagulant was processed with various hydrolases such as trypsin, DNase, RNase. and lysozome. When the digested samples were analyzed with an in vitro coagulation test measuring activated partial thromboplastin time (APTT) and agarose gel electrophoresis, the anticoagulant proved to be a relatively homogeneous DNA fragment with relative molecular size around 72 base pairs. Interestingly, the activity was further stimulated with a trypsin digestion. RNA. on the other hand, did not prolong the APTT. It was also demonstrated that the DNA accelerated the antithrombin III (AT-III) inhibition of thrombin from $IC_{50}$ of 0.34 to 0.16 unit determined with S-2238 as a substrate, whereas heparin, a popular anticoagulant. shifted the value to 0.05. Therefore, it is suggested that the DNA could be considered as an alternative antithrombotic agent to heparin, which would exhibits bleeding side effects.

A Case of Streptococcus pneumoniae associated Hemolytic Uremic Syndrome with DIC

  • Streptococcus pneumoniae associated hemolytic uremic syndrome (SpHUS) is one of the causes of atypical hemolytic uremic syndrome, and increasingly reported. They are more severe and leave more long-term sequelae than more prevalent, typical hemolytic uremic syndrome. But it is not so easy to diagnose SpHUS for several reasons (below), and there was no diagnostic criteria of consensus. A 18 month-old-girl with sudden onset of oliguria and generalized edema was admitted through the emergency room. She had pneumonia with pleural effusion and laboratory findings of HUS, DIC, and positive direct Coombs' test. As DIC or SpHUS was suspected, we started to treat her with broad spectrum antibiotics, transfusion of washed RBC and replacement of antithrombin III. On the $3^{rd}$ day, due to severe hyperkalemia and metabolic acidosis, continuous renal replacement therapy (CRRT) was started. She showed gradual improvement in 4 days on CRRT and discharged in 16 days of hospital care. At the follow up to one year, she has maintained normal renal function without proteinuria and hypertension. We report this case with review of articles including recently suggested diagnostic criteria of SpHUS.

Single Oral Dose Toxicity Study of an Alcohol Extract of Bumblebee, Bombus ignitus Larvae in Rats

  • The alcohol extract of the larvae of Bombus ignitus, otherwise known as the Bumblebee, was orally administered to rats at doses of 0, 0.04, 0.2, 1 or 2 g/kg as a single oral dose. There were no observed clinical signs or deaths related to treatment in all the groups tested. Therefore, the approximate lethal dose of the alcohol extract of B. ignitus was considered to be higher than 2 g/kg in rats. Mild decreases in body weight gain in male rats were observed dose-dependently within the B. ignitus treated groups over 2 weeks. Throughout the administration periods, no significant changes in diet consumption, ophthalmologic findings, clinical pathology (hematology, clinical chemistry and coagulation) or gross pathology were detected. Minor changes in male rats were found with in the hematological parameters in groups treated with the 0.04 g/kg, 1 g/kg or 2 g/kg of B. ignitus larvae extract, however, all the changes observed were within the physiological range. From these results, it was concluded that there was no evidence of specific toxicity related to the ingestion of alcohol extract of B. ignitus larvae.

  • Habitual pregnancy loss has been defined as three or more consecutive spontaneous abortions. The rate of recurrent pregnancy loss is 2% to 5% of reproducible women. Half of this failure can be explained by genetic, hormonal, infectious, and anatomic factors. And eighty percent of the unexplained failures are proposed to have an immunologic cause. The antiphospholipid antibodies are characterized by prolonged phospholipid-dependent coagulation test (known as APTT or Russel viper venom), thrombosis, thrombocytopenia, and fetal loss. The association of antiphospholipid antibodies with one or more of these characteristic clinical features has been termed the antiphospholipid syndrome. We have experienced a case of successful live birth after treated a woman with heparin and aspirin who has experienced spontaneous abortion four times with antiphospholipid antibodies and present it with the review of literature.

A Study on Possibility of Sedimentation Basin Omission After Installed Membrane System in Drinking Water Treatment (정수처리시설에서 막공정 도입시 침전공정생략 가능성에 관한 연구)

  • The objective of this study was to evaluate the possibility of sedimentation basin omission when installed hybrid membrane filtration process in the field plant with the capacity of $500m^3/day$ for 11 months in the "G", water purification plant in Seoul. In order to evaluate the possibility of the sedimentation basin omission, we measured the change of DOC by coagulant dosage. Dosage of PAC(power activated carbon) 4mg/L and coagulant($AI_2O_3$ 10%) 1.67mg/L were compatible to meet the water quality. Also according to the experiment without settlement process, optimization G values were determined to be 300/s, 64/s, and 32/s at the mixing tank, the first flocculator and the second flocculator, respectively. The test was performed under the conditions PAC-coagulation-no settlement-MF. As a result, a dosage of 4.0mg/L as PAC and 0.86 to 1.22mg/L as $Al_2O_3$(10%) in the condition of flux of 62.5LMH were determined to keep TMP value less than $1.0kg_f/cm^2$.

Anesthetic Management of the Dental Treatment in a Child with Liver Failure Scheduled for Liver Transplantation - A case report - (간이식 예정인 간부전 환아의 치과치료 시 마취관리 -증례 보고-)

  • Special anesthetic considerations were required for children with acute or chronic liver disease. We experienced a case of dental treatment to control infection under general anesthesia in the 2-year-old girl with liver failure. She was also scheduled for liver tansplantation. Her preanesthetic results of liver function test, electrolytes, and coagulation panel were unstable and out of normal ranges. Uneventful anesthetic induction using isoflurane and atracurium and nasotracheal intubation were carried out. General anesthesia was maintained with isoflurane for 2 hours. Oozing from multiple extraction sites was sustained, so the transfusion of platelet concentration 1 units, fresh frozen plasma 1 unit, and packed red blood cell 1 unit was done. She was recovered without complication but was transferred to pediatric intensive care unit for wound care with her endotracheal tube kept. She was transferred to a ward without noticeable complications next day. So we report this successful case of anesthetic management for dental treatment in a child with liver failure.

Electrodes for contact electric welding of aluminium alloys

  • Aluminium and aluminium alloys have the high electrical and heat conductivity. It gives rise to difficulties for a choice of electrodes material for their contact electric welding. This paper describes the investigations performed to solve the above problem. The purpose of this investigation was to obtain dispersion-hardening alloys by the internal oxidation method, to optimize their contents and treatment modes, to produce electrodes of these alloys and to test them. The strengthing effect of alloys with oxide particles depends on their size stability at high temperatures. Despite of the fact, that oxides are the most stable of all the non-metallic phases their coagulation takes place. Based on the early results, we chose two types of alloys, first No. 1 Cu - 0,4%Al and second No. 2 Cu - 0,2%Be for production of electrodes. These alloys had not additional alloying elements. These alloys were prepared as 1 mm plates and flake-formed 200 m thick, and also No. 1 as a powder of size 100 mkm (received from Korea). The large samples for electrodes were produced by three methods : explosive welding method, dynamic one including the explosion compression of electrode blank and the quasi-dynamic method including the high-speed compression of dense briquest and the further hot extrusion of a rod.

Variation of Sedimentation & Dewaterability Characteristics of Sewage Sludge under Various Coagulants (응집제 종류에 따른 하수 슬러지의 침강 및 탈수 특성 변화)

  • The purpose of this study is to investigate the effect of various types of coagulant on dewaterability and settleability of sewage sludge for the application of dewatering process. Cationic organic coagulants and inorganic coagulants of the aluminium base were used; PAC (Poly Aluminium chloride, $Al_2O_3$ 17%) and C-210P (0.2%). After Jar test, PAC 26 mg/L and 0.2% C-210P 55 mg/L was decided as the optimum concentration of the coagulant according to zeta potential measurement. pH, alkalinity and viscosity were measured in all experiments and the data on sedimentation characteristics is analyzed by SDI, SVI sedimentation rate and solid flux. The SRF(Specific Resistance of Filtration) experiment was conducted with the result of single and dual injection system as the dewaterability experiment. As a result, the organic coagulant making large floc has good characteristics of sedimentation and agglutination. Also, it is observed that the organic coagulants injection has a better dewaterability efficiency of coagulants under the condition of the lowest SRF value, followed by dual and inorganic coagulants injection.

Chemical Precipitation Treatment for the Disperse Dyes Removal (분산성 염료의 제거를 위한 응집처리)

  • In order to remove the pollutants effectively in the dye wastewater by chemical precipitation process, coagulation arid flocculation test were carried out using several coagulants on various reaction conditions. It was found that the Ferrous sulfate was the most effective coagulant for the removal of disperse dye(B79), and we could get the best result lot the removal of disperse dye(B56) in the aspects of TOC removal efficiency and sludge field. When the Ferrous sulfate dosage was $800mg/\ell$, the sludge settling velocity was very fast>, and the color was effectively removed in the disperse dye(B79) solution. Although the color removal was ineffective when the Alum was used as a coagulant, the sludge field was decreased in comparison with the Ferrous sulfate or the Ferric sulfate was used in the disperse dye(B56) solution. The general color removal effect for the disperse dye(B56 and B79) solutions, the Ferric sulfate was more proper coagulant than the Alum. It was showed that TOC removal was improved 5% and over by the addition of Calcium hydroxide, and $30mg/\ell$ of sludge yield was decreased(B79). When Alum or Ferric sulfate was used as a coagulant, pH condition for most effective color removal was 5 in B56 solution. In case of Ferrous sulfate as a coagulant, most effective pH condition for color removal was 9. When Ferric sulfate or Ferrous sulfate was used as a coagulant, pH condition for most effective color removal was 9 in B79 solution.

Phenobarbital and zonisamide treatment of a cat with epilepsy of unknown cause

  • A Korean domestic short hair (1-year-old, male) presented with 2 to 3 weeks of seizures, aggressive behavior, vomiting, anorexia, and lethargy. The frequency of seizure had gradually increased from once a week to once every 3 hours. Physical and neurologic examination, diagnostic screening tests, including complete blood count (CBC), serum chemistry, electrolyte, coagulation test, X-ray, ultrasonography, and urinalysis were performed. Feline Leukemia Virus (FeLV), Feline Immunodeficiency Virus (FIV) and Toxoplasma spp. All tested negative, but the Feline Corona Virus (FCoV) kit revealed a positive result. To determine the exact diagnosis, magnetic resonance imaging (MRI) was performed but yielded no specific findings. The patient was then diagnosed with idiopathic epilepsy and treatment of phenobarbital was initiated. A month's treatment with phenobarbital proved ineffective as symptoms worsened. Zonisamide was then selected as an additional anticonvulsant. After adding zonisamide, symptoms improved, and seizures abated for 15 months. This is the first case report in South Korea describing the use of phenobarbital and zonisamide in the treatment of a cat with idiopathic epilepsy.
