Patients with Pressure injuries (PIs) may have pain and discomfort, which results in poorer patient outcomes and additional cost for treatment. This study was a part of larger research project that aimed at prediction modeling using a big data. The purpose of this study were to describe the characteristics of patients with PI in critical care; and to explore comorbidity and diagnostic and interventive procedures that have been done for patients in critical care. This is a secondary data analysis. Data were retrieved from a large clinical database, MIMIC-III Clinical database. The number of unique patients with PI was 2,286 in total. Approximately 60% were male and 68.4% were White. Among the patients, 9.9% were dead. In term of discharge disposition, 56.2% (33.9% Home, 22.3% Home Health Care) where as 32.3% were transferred to another institutions. The rest of them were hospice (0.8%), left against medical advice (0.7%), and others (0.2%). The top three most frequently co-existing kinds of diseases were Hypertension, not otherwise specified (NOS), congestive heart failure NOS, and Acute kidney failure NOS. The number of patients with PI who have one or more procedures was 2,169 (94.9%). The number of unique procedures was 981. The top three most frequent procedures were 'Venous catheterization, not elsewhere classified,' and 'Enteral infusion of concentrated nutritional substances.' Patient with a greater number of comorbid conditions were likely to have longer length of ICU stay (r=.452, p<.001). In addition, patient with a greater number of procedures that were performed during the admission were strongly tend to stay longer in hospital (r=.729, p<.001). Therefore, prospective studies focusing on comorbidity; and diagnostic and preventive procedures are needed in the prediction modeling of pressure injury development in ICU patients.

This study was conducted to identify the relationship between sleep quality, stress, depression, and eating attitudes of shift work nurses, and to determine the influencing factors of eating attitudes. Participants were 152 shift work nurses working at a university hospital and data collection was performed from July 1 to July 20, 2021. The collected data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression. In this study, the prevalence of abnormal eating attitudes at clinical and sub-clinical levels was 7.9% and 19.1%, respectively. There was a positive correlation between eating attitudes and depression, and the influencing factors of eating attitudes were alcohol drinking(1 time/month) and depression. Therefore, it is necessary to find strategies to prevent depression in order to improve the eating attitude of nurses working in shifts, and repeated research on the relevance of alcohol drinking and eating attitudes is needed.
This study is a descriptive research study to confirm the mediating effect of work-life balance in the relationship between grit and job embeddedness of clinical nurses. Participants included 155 nurses from four hospitals located in D city. Data was collected from July 29 to August 19, 2022 using a self-report questionnaires. The collected data were analyzed by descriptive statistics, Pearson's correlation coefficient and Baron and Kenny's three-step hierarchical regression using the SPSS 22.0 program. In this study, it was found that there was a significant positive correlation between the nurse's grit, job embeddedness, and work-life balance, and it was confirmed that work-life balance partially mediated the relationship between grit and job embeddedness. Therefore, it is suggested that grit and work-life balance should be improved in order to improve the job embeddedness of nurses.
Objective: This study investigated the awareness of occupational therapy in the community in occupational therapy majors through a survey. This purpose is to investigate the awareness of occupational therapy in the community among occupational therapy majors and establish a theoretical foundation. Methods: The research subjects were surveyed among occupational therapy majors at M University living in Gyeongsangnam-do, and analyzed 141 questionnaires from September 2023 to December 2023. Results: The results of this study that education and awareness improvement are needed to increase awareness of occupational therapy in the community, and it was found that continuing education and case sharing are effective. Activation of home rehabilitation and continuous health management. This institutional development can induce employment activity through community rehabilitation, and activate programs in connection with adult day care centers. For the development of community occupational therapy, participation in education and development of customized treatment are necessary, and patient It should be developed to help with movement and movement, and it has been shown that it can also affect the quality of life of patients. In addition, cutting-edge technologies such as artificial intelligence are expected to be applied to remote support, telemedicine, etc., and are applied to dementia, cognitive patients, and central nervous system patients. Conclusion: This study investigated the awareness of occupational therapy in the community among occupational therapy majors. Education and awareness improvement are needed to increase awareness of occupational therapy in the community. Education to improve the professionalism of occupational therapists, strengthening connectivity with other majors, and local organizations. It is believed that collaboration with the local community and institutional supplementation tailored to the needs of the local community were necessary.

The study aimed to investigate the basic data to derive plans for snack provision to improve the nutritional status of older adults living in long-term care facilities (LFs) or long-term care hospitals (LHs). The 252 respondents (118 from LHs and 134 from LFs) were included in the study. The questionnaire of nationwide cross-sectional survey was developed by the authors and registered dietitians. The written questionnaire was sent to the food service managers across 800 LFs or LHs. The online survey was introduced using the online platform and network site for dietitians. More than 70% of live-in and non-live-in LFs provided snacks, which were mainly provided one to two times a day. Most institutions provided fruits one to three times a week. The main considerations when providing fruit were in the order of residents' preference, cost, and ease of consumption. The reasons for not serving fruit included cost and differences in the residents' eating and mastication abilities. Most institutions also provided dairy products at a frequency of one to three times a week. The reasons for not serving dairy products included cost and the lack of awareness of the need to provide them. To improve the quality of life and the offer benefits of fruits and dairy products to older people, efforts are needed to propose a plan to expand the provision of snacks in appropriate quantities and varieties.
This study is designed to estimate an appropriate level of patient's cost-sharing for oriental medical services in the Korean National Health Insurance. The findings of this study can be summarized as follows: 1) The current co-payment system for oriental medical services does not reflect its cost structure in clinical practice due to inconsistency of cost-sharing plan in the NHI. 2) Both oriental medical institutions and their patients, as a result, are at a relative disadvantage in financial burden, compared with other services. 3) The substantial proportion of patients' cost-sharing depends on the amount of co-payment and the range of medical cost that a flat rate is applied to. 4) The extension of the range doesn't make any substantial decrease in patient's cost-sharing. 5) The fixed amount of co-payment is more sensitive than a range to total variations of patient's cost-sharing. Based on the above, the budget impacts of a new co-payment system were estimated for each co-insurance rate, according to given scenarios. The results range from -59 billion Won (-8.5%) to 16 billion Won(2.3%).
Entering the fourth industrial revolution era, health technology is rapidly developing and the people's needs for medical services are gradually increasing. Establishing a life cycle management of health technology has emerged as a new policy agenda to cope with these changes. However, the management of health technology have been conducted without continuity and with several problems pointed out. Therefore, we suggest the reform agendas by stages to establish system for a life cycle management of health technology in the fourth industrial revolution era as follows. In the stage of development, it is important not only to provide research funding, but also consulting by professional about whole cycle of health technologies. In the phase of market entry, there are needs for enhance the system that would expand the early adoption for innovative technology and increase its effectiveness. After the spread of health technology to clinical settings, a reassessment and post management system should be established that have an institutional framework with strong price adjustment and exit mechanism. Furthermore, we hope that discussions will be brisk in macro perspective on the balancing of development in healthcare industry, health of people and national health insurance finance.
BACKGROUND/OBJECTIVES: Nutrition is a determinant factor of health in elderly people. Independent living in elderly people can be maintained or enhanced by improvement of nutritional behavior. Hence, the present study was conducted to determine the impact of Health Belief Model (HBM)-based intervention on the nutritional behavior of elderly women. SUBJECTS/METHODS: Cluster-random sampling was used to assess the sample of this clinical trial study. The participants of this study attended a 12-week nutrition education program consisting of two (2) sessions per week. There was also a follow-up for another three (3) months. Smart PLS 3.5 and SPSS 19 were used for structural equation modeling, determination of model fitness, and hypotheses testing. RESULTS: The findings indicate that intervention had a significant effect on knowledge improvement as well as the behavior of elderly women. The model explained 5 to 70% of the variance in nutritional behavior. In addition, nutritional behavior was positively affected by the HBM constructs comprised of perceived susceptibility, self-efficacy, perceived benefits, and barriers after the intervention program. CONCLUSION: The results of this study show that HBM-based educational intervention has a significant effect in improving nutritional knowledge and behavior among elderly women.
This study was performed to determine the optimal doses of alfentanil or remifentanil (effect-site concentrations) required to prevent pain and other suffering after abdominal general surgery in ICU patients. A total of 52 general abdominal surgical patients (ASA IIIII) requiring artificial ventilatory care in the ICU were provided with either alfentanil (24 patients) or remifentanil (28 patients) through target controlled infusion (TCI). Alfentanil and remifentanil concentrations were titrated up and down until the pain score became less than 3 (VAS; Visual Analogue Score < 3). The effect-site concentrations (ng/ml) of alfentanil or remifentanil required to adequately control postoperative pain in the ICU were 64 +/- 12 and 1.9 +/- 0.5 for intubation with artificial ventilation, 57 +/- 9 and 1.7 +/- 0.7 for intubation with spontaneous ventilation, and 41 +/- 10 and 1.2 +/- 0.5 after extubation, respectively. Pain scores and the corresponding opioid concentrations were independent from respiratory condition. The three effect-site concentrations of alfentanil and remifentanil obtained from this clinical trial using the TCI technique can be a guideline in the administration of the same opioids to relieve the discomfort of ICU patients who have undergone abdominal general surgery.
Purpose: This study was conducted to compare pain, sense, bleeding, and hematoma that patients feel depending on the process of eliminating air of pressure band that was applied to patients after percutaneous coronary intervention (PCI) through radial artery. Methods: It was a nonequivalent control group pretestposttest design to compare pain, sense, bleeding, and hematoma by amount and time of eliminating air of pressure band after PCI through radial artery. Results: On arrival at Coronary Care Unit, meaningful difference didn't exist between pain and sense, however, after two hours of starting eliminating air, the points of pain and sense were meaningfully low (p<.001). Also, there was meaningful difference in reciprocal action between group and time (p<.001). Conclusion: After PCI, the conclusion showed there is positive effect in decreasing wrist pain and sense without any influence of bleeding by eliminating air from patients' pressure bands.
