The status of clinical practice for students in the department of dental hygiene (치위생과 학생의 임상실습 교육 실태에 관한 조사연구)

  • The number of dental hygiene students is increasing as dental hygiene departments are extended or newly installed continuously, and in other to keep up with the quantitative increase, we need to standardize dental hygiene curriculums. The present study conducted a questionnaire survey with dental hygiene students who had completed clinical practice. The obtained results as follows. 1. According to the contents of clinical practice at dental clinics, the frequency of dental hygiene students' observation practice was high in basic medical service, dental prosthesis, and orthodontics. 2. The frequency of performance practice was high in basic medical service, oral medicine, preventive dentistry, pediatric dentistry, periodontology, and oral surgery. According to the area of clinical practice. 3. According to the contents of clinical practice at university hospitals, the frequency of dental hygiene students' observation practice was high in basic medical service, dental prosthesis, and orthodontics. 4. The frequency of performance practice was high in basic medical service, oral medicine, preventive dentistry, pediatric dentistry, periodontology, and dental prosthesis. 5. The students' satisfaction was high in basic medical service, oral medicine, preventive dentistry, and periodontology. The period of clinical practice varies according to school curriculum and circumstance among dental clinics and university hospitals where clinical practices are performed, students' satisfaction with their observation practice and performance practice may be different. Thus, for dental hygiene students' clinical practice, it is considered desirable to prepare integrated education programs that standardize the period and contents of clinical practice.

  • 'Personal Healthcare Records' (PHR) is a service for providing individual clinical data to patients. PHR service should be useful for the patient and healthcare service providers. This study has aimed at not only providing patients' clinical data but also developing optimized healthcare service for every patients. The research has been conducted as 3 phases: formal case analysis, caregivers interviews and patients interviews. The patients interviews were limited to cancer patients. As results, 3 key functions have been developed. First, it offers patient's clinical pathway as a personalized medical treatment scheduler. Second, it supplies Question & Answer board on online. Last, it supports patients to input their healthcare record. This Myongji PHR service has 3 months of pilot test on web and mobile application(android version). For further commercialized PHR service, the standardization for clinical pathway registration and user convenience need to be considered.

Objects The objectives of this study were to examine the present state of Oriental clinical service based on Constitutional medicine and to make an accurate estimate of it’s scale after 10 years. 2. Methods The data for this study were collected from a questionnaire survey to 319 Traditional Korean Medical doctors who were selected as random samples. And this questionnaire is consist of several inquiries related with management and application of constitutional medicine. 3. Results and Conclusions The major results of this survey are following: l. The 52.8% facilities of all Oriental medical centers and Clinics in the whole country are applying Constitutional medicine for treating patients. 2. The 24.8% persons of all health workers are engaged in Constitutional clinical service. 3. The proportion of sales by Constitutional clinical services to the total sales by Traditional clinical service is 23.5% in 2005. 4. The proportion of application to medical care insurance by Constitutional clinical service is 17.9% of all amounts After taking these results into consideration, the sales of Constitutional clinical service now are estimated to make up 23.8% percent of the total traditional medical service and the amount of it’s sales after 10 years is expected to increase up to 3.6 times as much as the current quantity.

Clinical sign and transmission of foot-and-mouth disease in deer, Review (사슴에서의 구제역 증상과 전파 가능성)

  • Foot-and-mouth disease (FMD) commonly infects cloven-hoofed livestock animals such as cattle, pig, sheep, and goat and its clinical signs are well-known. Besides livestock, FMD can be transmitted among cloven-hoofed animals in the wild. FMD mostly affects livestock animals in farms, but, wild animals are likely to play a pivotal role in spreading the disease due to their way of free living. In the case of deer, the clinical signs of FMD vary widely from subclinical to severe infections. Thus, in some deer species, it may be hard to verify clinical signs of FMD. A deer may carry the virus up to 11 weeks after exposure, shedding the virus during the period. However, deer is not considered as a typical host for persistent infection like buffalo, cattle or sheep. In Korea, small-scale livestock farms which have less than 10 animals make up 63.6% of the entire livestock farms. Considering raising environment in deer farms, it is assumed that the risk of virus excretion and consequent transmission of FMD among deers is relatively lower than other cloven-hoofed animals. However, Sika deer and Elk which are typical deer species in Korea would manifest mild to subclinical symptoms upon FMD infection. Therefore, laboratory testing is necessary to confirm FMD in these animals because of difficulty in verifying clinical signs and the risk of virus shedding during inapparent infection.

  • Zoster-associated limb paresis is a relatively uncommon complication of herpes zoster that is characterized by focal motor weakness. Awareness of this disorder is important to avoid unnecessary invasive investigations and to ensure appropriate treatment. We report a case of a herpes zoster involving the femoral nerve.

An App-based Evolving Medical Nomogram Service System (앱기반 진화 의료 노모그램 서비스 시스템)

  • Clinical nomogram is a graphical representation of numeric formula, constructed from clinical cases database of followed patients' treatment, which is used for medical predication. For a clinical nomogram to contribute patient care, it is required to accumulate as many as clinical cases and to extract medical prediction knowledge. It needs to be equipped with an effective method to build medical nomogram with high predication accuracy. It is desirable for medical nomogram to be accessible at patient care point. This paper proposes a medical nomogram service system architecture which takes into account the above-mentioned issues. The proposed system architecture includes a web-based database subsystem to maintain and keep track of clinical cases. On the periodic basis, a new clinical nomogram is reconstructed for the updated clinical database. For the convenient use of patient care practice environment, an app-based program is provided which makes prediction based on the most recent clinical nomogram constructed in the service system. The proposed method has been applied to a clinical nomogram service system development for recurrence and survival prediction in bladder cancer patients.

  • With the advances in ubiquitous service infrastructure, healthcare services have drawn attention as one of promising application domains. In ubiquitous healthcare services, patients or care-givers as well as medical personnel are asked to play their roles and, in addition, the information system is supposed to have active roles. In medicine, clinical protocols have been developed and put into practice in order to reduce treatment variances and assure the service quality. In the same token, clinical protocols on ubiquitous service practices are need to be developed which takes into account both the clinical details and the ubiquitous service functionality. This paper introduces a clinical protocol modeling methodology which pays attention to participants and their tasks including contextual information. The proposed method has been successfully applied to a real application domain, OAB(overactive bladder) syndrome patient care to see how it builds a clinical protocol.

Factors Affecting Clinical Competence among Nursing Students (간호대학생의 임상수행능력에 영향을 미치는 요인)

  • The purpose of this study was to identify the association of factors related to clinical practice training for clinical competence among nursing students, and to analyze those factors influencing clinical competence, providing recommendations for improving their clinical competence and clinical learning environment. This descriptive correlative study completed organized questionnaires from 557 nursing students. The data was analyzed by SPSS 19.0. The most important factor affecting the clinical competence among nursing students was teaching effectiveness (${\beta}=.22$). followed by critical thinking (${\beta}=.19$). and the professional self-concept (${\beta}=.19$). The explained variable for clinical competence was 45.2% in nursing students. Thus, the development of an effective clinical internship program is important for strengthening nursing students' clinical competence. We suggest that the capability of nursing students should be strengthened and effective clinical internship programs should be developed to improve the clinical competence of nursing students.

The Major Barriers to Provision of Pharmaceutical Care Service in Compliance with Internationally Recognized Good Pharmacy Practice Standards in Community Pharmacies in Korea: A Questionnaire Survey (한국의 지역약국에서 국제통용 우수약무기준의 약료서비스 제공을 어렵게 하는 주요 장애요인의 분석: 설문조사 연구)

  • Objective: The provision of pharmaceutical care service in compliance with good pharmacy practice (GPP) standards is important, but there is lack of studies to investigate the barriers that significantly hinder community pharmacies in Korea from adhering to the standards. This study was aimed to identify the major barriers to provision of pharmaceutical care service in compliance with the proposed pharmacy practice standards which have been developed based on the GPP standards recommended jointly by WHO and FIP. Methods: Questionnaires reviewed by the expert committee were posted for 32 days on the website which is most frequently accessed by community pharmacists. The respondents completed them by checking the scores (Max=5, Min=1) for major barriers to provision of pharmaceutical care service focusing on patient information management and drug use review process in prescription fillings. The answered scores were automatically collected using online data processing. Mean differences between scored data were assessed by ANOVA. Results: Total 321 pharmacists participated in the survey. Results indicated that 'difficulty of diagnosis identification' (m=3.92, SD=1.21), 'lack of time' (m=3.48, SD=1.22) and 'lack of updated clinical information' (m=3.17, SD=1.10) were the major barriers to provision of pharmaceutical care service in patient information management. The main barriers to drug utilization review were 'lack of time' (m=3.32, SD=1.21), 'lack of updated clinical information' (m=3.11, SD=1.17), and 'negative feedbacks or refusals from prescribers' (m=3.00, SD=1.38). There were significant differences among the groups by location, employed number of pharmacists and acceptability to the proposed GPP standards. Conclusion: Difficulties in managing patient clinical information and lack of time were found to be the major barriers in providing pharmaceutical care services in community pharmacies in Korea. Further research is recommended to determine ways to reduce these barriers in order to provide quality pharmaceutical care service that is in compliance with the internationally recognized GPP guidelines.

The Effects of Introductory Clinical Practice Education Program (임상입문 특별실습교육프로그램의 운영 및 효과)

  • The purpose of this study was to evaluate the effects of introductory clinical practice education program for nursing students. The nursing students of 94 participated the introductory education program of 80 hours. The questionnaire survey on self efficacy and clinical competency were conducted 3 times(before education, after education, 2 months after education) to the nursing students. The data were analyzed by the SPSS 17.0 program. The results were as follows; The self efficacy and the clinical competency score showed a significant change in the posttest. Self efficacy and clinical competency have significant relationships with college life satisfaction, interpersonal relationship and academic aptitude satisfaction. Also, self efficacy has significant relationship with clinical competency and clinical practice satisfaction. As a result, the introductory clinical practice education program can be helpful to adjustment for nursing students before clinical practice.
