Use of Ultrasonography for Foot and Ankle Sports Injuries (족부 및 족관절 스포츠 손상에서 초음파의 활용)

  • Sports injuries of the foot and ankle are commonly encountered in clinical practice. Ultrasound is very useful for the diagnosis of such injuries, because it is more economical, readily accessible, and can perform a dynamic study compared to magnetic resonance imaging. This review focused on the sonographic features of common foot and ankle sports injuries.

The Clinical Study on Measurement of Foot Reflex Zone Acupoint Detection of Facial Paralysis Patients by Acupoints Detector (경혈탐측기를 이용한 말초성 안면신경마비환자의 족부반사구 변화에 대한 임상적 고찰)

  • Objectives : We investigate the characteristics of foot reflex zone acupoint of facial paralysis patients. Methods : In order to make a comparison between facial nerve paralysis patient group and non-facial paralysis group, we measured foot reflex zone acupoint detection in both group of 18 patients who were diagnosticated to facial nerve paralysis and 18 persons who were not. Results : 1. In comparing the means of the foot reflex zone, the measurements of facial nerve paralysis group is different significantly from non-facial paralysis group(p<0.05). 2. The measurement of detection of foot reflex zone acupoints, such as hypophysis(垂體), nose(鼻), cerebrum(大腦), neck(頸項), Trapezius muscle(僧帽筋), eye(眼) and ear(耳) of the facial nerve paralysis group is different significantly in comparison with non-facial paralysis group(p<0.05). But the measurement of detection of foot reflex zone acupoints, such as trigeminal nerve(三叉神經), cerebellum (小腦), kidney(腎), ureter(輸尿管) and urinary bladder(膀胱) of the facial nerve paralysis group is not defferent significantly in comparison with non-facial paralysis group(p>0.05). Conclusions : The results suggest that foot reflex zone can be used in the diagnosis and treatment of facial nerve paralysis.

Clinical Features of Tibialis Anterior Tendon Rupture (전경골건 파열의 임상 양상)

  • Purpose: Tibialis anterior tendon rupture is uncommon and usually caused by laceration. Rupture with an open window is often considered simple laceration, and thus diagnosis is often overlooked or delayed. The purpose of this study was to analyze the clinical features of tibialis anterior tendon rupture. Materials and Methods: Twenty-two patients treated for tibialis anterior tendon rupture from March 2015 to December 2019 were examined. Age, sex, rupture etiology, rupture location, and diagnostic and treatment delays were investigated. Results: Mean patient age was 45.7 years, and there were 14 males and 8 females. In 18 cases, rupture was caused by laceration and in 4 by spontaneous rupture. Of the 18 cases caused by laceration, 8 were lawnmower related, 8 were glass injuries, and 2 were caused by crush or degloving injuries, respectively. Three of the 4 spontaneous rupture cases and 4 of the 18 caused by laceration were overlooked. Conclusion: Tibialis anterior tendon rupture is rare and is easily overlooked. Close physical examination is essential to arrive at a correct initial diagnosis in patients with acute or chronic rupture, and greater care is needed in cases of glass injury.

The Results of Modified $Brostr{\ddot{o}}m$ Procedure for the Treatment of Chronic Lateral Ankle Instability (만성 족근 관절 외측 불안정성 환자에서 $Brostr{\ddot{o}}m$ 변형 술식의 치료 결과)

  • Purpose: This study was performed to evaluate clinical and radiographical results of treatment of chronic lateral ankle instability with modified Brostrom procedure. Materials and Methods: This is a retrospective study of 22 cases in 22 patients treated with modified $Brostr{\ddot{o}}m$ procedure under the diagnosis of chronic lateral ankle instability from May 2000 to August 2004. Average age was 32.3 years. Average follow-up period was 23 months. Preoperative and postoperative radiographs of ankle anteroposterior view, lateral view and varus stress view were analyzed. The clinical evaluation was performed according to the Americal Orthopaedic Foot and Ankle Society scales. Results: After the modified $Brostr{\ddot{o}}m$ procedure, Americal Orthopaedic Foot and Ankle Society scales score was improved by average 28 points. Among the 22 cases, 13 cases were excellent, 3 cases good, 4 cases fair, and 2 cases poor with satisfaction of 73%. Five cases had intermittent ankle pain of whom 3 cases had difficulty during competitive sports activity. Lateral tilting of talus improved by average 5.4 degrees on varus stress view. Conclusions: The modified $Brostr{\ddot{o}}m$ procedure is one of the most effective methods for treating of chronic lateral ankle instability.

A study on the traceability and uncertainty of skin hydration measurement on the sole of the foot

  • Objectives: In order to accurately measure skin moisture and accumulate normal skin moisture data, analyzing the uncertainty of the skin moisture measurement and establishing standard reference data is essential. It helps to evaluate the skin's condition and has great significance in diagnosis and treatment. In this study, the traceability of the equipment and the uncertainty of the results were analyzed for their potential use in the field of Korean medicine. The data was collected measuring skin moisture on the sole of the foot. Methods: One professional measured the moisture of the adult male's foot 10 times following a determined protocol. The standard uncertainty of repeated measurements was calculated from the mean value of 10 repeated measurements. Ten trainee participants also measured the adult male's foot following a determined protocol. The standard uncertainty by tester was calculated from the value of repeated measurements. Results: As a result of analyzing the difference between the mean and standard uncertainties in both the expert and trainees, it was confirmed that the variance in this study did not satisfy the normal distribution. In addition, the Mann-Whitney U-test was carried out, and it was found that there was no significant difference in the measured values of the two groups. The authorized uncertainty of measurements and traceability of all the equipment was not confirmed. Conclusions: This paper establishes the basis for later measurement-equipment research to provide the objective indicators to approach the dryness of plantar skin from dehydration, the Korean medicine perspective.

Subungal Exostosis of the Hallux (족모지 조갑하 외골종)

  • Subungal exostosis is a rare benign cartilaginous tumor affecting the distal phalanx. From 1995 to 1996, six subungal exostoses of the hallux were treated in the Chonnam national university hospital. The symptoms were subungal pain, mass on the distal phalanx elevating the nail or causing ulceration of the nail bed. The plain radiological examination showed a bony mass occurring on the dorsomedial or medial aspect of the distal phalanx. The diagnosis of the subungal exostosis of the hallux were suspected from clinical presentation and confirmed with radiographic examination. Histological patterns were fibrocartilaginous cap with the mature trabecular bone. Complete excision of the lesion including overlying nail bed was curative without recurrence in all cases.

Fistula of the ankle : A complication of ankle sprain (염좌 후에 발생한 족관절 누공)

  • Purpose: To investigate the clinical features of fistulas of the ankle joints. Materials and Methods: Seven fistulas in seven patients were reviewed during Apr. 2000 to Mar. 2002, retrospectively. There are five men and two wemen. Average age was 47.7 years (range, 42-65 years). Average follow-up period was 1.4 years. There were six cystic lesions after ankle sprain and one patient with persistent discharge after excision of bursa over lateral malleolus. Results: Duration from injury to presentation was average 9.8 years. The site of preoperative swelling was mostly over the lateral malleolus in five patients. In one patient, the area of swelling was extended to the anterolateral ankle joint and in another patient there was extensive swelling from Achilles tendon to the anterolateral ankle joint. Concomitant symptoms were instability in three patients, pain and instability in three patients. Methods of surgery were simple repair in one, modified Brostrom in three, augmentation with periosteal flap in addition to modified Brostrom in two and Chrisman-Snook in addition to augmentation with periosteal flap and modified Brostrom in one. There were no recurrence of instability as well as fistula. Conclusion: We think that the fistula of the ankle joint should be included in the differential diagnosis of the cystic lesion over the lateral malleolus and the result of surgical treatment would be satisfactory in most cases.

Kohler's Disease of the Tarsal Navicular: Long-Term Follow-up of 12 Years - A Case Report - (족부 주상골 무혈성 괴사: 12년 장기 추시 결과 -1예 보고-)

  • Kohler's disease is an uncommon idiopathic osteochondrosis of the tarsal navicular. The diagnosis is clinical and the natural history is benign. The treatment is symptomatic, including shoe supports when the symptoms are mild and initial cast immobilization for at least 8 weeks when the symptoms are more intensive. The final clinical outcome is always favorable. Although it has been fully described both clinically and radiologically, There have been a few long-term follow-up reports in worldwide. This case report presents a Kohler's disease of the tarsal navicular with long-term follow-up of 12 years.

Stiff-Person Syndrome: Diagnostic Difficulty (근육 강직 증후군: 진단적 어려움)

  • Stiff-person syndrome is a rare disorder, and the natural history of the syndrome has not been completely ascertained. The symptoms range from mild to severe and can progress over time: the final result can be significant disability. However, this syndrome is often misdiagnosed due to a lack of understanding of the clinical manifestations. We report the case of a patient who presented with slowly progressing gait disturbance and lower extremity pain and was later diagnosed as suffering from stiff-person syndrome. The patient experienced symptomatic improvement after the administration of benzodiazepines. No recurrence of symptoms has been reported. If the characteristic clinical features and electromyography findings of the syndrome are accurately interpreted, diagnosis of patients with abnormal muscle tension can be effectively done.

Broström Procedure and Fibula Periosteal Turn Down Augmentation for the Ball-and-Socket Ankle Accompanying Lateral Ankle Instability: A Case Report (Broström 술식과 비골 골막 젖힘 보강술로 치료한 구상 발목관절에 동반된 족근관절 외측 불안정증: 증례 보고)

  • We report on the case of a patient with chronic instability of the ball-and-socket ankle joint. The patient, a 21-year-old male, was diagnosed 10 years previously with chronic instability of the ball-and-socket ankle joint. He underwent Broström procedure and augmentation using a periosteal turn down from the fibula for this chronic instability despite having received conservative treatment since the diagnosis. After the procedure, the clinical symptoms of ankle instability were improved and the patient is being periodically followed-up. We report on this case of using a Broström procedure as a treatment option for patients with instability of the ball-and-socket ankle joint with normal range of foot and normal ankle joint alignment without damage in the joint and cartilage.
