High Rate of Gangrenous Adnexal Torsion: Dilemma of a Missing Silent Cancer

  • Background: Adnexal torsion results in ischemia of structures distal to twisted pedicle and acute onset of pain is responsible for about 3% of all gynecologic emergencies. Ovarian torsion classically occurs in a pathological enlarged ovary, as with cancer, but diagnosis remains a challenge. Objective: Our purpose was to evaluate clinical risk factors predictive of torsion with gangrenous adnexa. Material and methods: A retrospective descriptive study and chart review of surgically proven ovarian torsion/adnexal torsion cases at the Obstetrics and Gynecology Department of Prapokklao Hospital, Chanthaburi, Thailand between January 2011 and December 2015 was conducted. Result: Seventy-eight cases were identified. Mean age at presentation was 35.5 years. The average maximum diameter of the ovarian tumors was 10.8 cm. The percentage of gangrenous ovarian cysts in this study was 46.2 (36/78). The precision to determine the pathological site by patient, physician and ultrasonography was 8.5, 24.2 and 83.3 percent, respectively with statistically significant variation. Conclusion: Ovarian/adnexal torsion remains a challenge condition especially in young nulliparous women. Sophisticated investigation does not guarantee ovary preservation. Combining clinical acumen, appropriate tests and detailed consideration may be the best practice at the present time.

Dexamethasone Release from Glutaraldehyde Cross-Linked Chitosan Microspheres: In Vitro/In Vivo Studies and Non-Clinical Parameters Response in Rat Arthritic Model

  • The Dexamethasone (DEX) loaded chitosan microspheres were prepared by thermal denaturation and chemical cross-linking method using a dierent concentration of glutaraldehyde as chemical cross-linking agent. The prepared microspheres were evaluated for the percentage of Drug Loading (DL), Encapsulation Efficiency (EE) and surface morphology by Scanning Electron Microscopy (SEM). DL and EE were found to be maximum range of 10.0 to 10.79 % and 58.19 to 64.73 % respectively. The SEM Photographs of the resultant microspheres exhibited fairly smooth surfaces and predominantly spherical in appearance. In addition, Fourier Transform Infrared Spectroscopy (FTIR) and Differential Scanning Calorimetry (DSC) shown that there was no interaction between the drug and polymer. In vitro and in vivo release studies revealed that the release of dexamethasone was sustained and extended up to 63 days and effectively controlled by the extent of cross-linking agent. Non-clinical parameters such as paw volume, hematological parameters like Erythrocyte Sedimentation Rate (ESR), Paced Cell Volume (PCV), Total Leucocytes Count (TLC), Hemoglobin (Hb), Differential Cell Count (DCC) were investigated in Fruend's Complete Adjuvant (FCA) induced arthritic rats. Radiology and histopathological studies were also performed in order to evaluate the therapeutic efficacy of the DEX-loaded microspheres in extenuating the rat arthritic model.

Moyamoya Syndrome : A Window of Moyamoya Disease

  • Moyamoya-like vasculopathy develops in association with various systemic diseases and conditions, which is termed moyamoya syndrome. Relatively common diseases and conditions are related to moyamoya syndrome, including neurofibromatosis type 1, Down syndrome, thyroid disease, and cranial irradiation. Moyamoya syndrome shares phenotypical characteristics with idiopathic moyamoya disease. However, they differ in other details, including clinical presentations, natural history, and treatment considerations. The study of moyamoya syndrome can provide clinicians and researchers with valuable knowledge and insight. Although it is infrequently encountered in clinical practice, moyamoya-like vasculopathy can severely complicate outcomes for patients with various underlying diseases when the clinician fails to expect or diagnose moyamoya syndrome development. Furthermore, moyamoya syndrome could be used as a doorway to more enigmatic moyamoya disease in research. More comprehensive survey and investigation are required to uncover the secrets of all the moyamoya-like phenomena.

노령견에서 발생한 거대 비장 혈종 증례 보고 (A Case of Massive Splenic Hematoma in a Geriatric Dog)

  • A 10-year-old intact female, mixed breed dog presented with abdominal distention of 2 months duration and three days of decreased appetite. The patient was conscious and no other gross abnormalities were noted. A tentative diagnosis of idiopathic massive hematoma or hemangiosarcoma was made on evaluating all clinical findings. Splenectomy was performed for treatment of abdominal distention and histopathological investigation was initiated to confirm the diagnosis. The hematoma was extremely massive on gross morphology, with the size of $20.2{\times}12.4cm$ and the splenic mass was diagnosed as hematoma formation, with moderate to marked lymphoid hyperplasia and adjacent moderate extramedullary hematopoiesis, based on microscopic description of spleen histology. This is the first case report in veterinary literature of a dog with extremely rare splenomegaly, an unusually large hematoma, with no malignancy or remarkable clinical signs.

전남지역의 유산양 관절염.뇌염 감염 실태조사 (Investigation of carprine arthritis-encephalitis from dairy goat in Jeonnam province)

  • This survey was conducted to investigate the infection of caprine arthritis encephalitis virus (CAEV). From April to October 2007, we examined a total of 173 goats in 7 dairy goat-breeding farms located in Na-ju, Hae-nam, Young-am, and Young-gwang area of Jeonnam province. The results show that 41 (23.7%) goats are positive for CAEV, confirmed by nested PCR of 173 blood samples. Regional analysis revealed that a positive proportion for CAEV was 50.0% in Young-gwang, 25.6% in Young-am, 25.0% in Hae-nam, and 10.9% in Na-ju. Clinical signs were observed in 17 (9.8%) goats, of which 6 suffered from arthritis, 7 from mastitis, and 4 from pneumonia. Among the examined 173 goats, 8(4.6%) goats are positive for CAEV accompanied with CAE symptoms. There was a tendency to be much higher(p<0.05) levels of natrium(Na), chlorine(Cl) in CAEV-positive than in CAEV-negative goats. However, other serum biochemical values were no statistically significant effect.

Clinical Impact and Reliability of Carbonic Anhydrase XII in the Differentiation of Malignant and Tuberculous Pleural Effusions

  • Objective: To assess the practical utility of pleural fluid carbonic anhydrase XII (CAXII) quantification for differential diagnosis of effusions. Materials and Methods: Fluid was collected prospectively from fifty patients presenting with lymphocytic pleural effusions for investigation and CAXII was quantified by ELISA. Results: Pleural fluid CAXII concentrations were significantly higher in lung cancer patients (n=30) than in tuberculous controls (n=20). The sensitivity and specificity of this biomarker were 60%and 75%, respectively. CAXII measurement was not inferior to cytological examination in the diagnosis and exclusion of pleural effusions from lung cancer patitents (sensitivity 60% vs. 57%; specificity 75% vs. 100%; positive predictive value 77%; negative predictive value 54%). In patients with negative cytology, it offered a sensitivity of 54%. Conclusions: Pleural fluid CAXII is elevated in pleural effusions from lung cancer patients. Measurement of CAXII may be used in the future as a valuable adjunct to cytology in the diagnostic assessment of patients with pleural effusions related to lung cancer, especially when cytological examination is inconclusive.

Molecular Diagnosis for Personalized Target Therapy in Gastric Cancer

  • Gastric cancer is the second leading cause of cancer-related deaths worldwide. In advanced and metastatic gastric cancer, the conventional chemotherapy with limited efficacy shows an overall survival period of about 10 months. Patient specific and effective treatments known as personalized cancer therapy is of significant importance. Advances in high-throughput technologies such as microarray and next generation sequencing for genes, protein expression profiles and oncogenic signaling pathways have reinforced the discovery of treatment targets and personalized treatments. However, there are numerous challenges from cancer target discoveries to practical clinical benefits. Although there is a flood of biomarkers and target agents, only a minority of patients are tested and treated accordingly. Numerous molecular target agents have been under investigation for gastric cancer. Currently, targets for gastric cancer include the epidermal growth factor receptor family, mesenchymal-epithelial transition factor axis, and the phosphatidylinositol 3-kinase-AKT-mammalian target of rapamycin pathways. Deeper insights of molecular characteristics for gastric cancer has enabled the molecular classification of gastric cancer, the diagnosis of gastric cancer, the prediction of prognosis, the recognition of gastric cancer driver genes, and the discovery of potential therapeutic targets. Not only have we deeper insights for the molecular diversity of gastric cancer, but we have also prospected both affirmative potentials and hurdles to molecular diagnostics. New paradigm of transdisciplinary team science, which is composed of innovative explorations and clinical investigations of oncologists, geneticists, pathologists, biologists, and bio-informaticians, is mandatory to recognize personalized target therapy.

추나요법을 시행한 요통환자의 호전도와 Moire 영상 개선도와의 상관관계 (Investigation on the correlation Improvement Rate of Symptoms with Moire Topography Analytic Improvement Rate)

  • The present study retrospectively investigated clinical outcome at patients with low back pain or sciatica during Chuna treatment (flexion-distraction technique). The study population consisted of 29 patients. Discogenic group consisted of 21 patients who were already diagnosed as HNP of lumbar spine with serial MRIs(magnetic resonance imaging) or CTs(computerized tomography). Simple LBP group consisted of 8 patients with low back pain & sciatica who were non-specific disorder on radiologic examination. All patients were treated with flexion-distraction technique, one of Chuna technique, under analysis of Moire Topography. And the evaluation of clinical outcome was done twice during this study by Moire Topography Analytic Point and Low Back Pain Assesment, Visual Analogue Scale. The results were summarized as follows; Total improvement rate of Moire Topography was $25.8{\pm}17.8%$, and the rate of Low Back Pain Assesment was $56.5{\pm}23.0%$, Visual Analogue Scale of post-treatment was $32.6{\pm}22.5$ Between Improvement rate of Moire Topography and improvement rate of Low Back Pain Assesment, significant correlation was proved(Person's coefficient was 0.381, p<0.05). After all, it is certain improvement of Moire Topography represents symptom's improvement.

화병환자의 불안증 해소에 대한 침치료의 유효성 평가를 위한 단일군, 공개, 단일기관 임상시험 (The clinical trial for the significant effects of acupuncture on decreasing anxiety symptom of Hwa-Byung in a single institute- single-arm with Hwa-Byung, open lable)

  • Objectives : The purpose of this research is to investigate the effects of acupuncture on the anxiety symptoms regarding Hwa-Byung. Methods : Acupuncture was applied on 15 participants, 4 males and 11 females. Hwa-Byung was diagnosed by Hwa-Byung Diagnostic Interview Schedule(HBDIS). Then clinical research STAI, BDI, VAS, HRV was carried out for investigation to analyze the change of each symptom between pre-acupuncture and post-acupuncture. Results : STAI, VAS were all significantly decreased. BDI decreased, but the decrease was not significant. All physical and psychological scores of HBDIS were significantly decreased excluding A2, B2, B3, B5, C1, C5. Subjects of HRV were different between pre-acupuncture and post-acupuncture, but the difference was not significant. Conclusions : The results suggest that acupuncture has positive effects on anxiety and other symptom of Hwa-Byung.

  • 저자들은 14세 남아에서 4년간 3차례 발병한 Mollaret meningitis 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. 아울러 정확한 진단과 감별을 위해 뇌척수액의 세포학적 검사, herpes virus 동정, 뇌자기공명촬영 추적 검사 등이 필요할 것으로 사료된다.
