$Papillon-Lef\`{e}vre$ Syndrome의 임상 및 미생물학적 검사와 치료 (A CLINICAL AND BACTERIOLOGICAL EXAMINATION AND TREATMENT OF $PAPILLON-LEF\`{E}VRE$ SYNDROME)

  • The $Papillon-Lef\`{e}vre$ Syndrome(PLS), a disease with autosomal recessive inheritance, is characterized by diffuse hyperkeratosis of the palms and soles, mostly prepubertal periodontitis and premature loss of primary and permanent dentition. The etiology of the destruction of periodontal tissues has not been completely clarified. In recent years, two main factors are suggested to be responsible for tooth loss ; firstly, the presence of gram negative microorganisms in the periodontal pockets of the patients. The other factor suggested is cellular deficiency in chemotaxic and phagocytic function of neutrophylic granulocytes. Resent data suggestes that mechanical debridement in conjunction with antibiotic therapy may be successful in periodontal management of $Papillon-Lef\`{e}vre$ Syndrome, particularly if administered early. In this study, a $Papillon-Lef\`{e}vre$ Syndrome patient was studied clinically, radiologically, histopathologically and microbiologically. 5 years female patient with gingival swelling and destruction of periodontal structure on the whole dentition were examined and palmar and plantar hyperkeratosis were can be seen. On microbiological analysis, Actinobacillus actino-mycetemcomitans was performed. Concurrently, the children recieved extraction of maxillary anterior teeth and construction of removable prosthetis. The combination of professional oral hygiene care and antibiotic therapy improved the dermatologic and periodontal condition.

Rapid One Step Detection of Pathogenic Bacteria in Urine with Sexually Transmitted Disease (STD) and Prostatitis Patient by Multiplex PCR Assay (mPCR)

  • We developed a multiplex PCR (mPCR) assay to simultaneously detect Chlamydia trachomatis, Neisseria gonorrhoeae, Mycoplasma genitalium, Ureaplasma urealyticum, Corynebacterium spp. and seudomona aeruginosa. This method employs a single tube and multiple specific primers which yield 200, 281, 346, 423, 542, and 1,427 bp PCR products, respectively. All the PCR products were easily detected by agarose gel electrophoresis and were sequenced to confirm the specificity of the reactions. To test this method, DNA extracted from urine samples was collected from 96 sexually transmitted disease or prostatitis patients at a local hospital clinical center, and were subjected to the mPCR assay. The resulting amplicons were cloned and sequenced to exactly match the sequences of known pathogenic isolates. N. gonorrhoeae and Corynebacterium spp. were the most frequently observed pathogens found in the STDs and prostatitis patients, respectively. Unexpectedly, P. aeruginosa was also detected in some of the STD and prostatitis samples. More than one pathogen species was found in 10% and 80.7% of STD and prostatitis samples, respectively, indicating that STD and prostatitis patients may have other undiagnosed and associates. The sensitivity of the assay was determined by sing purified DNA from six pathogenic laboratory strains and revealed that this technique could detect pathogenic DNA at concentrations ranging from 0.018 to $1.899\;pg/{\mu}l$. Moreover, the specificities of this assay were found to be highly efficient. Thus, this mPCR assay may be useful for the rapid diagnosis of causative infectious STDs and prostatitis. useful for the infectious STDs and prostatitis.

Investigation of bacteremia after toothbrushing in orthodontic patients

  • Objective: The aim of this study was to investigate the occurrence rate of bacteremia following toothbrushing with toothpastes composed of several antibacterial agents and compare the results with the conventional oral hygiene maintaining methods in orthodontic patients. Methods: This clinical study included 100 adult orthodontic patients who were divided into 4 groups. Each group comprised of 25 patients, wearing fixed orthodontic appliances. In the first group, bacteremia was assessed after toothbrushing without using any toothpaste. In the second group, a 0.2% chlorhexidine gluconate mouthrinse was used before brushing with no toothpaste. In the third group, subjects brushed with a commonly used toothpaste which did not include an additional antimicrobial agent. The fourth experimental group used toothpaste which included tea tree oil, clove oil, peppermint oil and bisabolol as antimicrobial elements. Pre- and post-brushing blood samples were obtained using a strict aseptic technique. All samples were microbiologically evaluated using blood culture bottles. Results: Toothbrushing in orthodontic patients yielded to an increase in the occurrence rate of bacteremia when using normal toothpaste or no toothpaste at all. Conclusions: The use of chlorhexidine mouthwash before toothbrushing, and brushing with antimicrobial toothpaste did not show a statistically significant difference in preventing the occurrence of bacteremia (p > 0.05).

Effects of Soybean Peptide Intake on Antioxidant Enzyme, Cortisol and Inflammatory Cytokines in High School Judo Athletes

  • The purpose of this study was to investigate the effect of soybean peptide on antioxidant enzymes, cortisol hormone and inflammatory cytokine levels. 19 high school male judo athletes participated in the experiments. They were randomly divided into two groups, one group took soybean peptide (S-peptide, n=10) 4 g a day for 4 weeks and the other group placebo (placebo group, n=9) for the same time. Blood samples were collected before intake, after 2 weeks intake and 4 weeks intake and these were analyzed for total antioxidant status (TAS), catalase (CAT), levels of cortisol hormone, tumor necrosis factor-alpha (TNF-${\alpha}$) and interleukin-6 (IL-6). As a result, the S-peptide group was significantly increased in TAS and CAT (P<0.05). The malondialdehyde (MDA) levels showed decrease after soybean peptide intake but there was no significant difference. In the levels of plasma cortisol which reflect stress status, there was significantly decreased in the S-peptide and placebo group after 4 weeks (P<0.05). There were significant decreases of TNF-${\alpha}$ and IL-6 after 4 weeks in S-peptide group (P<0.05). These results suggest that the intake of soybean peptide can activate antioxidant defenses and decrease exercise-induced oxidative stress.

Transferrin-Conjugated Liposome/IL-12 pDNA Complexes for Cancer Gene Therapy in Mice

  • Transferrin ($T_{f}$) has been used as a targeting ligand for delivering liposome/interleukin-12 (IL-12) pDNA complexes to cancer cells mostly due to the greater number of transferrin receptors ($T_{f}R$) found on tumor cells than on normal cells. $T_{f}$ was conjugated to liposomes via the reaction of MPB-PE with thiol groups of $T_{f}$ introduced by a heterobifunctional cross-linking agent, N-succinimidyl-3-(2-pyridyldithio)propionate (SPDP). Four days after C26 inoculation when the tumor volume reached ${\sim}100mm^{3}$, tumor-bearing Balb/c mice were injected intravenously with $T_{f}-liposome/IL-12 pDNA$complexes twice a week for 3 weeks. Significant suppression of tumor growth was achieved in the group treated with the $T_{f}-liposome/IL-12 pDNA$ complexes, with a dose of $10{\mu}g$ of IL-12 pDNA showing the highest suppression effect among the tested doses. Similar results were obtained when the therapy was initiated one day after tumor inoculation, although in this case $30{\mu}g$ IL-12 pDNA/$T_{f}-liposome$ complexes showed a significant suppression of tumor growth between 19 and 23 days after tumor inoculation. This result indicates that the transferrin receptor-targeted liposomal system is an efficient delivery agent of therapeutic genes, such as IL-12, in mice and that its potential clinical use warrants further research investigation.

Reovirus and Tumor Oncolysis

  • REOviruses (Respiratory Enteric Orphan viruses) are ubiquitous, non-enveloped viruses containing 10 segments of double-stranded RNA (dsRNA) as their genome. They are common isolates of the respiratory and gastrointestinal tract of humans but are not associated with severe disease and are therefore considered relatively benign. An intriguing characteristic of reovirus is its innate oncolytic potential, which is linked to the transformed state of the cell. When immortalized cells are transfected in vitro with activated oncogenes such as Ras, Sos, v-erbB, or c-myc, they became susceptible to reovirus infection and subsequent cellular lysis, indicating that oncogene signaling pathways are exploited by reovirus. This observation has led to the use of the virus in clinical trials as an anti-cancer agent against oncogenic tumors. In addition to the exploitation of oncogene signaling, reovirus may further utilize host immune responses to enhance its antitumor activity in vivo due to its innate interferon induction ability. Reovirus is, however, not entirely benign to immunocompromised animal models. Reovirus causes so-called "black feet syndrome" in immunodeficient mice and can also harm neonatal animals. Because cancer patients often undergo immunosuppression due to heavy chemo/radiation-treatments or advanced tumor progression, this pathogenic response may be a hurdle in virus-based anticancer therapies. However, a genetically attenuated reovirus variant derived from persistent reovirus infection of cells in vitro is able to exert potent anti-tumor activity with significantly reduced viral pathogenesis in immunocompromised animals. Importantly, in this instance the attenuated, reovirus maintains its oncolytic potential while significantly reducing viral pathogenesis in vivo.

Co-infection of Aspergillus spp. with Erysipelothrix rhusiopathiae in a red-crowned crane: a case report

  • This case describes outbreaks of acute aspergillosis in a red-crowned crane. A six-month-old, male, crane had showed clinical signs (i.e. anorexia, performance loss, ruffled feathers and drooped wings and open mouth breathing, etc.) before death. In necropsy examination, spherical to oval nodules disseminated from the respiratory tract to other organs. Those nodules were formed predominantly in air sacs, lung, peritoneum, serosa of esophagus and trachea. The nodules varied in size from 1 mm to over 1cm and the color was white to yellow. Microscopically, most of lung architecture were replaced by multiple foci which were characterized by well demarcated eosinophilic and karyorrhetic debris and surrounded by numerous Inflammatory cell. Most within necrotic center of the nodules, large numbers of fungal hyphae were present. Microbiology result indicated fungal growths on sabroud dextrose agar and bacterial growths on blood agar. Bacteria identified as E. rhusiopathiae using MALDI-TOF (microflex, BRUKER, USA) and fungi identified as A. fumigatus, A. terreus by sequencing the ITS1 and ITS4 regions. To confirm the route of infection, we checked the existence of the same pathogens in cohabitant (i.e. mother crane). The young age and weakened immunity (i.e. bacterial infection, etc.) causes fatal aspergillosis in birds.

Iron Increases Susceptibilities of Pseudomonas aeruginosa to Ofloxacin by Increasing the Permeability

  • Iron increased the susceptibilities of clinical isolates Pseudomonas aeruginosa to quinolones. In the presence of iron, increased susceptibilities to ofloxacin were observed in twenty-six out of thirty isolates and with no change in four isolates. In the case of norfloxacin, iran increased susceptibilities of twelve isolates but did not render any change in eighteen isolates. In the case of ciprofloxacin, iron decreased the MICs (Minimal Inhibitory Concentration) of twenty isolates, increased the MIC of one isolate, and did net change the MICs of nine isolates. To find out how iron increased susceptibility to ofloxacin, bacterial cells were grown in Muller Hinton (MH) media and succinate minimal media (SMM) to induce iran acquisition systems and the intracellular ofloxacin concentrations were assayed in the presence of iron. The addition of iron to the media decreased the MICs of cells whether they were grown in MH or SMM. Siderophores, carbonyl cyanide m-chlorophenylhydrazone (an inhibiter of proton motive force), and ouabain (an inhibitor of ATPase) did not decrease the effect of iron. Results suggested that the increase in the intracellular ofloxacin concentration by iron is accomplished not by decreasing the efflux but by increasing the of ofloxacin permeability.

메티실린 내성 황색포도알균 판독에 있어 항균제 감수성 검사와 mecA PCR법의 비교 (Comparison Between Antimicrobial Susceptibility Test and mecA PCR Method for Reading of Methicillin-Resistant Staphylococcus aureus)

  • Methicillin-resistant Staphylococcus aureus (MRSA)는 화농성 질환, 균혈증을 유발하고 병원내 감염의 주요 원인균으로 알려져 있다. 병원에서의 MRSA 분리율은 점차 증가하여 80% 이상으로 보고 되고 있으며 Methicillin 뿐만 아니라 다른 항균제에도 내성을 나타냄으로 치료를 위한 항균제 사용에 제한을 받고 있다. 이에 본 연구에서는 MRSA의 정확한 판정을 통해 항생제 남용을 막고자 대부분의 병원에서 사용하는 항균제 감수성 검사법과 경제적인 면으로 인해 병원내에서 많이 사용되고 있지 않지만 정확도가 높은 mecA 유전자 검출법을 서로 비교하였다. 그 결과 대조군인 Methicillin-susceptible Staphylococcus aureus와 실험군 MRSA 20 균주를 대상으로 항균제 감수성 검사법과 mecA 유전자 PCR 검출법을 실시한 결과 MRSA 20 균주는 Oxacillin과 Cefoxitin에 모두 내성을 나타냈으나 mecA 유전자 검출에서는 20 개 중 17 개에서만 유전자가 검출되어 염기서열분석 결과 mecA 유전자임을 확인하였다. 이런 결과로 보아 mecA(-) 3 균주는 mecA 유전자의 변이로 추측할 수 있기에 임상에서의 MRSA의 판정은 항균제 디스크법과 mecA 유전자 PCR 검출법을 동시에 사용함으로 정확한 MRSA 진단에 도움을 주고자 한다.

Pathogenic free-living amoebae in Korea

  • Acanthamoeba and Naegleria are widely distributed in fresh water, soil and dust throughout the world, and cause meningoencephalitis or keratoconjunctivitis in humans and other mammals. Korean isolates, namely, Naegleria sp. YM-1 and Acanthamoeba sp. YM-2, YM-3, YM-4, YM-5, YM-6 and YM-7, were collected from sewage, water puddles, a storage reservoir, the gills of a fresh water fish, and by corneal washing. These isolates were categorized into three groups based on the mortalities of infected mice namely, highly virulent (YM-4), moderately virulent (YM-2, YM-5 and YM-7) and nonpathogenic (YM-3). In addition, a new species of Acanthamoeba was isolated from a freshwater fish in Korea and tentatively named Korean isolate YM-4. The morphologic characters of its cysts were similar to those of A. culbertsoni and A. royreba, which were previously designated as Acanthamoeba group III. Based on experimentally infected mouse mortality, Acanthamoeba YM-4 was highly virulent. The isoenzymes profile of Acanthamoeba YM-4 was similar to that of A. royreba. Moreover, an anti-Acanthamoeba YM-4 monoclonal anti-body reacted only with Acanthamoeba YM-4, and not with A. culbertsoni. Random amplified polymorphic DNA marker analysis and RFLP analysis of mitochondrial DNA and of a 188 small subunit ribosomal RNA, placed Acanthamoeba YM-4 in a separate cluster based on phylogenic distances. Thus Acanthamoeba YM-4 was identified as a new species, and assigned Acanthamoeba sohi. Up to the year 2002 in Korea, two clinical cases were found to be infected with Acanthamoeba spp. These patients died of meningoencephalitis. In addition, one case of Acanthamoeba pneumonia with an immunodeficient status was reported and Acanthamoeba was detected in several cases of chronic relapsing corneal ulcer, chronic conjunctivitis, and keratitis.
