절단면 재렌더링 기법을 이용한 GPU 기반 MIP 볼륨 렌더링 (GPU based Maximum Intensity Projection using Clipping Plane Re-rendering Method)

  • 의료영상에서 사용하는 MIP 볼륨 렌더링은 CT나 MR 등의 볼륨데이터에서 시각 광선으로부터 높은 밝기 값을 추출하여 혈관과 뼈와 같은 환자의 조직을 보여주는 볼륨 렌더링 기법이다. 최근 GPU를 MIP 볼륨 렌더링에 사용하여 대용량 의료영상 데이터에 대해서도 속도가 빠른 렌더링이 가능하게 되었다. 볼륨데이터를 여러 각도에서 관찰하면, 일반적으로 시각과 동일한 방향의 텍스쳐 평면과 볼륨 경계평면이 비스듬하게 교차한다. 볼륨데이터의 외부에는 값이 존재하지 않으므로 경계부분에서 공간 주파수가 높게 나타난다. 기존의 MIP 렌더링은 샘플링 간격이 일정하기 때문에 경계부분에서 데이터의 손실이 생겨 알리아싱이 나타나는 문제가 있다. 화질을 개선하기 위해 샘플링 간격을 줄여 슬라이스수를 증가시킬 수 있으나, 이때는 렌더링 수행 시간이 길어지게 된다. 이 논문에서는 기존 렌더링 결과에 볼륨 경계 평면을 추가로 렌더링하는 방법을 제안한다. 이 방법은 주파수가 높은 경계 부분의 샘플링 간격을 줄여 화질을 향상시킨다. 한편 MIP는 샘플링 순서에 무관하므로 추가된 슬라이스는 기존 렌더링 영상을 손실시키지 않는다. 증가된 슬라이스는 경계부분인 여섯 평면에 불과하므로 렌더링 수행시간에는 거의 영향을 주지 않고 화질을 개선할 수 있다.

Modified Arachnoid Plasty Reduces Chronic Subdural Hematoma after Unruptured Aneurysm Clipping : Technical Note

  • Objective : Chronic subdural hematoma (CSDH) is a rare complication of unruptured intracranial aneurysm (UIA) clipping surgery. To prevent postoperative CSDH by reducing subdural fluid collection, we applied the modified arachnoid plasty (MAP) during the UIA clipping surgery to seal the dissected arachnoid plane. Methods : This retrospective study included 286 patients enrolled from July 2012 to May 2015. We performed arachnoid plasty in all patients, with MAP used after June 17, 2014. Patients were divided into two groups (non-MAP vs. MAP), and by using uni- and multivariate analyses, baseline characteristics, and relationships with postoperative CSDH between the two groups were analyzed. The degree of preoperative brain atrophy was estimated using the bicaudate ratio (BCR) index. Results : Ten patients (3.5%) among 286 patients had postoperative CSDH after clipping. Nine (3.1%) were in the non-MAP group, and one (0.9%) was in the MAP group. The higher BCR index showed statistical significance with occurrence of postoperative CSDH in both uni- (p=0.018) and multivariate (p=0.012; odds ratio [OR], 8.547; 95% confidence interval [CI], 1.616-45.455) analyses. MAP was associated with a lower risk of postoperative CSDH (p=0.022; OR, 0.068; 95% CI, 0.007-0.683). Conclusion : This study shows that the degree of preoperative brain atrophy is associated with an increased occurrence of CSDH after clipping and that MAP could help reduce the risk of postoperative CSDH after unruptured aneurysm clipping via a lateral supraorbital approach.

Added Predictive Values of Proton Density Magnetic Resonance Imaging on Posterior Communicating Artery Aneurysms and Surrounding Soft Tissues with Simple Classification

  • Objective : Deciphering the anatomy of posterior communicating artery (PCoA) aneurysms in relation to surrounding structures is essential to determine adjuvant surgical procedures. However, it is difficult to predict surgical structures through preoperative imaging studies. We aimed to present anatomical structures using preoperative high-resolution three-dimensional proton density-weighted turbo spin-echo magnetic resonance (PDMR) imaging with simple classification. Methods : From January 2020 to April 2022, 30 patients underwent PDMR before microsurgical clipping for unruptured PCoA aneurysms in a single tertiary institute. We retrospectively reviewed the radiographic images and operative data of these patients. The structural relationship described by PDMR and intraoperative findings were compared. Subsequently, we classified aneurysms into two groups and analyzed the rate of adjuvant surgical procedures and contact with the surrounding structures. Results : Correlations between preoperative PDMR predictions and actual intraoperative findings for PCoA aneurysm contact to the oculomotor nerve, temporal uncus, and anterior petroclinoid fold (APCF) reported a diagnostic accuracy of 0.90, 0.87, and 0.90, respectively. In 12 patients (40.0%), an aneurysm dome was located on the plane of the oculomotor triangle and was classified as the infratentorial type. Compared to the supratentorial type PCoA aneurysm, adjuvant procedures were required more frequently (66.7% vs. 22.2%, p=0.024) for infratentorial type PCoA aneurysm clipping. Conclusion : Preoperative PCoA aneurysm categorization using PDMR can be helpful for predicting surgical complexity and planning of microsurgical clipping.

Development of paint area estimation software for ship compartments and structures

  • The painting process of large ships is an intense manual operation that typically comprises 9-12% of the total shipbuilding cost. Accordingly, shipbuilders need to estimate the required amount of anti-corrosive coatings and painting resources for inventory and cost control. This study aims to develop a software system which enables the shipbuilders to estimate paint area using existing 3D CAD ship structural models. The geometric information of the ships structure are extracted from the existing shipbuilding CAD/CAM system and used to create painting zones. After specifying the painting zones, users can generate the paint faces by clipping structural parts inside each zone. Finally, the paint resources may be obtained from the product of the paint areas and required paint thickness. Implementing the developed software system to real shipbuilders' operations has contributed to improved productivity, faster resource estimation, better accuracy, and fewer coating defects over their conventional manual calculation methods for painting resource estimation.

다수의 평면을 가지는 모델기반 카메라 추적방법 (Camera Tracking Method based on Model with Multiple Planes)

  • 본 논문에서는 다수의 평면을 가지는 모델기반 카메라 추적 시스템이 제안된다. 상품의 정보를 표기하기 위한 2차원 바코드(barcode)로 널리 사용되는 QR(Quick Response) 코드를 인식하여 해당 물체의 3차원 모델을 임포팅한다. 그리고 관련 기하정보를 이용하여 모델의 주요 정점(vertex)을 추출하고 옵티컬 플로우(optical flow)를 이용하여 추적한다. 클리핑 알고리즘으로 다수의 평면을 가지는 물체의 평면 영역을 구별하고 매칭된 특징으로부터 호모그래피를 계산하여 초기 단계의 대략적인 카메라 움직임 파라미터를 추정한다. 이후 카메라의 움직임에 따라 다양한 평면에 존재하는 특징점과 해당 3차원 정보를 선형 방정식으로 구성하고 DLT(Direct Linear Transform) 방법을 적용한다. 최종 단계에서 번들 조정(Bundle Adjustment) 알고리즘을 이용해 카메라의 움직임 파라미터에 포함된 에러를 최소화 한다.

Four-Year Experience Using an Advanced Interdisciplinary Hybrid Operating Room : Potentials in Treatment of Cerebrovascular Disease

  • Objective : To describe our experiences with a fully equipped high-end digital subtraction angiography (DSA) system within a hybrid operating room (OR). Methods : A single-plane DSA system with 3-dimensional rotational angiography, cone-beam computed tomography (CBCT), and real-time navigation software was used in our hybrid OR. Between April 2014 and January 2018, 191 sessions of cerebrovascular procedures were performed in our hybrid OR. After the retrospective review of all cases, the procedures were categorized into three subcategorical procedures : combined endovascular and surgical procedure, complementary rescue procedure during intervention and surgery, and frameless stereotaxic operation. Results : Forty-nine of 191 procedures were performed using hybrid techniques. Four cases of blood blister aneurysms and a ruptured posterior inferior cerebellar artery aneurysm were treated using bypass surgery and endovascular trapping. Eight cases of ruptured aneurysm with intracranial hemorrhage (ICH) were treated by partial embolization and surgical clipping. Six cases of ruptured arteriovenous malformation with ICH were treated by Onyx embolization of nidus and subsequent surgical removal of nidus and ICH. Two (5.4%) of the 37 cases of pre-mature rupture during clipping were secured by endovascular coil embolization. In one (0.8%) complicated case of 103 intra-arterial thrombectomy procedures, emergency surgical embolectomy with bypass surgery was performed. In 27 cases of ICH, frameless stereotaxic hematoma aspiration was performed using $XperGuide^{(R)}$ system (Philips Medical Systems, Best, the Netherlands). All procedures were performed in single sessions without any procedural complications. Conclusion : Hybrid OR with a fully equipped DSA system could provide precise and safe treatment strategies for cerebrovascular diseases. Especially, we could suggest a strategy to cope flexibly in complex lesions or unexpected situations in hybrid OR. CBCT with real-time navigation software could augment the usefulness of hybrid OR.

Quantitative Analysis of Factors Affecting Cobalt Alloy Clip Artifacts in Computed Tomography

  • Objective : Clip artifacts limit the visualization of intracranial structures in CT scans from patients after aneurysmal clipping with cobalt alloy clips. This study is to analyze the parameters influencing the degree of clip artifacts. Methods : Postoperative CT scans of 60 patients with straight cobalt alloy-clipped aneurysms were analyzed for the maximal diameter of white artifacts and the angle and number of streak artifacts in axial images, and the maximal diameter of artifacts in three-dimensional (3-D) volume-rendered images. The correlation coefficient (CC) was determined between each clip artifact type and the clip blade length and clip orientation to the CT scan (angle a, lateral clip inclination in axial images; angle b, clip gradient to scan plane in lateral scout images). Results : Angle b correlated negatively with white artifacts (r=-0.589, p<0.001) and positively with the angle (r=0.636, p<0.001) and number (r=0.505, p<0.001) of streak artifacts. Artifacts in 3-D images correlated with clip blade length (r=0.454, p=0.004). Multiple linear regression analysis revealed that angle b was the major parameter influencing white artifacts and the angle and number of streak artifacts in axial images (p<0.001), whereas clip blade length was a major factor in 3-D images (p=0.034). Conclusion : Use of a clip orientation perpendicular to the scan gantry angle decreased the amount of white artifacts and allowed better visualization of the clip site.

Edge 정보에 근거한 피라미드 영상부호화 (Pyramid Image Coding Using Edge Information)

  • 본 논문에서는 피라미드 부호화에서 몇가지 수행이 간단한 방법을 사용하여 0.3~0.5bits/pel의 고감축율에서도 양호한 품질의 영상을 얻고자 하였다. 이를 위하여 Laplace 평면 구성의 궤환방식, 3차원 예측기, 그리고 중앙집중 양자화기를 부호화시 도입하였다. 또한 최하층의 비트율을 대폭 감축하기 위해 조건부로 전송하는 방식과 전송하지 않고 에지 선명화하는 방식을 새로이 적용하였다. 실험 결과, 에지 선명화 방식에 의한 부호화시 0.3bits/pel의 엔트로피로 구해진 비트율에서도 비교적 양호한 품질의 영상을 얻을 수 있었다.
