A Study on Influencing Factors in Health Promoting Lifestyles of College Students (대학생의 건강증진 생활양식과 관련요인 연구)

  • The primary purpose of this study was to identify factors affecting health promoting lifestyles of college students on the basis of Pender's Health Promotion Model. The subjects were 1,159 students of one university in Seoul. These data were collected by self reporting questionnaire from April 19 to May 3, 1993. This study examined health promoting lifestyles, cognitive-percep-tual factors: perceived health status, perceived importance of health, health locus of control, and health conception, and modifying factors: sex, grade, major, residence type, and attendance at a health care course. The instruments used in this study were Health Promoting Lifestyle Profile by Walker et al. (1987), Health Value Scale by Wallston et al. (1976), Health Locus of Control by Wallston et al. (1978), and Health Conception Scale by Laffrey(1986). The data were analyzed by Cronbach's $\alpha$, mean, standard deviation, percentage, t-test, ANOVA, Pearson's Correlation Coefficient, and Stepwise Multiple Regression with SPSS PC+ Program. The results were as follows : 1. The means of health promoting lifestyles revealed total 3.33, self actualization 3.74, health responsibility 2.72, exercise 2.80, nutrition 3.55, interpersonal surpport 3.76, and stress management 2.96. Interpersonal support showed the highest score and health responsibility showed the lowest score. 2. No significant differences between sex, grade, major, and residence type, and health promoting lifestyles Were founded(p>.05). Attendants at a health care course showed a significant higher score than nonattendants (p<.001). Male showed a significant higher score in exercise subscale, female showed significant higher scores in health responsibility, nutrition, interpersonal support, stress management subscales(p<.001), residence type showed a significant difference in nutrition subscale(p<.001). 3. No significant differences between perceived health status and sex, grade, major, residence type, and attendance at a health care course were founded(p>.05). Perceived importance of health showed no differences among sex, grade, major, and residence type(p>.05), showed only in attendance at a health care course (p<.001). Attendants at a health care course showed a significant higher score than nonattendants(p<.001). No significant differences between health conception and sex, grade, major, and residence type were founded (p>.05), only significant difference between health conception and attendance at a health care course was founded(p<.05). Nonattendants showed a significant higher score in clinical health conception, attendants showed a significant higher score in wellbeing health conception(p<.05). 4. A significant positive correlation between health promoting lifestyles and perceived health status was founded(r=.2415, p<.001). A significant positive correlation between health promoting lifestyles and perceived importance of health was founded (r=.1475, p<.001). The health promoting lifestyles revealed significant positive correlations in internal and powerful others locus of control (r=.3187, p<.001: r=.1475, p<.001), but revealed a significant negative correlation in chance locus of control(r=-.997, p<.001). A significant positive correlation between health promoting lifestyles and clinical health conception and wellbeing health conception were founded (r=.1241, p<.001 : r=.3047, p<.001). 5. Perceived health status was the highest factor predicting health promoting lifestyles of college students(R=.3415, $R^2=11.62$). Perceived health' status, perceived improtance of health, internal locus of control, wellbeing health conception, powerful others locus of control accounted for 28.19% in health promoting lifestyle patterns. In conclusion, college students who reported more helath promoting lifestyles evaluated their health positively, perceived importance of health highly, perceived their health as affected by theirselves, powerful others but not by chance or luck, and accepted health as high level wellness rather than merely the absence of illness. Those who attending at a health care course had healthier lifestyle patterns. And attendance at a health care course had significant. correlations in these cognitive-perceptual factors. Further studies are required to identify reasons of attending a health care course, and to compare health promoting lifestyles pre-post attending a course related to health care.

MDM2, p53 and pRb Expression Prior to Definitive Chemoradiotherapy in Esophageal Carcinoma (식도암에서 MDM2, p53, pRb 발현과 동시적 항암화학방사선요법의 결과)

  • Purpose: This study evaluated the pretreatment expression patterns of MDM2, p53, and pRb proteins to determine if the expression patterns could predict the outcome of concurrent chemoradiotherapy (CCRT) for esophageal squamous cell carcinoma and aid in the decisions for the selection of treatment modalities. Materials and Methods: Fifty-one patients that were treated with definitive chemoradiotherapy for stage $I{\sim}IVa$ esophageal squamous cell carcinoma were selected for this study. Radiotherapy was administered with daily $1.8{\sim}2\;Gy$ fractions up to a median dose of 54 Gy for primary tumors, and with four cycles of cisplatin/5-fluorouracil chemotherapy that was administered every 4 weeks, the first two cycles of which were administered concurrently with radiotherapy. Expression of MDM2, p53, and pRb was investigated by immunohistochemical analysis using pretreatment biopsy specimens. Results: MDM2, p53, and pRb were detected with high immunoreactivity in 19.6%, 27.5%, and 66.7% of the patients, respectively. However, there was no significant correlation between expression of these factors and clinical outcome. By the use of multivariate analysis with nine covariates-age, tumor location, tumor length, stage, pathological response, clinical response, MDM2 expression, p53 expression, and pRb expression, only pathological response and stage were significant factors for cause-specific survival. Conclusion: Expression of MDM2, p53, and pRb was not found to be clinically significant for predicting outcomes after CCRT in this study. Further studies with a larger patient population and longer follow-up periods are needed to re-evaluate the expression pattern and to identify new predictors for CCRT response.

Radiotherapy Results of Uterine Cervix Cancer Stape IIB : Overall Survival, Prognostic factors, Patterns of Failure and Late Complications (자궁경부암 병기 IIB에서의 방사선 치료 성적: 생존율 및 예후인자, 치료 후 실패양상, 만성 합병증)

  • Purpose : Treatment of choice for uterine cervix cancer stage IIB is radiotherapy. We analyzed survivals, Prognostic factors, patterns of failure and complications. Materials and Methods : This is a retrospective analysis of 167 patients with stage IIB carcinoma of uterine cervix treated with curative external pelvic and high dose rate intracavitary radiotherapy at the Department of Therapeutic Radiology, Soonchunhyang University Hospital from August 1985 to August 1994. All the patients followed up from 3 to 141 months(mean 60 months) and age of patients ranged from 31 to 78 years at presentation(mean : 55 years). Results : Overall complete response rate was $84\%$. The response rate for squamous cell carcimoma and adenocarcinoma were $86\%$ and $60\%$, respectively. Overall 5-years survival rate and disease free survival rate was 62 and $59\%$, respectively Mass size and treatment response were significant Prognostic factors for survival Pathologic type and parametrial involvement were marginally significants Prognostic factors. Local failure was 43 cases, distant metastasis was 14 cases and local failure plus distant metastasis was 3 cases, and most of local failures occurred within 24 months, distant metastasis within 12 months after treatment Twenty eight($16.8\%$) patients developed late rectal and urinary complications There were tendency to increasing severity and frequency according to increased fractional dose and total(rectal and bladder) dose. Conclusions : Survival rate was significantly related to tumor size and radiotherapy response. Tumor size should be considered in the clinical s1aging. To increased survival and local control, clinical trials such as decreasing duration of radiotherapy or addition of chemotherapy is needed. To detect early recurrence, regular follow up after RT is important. Because total rectal and bladder dose affected late complications. meticulous vaginal packing is needed to optimize dose of normal tissues and to decrease late complications.

Characteristics of Brain Perfusion in Patients of Parkinson's Disease (파킨슨병 환자의 뇌관류 분석)

  • Purpose: It was well known that cerebral blood perfusion is normal or diffusely decreased in the majority of patients with Parkinson's disease (PD). Actually we interpreted brain perfusion SPECT images of PD patients in the clinical situation, we observed various cerebral perfusion patterns in patients with PD. So we performed brain perfusion SPECT to know the brain perfusion patterns of PD patients and the difference of perfusion patterns according to the sex and the age. Also we classified PD patients into small groups based on the brain perfusion pattern. Methods and Materials: Two hundred nineteen patients (M: 70, F: 149, mean age: $62.9{\pm}6.9$ y/o) who were diagnosed as PD without dementia clinically and 55 patients (M:15, F:40, mean age: $61.4{\pm}9.2$ y/o) as normal controls who had no past illness history were performed $^{99m}Tc$-HMP AO brain perfusion SPECT and neuropsychological test. Results: At first, we compared all patients with PD and normal controls. Brain perfusion in left inferior frontal gyrus, left insula, left transverse temporal gyrus, left inferior parietal lobule, left superior parietal lobule, right precuneus, right caudate tail were lower in patients with PD than normal controls. Secondly, we compared male and female patients with PD and normal controls, respectively. Brain perfusion SPECT showed more decreased cerebral perfusion in left hemisphere than right side in both male and female patients compared to normal controls. And there was larger hypoperfusion area in female patients compared with male. Thirdly, we classified patients with PD and normal controls into 4 groups according to the age and compared brain perfusion respectively. In patient below fifties, brain perfusion in both occipitoparietal and left temporal lobe were lower in PD group. As the patients with PD grew older, hypoperfusion area were shown in both frontal, temporal and limbic lobes. Fourthly, We were able to divide patients into small groups based on cerebral perfusion pattern. There was normal cerebral blood perfusion in 32 (14.7%) of 219 patients with PD, decreased perfusion on the frontal lobe in 45 patients (20.6%), the temporal lobe in 39 patients (17.4%), the parietal lobe in 39 patients (17.9%), the occipital lobe in 40 patients (18.3%), diffuse area in 14 patients (6.4%) and unclassified in 10 patients (4.6%). Fifthly, we compared the results of the neuropsychological test and cerebral perfusion pattern. There was no correlation between two tests except visuospatial function. Conclusion: Various perfusion state were found in patients with PD according to the age and sex. Also we were able to classify perfusion state into several groups and compare the neuropsychological test with cerebral perfusion.

Shear bond strength of rebonded orthodontic bracket with flowable resin (Flowable resin을 이용한 브라켓의 재접착 시 전단결합강도에 대한 연구)

  • This study was performed to evaluate clinical practicality of the rebonding method with flowable resin without the removal of the residual resin on the debonded theeth and debonded bracket base after debonding. The samples of the control group (group I) were rebonded with Transbond XT using the usual rebonding method after the residual resin was removed. At experimental group, the brackets were rebonded with Transbond XT(group II) and CharmFil Flow (group III) without removal of residual resin which is the possibility becoming the index (or rebonding to similar position With initial bonding. The Shear bond Strength of the each group was measured. Patterns of bonding failure were evaluated with modified ARI score. and the shear bond strength according to patterns of bonding failure at experimental group was compared. Between the control group $(6.51\pm1.21MPa)$ and the group II rebonded with Transbond XT $(6.30\pm1.01MPa)$ did not have significantly difference in the shear bond strength (p=0.534), and the shear bond strength of group II was Significantly lower 4han the group III rebonded With CharmFil Flow $(7.29\pm1.54 MPa)$ (P=0.009). At control group, there was not large difference if distribution of bending failure pattern. But at experimental group, bond failure did not occur in interface between the resin-enamel. and bond failure between the resin-bracket, within the resin was distributed similarly. There was not significantly difference in the shear bond strength according to patterns of bonding failure at experimental group (P>0.05) The result of this study showed that the method suggested in this study aid flowable resin as rebonding adhesive could be useful in clinically.

Study on purchase and intake patterns of individuals consuming dietary formula for weight control or health/functional foods (체중조절용 조제식품과 다이어트 건강기능식품 섭취자의 제품구매 및 취식 행태에 관한 연구)

  • In our previous work, we reported consumers' perceptions of body shape and weight control. In an ongoing effort, we analyzed the purchasing behavior, intake patterns, future purchasing decisions, and degree of satisfaction in individuals consuming dietary formula for weight control (DF) or heath/functional foods (HFFs) by using the same survey questions. Portfolio analysis for marketing strategy was also investigated. Subjects were divided into two groups according to consumption of DF or HFF during the previous year : DF group (n = 89) and HFF group (n = 110). Average intake frequency was $1.7{\pm}0.7$ per day for HFFs and $1.5{\pm}0.9$ per day for the DF, and the most prevalent form was pill (58.2%) for HFFs and bar (42.7%) for DF. Duration of intake was $3.1{\pm}2.3$ months for HFFs versus $3.9{\pm}3.5$ months for DF. The average degree of satisfaction was $3.6{\pm}0.6$ on a 5-point scale, meaning 'relatively satisfied'. For the weight control method to be used in the future, 44.5% of the HFF group selected 'HFFs' while 47.2% of the DF group selected 'DF', showing a tendency to use the current product type in the future. The average planned period for the intake was $3.8{\pm}3.7$ months for HFFs and $3.0{\pm}2.4$ months for DF (p < 0.05). The HFF group emphasized efficacy, functional ingredients of the products, reliable products, and higher satisfaction, whereas the DF group emphasized the added materials in addition to weight control effects.

Automatic Interpretation of Epileptogenic Zones in F-18-FDG Brain PET using Artificial Neural Network (인공신경회로망을 이용한 F-18-FDG 뇌 PET의 간질원인병소 자동해석)

  • For the objective interpretation of cerebral metabolic patterns in epilepsy patients, we developed computer-aided classifier using artificial neural network. We studied interictal brain FDG PET scans of 257 epilepsy patients who were diagnosed as normal(n=64), L TLE (n=112), or R TLE (n=81) by visual interpretation. Automatically segmented volume of interest (VOI) was used to reliably extract the features representing patterns of cerebral metabolism. All images were spatially normalized to MNI standard PET template and smoothed with 16mm FWHM Gaussian kernel using SPM96. Mean count in cerebral region was normalized. The VOls for 34 cerebral regions were previously defined on the standard template and 17 different counts of mirrored regions to hemispheric midline were extracted from spatially normalized images. A three-layer feed-forward error back-propagation neural network classifier with 7 input nodes and 3 output nodes was used. The network was trained to interpret metabolic patterns and produce identical diagnoses with those of expert viewers. The performance of the neural network was optimized by testing with 5~40 nodes in hidden layer. Randomly selected 40 images from each group were used to train the network and the remainders were used to test the learned network. The optimized neural network gave a maximum agreement rate of 80.3% with expert viewers. It used 20 hidden nodes and was trained for 1508 epochs. Also, neural network gave agreement rates of 75~80% with 10 or 30 nodes in hidden layer. We conclude that artificial neural network performed as well as human experts and could be potentially useful as clinical decision support tool for the localization of epileptogenic zones.

Relationship among porcine lncRNA TCONS_00010987, miR-323, and leptin receptor based on dual luciferase reporter gene assays and expression patterns

  • Objective: Considering the physiological and clinical importance of leptin receptor (LEPR) in regulating obesity and the fact that porcine LEPR expression is not known to be controlled by lncRNAs and miRNAs, we aim to characterize this gene as a potential target of SSC-miR-323 and the lncRNA TCONS_00010987. Methods: Bioinformatics analyses revealed that lncRNA TCONS_00010987 and LEPR have SSC-miR-323-binding sites and that LEPR might be a target of lncRNA TCONS_00010987 based on cis prediction. Wild-type and mutant TCONS_00010987-target sequence fragments and wild-type and mutant LEPR 3'-UTR fragments were generated and cloned into pmiRRB-REPORTTM-Control vectors to construct respective recombinant plasmids. HEK293T cells were co-transfected with the SSC-miR-323 mimics or a negative control with constructs harboring the corresponding binding sites and relative luciferase activities were determined. Tissue expression patterns of lncRNA TCONS_00010987, SSC-miR-323, and LEPR in Anqing six-end-white (AQ, the obese breed) and Large White (LW, the lean breed) pigs were detected by real-time quantitative polymerase chain reaction; backfat expression of LEPR protein was detected by western blotting. Results: Target gene fragments were successfully cloned, and the four recombinant vectors were constructed. Compared to the negative control, SSC-miR-323 mimics significantly inhibited luciferase activity from the wild-type TCONS_00010987-target sequence and wild-type LEPR-3'-UTR (p<0.01 for both) but not from the mutant TCONS_00010987-target sequence and mutant LEPR-3'-UTR (p>0.05 for both). Backfat expression levels of TCONS_00010987 and LEPR in AQ pigs were significantly higher than those in LW pigs (p<0.01), whereas levels of SSC-miR-323 in AQ pigs were significantly lower than those in LW pigs (p<0.05). LEPR protein levels in the backfat tissues of AQ pigs were markedly higher than those in LW pigs (p<0.01). Conclusion: LEPR is a potential target of SSC-miR-323, and TCONS_00010987 might act as a sponge for SSC-miR-323 to regulate LEPR expression.

Patterns of Pulsatile and Surge Modes of Follicle-Stimulating Hormone Treated with Different Progesterone Levels in Ovariectomized Goats (난소제거된 Shiba 염소에서 다른 농도의 Progesterone 처치에 의한 FSH Surge 및 Pulse 분비에 미치는 영향)

  • The objective of the present study was to determine the progesterone levels that effects on the pulsatile and surge modes of FSH secretion. In previous studies we have shown that LH surge occurred in the follicular levels of progesterone, whereas there was no surge mode secretion of LH in either the sub luteal or luteal levels of progesterone. LH pulsatile frequencies were high in two groups such as follicular level and sub luteal level. But in the luteal level of progesterone the pulsatile pattern of LH were strongly suppressed. Namely, sub luteal levels of progesterone, around 1 ng/ml, completely suppressed the LH surge but did not affect the pulsatile frequency of LH secretion. Because of this we hypothesized that the two secretory patterns of FSH are similar to that of LH. Long-term ovariectomized Shiba goats that had received implants of estradiol capsules and three different progesterone silastic packet inducing follicular, subluteal and luteal levels of progesterone were divided into three groups such as non-P, low-P and high-P group. Blood samples were collected daily throughout the experiment for the analysis of gonadal steroid hormone levels and at 10-min intervals for 8 h on Days 0, 3, and 7 (Day 0: just before progesterone treatment) for analysis of the pulsatile frequency of FSH secretion. Then estradiol was infused into the jugular vein of all animals at a rate of 3 ${\mu}/h$ for 16 h on Day 8 to determine whether an FSH surge was induced. Blood samples were collected every 2 h from 4 h before the start of the estradiol infusion until 48 h after the start of the infusion. In each group, the mean ${\pm}$ SEM concentration after progesterone implant treatment was 3.3 ${\pm}$ 0.1 ng/ml for the high P group, 1.1 ${\pm}$ 0.1 ng/ml for the low P group, and < 0.1 ng/ml for the non-P group, concentrations similar to the luteal levels, subluteal levels, and follicular phase levels of the normal estrous cycle, respectively. The FSH pulse frequency was maintained highly in all groups on Day 0, Day 3 and Day 7. An FSH surge was induced in all 4 cases of the Non-P group. In the High P and Low P groups, the plasma concentrations of FSH remained low until 48 h after the start of estradiol infusion, and no occurrence of FSH surge was found in any of the animals. The results of this study not only confirm that the pulsatile patterns of FSH were not inhibited strongly relative to LH, they also suggest that some other mechanism and factor may be controlling the FSH secretion.

Birth weight for gestational age patterns by sex, plurality, and parity in Korean population (한국의 성별, 태아수별, 출산수별 임신주수에 따른 출생체중)

  • Purpose : A universal standard of the birth weight for gestational age cannot be made since birth weight distribution varies with races, nations and eras. This report aims to establish the birth weight for gestational age patterns by sex, plurality, and parity, specific for Korean live births. Methods : The national birth certificate data of all live births in Korea from January 2000 to December 2004 were used: for live births with gestational age 24 weeks to 42 weeks (n=2,585,5160), mean birth weight, standard deviation and 10th, 25th, 75th and 90th percentile values were obtained for each gestational age group by one week increment. To establish final standard values of Korean birth weight distribution by gestational age, the finite mixture model to eliminated erroneous birth weights was used for respective gestational age. Same as above method the birth weight for gestational age standard by sex, plurality, and parity were completed. Results : The male newborns are more heavier than female during the entire gestational age. The singletons are more heavier than twin during the entire gestational age. The para 2 are more heavier than the para 1 during the entire gestational age. Korean standard was more heavier in 10th and 50th percentile than Lubchenco's standard. Alexander's standard was more heavier in 50th and 90th percentile than Korean standard. Conclusion : These birth weight for gestational age patterns by sex, plurality, and parity are similar to the other standards. I hope that for Korean infants, this curve will help clinicians in defining and managing the large for gestational age infants and also for infants with intrauterine growth retardation.
