Prevalence and Risk Factors of Clonorchiasis among Residents of Riverside Areas in Muju-gun, Jeollabuk-do, Korea

  • We evaluated the status of Clonorchis sinensis infection and potential risk factors among residents of riverside areas (Geumgang) in Muju-gun, Jeollabuk-do (Province), Korea. From January to February 2010, a total of 349 (171 males, 178 females) stool samples were collected and examined by the formalin-ether concentration technique. Also, village residents were interviewed using questionnaires to obtain information about C. sinensis infection-related risk factors. Overall egg-positive rate of C. sinensis was 13.2%. Egg-positive rates were significantly higher in males, farmers, and residents who had lived there more than 20 years, and in residents who had eaten raw freshwater fish than in opposite groups, respectively. However, there was no significant difference between age groups, education levels, cigarette smoking, alcohol drinking, health status, past history of infection, and experience of clonorchiasis medication and examination. Logistic regression analysis was performed to determine risk factors for clonorchiasis. On univariate analysis, the odds ratios for males, farmers, those who had lived there more than 20 years, and who had eaten raw freshwater fish were 2.41, 4.44, 3.16, and 4.88 times higher than those of the opposites, respectively. On multivariate analysis, the odds ratio of residents who had eaten raw freshwater fish was 3.2-fold higher than that of those who had not. These results indicate that residents living in Muju-gun, along the Geum River, Korea, have relatively high C. sinensis egg-positive rates, and the habit of eating raw freshwater fish was the major factor for the maintenance of clonorchiasis.

Changing patterns of Clonorchis sinensis infections in Kyongbuk, Korea (경북지역에 있어서 간흡충 감염상의 변화)

  • Studies were conducted from May, 1993 to April, 1995 to determine the changing patterns of infection Ity the liver fluke, Clonorchis sinensis, among residents and fish hosts in Kyongbuk Province. The infection rate among residents was 7.7% by stool examination. The rate in males (11.3%) was significantly higher than females (4.1%) Positive rate of intradermal test was 27.6% in the same population. The special type of a simple catalytic model was applied for the analysis of intradermal positive reactors by age arid sex, ann the equation was Y= 0.4776 t (1 -e-0.0375t) for mates ann, y : 0.2085 (1-e-0.0138t) for females. Analysis of stool examination data by two-stage catalytic model revealed y : 0.025 (e-0.0047t _ e-0.0235t). The annual Clonorchis infection rate was 4.7 per 1,000 susceptibles and the annual loss rate was 23.5 per 1,000 infected. The frequency distribution by the eggs per gram (EPG) was calculated as well as the cumulative percentages of positives. The regression equations were y : 0.929+ 1.506 log x for males and, y : 0.473 + 1.767 log x for females. Of the 25 fish species, 7 species were infected with Clonorchis mrtacercariae. Infection rates varied by the species, and ranged from 2.8% in Punfun,Bia herzi to 30.0% in Pseudorasbora panic. Average number of the matacercariae per gram of flesh was 58.1 in p. pnnpa, followed by 10.2 in Gncthopogon atromoculntus, 7.0 in Saurosobio dobryj, and 3.0 in Pcrccheilogncthus rhonlben. The present study indicates that clonorchiasis in Kyongbuk Province is less prevalent than that of several decades ago.

Effect of Clonorchis sinensis Excretory-secretory Product on the Cultured SD Rat Bile Duct Fibroblast (배양된 흰쥐 담관 섬유모세포에 대한 간흡충 분비배설 물질의 영향)

  • Clonorchis sinensis is the most important widely distributed parasite of the human bile duct in East Asia and the most prevalent parasitic helminth in Korea. The prevalence rate of human clonorchiasis has remained at about 2.9% in Korea. C. sinensis induces dilatation of the duct, hyperplasia of the mucosa, metaplasia or neoplasia of the mucosal epithelium, periductal inflammation and fibrosis, and thickening of the ductal wall. Fibroblast are the most common cells in connective tissue and are responsible for the synthesis of extracellular matrix components. The fibrosis associated with chronic inflammation and injury may also contribute to cholangiocarcinoma pathogenesis, particularly through an increase in extracellular matrix components, which participate in the regulation of bile duct differentiation during development. In this study, ultrastructural changes, the distribution of lectin receptors and actin protein in cultured SD rat bile duct fibroblast after infection of C. sinensis were observed. Experimental group had been divided into four groups: normal bile duct fibroblast cultured in basal media (G1); C. sinensis infected bile duct fibroblast cultured in basal media (G2); normal bile duct fibroblast cultured in basal media containing excretory-secretory product (ESP) (G1-1); C. sinensis infected bile duct fibroblast cultured in basal media containing ESP (G2-1). Overall, once a host is infected by C. sinensis, it affects the host to the extent that sialic acid of ductal fibroblast is increased. Number of cytoplasmic process of SD rat bile duct fibroblast was increased. Actin protein and sialic acid were located in cell surface. Fibroblast induced by C. sinensis was not recovered to normal fibroblast. The cytoplasm bulk and cytoplasmic process were increased whereas the growth rate of the fibroblast of infected SD rat was reduced rather than that of normal fibroblast. In result, it inhibits fibroblast proliferation and increases actin protein on fibroblast cytoplasm, and so causes fibroblast metamorphosis and cellular mutation.

A Study on Factors Related to of Clonorchiasis Infection among the Inhabitants in the Nakdong Riverside Area (낙동강 유역 주민들의 간흡충 감염에 영향을 주는 개인요인 및 지역요인 분석)

  • Purpose: This study investigated the factors of C. sinensis infection among the residents in vicinity of the Nakdong River and the infection status of fishes in the stream with C. sinensis maetacercaria. Methods: Selected five regions of the Nakdong River and collected freshwater fishes from the stream to examine their metacercarial infections; and studied the status and causes of C. sinensis infection in 552 residents living within 5 kilometers from the river. Results: Among the 552 residents studied, 13.8% were infected with C. sinensis, and among 379 freshwater fishes of 20 species collected from the five regions, 41 fishes of 8 species were infected with C. sinensis metacercaria. Conclusion: In order to prevent human C. sinensis infection and transition to chronic liver ions, we need to develop and apply good programs such as continuous health education, personal hygiene improvement, active medical treatment for those infected with C. sinensis, and residents’ voluntarily stop of eating freshwater fishes in the raw state.

  • We examined stools of 113 soldiers of Korean Army in Whachon-gun, Korea in 1992. The eggs of Clonorchis sinetris (71113) and Metqsonimw sp. (31113), and cysts of Ginrdia lomblic (41113) were detected. No eggs of soil-transmitted helminths were found. Most of the CLonorchis-infected soldiers (6171 were recruited from Kyongsangnam-do.

Image analytical observation on the growth and development of Clonorchis sinensis in rats (흰쥐 감염 간흡충의 성장 및 발육에 대한 형태계측학적 관찰)

  • For the two-dimensional observation on the growth and development of CLonorchis sinensis, an image analysis system (IBAS 2000, Kontron, Germany) was used in this study. On 3, 6, 9, 12, 15, 21, 30 and 90 days of infection, 474 worms were collected from rats infected with 50 metacercariae each. The overall recovery rate was 31.6%. The worms grew rapidly in their length and width up to 30 days of infection and then, did slowly to 90 days of infection. However, the growth pattern of body area was not similar to that of body length and width, because the body area increased continuously even after 30 days of infection. With the image analysis system, the sizes of irregular-shaped genital organs were measured easily, which showed sigmoidal growth patterns . The growth rate of genital organs increased rapidly. until 21 days of infection when the uterus was filled with numerous eggs, and then gradually slowed down to 90 days. There was no difference in growth pattern between the anterior and posterior testis. The seminal receptacle , however, showed an abrupt increase in size between 15 to 21 days of infection when it was filled with condensed semen. Therefore, the growth pattern of seminal receptacle might be used as another criteria to estimate the extent of sexual maturation of C. sinensis. From this study, it is suggested that image analysis system is very useful to reveal the growth and development pattern of C. sinensis, especially of their internal organs.

  • The authors characterized the proteins of the crude antigen obtained from Clonorchis sinensis worm and excretory-secretory and billis from rabbits, experimentally infected for 3 months. Protein composition was observed after adding a cysteine proteinase inhibitor E-64 and a serine proteinase inhibitor PMSF, respectively. SDS-PAGE of the crude antigen from C. sinensis recovered from the infected rabbits, the crude antigen from the adult worm excretory-secretory, and the crude antigen from billis of the rabbits resolved 26, 27 and 19 profiles between 200-9 kDa, respectively. When E-64 supplemented 29, and 22 bands, respectively. More study should be carried out in the future on the immunological characteristics and the monoclonal antibody of the each antigen.

  • C. sinensis total RMh was containing large amount of 185 rRNA but little 285 rRNA. The size of the double-stranded cDNA synthesized from poly $(A)^{+}$ mRNA was 0.4-4.2 kb long with tapering unto 9.5 kb. Degenerated oligonucleotides (as 2 sense and 3 antisense Primers) were designed on the conserved regions of the known tropomyosin amino acid sequences. From one out of the PCR amplifications using total CDNA and matrix of primers, a specific gene product, 580 bp in size, was produced. Upon Southern hybridization of the PCR products with Schistosomn mnnsoni tropomyosin (SMTM) CDNA, only one signal appeared at the band of 580 bp product. This 580 bp product was considered to encode C. sinensis tropomyosin (CSTM) and cloned in pGEM-3Zf(-) for DNA sequencing. CSTM cDNA was 575 bp containing one open reading frame of 191 predicted amino acids, which revealed 86.3% homology with SMTM and 51.1% with rrichostronsylur coeubnlormis tropomyosin. CSTM cDNA obtained will serve as a probe in the studies of molecular cloning of CSTM.

Analysis of Clonorchis sinensis antigens and diagnosis of clonorchiasis using monoclonal antibodies (단세포군 항체를 이용한 간흡충 항원의 분석 및 간흡충증의 진단)

  • Clonorchis sinensis is a common parasite of man in Korea. Researches on the specific antigens of C. sinensis would be valuable not only because those elucidate the molecular characteristics of this fluke but also because it is applicable to immunodiagnosis. Although many monoclonal antibodies have been used in the field of parasite immunology, few articles on monoclonal antibodies against C. sinensis have been published so far. The aim of this study was to analyse C. sinensis antigens recognized by monoclonal antibodies, and to set up ELISA-inhibition test using C. sinensis specific monoclonal antibodies for improved specificity of immunodiagnostic tests. By fusion between spleen cells of the mice immunized with C. sinensis water-soluble crude adult worm antigens and plasmacytoma cells of mouse origin, 29 hybridoma clones secreting anti-C. sinensis monoclonal antibodies were made, and 8 clones among those were found specific. After cell cloning, isotypes of 6 selected specific monoclonal anti- bodies were determined to be IgGl, IgG2b and IgA. Four exposed antigenic determinants of natural infection were recognized by different specific monoclonal antibodies. By enzyme-immunoelectrotransfer blot, 10 KD, 34 KD antigenic determinants were found to be reacted with CsHyb 0714-20, CsHyb 0605-10 monoclonal antibodies, respectively, The antigenic determinant recognized by CsHyb 0714-20 monoclonal antibody was revealed to be located at the surface and parenchyme of a parasite by indirect immunoauorescent antibody technique, and those reacted with CsHyb 0605-10, CsHyb 0714-25 monoclonal antibodies were found at the parenchyme and intestine. The antigenic determinant reacted with CsHyb 0605-23 monoclonal antibody was found mainly around the uterine eggs. Four antigenic determinants recognized by specific monoclonal antibodies were all found to be present in the early eluted fractions of C. sinensis antigens separated by Sephadex G-200 gel filtration. By conventional ELISA, 75% of clonorchiasis cases were found positive, but 7.1% of normal controls and 37.5% of paragonimiasis cases showed false positives. However, by ELISA-inhibition test using C. sinensis specific monoclonal antibody (CsHyb 0605-23), 77.1% of clonorchiasis cases were found positive, and there were no false positives in normal controls or paragonimiasis cases, indicating 100% specificity. The ELISA- inhibition test using monoclonal antibodies was found to have same sensitivity and definitely high specificity in comparison with conventional ELISA for serodiagnosis of human clonorchiasis.
