Biomechanical Evaluation of Cement type hip Implants as Conditions of bone Cement and Variations of Stem Design (골시멘트 특성 및 스템 형상에 따른 시멘트 타입 인공관절의 생체역학적 평가)

  • The total hip replacement (THR) has been used as the most effective way to restore the function of damaged hip joint. However, various factors have caused some side effects after the THR. Unfortunately, the success of the THR have been decided only by the proficiency of surgeons so far. Hence, It is necessary to find the way to minimize the side effect caused by those factors. The purpose of this study was to suggest the definite data, which can be used to design and choose the optimal hip implant. Using finite element analysis (FEA), the biomechanical condition of bone cement was evaluated. Stress patterns were analyzed in three conditions: cement mantle, procimal femur and stem-cement contact surface. Additionally, micro-motion was analyzed in the stem-cement contact surface. The 3-D femur model was reconstructed from 2-D computerized tomography (CT) images. Raw CT images were preprocessed by image processing technique (i.e. edge detection). In this study, automated edge detection system was created by MATLAB coding for effective and rapid image processing. The 3-D femur model was reconstructed based on anatomical parameters. The stem shape was designed using that parameters. The analysis of the finite element models was performed with the variation of parameters. The biomechanical influence of each parameter was analyzed and derived optimal parameters. Moreover, the results of FE A using commercial stem model (Zimmer's V erSys) were similar to the results of stem model that was used in this study. Through the study, the improved designs and optimal factors for clinical application were suggested. We expect that the results can suggest solutions to minimize various side effects.

EFFECT OF SURFACE DEFECTS AND CROSS-SECTIONAL CONFIGURATION ON THE FATIGUE FRACTURE OF NITI ROTARY FILES UNDER CYCLIC LOADING (전동식 니켈 티타늄 파일의 표면 결함 및 단면 형태가 반복응력 하에서 피로 파절에 미치는 영향)

  • The purpose of this in vitro study was to evaluate the effect of surface defects and cross-sectional configuration of NiTi rotary files on the fatigue life under cyclic loading. Three NiTi rotary files ($K3^{TM},{\;}ProFile^{\circledR},{\;}and{\;}HERO{\;}642^{\circledR}$) with #30/.04 taper were evaluated. Each rotary file was divided into 2 subgroups : control (no surface defects) and experimental group (artificial surface defects), A total of six groups of each 10 were tested. The NiTi rotary files were rotated at 300rpm using the apparatus which simulated curved canal (40 degree of curvature) until they fracture. The number of cycles to fracture was calculated and the fractured surfaces were observed with a scanning electron microscope. The data were analyzed statistically. The results showed that experimental groups with surface defects had lower number of cycles to fracture than control group but there was only a statistical significance between control and experimental group in the $K3^{TM}$ (p<0.05), There was no strong correlation between the cross-sectional configuration area and fracture resistance under experimental conditions. Several of fractured files demonstrated characteristic patterns of brittle fracture consistent with the propagation of pre-existing cracks. This data indicate that surface defects of NiTi rotary files may significantly decrease fatigue life and it may be one possible factor for early fracture of NiTi rotary files in clinical practice.

  • The objective is to establish the standard of criteria for differential diagnosis of signs and symptoms. This study selected signs and symptoms related to heart which stands for Fire(火) as a kind of five phase(五行). Eleven experts was asked to evaluate the adequateness of criteria which was developed by Korea Institute of Oriental Medicine(KIOM) and to suggest the amendment of them. To implement the study, we used the questionnaire which asks about the diagnosis criteria for an insufficiency of the heart-qi(心氣虛證), deficiency of the heart blood(心血氣證), deficiency of the heart-yin(心陰虛證), insufficiency of the heart-yang(心陽虛證), exuberant fire due to hyperactivity of the heart(心火亢盛證), stagnation of the heart blood(心血瘀阻證), heart disturbed by phlegm-fire(痰火擾心證), attack of the heart by retainedfluid(水氣凌心證). Every criteria consists of primary symptoms, secondary symptoms, tongue findings. and pulse findings. In perspectives of the classification of patterns for signs and symptoms and criteria for diagnosis, the result shows that the previous standard doesn't have so many problem. So many of experts were agree with the criteria which was suggested but the trend is that they use, in their actual practice, less than the criteria. Additionally, they pointed that every element in a criterion should have the different weight value, criteria for the overlapped pattern should be added, and, in future, criteria which are based on clinical investigation should be established.

  • The goal of this study is to develop knowledge representation method for the construction and evaluation of ontology for diagnosis in oriental medicine. To develop the expert system for decision making on diagnosis and treatment, the systematic and structural knowledge which can be processible in EMR(Electronic Medical Record) must be precedent, and the Computational Process which control the system as well. This study set up an ontology as a trial model to represent the oriental medical knowledge into the machine processible one. Protege 2.1 has been used to build the ontology, and the serialization format of our ontology is the XML document based on OWL. The components of oriental medical diagnosis was arranged with the combination of symptoms which belong to the certain symptom patterns. Then natural language which expresses the oriental medical diagnosis components were converted into the logical sentence, and individual characteristic symptoms into each values of specific properties. In addition to the study, the diagnosis software for oriental medicine was developed and it used the ontology which we developed. Sequently, we tested the software to confirm the appropriateness of ontology. The result of the test shows that diagnostic questions are automatically formulated according to the diagnosis components of this ontology and that as such diagnostic results are induced. Therefore, the ontology system in this study will be efficient to develop the diagnosis program and useful as a tool for doctors to make decision. But, it is not recommendable to apply the system to the clinical environment until the clear diagnosis standards are introduced, and the more reliable diagnosis program can be developed based on the more appropriate ontology mentioned above.

An Epidemiological Study of Cutaneous Leishmaniasis in Al-Jabal Al-Gharbi, Libya

  • Cutaneous leishmaniasis (CL) is an endemic parasitic infection in the Mediterranean region, including Libya and its Al-jabal Al-gharbi province. We aimed at studying the occupational relevance as well as other epidemiological aspects of CL. We investigated 140 CL cases who attended at Gharyan outpatient polyclinic during a period of 6 months in 2009. CL infection was clinically diagnosed and confirmed by demonstration of Leishmania parasites on smears from lesions. Our findings showed that males were more affected than females (P=0.04), and people above 10-years were more affected than younger ones (P=0.0001). A significant percent of CL cases belonged to Al-Kawasem subprovince (P=0.0001). Farm-related activities were the most frequent occupations among CL cases (P=0.04). In addition to farm workers, housewives and students are at risk groups since they are engaged at farm activities. Moreover, those who have occupations that require staying outdoors for a part of night, e.g., policemen, are also at risk. Compared to children, adult CL patients had multiple lesions (P=0.001) that were more prevalent in their upper and lower extremities than the face (P=0.0001). We conclude that CL is a major health problem in Al-jabal Al-gharbi province of Libya. The presence of rodents and sandflies makes it a suitable environment for Leishmania to spread in an endemic epidemiological pattern. Being engaged in farming activities or outdoor occupations increases the risk of infection. Various clinical patterns of CL suggest the presence of more than 1 species of Leishmania at Al-jabal Al-gharbi province. We propose that the 2 species responsible for CL in this area are L. major and L. tropica. Further investigations to identify the leishmanial species responsible for CL at Al-jabal Al-gharbi together with adoption of preventive and control programs are needed.

Patterns of Calcification in Thyroid Nodules; Significance and Malignant Potentiality (갑상선결절내 석회화소견과 갑상선암종과의 연관성)

  • Objectives: Calcification of the thyroid nodule has been reported to have a close relationship with differentiated thyroid carcinoma, however there are limited studies on the relationship between the calcified thyroid nodule and thyroid malignancy. The authors studied the clinical significance of calcification within the thyroid nodule. Materials and Methods: There were total of 60 patients who underwent surgery for calcified thyroid nodules which were identified from plain neck X-ray, ultrasound and computed tomography during the period January 1991 to June 1996 at the Department of Otorhinolaryngology-Head and Neck Surgery of Korea University Hospital. Six patients were not included because of recurrence and previous thyroid surgery. Histopathologic and radiologic analysis was done on the remaining 54 patients. Results: Results showed that 25 of 54 cases(46%) to be malignant histopathologically. Of the 25 malignant cases, papillary carcinoma was the most common with 22 cases followed by 2 cases of undifferentiated carcinoma and I case of medullary carcinoma. Statistically high incidence of malignancy was observed when the consistency of calcified thyroid nodule was solid, shape of calcification was irregular and inhomogenous, and adhesion of calcified thyroid nodule to the regional structure was present. Conclusion: The probability that a thyroid nodule is malignant has been reported to be 3­20%. However, about half of the thyroid nodules with calcification found to be malignant on this study, calcification of the thyroid nodule can be used as a guideline for detecting thyroid malignancy.

Recent Development of Rapid and Automation Technology for Food Microbiological Examination

  • Interests in the field of rapid methods and automation in microbiology have been growing steadily on an international scale in recent years. International meetings concerned this problem have been held in elsewhere in the world countries since the past twenty years. But, unfortunately in the field of microbial examination in food hygiene, this problem have not yet been developed so much as in the field of clinical microbiology. Today, I would like to introduce you here present aspects of rapid and automation technologies, those which are manly carrying in milk and meats industries. My illustration will be given recent improved technologies using automatic apparatus and instruments along with process of microbial count procedure. Recent direct microbiological counting system (ChemeScan \ulcorner) as real time ultrasensitive analysis created by Cheminex Ltd., France is now most evolutional instrument to provide direct microbial counts, down to one cell, within 30 minutes. The results from these evaluations how a good correlation between the ChemScan system and the standard plate count method. This system will be successful application for not only in the field of pharmacology but also food microbiology. In addition, current identification of microbes by sophisticated instruments suitable for food microbiology, one of which Biology is manual system (BIOLOG\ulcorner), provides reference-level capability at a modes price. For the manual system, the color reactions in the microplate are read by eye and manually keyed into personal computer. Species identification appears on the computer screen within seconds, along with biotype patterns, a list of closely related species, and other useful statistics. In present this is useful application for microbial ecology and epidemiological survey. RiboPrinter system newly produced by DuPont is now focusing among microbiologists in the world, and is one of the biggest microbial characterization system using a DNA-based approach. The technology analyzer is bacterial culture for its genetic fingerprint or riboprint pattern. Finally Bio-cellTracer system for automatic measurement of fungal growth and Fukitori-Maseter, a Surface Hygiene Monitoring Kit by using swabe procedure in food processing environment are briefly illustrated in this presentation.

Characteristics of Antimicrobial Susceptibility of Enterobacter Species (Enterobacter균종의 항균제 감수성의 본태)

  • A total of 58 strains of Enterobacter species isolated from clinical specimens at Kyungpook National University Hospital in Taegu and Yonsei University Hospital in Seoul were tested for the molecular characterization to investigate the nosocomial infection through the study of R plasmids which might spread among Gram negative organisms regardless of their originated strains. All strains resistant to ampicillin, cefoxitin and cephalothin but susceptible to moxalactam were subjected to the further test for the determination of in detail MIC value against 23 drugs of common use including beta-lactam antibiotics and R plasmid profile analysis. The reistance frequency of strains against carbenicillin (53.4%) was similar to those against chloramphenicol, tobramycin, and sulfisomidine. Though the MIC values of resistance criteria against ceftazidime, aztreonam, imipenem, and norfloxacine in NCCLS manual were not available but MIC ranges of strains tested were very low. There were differences in patterns and frequencies of resistance between the strains isolated in Seoul and Taegu isolates. Seoul isolates showed a tendency of higher multiplicity of resistance than those of Taegu isolates. The resistances against cefoxitin, cephalothin, cefoperazone, cefotaxime, nalidixic acid, and rifampin were not conferred to the conjugally transferable R plasmid. The approximate molecular size of conjugally transferable R plasmids ranged 30 to 151 megadalton, and one or 2 to 3 R plasmids were identified in each transconjugants.

In Vitro Susceptibility of Diarrhea-Causing Escherichia coli to 9 Antibacterial Agents in Clinical Use (최근 분리된 장내 병원성 대장균의 항균제 감수성)

  • To determine the prevalence of antibiotic resistance in fecal E. coli and to investigate possible associations between antibiotic resistance and other plasmid-mediated virulence properties, antibiotic disk susceptibility tests for nine antibiotics were done on 141 strains of E. coli isolated from diarrheal children and well controls. Eighty two percent of the test strains were resistant to one or more antibiotics. Antibiotics to which the test strains were most resistant in descending order were ampicillin (85%), trimethoprim/sulfamethoxazol (60%), and cephalothin (55%). Seventy nine percent of these resistant strains were resistant to two or more antibiotics. All 141 test strains were sorted into enterotoxigenic E. coli (ETEC), enteropathogenic E. coli (EPEC), enteroadherent E. coli (EAEC) and non-pathogenic E. coli and the percentages of strains resistant to multiple antibiotics were compared. Among ETEC regardless of its source, multiple drug resistance was more frequent in strains producing heatstable enterotoxin (ST) only than in strains producing only heat-labile enterotoxin (LT) or both. In EAEC, multiple resistance was more frequently associated with strains isolated from diarrheal patients than with those from well controls. The major antibiotic resistance patterns possessed by multiple resistant enteropathogenic strains were $SXT^R$ $AM^R$, $CR^R$, and $SXT^R$ $AM^R$ $CR^R$. Of 28 ST- producing $SXT^R$ ETEC, 26(96%) were also resistant to ampicillin and 17 (61%) were resistant to cephalothin. The similar pattern was observed in EAEC and EPEC as well. This study has important implications for the treatment of E. coli diarrhea with antibiotics because it is possible that dissemination of virulence could occur under the force of selective antibiotic pressure. In addition, this study suggests that the in vivo efficacy of SXT in treating diarrheal illness be reevaluated.

  • This paper presents a parameter visualization technique to overcome the limitation of the naked eye in contrast-enhanced ultrasonography. A method is also proposed to compensate for the distortion and noise in ultrasound image sequences. Meaningful parameters for diagnosing liver disease can be extracted from the dynamic patterns of the contrast enhancement in ultrasound images. The visualization technique can provide more accurate information by generating a parametric image from the dynamic data. Respiratory motions and noise from micro-bubble in ultrasound data may cause a degradation of the reliability of the diagnostic parameters. A multi-stage algorithm for respiratory motion tracking and an image enhancement technique based on the Markov Random Field are proposed. The usefulness of the proposed methods is empirically discussed through experiments by using a set of clinical data.
