Establishment and Clinical Application of an Electronic Database for Breast Cancer in China

  • Purpose: To establish a database for breast cancer patients to save and manage clinical data and to preliminarily investigate its clinical application. Materials and Methods: Information on breast cancer patients hospitalized in our department from 2008.01 to 2013.01 were input into our breast cancer management system. SPSS 16.0 software was used as a convenient reference to evaluate the accuracy of the newly built database. Results: A database of 2403 breast cancer patients was successfully established. Information in the database clearly displayed capabilities of storage, addition, retrieval, statistical analysis and other functions. As the continuously updated database showed, the distribution of age, sex, nationality, allergy history, pausimenia and marriage of patients was identical to that achieved by SPSS analysis, indicating reliable and accurate data analysis. Conclusions: The described database is easy and convenient to operate and manage, and should prove suitable for application in clinical research and treatment.

복부수술환자의 간호진단, 간호중재, 간호결과 연계 데이터베이스 구축 (Construction of Linkage Database on Nursing Diagnoses, Interventions, Outcomes in Abdominal Surgery Patients)

  • This reserch was to develop database software in order to handle a lot of clinical nursing data with nursing diagnoses, related factors, defining characteristics, nursing interventions, nursing activities and nursing outcomes. MS Access2000 and SQL was selected to use a general purpose database logic with an efficiency. MS Visual Basic 6.0 was used to construct the circumstance of Graphic User Interface. The Linkage Database of abdominal surgery patients was constructed from the clinical data and questionnaire. This database system could add related factors, defining characteristics, nursing activities in the database and analyze the statistical results through Access query. In the final stage, end-users satisfaction analysis using 5 points Likert scale was dong with the response of using the database system. The accuracy/trustworthiness of the database system was verified with the highest average scores as 4.42 and also, the efficiency as 4.21, user friendly function as 4.1.

Construction of an PFT database with various clinical information using optical character recognition and regular expression technique

  • The pulmonary function test (PFT) is an essential data source for evaluating the effect of drugs on the lungs or the status of lung function. However, the numeric values of PFT cannot be easily used for clinical studies without labor-intensive manual efforts, because PFTs are usually recorded as image files. This study was aimed at constructing a de-identified, open-access PFT database with various clinical information. For constructing the PFT database, optical character recognition (OCR), regular expression, and the parsing technique were used to extract alphanumeric data from the PFT images in a Korean tertiary teaching hospital. This longitudinal observational database contains 413,000 measurements of PFT from 183,000 patients.

  • 인구의 고령화와 의료서비스 수요의 증가로 한의학의 이론을 기초로 한 의료기기 개발이 증가하고 있다. 개발한 의료기기의 임상적 유효성 확보를 위한 다양한 임상시험들이 수행되고 있으며, 이 과정에서 다양하게 수집되는 정보들의 체계적인 관리의 중요성이 증가하고 있다. 본 논문에서는 한의학의 대표적인 진단 방법인 맥진을 기기화 한 맥진기를 이용하여 임상시험을 진행하는데 있어 환자의 의학적 정보, 맥진기의 측정 정보들을 체계적으로 관리할 수 있는 데이터베이스를 설계하고 구축하였다. 구축된 데이터베이스는 임상시험에 대한 다양한 정보 및 측정된 정보의 질관리를 통해 기존 알고리즘 검증 또는 새로운 알고리즘 개발 시 효율적인 대응이 가능하고, 임상 데이터의 통제 측면에서도 여러 장점들을 가지고 있다. 본 연구의 결과는 이종의 맥진기나 다른 생체신호 측정기와의 정보 공유가 가능한 통합 데이터베이스 관리 시스템 구축을 위한 한의 의료기기 데이터 표준안 구성에 기여할 수 있을 것이다.

  • The Korean functional Genomics Project focuses on stomach and liver cancers. Specimens collected by six hospital teams are used in BNA microarray experiments. Experimental conditions, spot measurement data, and the associated clinical information are stored in a relational database. Microarray database schema was developed based on EBI's ArrayExpress. A diagrammatic representation of the schema is used to help navigate over marty tables in the database. Field description, table-to-table relationship, and other database features are also stored in the database and these are used by a PERL interface program to generate web-based input forms on the fly. As such, it is rather simple to modify the database definition and implement controlled vocabularies. This PERL program is a general-purpose utility which can be used for inputting and updating data in relational databases. It supports file upload and user-supplied filters of uploaded data. Joining related tables is implemented using JavaScripts, allowing this step to be deferred to a later stage. This feature alleviates the pain of inputting data into a multi-table database and promotes collaborative data input among several teams. Pathological finding, clinical laboratory parameters, demographical information, and environmental factors are also collected and stored in a separate database. The same PERL program facilitated developing this database and its user-interface.

The National Clinical Database as an Initiative for Quality Improvement in Japan

  • The JCVSD (Japan Cardiovascular Surgery Database) was organized in 2000 to improve the quality of cardiovascular surgery in Japan. Web-based data harvesting on adult cardiac surgery was started (Japan Adult Cardiovascular Surgery Database, JACVSD) in 2001, and on congenital heart surgery (Japan Congenital Cardiovascular Surgery Database, JCCVSD) in 2008. Both databases grew to become national databases by the end of 2013. This was influenced by the success of the Society for Thoracic Surgeons' National Database, which contains comparable input items. In 2011, the Japanese Board of Cardiovascular Surgery announced that the JACVSD and JCCVSD data are to be used for board certification, which improved the quality of the first paperless and web-based board certification review undertaken in 2013. These changes led to a further step. In 2011, the National Clinical Database (NCD) was organized to investigate the feasibility of clinical databases in other medical fields, especially surgery. In the NCD, the board certification system of the Japan Surgical Society, the basic association of surgery was set as the first level in the hierarchy of specialties, and nine associations and six board certification systems were set at the second level as subspecialties. The NCD grew rapidly, and now covers 95% of total surgical procedures. The participating associations will release or have released risk models, and studies that use 'big data' from these databases have been published. The national databases have contributed to evidence-based medicine, to the accountability of medical professionals, and to quality assessment and quality improvement of surgery in Japan.

한의 임상 정보의 효율적 통합을 위한 한의임상 데이터베이스 및 E-CRF 입력 시스템 구축 (Implementation of database and E-CRF for efficient integration of Korean clinical data)

  • 최근 의학기술의 발전과 더불어 서양의학 뿐만 아니라 한의학 분야에서도 임상 데이터에 대한 통합 및 표준화에 관한 연구가 활발히 진행 중에 있다. 유사한 임상시험 뿐만 아니라 전혀 다른 임상시험의 데이터도 하나의 표준에 맞춰 통합 구축된다면 통합된 의료데이터는 암묵적 한의의료지식 도출연구에 활용될 수 있다. 따라서 본 논문에서는 한의임상 정보를 효율적으로 저장하기 위하여 국제표준으로 널리 사용되는 CDISC 표준안을 기반으로 한의임상 데이터베이스를 구축하였고, 임상현장에서 편리한 데이터 입력을 위해 E-CRF를 구축하였다. 아울러, 실제 4개의 임상연구에 대한 데이터 저장과정을 거쳐 한의임상 데이터 통합에 대한 예를 보였다. 우리의 연구 결과는 통합된 데이터로부터 암묵적 의료지식도출을 위한 기반을 마련하였고, 데이터 통합을 통한 효율적 관리뿐만 아니라 반복적이거나 불필요한 임상시험 방지, 정제 된 데이터의 재배포를 통하여 연구의 편리성과 협업을 촉진할 수 있다.

Collective Experience: A Database-Fuelled, Inter-Disciplinary Team-Led Learning System

  • We describe the framework of a data-fuelled, interdisciplinary team-led learning system. The idea is to build models using patients from one's own institution whose features are similar to an index patient as regards an outcome of interest, in order to predict the utility of diagnostic tests and interventions, as well as inform prognosis. The Laboratory of Computational Physiology at the Massachusetts Institute of Technology developed and maintains MIMIC-II, a public deidentified high- resolution database of patients admitted to Beth Israel Deaconess Medical Center. It hosts teams of clinicians (nurses, doctors, pharmacists) and scientists (database engineers, modelers, epidemiologists) who translate the day-to-day questions during rounds that have no clear answers in the current medical literature into study designs, perform the modeling and the analysis and publish their findings. The studies fall into the following broad categories: identification and interrogation of practice variation, predictive modeling of clinical outcomes within patient subsets and comparative effectiveness research on diagnostic tests and therapeutic interventions. Clinical databases such as MIMIC-II, where recorded health care transactions - clinical decisions linked with patient outcomes - are constantly uploaded, become the centerpiece of a learning system.

한약 임상시험의 특성 : 질병관리본부 임상연구정보서비스(CRIS)를 중심으로 (The Characteristics of KM Clinical Trials - Focusing on the Clinical Research Information Service (CRIS))

  • Objectives: To examine the fundamental characteristics of interventional clinical trials registered in the CRIS (Clinical Research Information Service) database. Methods: We systematically analyzed the status of Korean medical interventions using the search function of the CRIS database. Results: From 2010 to January 2020, 267 Korean medicine-related clinical trials were registered with CRIS. The studies, which included many randomized trials (80.14%), focused primarily on demonstrating the effectiveness and safety of acupuncture and Korean herbal remedies. Most of these studies were government-sponsored and researcher-led clinical trials, but a few were company-sponsored trials. All trials were small and need improvement. Conclusion: This study reviews the current status of clinical trials in the field of Korean medicine and the basic data that can be used to create evidence-based Korean medicine.

국내 한약-양약 상호작용 연구논문 분석을 통한 상호작용 정보 현황파악 및 제공방안에 대한 연구 (Current Status of Herb-Drug Interaction Information and Information Database in Korea)

  • The objectives of this study are to examine the current status and problems of herb-drug interaction (HDI) information and information database in Korea and suggest the better way to establish useful HDI database. We collected HDI studies that published in Korea and analyzed according to objective, methods, selection criteria of herbs, number of study, correlation between study subject and frequently used herbal medicine (HM). Then we selected representative HM database on the internet made by Korea Food and Drug Administration (KFDA) among the several databases and analyzed its contents related to HDI. Several HDI studies were carried out from laboratory based research to clinical trials and HM databases have been developed for providing information about different aspects of traditional Korean medicine. But the information of HDI and information database are still far from practical applications because there are no coherence to select study subjects and methods among researchers. So, it is necessary to build up HDI database led by the government for providing systematic HDI information. HDI information database is expected to be able to provide useful evidence for health professionals in prescription and consultation to reduce the chance of adverse effects and improve the quality of medical care.
